BACKGROUND: Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a genetic cardiomyopathy characterized by fibrofatty replacement of ventricular myocardium, associated with ventricular arrhythmias and sudden cardiac death (SCD). While supraventricular arrhythmias occur in approximately 25% of ARVC patients, the coexistence of accessory pathways is exceptionally rare, particularly concealed pathways without ventricular pre-excitation.
CASE PRESENTATION: We describe two young patients with ARVC and concealed accessory pathways. Case 1: A 27-year-old male with family history of SCD presente

Diabetic cardiomyopathy increases the risk of ventricular tachycardia and fibrillation through ionic and structural remodeling, but whether the reentrant substrate arises from a diffuse slow-conduction (fibrotic) substrate, a focal scar, or from repolarization remodeling remains unclear. Using a three-dimensional monodomain model (truncated-ellipsoid left ventricle; ToR-ORd ionic model with six diabetic current-scaling factors), I performed a controlled ablation study (in-silico knockout, not catheter ablation) in which diffuse fibrosis and connexin-43 remodeling were reduced tissue conductivi

The prognosis of patients with mitral valve prolapse (MVP) largely depends on the presence of ventricular arrhythmias. This study aims to identify association between Cardiac MRI (CMR) based findings with ventricular arrhythmias in MVP patients. Consecutive patients (n = 45, mean age 56 ± 17 y) with MVP underwent CMR scans. Premature ventricular beats (PVC) and ventricular tachycardia (VT) were recorded by 24-hour Holter monitoring and exercise stress testing. Myocardial late gadolinium enhancement (M -LGE) was calculated as percentage of left ventricular (LV) mass and was analyzed by tertiles

BACKGROUND: Pulsed field ablation (PFA) is a novel and rapidly expanding energy for the ablation of atrial fibrillation (AF). The relationships between the characteristics of pulmonary vein (PV) and extra-PV PFA applications, and the type of arrhythmia recurrence or durable lesion observed during redo procedures remain unclear.
METHODS: In this substudy of the FRANCE PFA national exhaustive multicentric registry, we collected detailed data on PFA applications delivered during the initial AF ablation procedure. Among patients with arrhythmia recurrence undergoing redo procedures with 3-dimensi

INTRODUCTION: Cardiac magnetic resonance imaging (CMR) is a powerful tool in evaluating myocarditis. The role of fluorodeoxyglucose-positron emission tomography/computed tomography (FDG PET/CT) in known or suspected myocarditis remains unknown. We aimed to investigate whether FDG PET/CT adds prognostic value to CMR in patients with known or suspected myocarditis.
METHODS: From a registry of 1,646 patients at Brigham and Women's Hospital who underwent FDG PET/CT for known or suspected inflammatory cardiomyopathy, we applied the following criteria: 1) had a primary clinical diagnosis of myocard

INTRODUCTION: Chagas disease is an under-recognized cause of cardiomyopathy in the United States, and cardiac involvement may present decades after exposure with heart failure or ventricular arrhythmias.
CASES: A 52-year-old man from El Salvador presented with progressive dyspnea and volume overload. Evaluation revealed severe biventricular dysfunction, elevated filling pressures, no obstructive coronary disease, and nonischemic late gadolinium enhancement. Trypanosoma cruzi serology was positive. He required inotropic support and was discharged on guideline-directed medical therapy with a we

BACKGROUND: Catheter ablation of accessory pathways has high acute success rates; however, accessory pathway (AP) recurrence remains a clinically relevant issue. Although intra-procedural AP reconduction is frequently encountered during ablation procedures, its timing, predictors, and clinical significance remain insufficiently characterized.
OBJECTIVES: This study aimed to investigate the timing, electrophysiologic characteristics, and clinical implications of intra-procedural AP reconduction during catheter ablation of manifest and concealed accessory pathways.
METHODS: We retrospectively

BACKGROUND: Pregnancy in Eisenmenger syndrome is associated with very high maternal and fetal risk. Complex congenital anatomy further complicates diagnosis and peripartum management, particularly where advanced imaging and pulmonary hypertension therapies are limited. Published evidence specifically addressing pregnancy with uncorrected DORV and Eisenmenger physiology remains sparse.
CASE PRESENTATION: A 25-year-old primigravida at 34 weeks' gestation presented with progressive dyspnoea and generalized oedema. The available record documented a heart rate of 130 beats/min, respiratory rate of

Acute caffeine intoxication has become a rare but life-threatening clinical and forensic concern, particularly with the increasing availability of highly concentrated pure caffeine preparations. This report presents a fatal case following the intentional ingestion of a high dose of powdered pure caffeine, evaluated through an integrated assessment of the clinical course, autopsy findings, histopathological examination, and postmortem toxicological analyses. A man in his late twenties presented to the emergency department with nausea, vomiting, and tachycardia. Although transient clinical impro

Atypical pulmonary carcinoids may recur many years after initial treatment, but very late central-airway recurrence is uncommon and its optimal endoscopic management is uncertain. We report a 67-year-old man with metastatic atypical pulmonary carcinoid who developed recurrent subglottic disease almost 20 years after pulmonary resection. Following mild haemoptysis and inspiratory dyspnoea, bronchoscopy showed progression of a known subglottic lesion causing approximately 80% tracheal obstruction. Laser photoresection achieved rapid debulking, haemostasis and marked airway recanalization. Residu

BACKGROUND: Cardiac stereotactic body radiation therapy (cSBRT) is a noninvasive, investigational treatment for refractory ventricular tachycardia (VT). The antiarrhythmic effects and time course of action of cSBRT on abnormal ventricular substrate remain poorly understood.
OBJECTIVES: This study aimed to investigate the electrophysiological, molecular, and structural changes following cSBRT in a porcine infarct model.
METHODS: Anterior myocardial infarction was created in a porcine model of infarct-related re-entrant VT (n = 8). Swine were randomized to cSBRT (n = 4) or control infarct (n =

The circling rat is an autosomal recessive mutant (homozygous ci2/ci2) that displays lateralized circling behaviour, locomotor hyperactivity, ataxia and stereotypic head-movement. These abnormal behaviours occur in phases or bursts either spontaneously or in response to stress. Heterozygous (ci2/+) littermates display normal spontaneous behaviours. We have previously found that ci2/ci2 rats of both genders have a lower tissue content of dopamine in the striatum ipsilateral to the preferred direction of rotation, indicating that the rats turn away from the brain hemisphere with higher striatal

Lutembacher syndrome is a rare clinical entity, characterized by the coexistence of atrial septal defect and mitral stenosis. The hemodynamic interplay between these lesions often delays symptom onset, making late presentations uncommon. Advanced disease may result in severe pulmonary hypertension, right heart failure, and poor clinical outcomes, especially when compounded by superimposed infections. A 68-year-old female presented with progressive breathlessness for 2 years, recently worsening, along with cough with expectoration and bilateral pedal edema. Clinical examination revealed raised
