Cardiopulmonary Exercise Testing in Patients with Cardiac Pacemakers: From Device Optimization to Individualized Cardiac Rehabilitation.

Permanent cardiac pacing abolishes symptomatic bradycardia, yet a substantial proportion of recipients remain limited on exertion. Chronotropic incompetence is present in about half of pacemaker-wearing patients when assessed by the Wilkoff index, and the rate-responsive function intended to correct it is frequently left at the settings applied at implantation. Read across the whole cardiopulmonary exercise test rather than at peak effort, the relationship between heart rate and oxygen consumption distinguishes an inadequate sensor slope, an activity threshold set too high, a reaction time out

Reflex-Mediated Bradycardia and Asystole During Skull Base and Parapharyngeal Surgery: A Three-Case Series Highlighting Anatomical Predictability.

Intraoperative bradycardia and asystole during cranial neurovascular surgery are often regarded as unexpected events. In clinical practice, however, these episodes may occur during manipulation of specific cranial nerve-rich anatomical regions, suggesting that the risk may be anticipated and clinically actionable. We report three patients who developed suspected reflex-mediated cardioinhibitory events during surgery involving the jugular foramen, pericarotid skull base, and parapharyngeal space. The events occurred during operations involving regions in which glossopharyngeal or vagal pathways

Effects of atropine on hemodynamic parameters following electroconvulsive therapy: A systematic review and meta-analysis.

INTRODUCTION: Electroconvulsive therapy (ECT) plays an important role in the treatment of refractory psychiatric disorders. However, it poses hemodynamic risks, such as blood pressure and heart rate elevation, arrhythmias, and transient asystole. Although atropine administration during ECT was considered a standard procedure in Western countries until the 1970s, in recent years, its use is generally limited to patients at high risk of bradycardia and asystole. We performed this meta-analysis of randomized controlled trials (RCTs) to understand the effects of atropine on post-ECT hemodynamics a

Antepartum electronic fetal monitoring interpretation: multicenter observational study.

BACKGROUND: Antepartum EFM is the most widely used method worldwide to evaluate fetal well-being. However, neither NST nor other surveillance techniques can precisely determine the onset or degree of fetal hypoxemia and acidemia. The presence of accelerations seems to reflect fetal well-being; however, their absence does not predict fetal compromise. Evidence regarding the perinatal prognosis of normal NSTs with one or no accelerations compared with those classified as reactive remains insufficient. OBJECTIVE: To compare perinatal outcomes of the non-stress test (NST) interpreted by the usual

Homozygous LMNA (p.Arg216His) variant in an Ecuadorian patient with atrial flutter and high-grade atrioventricular block: a case report.

Cardiac laminopathies caused by LMNA variants may present with conduction disease, atrial arrhythmias, and ventricular dysfunction, sometimes before overt ventricular dilation. We report a 42-year-old Ecuadorian man with recurrent syncope and progressive exertional dyspnea who developed atrial flutter with a markedly slow ventricular response, high-grade atrioventricular block, and severe bradycardia. Next-generation sequencing using the TruSight Cardio panel identified NM_170707.4(LMNA):c.647G > A (p.Arg216His) in a homozygous state. Franklin by QIAGEN classified the variant as pathogenic und

Aveir VR for Atrial Leadless Pacing in Young Patients.

Atrial Leadless pacemaker implantspresent a new treatment for sinus node dysfunction in complex cardiac anatomies. The Aveir-VR single chamber pacemaker can be placed in the atrium to control atrial flutter, or in the ventricle to control bradycardia. In this case report, we present four cases of the implantation of the Aveir VR in the atrium of male pediatric patients to treat sinus node dysfunction. Retrospective review of patients with symptomatic sinus node dysfunction and atrially placed Aveir-VRs was performed at UC Davis between April 2024 and April 2026. Femoral access was the route of

Predictive Value of the Electrical Risk Score for Ventricular Arrhythmias in Heart Failure Patients with Primary Prevention Implantable Cardioverter Defibrillators.

Background: Implantable cardioverter defibrillators (ICDs) are essential for the primary prevention of sudden cardiac death in heart failure (HF); however, appropriate shocks increase mortality and impair quality of life. The electrical risk score (ERS) is an electrocardiographic (ECG)-based composite index with proven prognostic value, but its ability to predict ventricular arrhythmias in HF remains unclear. This study aims to evaluate the predictive value of the ERS for ventricular arrhythmia-related ICD shocks in HF patients undergoing primary prevention ICD implantation. Methods: In this s

Preoperative CT Suspicion of Main Pancreatic Duct Injury in Isolated Blunt Pancreatic Trauma: A Case Report With Operative Correlation.

An 18-year-old male had persistent tachycardia after blunt abdominal trauma. CT signs raised suspicion of main pancreatic duct disruption in the tail (suspected AAST Grade III). Exploratory laparotomy confirmed ductal transection, managed by spleen-preserving distal pancreatectomy. Six-month follow-up showed no fistula or clinical pancreatic insufficiency.

Spanish catheter ablation registry. 25th official report of the Heart Rhythm Association of the Spanish Society of Cardiology (2025).

INTRODUCTION AND OBJECTIVES: We report the results of the 2025 Spanish catheter ablation registry. METHODS: This retrospective registry included catheter ablation procedures performed at participating hospitals in Spain and recorded in the REDCap database using a dedicated data collection form. RESULTS: A total of 109 centers participated, reporting 33 435 ablations. Atrial fibrillation was the most commonly treated arrhythmia (14 452 cases), with single-shot techniques being the most frequently used approach (63%) and a marked increase in pulsed field ablation (37% vs 21% in 2024). The rema

Use of an abridged desensitization protocol to rechallenge oxaliplatin dosing after a suspected hypersensitivity reaction to FOLFOX: A case report.

IntroductionHypersensitivity reactions (HSRs) can occur in a consequential number of patients who receive oxaliplatin, a cornerstone agent of many chemotherapy regimens used to treat gastrointestinal malignancies. Premedications are not an infallible strategy for preventing HSR recurrence; thus, HSRs often preclude further treatment with this medication.Case reportWe present the case of an adult male who experienced clinical HSR symptoms (including rash, tachycardia and dyspnea) after receiving FOLFOX chemotherapy for adjuvant colon adenocarcinoma, and his subsequent clinical management.Manage

A Diphenhydramine Overdose Mimicking Encephalitis, Complicated by Acute Kidney Injury Associated with Anticholinergic Urinary Retention: A Case Report.

A 26-year-old man presented with fever, an altered consciousness, tremor, mydriasis, tachycardia, and a dry oral mucosa and was empirically treated for suspected encephalitis. A rapid neurological improvement and anticholinergic findings prompted the reconsideration of intoxication. A family assisted room search revealed empty over-the-counter medication bottles, and he later reported ingesting 2.4 g of diphenhydramine at once. Acute kidney injury developed in association with diphenhydramine-induced urinary retention, and his urine output and renal function improved after catheterization. In

Design and rationale of a multicenter randomized trial assessing strategies for catheter ablation of persistent atrial fibrillation- The STAR AF 3 trial.

RATIONALE: Pulmonary vein isolation (PVI) is the cornerstone of catheter ablation procedures for the treatment of atrial fibrillation (AF). However, the success rates of PVI are limited for patients with persistent AF. Yet, the optimal ablation strategy beyond PVI alone in patients with persistent AF remains to be established. STUDY DESIGN AND OBJECTIVES: The STAR AF 3 trial is an international multicenter prospective, single-blind, blinded endpoint, clinical trial that includes patients with persistent AF of more than 3 months duration referred for a first catheter ablation procedure. Pacemaker-associated bigeminy.

BACKGROUND: Pacemaker-associated bigeminy is an electrocardiographic pattern characterized by a regular alternation of two distinct cardiac events. It can arise from normal operation of device algorithms or indicate underlying dysfunction, such as sensing abnormalities. Accurate differentiation is crucial to avoid misdiagnosis and inappropriate clinical intervention. OBJECTIVE: To systematically elucidate the mechanisms underlying pacemaker-associated bigeminy and establish a practical diagnostic framework. METHODS: We conducted a retrospective analysis of six patients exhibiting pacemaker-a

Bilateral Ultrasound-Guided Erector Spinae Plane Block versus Serratus Anterior Plane Block for Postoperative Analgesia in Breast Reduction Surgery: A Prospective, Randomized, Outcome Assessor- Blinded, Controlled Trial.

BACKGROUND: Breast reduction surgery is a common aesthetic procedure associated with significant postoperative pain. Effective analgesia plays an important role in reducing opioid consumption, minimizing adverse effects, facilitating postoperative recovery, and improving patient satisfaction. Fascial plane blocks have recently gained popularity for breast surgery. This randomized study aimed to compare the effects of bilateral erector spinae plane block (ESPB) and serratus anterior plane block (SAPB) on postoperative analgesia in patients undergoing breast reduction surgery. METHODS: Seventy

Escalation of Resting Heart Rate and Tachycardia as a Response to Thoracic Radiotherapy: Implications for Cardiovascular Outcomes after Radiotherapy.

BACKGROUND: Cardiac toxicity is a well-recognized, but poorly understood complication of radiotherapy (RT). In non-oncological settings, there is an established relationship between resting heart rate and cardiovascular health. OBJECTIVES: To study the incidence and pattern of resting heart rate (rHR) as an early sign of cardiotoxicity among patients with non-metastatic non-small cell lung cancer (NSCLC) treated with RT. METHODS: We retrospectively analyzed serial rHR measurements of consecutive patients with locally advanced NSCLC treated with definitive chemoradiation (CRT) over 6-7 weeks

Hemobezoar-related jejunojejunostomy obstruction after minimally invasive Roux-en-Y gastric bypass: a systematic review.

BACKGROUND: Hemobezoar-related jejunojejunostomy (JJ) obstruction is a rare early complication after minimally invasive Roux-en-Y gastric bypass (RYGB). Although early small bowel obstruction after RYGB is well described, obstruction caused by hemobezoar formation remains poorly characterized. METHODS: This systematic review followed PRISMA 2020 guidelines and was registered in PROSPERO (CRD420261320591). PubMed/MEDLINE, Embase, and CENTRAL were searched from inception through February 2026. Eligible studies reported adults undergoing primary minimally invasive RYGB with early hemobezoar-rela

Pediatric restrictive cardiomyopathy with rapid complete atrioventricular block progression.

UNLABELLED: Restrictive cardiomyopathy (RCM) is a rare pediatric cardiomyopathy characterized by ventricular diastolic dysfunction, and poor prognosis that often requires heart transplantation. We report the case of a 7-year-old boy with RCM complicated by complete atrioventricular block (CAVB) in whom pathological examination of the explanted heart demonstrated extensive fibrosis involving the cardiac conduction system. At 1 year and 9 months of age, the patient presented with arrhythmia. Echocardiography revealed marked biatrial enlargement and small ventricles. Cardiac catheterization demon

Postextrastimulus Termination of Ventricular Tachycardia: An Indicator of a Good Ablation Site?

Entrainment mapping, or continuous resetting of a re-entrant tachycardia, provides a powerful tool for distinguishing between focal and re-entrant ventricular tachycardias (VTs) and for identifying critical circuit components, such as the entrance, critical isthmus (CI), and exit sites. However, this technique is not always feasible and, in some cases, may itself terminate VT during the entrainment attempt. Whereas local capture of the CI without global ventricular capture (nonglobal capture [NGC]) can terminate VT by creating a bidirectional block (orthodromic or nonorthodromic capture) withi

Matching Electrode Configuration to Signal Sensing for Cardiac Electroanatomic Mapping: A Critical Appraisal.

The management of cardiac arrhythmias has been transformed by electroanatomical mapping, yet it is often underappreciated how dependent mapping is to electrode configuration, sensing modality, catheter geometry, and signal-processing characteristics. This review systematically evaluates how these features influence mapping, in the context of catheters in contemporary linear, circular, grid, star-shaped and basket configurations. We compare direct sensing strategies in unipolar and bipolar configurations, monophasic action potentials, and discuss post-processing approaches including omnipolar,
