Growing up with Klinefelter syndrome: challenges of care in children and adolescents and patients advocacy.

Klinefelter syndrome (KS) is a common, yet underdiagnosed sex chromosome aneuploidy characterized by considerable phenotypic variability and multisystem involvement beginning in early life. The increasing use of noninvasive prenatal testing has led to earlier diagnosis, highlighting the need to redefine clinical management across the pediatric age. In this narrative review, pediatric endocrinologists, neuropsychiatrists, psychologists, and genetic experts critically examine current evidence on genetic mechanisms, growth, pubertal development, cardiometabolic risk, and neuropsychological featur

Parental and grandparental age in relation to Down syndrome risk and congenital heart disease in affected children: a case-control study in Peru.

UNLABELLED: This study aimed to assess the association between grandparental age and the risk of Down syndrome and evaluate the relationship between paternal age and congenital heart disease (CHD) among affected children in Peru. A retrospective case-control study was conducted at a national pediatric referral center in Peru, including 290 participants (145 cases with free trisomy 21 and 145 controls without genetic disorders). Logistic regression models were used to estimate the odds ratios (ORs) and 95% confidence intervals (CIs). Parental ages were higher in cases than in controls, but ther

Preserved social development but impaired executive function in a Shank3-deficient rat model of Phelan-McDermid syndrome.

Phelan-McDermid syndrome (PMS) is a genetic neurodevelopmental disorder caused by a microdeletion within chromosome 22q13.3 1-6, and accounts for ~1-3% of cases of autism spectrum disorders (ASD). Individuals with PMS typically present with neonatal hypotonia, severe speech delay, intellectual disability, motor impairments, and autism-related features, although additional manifestations such as epilepsy, sleep disturbances, and developmental regression are common. As in ASD, there is considerable heterogeneity in cognitive and behavioral impairments in PMS, making it difficult to determine whi

A dynamic "hormonal tug-of-war" driven by a functioning ovarian remnant after testis-preserving partial gonadectomy in ovotesticular disorders of sex development: a case report.

The endocrine behavior of residual ovarian tissue following testis‑preserving surgery in ovotesticular disorders of sex development (DSD) during adolescence remains poorly characterized. We report an adolescent male who presented with Tanner stage B4 gynecomastia despite male‑typical external genitalia. Baseline evaluation revealed elevated gonadotropins, relative hyperestrogenism (estradiol 58.7 pg/mL), and SRY‑negative chromosome mosaicism (47,XX,+der(8)/46,XX). Pelvic imaging demonstrated a small prostate and bilateral intratesticular cysts, and laparoscopy and cystoscopy confirmed a male i

Sleep disorders in adults with Cri du Chat syndrome: A questionnaire-based study.

BACKGROUND: Sleep disorders are increasingly recognized in individuals with Cri-du-Chat (CdC) syndrome, yet most available data focus on pediatric populations. The present study investigated the prevalence, characteristics, and potential determinants of sleep disturbances in adults with CdC syndrome using standardized and validated scales and questionnaires. Findings were also compared with previously published pediatric data to explore developmental trajectories and persistence of sleep problems into adulthood. METHODS: Adults (≥18 years) with genetically confirmed CdC syndrome were recruite

Mosaic Loss of the Y Chromosome: Mechanisms and Diseases.

Mosaic loss of the Y chromosome (mLOY) is a male‑specific somatic loss of the Y chromosome that is predominantly observed in peripheral blood leukocytes, but can also be found in other tissue types. It represents one of the most common male-specific somatic mutations, particularly prevalent in aging individuals. Accumulating evidence shows that mLOY markedly increases the incidence, progression, and adverse prognosis of multiple diseases, including, but not limited to, myocardial fibrosis, lung and bladder cancers, and Alzheimer's disease. Mechanistically, for extrinsic effects of mLOY, hemato

Sleep and circadian regulation of DNA repair and retrotransposons in neuronal genome stability and neurodegenerative disease mechanisms.

Sleep and wake are fundamental brain states that coordinate diverse physiological processes required for long-term health and survival. Recent research in molecular neuroscience and chronobiology has begun to clarify how sleep-wake cycles and circadian timing intersect with genomic maintenance mechanisms in the nervous system. In this Review, we summarize current evidence that sleep and circadian rhythms temporally regulate neuronal genome stability, with a particular focus on how they modulate neuronal retrotransposon activity. We discuss circadian clock-dependent regulation of DNA repair pat

Shared genetic architecture of schizophrenia and lupus identifies pleiotropic loci with distinct downstream mechanisms.

Schizophrenia (SCZ) and systemic lupus erythematosus (SLE) exhibit epidemiological overlap, suggesting shared genetic underpinnings. Using the largest SLE genome-wide association study (GWAS) to date, we identified a significant genetic correlation between SCZ and SLE (rg = 0.25, p = 9.0 × 10-4). Cross-trait analysis nominated multiple candidate shared loci outside the major histocompatibility complex (MHC), most with aligned effects. The strongest candidate shared association was found at the FURIN-FES locus, previously implicated in SCZ and newly associated with SLE. In addition to FURIN-FES

Phenotyping iPSC-derived neurons from Angelman patients and partial rescue via JNK pathway inactivation.

BACKGROUND: Angelman Syndrome (AS, MIM #105,830) is a rare neurodevelopmental disorder caused by imprinting defects, and it is marked by motor impairment, intellectual disability, speech limitations, and seizures. The condition arises due to the loss of function in neurons of the maternally-inherited UBE3A gene, mapping in the imprinted chromosomal region 15q11.2-q13, through a variety of genetic and epigenetic mechanisms. UBE3A encodes a ubiquitin E3 ligase, which is essential for synaptic plasticity. Most studies of AS have been performed on animal models, while induced pluripotent stem cell

Efficacy and safety of risk-stratified chemotherapy with azacitidine for children with myeloid leukaemia with Down syndrome: protocol for a multicentre phase II trial (JCCG AML-D24).

INTRODUCTION: The prognosis of myeloid leukaemia associated with Down syndrome (ML-DS) is excellent with reduced-intensity chemotherapy; however, that of refractory or relapsed ML-DS remains dismal. These contrasting outcomes highlight the importance of risk-stratified therapy, based on reliable prognostic factors, to prevent treatment-related mortality and relapse. In addition, novel therapeutic strategies capable of overcoming the poor prognosis of high-risk patients are eagerly anticipated. Based on the results of molecular analyses, in vitro functional assays and anecdotal case reports, az

22q11.2 Deletion Syndrome: A Comprehensive Review for Pediatricians From Diagnosis to Adult Care Transition.

Affecting ~1 in 4000 to 6000 live births, 22q11.2 deletion syndrome (22q11.2DS) is the most common chromosomal microdeletion syndrome. It results from a 1.5 to 3 Mb heterozygous deletion at chromosome 22q11.2 and is associated with a broad spectrum of clinical manifestations, including cardiac defects, palatal abnormalities, thymic hypoplasia, developmental delays, and neuropsychiatric symptoms. Multiple techniques such as chromosomal microarray and sequencing techniques such as fluorescence in situ hybridization help in the diagnosis of 22q11.2DS. A multidisciplinary approach to management is

Dynamic centrosomes: functional diversity and remodeling across cell types.

The centrosome is an evolutionarily conserved, microtubule-based organelle that has pronounced cell type-specific functions. In somatic cells, it serves as the primary microtubule-organizing center (MTOC), regulating chromosome segregation and contributing to asymmetric cell division and ciliogenesis. Although traditionally considered structurally stable, increasing evidence indicates that centrosomes can undergo extensive remodeling or even complete elimination in specialized cell types. Recent studies have begun to elucidate the molecular basis of these processes. For example, during oogenes

Mosaic Loss of Chromosome Y (LOY): From Genomic Instability to Disease and Single-Cell Detection.

Mosaic loss of chromosome Y (LOY) is the acquired absence of the Y chromosome in a fraction of cells, is most readily observed in the haematopoietic compartment, and represents the most common form of clonal mosaicism in older men. Once dismissed as a benign correlate of aging, LOY is now recognized as a clinically meaningful biomarker associated with reduced survival and an expanding range of age-related disorders, including multiple solid and haematological cancers, cardiovascular disease, Alzheimer's disease, and type 2 diabetes mellitus. Large-scale genome-wide analyses have shown that sus

Regulation of Human Telomerase: From Molecular Interactions to Population Genetics.

Human telomeres play critical roles in protecting chromosome ends and preserving genomic integrity. Telomerase, essential for maintaining telomere length and cellular replicative capacity, is only expressed in a small subset of human cells: stem and progenitor populations. Conversely, most somatic cells' telomeres shorten with each cell division; this shortening provides a potent tumor suppressor mechanism. Thus, telomerase regulation shapes not only cellular life span and differentiation, but also the regenerative capacity and long-term integrity of tissues. Here, we review the current unders

The transcriptional landscape of developing human trisomy 21 lungs.

RATIONALE: Trisomy 21 (T21), resulting in Down syndrome (DS), is the most prevalent chromosomal abnormality worldwide. While pulmonary disease is a major cause of morbidity and mortality in DS, the ontogeny of pulmonary complications remains poorly understood. We recently demonstrated that T21 lung anomalies, including airway branching and vascular lymphatic -abnormalities, are initiated in utero. Here, we aimed to describe molecular changes at the single-cell level in prenatal T21 lungs. METHODS: Single-cell RNA sequencing was used to generate transcriptomic profiles of individual human lung

CPX-351 in Down syndrome-associated myeloid leukemia: results and prognostic factors from the phase 3 ML-DS 2018 trial.

Myeloid leukemia of Down syndrome (ML-DS) is associated with an excellent prognosis but high treatment-related toxicity and mortality. The Phase 3 Clinical Trial for CPX-351 in ML-DS 2018 aimed to maintain the excellent event-free survival (EFS) achieved in the previous ML-DS 2006 trial while reducing the treatment intensity. Intensity-reduced induction and reinduction therapy with cytarabine and idarubicin with or without etoposide was replaced with CPX-351 (66 U/m2 on 3 days in course 1 and on 2 days in course 2). Risk stratification was based on flow cytometric measurable residual disease (
