Obstructive sleep apnoea severity and insulin resistance in children with Prader-Willi Syndrome: the SPISE index as a candidate metabolic marker.

UNLABELLED: We compared two surrogate markers of insulin sensitivity - the single-point insulin sensitivity estimator (SPISE) and HOMA-IR - and evaluated their associations with OSA severity in children with genetically confirmed PWS and a symptomatic non-syndromic referral group (NSR) with available polysomnography (PSG) and fasting biochemical data. In this single-centre retrospective cross-sectional study, OSA severity was indexed by the obstructive apnoea-hypopnoea index (oAHI), central AHI (cAHI) and oxygen desaturation index (ODI), and caregiver-reported symptoms by the PSQ-SRBD scale. SMCHD1 is a target for gene activation therapy to treat Prader-Willi syndrome.

Prader-Willi syndrome (PWS) is a neurodevelopmental disorder caused by loss of expression from the active paternal allele at an imprinted cluster on chromosome 15. Current treatments address individual symptoms rather than the underlying cause of disease. Because all patients preserve a normal but epigenetically silenced maternal copy of these genes, activation of this copy offers a potential disease-modifying therapy. Here, we investigated the epigenetic regulator SMCHD1. First, we demonstrated that SMCHD1 represses the PWS locus in both mice and humans, supporting its relevance as a therapeu

From iPSC Disease Modeling to Epigenome Editing: Toward Therapeutic Reversal of Genomic Imprinting Disorders.

Human induced pluripotent stem cells (iPSCs) and organoids are increasingly important components of new approach methodologies because they provide experimentally accessible human systems that preserve patient-specific genetic backgrounds and can be differentiated into disease-relevant cell types. Genomic imprinting disorders are particularly attractive targets for this approach because their pathogenesis depends not only on DNA sequence but also on parent-of-origin-specific epigenetic regulation. Prader-Willi syndrome (PWS) and Angelman syndrome (AS), which arise from distinct disruptions wit

[Mosaic Turner syndrome with a 45, X/46, XX/47, XXX karyotype presenting with recurrent fractures: a case report and literature review].

A case of a patient with Turner syndrome (TS) with a karyotype of 45, X/46, XX/47, XXX who was admitted to the Department of Endocrinology at Peking Union Medical College Hospital in March 2025 were reported. The patient presented with primary amenorrhea and recurrent fractures. Dual-energy X-ray absorptiometry revealed osteoporosis, and spinal X-rays showed multiple vertebral wedge-shaped deformities. Screening for other secondary causes of osteoporosis revealed no significant abnormalities. Based on these results, together with the patient's sex hormone levels indicating hypergonadotropic hy

Association between JAK2 mutations and thoracic aortic aneurysms in myeloproliferative neoplasms.

The Janus kinase 2 (JAK2) V617F mutation is recognized as a defining molecular lesion in Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs). This mutation was recognized in the context of clonal hematopoiesis of indeterminate potential, which has recently been linked with cardiovascular disease. Recent evidence has demonstrated a significant correlation between JAK2 mutations and thoracic aortic aneurysm (TAA) a condition frequently lacking identifiable risk factors. A scoping review, involving MEDLINE, Embase, Web of Science, Google Scholar, and prominent hematology and card

Sex differences in neuropsychiatric genetics: progress in understanding mechanisms of risk and protection.

Sex differences in the prevalence and clinical presentation of neuropsychiatric disorders suggest a corresponding divergence in underlying biological risk mechanisms between males and females. Advances in neuropsychiatric genetics have started to resolve these differences, defining sex-differential genetic architectures and risk loci across common and rare variants on the autosomes and sex chromosomes, with the study of major depressive disorder and neurodevelopmental disorders leading the way. As we look to translate sex-differential genetic signals into mechanistic understanding, emerging ev

A massively parallel reporter assay of MECP2cis-regulatory elements reveals genetic candidates for male-biased autism.

Autism affects male individuals four times more often than female individuals, yet the basis of this sex bias remains unclear. One hypothesis is that this sex bias is due to genetic variants in X chromosome genes where haploinsufficiency causes neurodevelopmental disorders (NDDs). In heterozygous XX female probands, a complete loss-of-function variant in one of these genes often causes syndromic NDD, but the same variant is too deleterious for survival in hemizygous XY males. A mild hypomorphic variant, however, may be tolerated in a female carrier but may produce milder neurological phenotype

Effects of Gallic Acid on Sleep and Circadian Rhythm in a Drosophila melanogaster Model of Fragile X Syndrome: An Integrative Behavioral and Bioinformatic Approach for Target Predictions.

Sleep and circadian rhythm (CR) disturbances are hallmark features of fragile X syndrome (FXS), a rare genetic disorder caused by a full mutation of the FMR1 gene, resulting in reduced or absent FMR1 protein (FMRP). Despite the clear genetic etiology, targeted treatment for FXS remains unavailable. Natural polyphenol gallic acid (GA) has great potential to modulate neurobiological effects and is attracting increased pharmacological interest due to its low toxicity and capacity to modulate neuronal signaling. Therefore, this study aimed to investigate the effects of GA on sleep and CR in the Dr

A Qualitative Study of Parental Decision-Making Following a Prenatal Diagnosis of Turner Syndrome: "How Serious Is This?".

A prenatal diagnosis of Turner syndrome (TS) presents expectant parents with complex and emotionally demanding decisions under conditions of uncertainty. However, there is limited insight into how parents interpret and integrate medical information, personal experiences, and values during this process. This qualitative study explores how parents who chose to continue a pregnancy following a prenatal TS diagnosis experienced and navigated decision-making. The study was conducted within the Danish healthcare system between September 2024 and October 2025. During this period, we conducted 12 semi

Growth and adiposity patterns in Argentine children with Prader-Willi syndrome without growth hormone treatment or individualised very low energy diets.

BACKGROUND: Children with Prader-Willi syndrome (PWS) show distinctive patterns of growth and body composition. Although growth hormone (GH) therapy is widely used, many patients continue to be monitored without treatment, making anthropometric data from untreated populations valuable for clinical follow-up. We previously developed Argentine references for height and body segments in GH-untreated children with PWS. AIM: To extend that work by characterising patterns of adiposity using weight, body mass index (BMI), and skinfold thickness measurements in children with PWS who had not received

Circulating BDNF in pediatric neurodevelopmental disorders: a meta-analysis of case-control differences and cognitive severity.

BACKGROUND: Brain-derived neurotrophic factor (BDNF) has been proposed as a neuroimmune biomarker of cognitive dysfunction in paediatric neurodevelopmental disorders other than autism spectrum disorder, but two related questions remain poorly characterised: whether circulating BDNF differs between affected and unaffected individuals, and whether it tracks the severity of cognitive impairment within affected populations. This systematic review and meta-analysis addressed both questions in children and adolescents with cerebral palsy (CP), intellectual disability (ID), or Down syndrome (DS). ME

Magel2-null mice demonstrate contradictory outcomes in body weight, impulsivity, and motivation during distinct fasted states.

Prader- Willi Syndrome (PWS) is a complex neuroendocrine disorder that presents with hyperphagia, obesity, and social and cognitive impairment. The cognitive impairment is comparatively under-researched in PWS and in the Magel2-null mouse model. Cognitive function is commonly trained and tested in mice after the manipulation of metabolic state to induce motivation, yet physiologic function in the Magel2-null mouse model is most commonly assessed in the fed state. This project evaluates Magel2-null mouse physiology in ad libitum, acute fast, and chronic restriction settings to establish a basel

Establishing consensus diagnostic criteria for ring chromosome 20 syndrome: A modified electronic Delphi consensus study.

OBJECTIVE: This study was undertaken to establish expert consensus clinical and diagnostic criteria for ring chromosome 20 syndrome using a modified electronic Delphi process. METHODS: In this modified two-round electronic Delphi consensus study, international experts rated candidate statements using a 9-point Likert scale. Consensus was predefined as at least 70% agreement. A disagreement index assessing rating dispersion was calculated for each statement. The international expert consensus study was conducted remotely using Research Electronic Data Capture. Thirteen international experts wi

Hyperphagia in Prader-Willi syndrome: linking hypothalamic dysfunction to clinical assessment and management across the lifespan.

BACKGROUND: Prader-Willi syndrome (PWS) is a rare genetic neurodevelopmental disorder characterised by hypothalamic dysfunction and profound dysregulation in hunger and appetite regulation, endocrine function, metabolism and behaviour. Hyperphagia is a defining clinical feature and a major contributor to morbidity, mortality and caregiver burden. Increasing evidence suggests that hyperphagia in PWS represents a multidimensional disorder rather than excessive appetite alone. RESULTS: In December 2024, an international meeting of clinical and scientific experts was convened in Toulouse, France,

Fragmentation patterns of human telomeric chromatin in plasma cfDNA.

The specialized chromatin structure at telomeres protects the linear ends of chromosomes from DNA surveillance machineries. Here, we demonstrate that circulating cell-free DNA (cfDNA) from plasma can map chromatin structure at telomeres. We find that the telomeric 6-mer repeats (TTAGGG/CCCTAA) are the most abundant circulating 6-mers in cfDNA. Telomeric sequences in cfDNA contain subnucleosomal footprints distinct from the rest of the genome, arising from specific cleavages in the C-rich strand, and a nucleosome repeat length of 145 bp, markedly shorter than the ~170 bp observed genome-wide. Negative symptom dimensions link cognitive impairment to global and social functioning in 22q11.2 deletion syndrome.

BACKGROUND: 22q11.2 deletion syndrome (22q11DS) confers a 20-30% lifetime risk of schizophrenia, with negative symptoms associated with poorer outcomes and higher psychosis risk. High rates of neurocognitive deficits, autism spectrum traits (ASD), and attention-deficit/hyperactivity disorder (ADHD) may further complicate symptom profiles and functional outcomes. We examined relationships among positive and negative symptom severity, ASD traits, ADHD symptoms, and global functioning in 22q11DS. METHODS: Individuals with 22q11DS (n = 38) and healthy comparison subjects (n = 34) completed clinic

Diagnostic and Clinical Outcomes of Sex Chromosome Abnormalities Detected on Noninvasive Prenatal Screening.

OBJECTIVE: Noninvasive prenatal screening (NIPS) screens for aneuploidy, but its positive predictive value for sex chromosome aneuploidies (SCA) is variable. NIPS reports include "atypical sex chromosome findings," which may indicate fetal/maternal SCAs or mosaicism, although sensitivity is unknown. Previous studies are limited by small cohorts and insufficient maternal testing and ultrasound data. This study evaluates confirmation rates and outcomes for screen-positive SCAs, including "atypical sex chromosome" on NIPS. STUDY DESIGN: This retrospective study included singleton pregnancies tha

Treating sleep problems in children with Prader-Willi syndrome.

The purpose of the present study was to investigate the effects of a sleep treatment package on the frequency of night awakenings, bedtime resistance, and caregiver stress with the caregivers of three child participants diagnosed with Prader-Willi syndrome (PWS) using a multiple baseline across subjects design. During baseline, child participants were not exposed to any procedure. During intervention, caregivers implemented a treatment package including graduated extinction, bedtime routines, and bedtime fading without response cost. All interventions were previously successful with children d
