Molecular risk markers define risk of relapse in myeloid leukemia of Down syndrome beyond measurable residual disease.

Myeloid leukemia of Down syndrome (ML-DS) is a distinct form of pediatric acute myeloid leukemia (AML) that responds to reduced-intensity chemotherapy, as compared with non-DS AML that requires intensive chemotherapy and often stem cell transplant. While most patients with ML-DS have a favorable prognosis, outcomes for those with refractory or relapsed disease are dismal. Children's Oncology Group study AAML1531 introduced the use of measurable residual disease by multiparameter flow cytometry at the end of the first course of induction therapy (EOI-1 MRD) for risk stratification of treatment

Optical genome mapping in myelofibrosis: enhancing prognostic accuracy through high-resolution genomic profiling.

Accurate genomic stratification is essential for guiding therapeutic decisions in myelofibrosis (MF), particularly regarding allogeneic stem cell transplant. Conventional chromosome banding analysis (CBA), however, has limited resolution and provides an informative karyotype in only about half of patients with MF. In a prospective series of 107 patients with primary or secondary MF, we compared optical genome mapping (OGM) plus targeted next-generation sequencing with CBA. OGM generated interpretable cytogenetic data for every sample, whereas CBA succeeded in fewer than half. In addition, OGM

Difference in Language Profiles of Children With Autism Spectrum Disorder and Down Syndrome Is Not Driven by Non-Verbal Cognition.

BACKGROUND: Autism Spectrum Disorder (ASD) and Down syndrome (DS) are among the most common types of neurodevelopmental conditions that have co-occurring language impairments. Usually, non-verbal IQ has been reported as one of the main predictors of language functioning in children with these conditions. Although language abilities of children with ASD and DS have been described in the previous studies, there is still a lack of direct comparisons of language profiles in the non-verbal IQ-matched groups of children with these disorders, and, therefore, it is largely unexplored whether language

Posterior Fossa Group A Ependymoma in Trisomy 21.

Brain tumors are the most common solid tumors in children. However, the risk of central nervous system tumors appears reduced in individuals with trisomy 21, particularly for tumors of nervous tissue. We present the first reported case of posterior fossa group A (PFA) ependymoma in a patient with trisomy 21. Including this case, only four cases of ependymoma in trisomy 21 have been reported. Similar to medulloblastoma, ependymoma may occur at a markedly reduced frequency in trisomy 21. Investigating the protective role of trisomy 21 in ependymoma development may provide new insights into its p

Leveraging clinical sleep data across multiple pediatric cohorts for insights into neurodevelopment: the Retrospective Analysis of Sleep in Pediatric (RASP) cohorts study.

Sleep disturbances are prominent across neurodevelopmental disorders (NDDs) and may reflect specific abnormalities in brain development and function. Overnight polysomnography (PSG) allows for detailed investigation of sleep architecture, offering a unique window into neurocircuit function. Analysis of existing pediatric PSGs from clinical studies could enhance the availability of sleep studies in pediatric patients with NDDs towards a better understanding of mechanisms underlying abnormal development in NDDs. Here, we introduce and characterize a retrospective collection of 1527 clinical pedi

Mutual learning for joint disease detection and severity prediction reveals multimodal pathogenesis for neurodegenerative disorders.

MOTIVATION: Neurodegenerative disorders influence millions of people worldwide, and uncovering the pathogenesis is of urgent need. Many efforts have been made to detect or predict neurodegenerative disorders, while exploring the pathogenesis has been ignored from a systemic perspective. RESULTS: To handle this issue, we propose a novel and powerful method, referred to as Pathogenesis-aware Mutual-Assistance Classification and Regression Optimization (Pa-MACRO). First, Pa-MACRO incorporates a mutual-assistance bidirectional mapping technique with a joint-embedding fine-grained interpretability

Application of single nucleotide polymorphism array (SNP array) derived median copy number state for monitoring the clonal burden of the chromosomal aberrations in myelodysplastic neoplasms.

OBJECTIVES: Our study integrates SNP array and metaphase cytogenetics (MC) to decode clonal architecture in myelodysplastic syndromes (MDS) and related disorders. METHODS: We retrospectively analyzed chromosomal aberrations in 33 patients diagnosed with MDS and related disorders by both SNP array and MC, of whom ten were detected with chromosomal copy number variations (CNVs) by MC. SNP array was used to quantify the clonal burden of CNVs using the metric median copy number state. RESULTS: We found that SNP array-derived median copy number state was associated with the abnormal/total ratio b

Clinical and Genetic Significance of Chromosomal Microarray Screening of Asymptomatic Newborns.

INTRODUCTION: In South Korea, low-resolution chromosomal microarray analysis (LR-CMA) is frequently used as a screening tool to identify chromosomal anomalies in asymptomatic newborns. However, its clinical utility remains controversial. METHODS: We retrospectively analyzed 99 asymptomatic newborns who underwent diagnostic CMA following abnormal LR-CMA screening results at a single tertiary hospital between 2019 and 2024. Clinical features, copy number variant (CNV) findings, and follow-up outcomes were assessed. RESULTS: Among the 99 patients (57.6% male), the median ages at the initial and

A chromosome region linked to neurodevelopmental disorders influences locomotor behavior through sex-specific neural circuits.

Biological sex shapes the manifestation and progression of neurodevelopmental disorders (NDDs), however, the underlying mechanisms remains unclear. Hemideletion of the 16p11.2 region (16p11.2 del/+) is associated with NDDs, and 16p11.2 del/+ mice exhibit sex-specific, striatum-related phenotypes relevant to NDDs. In this study, using snRNA-seq, we identify cell type- and sex-specific transcriptomic changes in D1- and D2-spiny projection neurons (SPNs), with greater impact in males. Fiber photometry recordings reveal reduced neuronal activity in the dorsal striatum of 16p11.2 del/+ males, but n

Survival in infants with trisomy 18, palliative care and ethical reflections: a single center considerations.

BACKGROUND: Trisomy 18 was once considered a fatal diagnosis due to the presence of cardiac and extracardiac lesions. However, with the increasing use of therapeutic management, 3% to 25% of infants with trisomy 18 may survive beyond their first year, depending on the interventions provided. Currently, there are no clear and widely accepted criteria to guide medical decisions for children with trisomy 18. This means that patients could often be at risk of either over-treatment or therapeutic abandonment. We aimed to explore the effectiveness of intensive and non-intensive treatments in enhanci

Developing Del2Phen: A Novel Phenotype Description Tool for Chromosome Deletions.

Information on the health-related consequences of rare chromosome disorders is often limited, posing challenges for both patients and their families. The Chromosome 6 Project aims to bridge this knowledge gap for structural aberrations involving chromosome 6 by providing parents of affected children with information on the expected phenotypes of their child. To achieve this, detailed phenotype and genotype data are collected directly from parents worldwide and supplemented with data from literature reports, resulting thus far in a dataset of over 500 individuals. This comprehensive data pool w

Mothers' and Fathers' Recode Use With Young Children With Down Syndrome.

PURPOSE: Parental recodes, a form of responsivity, help facilitate language development in children with Down syndrome (DS). However, little is known about the complexity and referents of recodes or differences between mothers and fathers and how this relates to child language. METHOD: This study included 15 children with DS (Mage = 39.67 months, range: 24-61 months) and their mothers and fathers. Mother-child and father-child play interactions were recorded in the home. RESULTS: We found no differences between mothers' and fathers' use of recodes. A moderate positive association existed bet

Alterations in electroencephalography signals in female fragile X syndrome mouse model on a C57BL/6J background.

Fragile X Syndrome (FXS), the most common monogenic cause of autism spectrum disorder, exhibits sex differences in prevalence and phenotypic severity. Electroencephalography provides translational insights into its pathophysiology, yet prior research focuses predominantly on males. In C57BL/6J mice, male Fmr1 knockout models show increased absolute gamma power across developmental stages, while female phenotypes, particularly in juveniles, remain uncharacterized. This study addresses this gap by comparing juvenile female Fmr1 knockout and wild-type mice. Frontal-parietal differential electroen

Elevated serotonin receptor 2A signaling restores learning and memory in a Fragile X syndrome model.

Serotonin (5-hydroxytryptamine, 5-HT) has central roles enabling learning and memory, particularly via serotonin receptor 2A (5-HT2AR) signaling. Drosophila Fragile X syndrome model (dfmr1 null mutant) studies reveal impaired learning and memory, which may reflect serotonergic signaling deficits. Here, we use classical olfactory T-maze conditioning to assess behavior, combined with imaging to assess 5-HT and 5-HT2AR levels within the underlying Mushroom Body (MB) brain circuitry. Null dfmr1 mutants exhibit learning and memory deficits that are corrected by elevating 5-HT signaling via 1) overe

T2/FLAIR Mismatch Subregion as an Independent Prognostic Biomarker in IDH-Mutant and 1p/19q Noncodeletion Astrocytoma.

BACKGROUND AND PURPOSE: While the global T2/FLAIR mismatch sign is a diagnostic hallmark of IDH-mutant and 1p/19q noncodeletion astrocytoma, the prognostic significance of partial mismatch patterns remains unclear. This study investigated the differences in MRI between subregions with a T2/FLAIR mismatch and a match, and the impact of these differences on astrocytoma clinical outcomes. MATERIALS AND METHODS: This retrospective study analyzed 221 patients with histopathologically confirmed IDH-mutant and 1p/19q noncodeletion astrocytoma, stratified by the presence (n=131) or absence (n=90) of

Omics analyses decoding mechanisms underlying the self-flocculating phenotype of yeast cells and stress tolerance for robust production.

A unique self-flocculating yeast strain SPSC01 was developed through protoplast fusion for fuel ethanol production with high product titers. In this study, we conducted comparative multi-omics analyses on SPSC01 to elucidate mechanisms underlying its self-flocculating phenotype and associated stress tolerance, the most desirable merit for robust production in industry. Leveraging two cutting-edge third-generation sequencing technologies, we achieved a gapless high-quality and chromosome-level assembly for the genomes of SPSC01 and its parental strains. Through comprehensive genome analyses, we
