From Feeding Difficulties to Nutritional Risk in Children with Down Syndrome.

Children with Down syndrome (DS) may experience feeding difficulties arising from interactions among hypotonia, cardiorespiratory and gastrointestinal conditions, swallowing dysfunction, oral-motor and sensory differences, delayed feeding skill development, and psychosocial or environmental factors. These difficulties may compromise feeding safety and efficiency, restrict dietary diversity, and contribute to inadequate fluid or nutrient intake, faltering growth, micronutrient deficiencies, or excess adiposity. This narrative review synthesized the literature selected for its relevance to feedi

The Role of the Immunoglobulin Superfamily of Cell Adhesion Molecules in Intellectual Disability.

Immunoglobulin superfamily (IgSF) cell adhesion molecules (CAMs) are cell surface glycoproteins that play critical roles during multiple stages of synapse formation, including axon and dendrite growth and guidance, target recognition, axo-dendritic contact formation and its maturation into a functional synapse. Intellectual disability is a condition characterized by significant limitations in intellectual functioning and adaptive behaviors. Growing evidence indicates that deficits in synapse formation underlie the pathogenesis of intellectual disability. In this review, we discuss studies show

Growth hormone therapy in Turner syndrome: Do cytogenetic subtypes influence outcomes?

Turner syndrome (TS) is a chromosomal disorder caused by complete or partial loss of one X chromosome, occurring in approximately 1 in 2500 live female births. Short stature, resulting from haploinsufficiency of the short stature homeobox-containing (SHOX) gene, absence of a pubertal growth spurt, and ovarian insufficiency are hallmark features. Recombinant human growth hormone (GH) therapy remains the mainstay of management, increasing adult height by approximately 5-8cm. This review examines how cytogenetic subtypes influence GH response, focusing on 45,X monosomy, mosaicism, and structural

Quantifying Physical Activity in Toddlers with Down Syndrome with Wearable Sensors During a Home-Based Mobility Intervention.

BACKGROUND: Quantifying movement during early mobility interventions is important for evaluating intervention participation and progression in toddlers with Down syndrome (DS), but behavioral video coding is time and labor-intensive. This study established and evaluated an activity count cutpoint for distinguishing moderate-to-vigorous physical activity (MVPA) and sedentary-to-light physical activity (SLPA) in toddlers with DS during a home-based mobility intervention using a wearable accelerometer integrated into a body-weight support harness system. METHODS: Video recordings behaviorally co

Epigenetic regulation of humoral immunity: molecular mechanisms, diseases and therapeutic targets.

Humoral immunity, a fundamental branch of adaptive immunity, relies on B lymphocytes and the antibodies they produce to eliminate extracellular pathogens and neutralize toxins. The development, activation, and differentiation of B cells are tightly regulated by epigenetic mechanisms-including DNA methylation, histone modifications, chromatin dynamics, RNA modifications, and noncoding RNAs-that modulate gene expression without altering the DNA sequence, orchestrating key events such as V(D)J recombination, class switch recombination, somatic hypermutation, germinal center reactions, and the gen

Introduction to epigenetic mechanisms in the brain.

This chapter provides an introductory overview of epigenetic mechanisms, focusing on the molecular processes that regulate gene expression without altering the DNA sequence. It outlines the foundational concepts of DNA methylation, histone modifications, chromatin remodeling, and noncoding RNAs, with emphasis on their roles in cell identity and transcriptional regulation. The interactions between these epigenetic mechanisms, such as the crosstalk of histone acetylation and DNA methylation, are also discussed. Key mechanisms of long-term gene inactivation, like X-chromosome inactivation and gen

Fertility preservation in girls and adolescents with Turner syndrome: lessons for reproductive care in differences of sex development - a mini-review.

Current knowledge regarding fertility preservation (FP) in girls is derived predominantly from pediatric oncology cohorts; however, FP is increasingly being applied to non-oncological conditions associated with impaired fertility, including differences of sex development (DSD). Turner syndrome (TS), characterized by accelerated germ cell depletion beginning during fetal life, represents the DSD condition with the greatest clinical experience in FP. This mini-review summarizes current evidence on ovarian tissue cryopreservation and oocyte cryopreservation in TS. Available data support the techn

Antidepressant use and Parkinson's disease risk: Overall protective effect with drug-specific variation.

BACKGROUND: Antidepressants can cause extrapyramidal side effects, but their impact on Parkinson's disease (PD) risk remains unclear. AIMS: We aimed to investigate the association between genetic liability to antidepressant use and PD. METHODS: We examined the causal effect of antidepressant use on PD using Mendelian randomisation (MR) analysis. We further performed multivariable MR (MVMR) analysis to determine the independent effect of genetic liability to antidepressant use on PD by accounting for the effect of major depressive disorder (MDD) on PD. The Spectrum of Mosaic Double Aneuploidy of Monosomy X and Trisomy 18: Two New Cases and Review of the Literature.

INTRODUCTION: Mosaic double aneuploidy (MDA) defines the presence of two chromosomally distinct cells lines derived from the same zygote arising from early cytogenetic events. MDA for monosomy X and trisomy 18 is a rare occurrence described in only 12 individuals in the medical literature to date. CASE PRESENTATION: Here, we present 2 individuals with MDA of monosomy X and trisomy 18 with a Turner predominant phenotype. Congruent with previous reports, the percentage of aneuploid cells is highly variable in differing tissues, without clear genotype-phenotype associations. Molecular mechanism

Yra2 regulates proteolysis of Cse4 to prevent its mislocalization to non-centromeric regions for chromosomal stability in budding yeast.

UNLABELLED: Restricting the localization of centromere-specific histone H3 variant Cse4 (CENP-A in humans) to centromeric chromatin is essential for chromosome segregation. Mislocalization of overexpressed Cse4/CENP-A to non-centromeric regions contributes to chromosomal instability (CIN) in model organisms and human cells. CIN is an important hallmark of many cancers and hence defining mechanisms that prevent mislocalization of Cse4 is clinically significant. Here we report a role for YRA2 ( Y east R NA A nnealing Protein 2) in ubiquitin mediated proteolysis of Cse4 to prevent its mislocaliza

Venetoclax combined with olverembatinib and prednisone as a frontline therapeutic strategy for Philadelphia chromosome-positive mixed-phenotype acute leukemia.

Mixed-phenotype acute leukemia (MPAL) is a rare hematological malignancy with poor prognosis, for which there are currently no standardized therapeutic guidelines. Preclinical and clinical studies have shown that venetoclax and BCR::ABL1 tyrosine kinase inhibitors have synergistic effects on Philadelphia chromosome-positive (Ph +) leukemias. We retrospectively analyzed five patients with de novo Ph + MPAL who received venetoclax, olverembatinib, and prednisone (VOP) therapy. All patients achieved complete remission after the initial induction therapy and tested negative for measurable residual

Meiosis and Ageing in Mammals.

Meiosis is a specialized form of cell division that plays an essential role in mammalian sexual reproduction to reduce the ploidy of the developing germ cells so that each gamete transmits one copy of each chromosome to the next generation. In humans, approximately 20% of conceptions inherit the wrong number of chromosomes, with the vast majority of these aneuploid embryos arising from de novo chromosome segregation errors during meiosis. These inherited autosomal aneuploidies usually arise maternally during oocyte meiosis, and are strongly affected by maternal age. Paternally-inherited autoso

Multicentric Longitudinal Performance Monitoring of Different Prenatal Cell-Free DNA Screening Technologies in Belgium.

OBJECTIVE: Prenatal cell-free DNA screening is widely used to assess the risk of fetal trisomy 21, 18, and 13. Although commercial and laboratory-developed tests (LDTs) show high accuracy in validation studies, real-world longitudinal performance data are lacking. METHOD: In Belgium, prenatal cell-free DNA screening is a population screening offered to all pregnant people, regardless of their a priori risk. Over 3 years, all confirmatory diagnostic tests after a positive prenatal cell-free DNA screening result were recorded along with the technologies used and their outcomes. Positive predict

Coarctation of the aorta as the presenting cardiovascular manifestation of Williams syndrome in an infant.

Williams syndrome (WS) is a multisystem genetic disorder caused by a 7q11.23 microdeletion and is classically associated with supravalvular aortic stenosis (SVAS) and peripheral pulmonary artery stenosis. We report a male infant in whom discrete juxtaductal coarctation of the aorta (CoA) was incidentally detected during pre-operative evaluation for left inguinal hernia. Echocardiography demonstrated a peak gradient of 25 mm Hg without SVAS or branch pulmonary artery stenosis. Recognition of characteristic facial features prompted genomic evaluation, which confirmed a pathogenic 1.6 Mb 7q11.23

A novel microdeletion syndrome involving 17q23.3 region in a child with short stature and multiple congenital anomalies: A Case Report.

Microdeletion syndromes are rare chromosomal abnormalities causing diverse clinical features due to gene dosage imbalances. Deletions in chromosome 17q23.3 have been linked to neurodevelopmental and growth disorders, although the genotype-phenotype correlations remain unclear. An eight-month-old male baby presented with short stature, low-set ears, cardiac anomalies, and multiple congenital malformations. He was born via C-section due to intrauterine growth restriction and exhibited craniofacial dysmorphism, lymphatic vessel dysplasia, micropenis, undescended testes, atrial septal defect, and

Pregnancy-related disorders: molecular mechanisms and management.

Pregnancy-related disorders (PRDs), including preeclampsia (PE), gestational diabetes mellitus (GDM), hyperemesis gravidarum (HG), and intrahepatic cholestasis of pregnancy (ICP), remain major causes of maternal and perinatal morbidity. This review synthesizes their molecular, endocrine, immune, and clinical features. We propose the Placental-Maternal Adaptation Failure Continuum (PMAFC), in which gestational/fetoplacental stress load interacts with maternal adaptive reserve. This load includes placenta-derived signals, the physiological demands of gestation and fetal growth, and secondary mat
