Bicuspid Aortic Valve with Giant Non-Coronary Sinus Aneurysm Compressing the Sinoatrial Node Causing Arrhythmia and Coarctation of the Aorta: A Case on Clinical Decision-Making and Surgical Challenges.

BACKGROUND: Bicuspid aortic valve, the most common congenital valvular disease, frequently coexists with aortic pathologies. Symptomatic arrhythmias from unruptured giant aortic sinus aneurysm compressing the sinoatrial node are exceedingly rare. The coexistence of bicuspid aortic valve, arrhythmogenic sinus aneurysm, coarctation of the aorta, and right coronary artery occlusion represents an extraordinarily complex malformation with scarce documentation and controversial management strategies. CASE PRESENTATION: A 67-year-old male patient presented with chest tightness and palpitations. Echo

A Case of MEN 2A Syndrome With Bilateral Recurrent Pheochromocytoma: A Case Report and Literature Review.

Pheochromocytoma is a neuroendocrine tumor arising from chromaffin cells and producing catecholamines. Due to the excessive release of catecholamines, these tumors can present with paroxysmal episodes of high blood pressure, headache, diaphoresis, and palpitations. We present the case of a 23-year-old female patient who presented to the clinic with a history of headache, palpitations, and vomiting for two months. She was admitted for further investigations and management. Laboratory studies revealed elevated urinary and plasma catecholamine levels. Further work-up revealed bilateral adrenal ma

Fluoroless Ablation of a Coronary Sinus-related Accessory Pathway Without Coronary Sinus Angiogram in a Patient with Multiple Prior Failed Ablations.

Accessory pathways (APs) associated with the coronary sinus (CS), often linked to diverticula, are rare and challenging to ablate, frequently requiring CS venography for anatomical guidance. We report the case of a 56-year-old woman with recurrent palpitations, manifest pre-excitation, and two prior failed ablations for a presumed posteroseptal pathway. The electrocardiogram showed an immediately negative QRS complex in the inferior leads, suggesting an epicardial CS-related pathway. Using solely three-dimensional (3D) electroanatomic mapping (CARTO™; J&J MedTech, New Brunswick, NJ, USA) witho

Mitochondrial DNA depletion syndrome 12B: role of photon-counting computed tomography in detection of cardiac involvement-a case report.

BACKGROUND: Mitochondrial DNA depletion syndrome 12B (MDS12B) is an ultra-rare autosomal recessive disorder caused by SLC25A4 mutations, which encode the mitochondrial adenine nucleotide translocator. Hypertrophic cardiomyopathy (HCM) is the predominant manifestation, yet detailed cardiac tissue characterization remains limited. CASE SUMMARY: A 28-year-old male presented with a 17-year history of exertional palpitations and progressive fatigue. Echocardiography revealed severe concentric left ventricular hypertrophy with moderately reduced ejection fraction (42%). Photon-counting computed tom

Pulsed field ablation for persistent atrial fibrillation with atrioventricular reentrant tachycardia under tubeless general anesthesia: a case report.

BACKGROUND: Radiofrequency ablation (RFA) combined with left atrial appendage closure (LAAC) has long been used in the treatment of atrial fibrillation (AF). Pulsed field ablation (PFA) is now increasingly adopted due to its great precision and improved safety profile. This report presents a rare case of PFA performed for persistent AF and atrioventricular reentrant tachycardia (AVRT), along with LAAC, under non-intubated general anesthesia. CASE DESCRIPTION: A 36-year-old male patient presented with a 10-year history of recurrent chest tightness and palpitations, which had worsened during th

Acute Lymphocytic Myocarditis Complicated By Bidirectional Ventricular Tachycardia Treated With Early Steroid Use.

Acute lymphocytic myocarditis can precipitate malignant ventricular arrhythmias, including the rare presentation of bidirectional ventricular tachycardia (BDVT), a rhythm associated with significant hemodynamic instability. Although BDVT is classically linked to catecholaminergic polymorphic ventricular tachycardia and digitalis toxicity, it is increasingly recognized in myocarditis, likely reflecting diffuse inflammatory involvement of the myocardium and Purkinje system. Emerging evidence suggests that early corticosteroid therapy may rapidly reduce arrhythmia burden and improve ventricular f

[Tsutsugamushi disease with left heart failure as the main manifestation complicated by multiple organ dysfunction: a case report and literature review].

Tsutsugamushi disease (TD) is a zoonotic and naturally focal infectious disease caused by Orientia tsutsugamushi (Ot), transmitted between humans and rodents through the bite of chigger mites. On August 17, 2025, a patient with TD complicated by acute left sided heart failure and multiple organ dysfunction was admitted to Wuhan Jinyintan Hospital. The patient presented to the emergency department with a 10 day history of fever, palpitations, dyspnea, and cough, and had a history of outdoor exposure. Physical examination revealed a temperature of 39.2 centigrade, a heart rate of 79 beats/min (s

Biatrial cardioneuroablation guided by robotic magnetic navigation and artificial intelligence-based mapping in vagal bradyarrhythmias: A controlled observational study.

PURPOSE: Reflex bradyarrhythmias and syncope related to excessive vagal tone may be refractory to conservative therapy and significantly impair quality of life. Cardioneuroablation (CNA) has emerged as a device-sparing alternative, but real-world outcome data remain limited. METHODS: We retrospectively evaluated 12 consecutive patients (aged 23-55 years) with drug-refractory, vagally mediated bradyarrhythmias confirmed by tilt-table testing. All underwent biatrial CNA guided by ultra-high-density electroanatomical mapping with automated fragmented-electrogram detection and robotic magnetic na

Biological age acceleration and bradyarrhythmia: evidence from clinical and epigenetic perspectives.

BACKGROUND: To date, effective preventive and therapeutic strategies for early-stage bradyarrhythmia remain limited. METHODS: We first calculated two well-established biological aging (BA) measures (Klemera-Doubal Method [KDM] and Phenotypic Age [PhenoAge]), with BA acceleration defined as the residual from regressing each BA measure on chronological age. Cox proportional hazards models were used to evaluate the effects of BA acceleration on bradyarrhythmia risk. We then conducted epigenetic Mendelian randomization (MR) and colocalization analyses to prioritize aging-related DNA methylation C

Oculocardiac Reflex during Electrode Placement in Trigeminal Schwannoma Surgery via Cavernous Sinus Approach.

The oculocardiac reflex (OCR) is well known in ophthalmic surgery, but its occurrence during neurosurgical procedures is rare and often unexpected. Mechanical stimulation of trigeminal pathways at the skull base can cause significant hemodynamic changes; however reflex activation during preparatory steps rather than tumor manipulation is rarely described. We report an intraoperative event in a 22-year-old male undergoing resection of a cystic trigeminal schwannoma via a subtemporal transcavernous approach under general anesthesia, during which periorbital needle electrodes were placed for cran

Percussion Pacing as a Rescue for Peri-Stimulus Asystole During Electroconvulsive Therapy.

Electroconvulsive therapy (ECT) can provoke a brief parasympathetic surge, commonly causing transient sinus bradycardia or a short sinus pause. Most events last only seconds and resolve without intervention. Rarely, however, bradyarrhythmia or asystole may persist long enough to threaten cerebral perfusion, requiring anesthesia teams to respond immediately. We report the case of a male patient with treatment-resistant major depressive disorder who underwent bilateral ECT under methohexital and succinylcholine. Baseline sinus bradycardia was present at 47 beats per minute. Immediately after the

Conduction Gap of Left Atrial Anterior Wall Linear Ablation Undetected by Left Atrial Appendage Pacing but Revealed by Left Lateral Ridge Pacing.

Confirmation of counterclockwise conduction block across the left atrial anterior wall linear lesion is usually performed during left atrial appendage pacing. However, even when endocardial conduction block across the left atrial anterior wall linear lesion is suggested during left atrial appendage pacing, an epicardial connection between the left lateral ridge and the left atrial anterior wall can be a cause of residual conduction gap. In such a situation, assessment during pacing with the left lateral ridge capture should be considered.

LAA occluder device impeding pulsed field ablation of an anterior mitral line: a case report about the role of a dual energy source.

BACKGROUND: Pulsed field ablation (PFA) and percutaneous closure of the left atrial appendage are increasingly used in patients with atrial fibrillation. An interaction between this form of energy and metallic structures in the heart is an emerging problem. We first report a left atrial appendage closure (LAAC)-limiting PFA using a novel dual energy ablation catheter, solved by switching to the classic radiofrequency ablation. CASE SUMMARY: An 85-year-old woman with heart failure with preserved ejection fraction and a history of persistent atrial fibrillation (status post pulmonary vein isola

Impact of 18FDG-Avidity and Immunosuppression on Idiopathic and Genetic Cardiomyopathies.

BACKGROUND: Myocardial 18fluorodeoxyglucose (18FDG)-avidity is frequently seen in patients with genetic cardiomyopathy, as well as a growing "idiopathic 18FDG-avidity" group of genotype-negative patients who do not clearly have cardiac sarcoidosis. The prognostic implications of 18FDG-avidity in patients with and without genetic cardiomyopathy, and the effects of immunosuppression in the latter, remain uncertain. METHODS: This multicenter, retrospective study included all patients who were referred for both 18FDG-PET and cardiomyopathy genetic testing. Patients with acute myocarditis, biopsy-

Refractory Ventricular Tachycardia-Induced Severe Cardiomyopathy in a Child: Successful Radiofrequency Ablation With ECMO Support.

BACKGROUND: Ventricular tachycardia (VT)-induced cardiac dysfunction is a recognized but uncommon cause of cardiomyopathy in children. CASE SUMMARY: A 7-year-old girl presented with severely depressed left ventricular function and incessant VT that was refractory to multiple antiarrhythmics and attempted cardioversions. Because of hemodynamic instability, the patient required extracorporeal membrane oxygenation (ECMO) support and ultimately radiofrequency ablation of a focal VT that was localized to the right midventricular septum near the moderator band. The patient had a subsequent return o

Identifying the High-Risk Arrhythmic Mitral Valve Prolapse Phenotype by Imaging.

Within arrhythmogenic mitral valve prolapse (AMVP), a distinct subset of mitral valve prolapse (MVP), there is a small group that carries a higher risk of malignant ventricular arrhythmias and sudden cardiac death called malignant MVP. The malignant MVP phenotype is over-represented in young women with unexplained sudden cardiac death on autopsy. AMVP is defined by myxomatous, often bileaflet MVP with mitral annular disjunction, abnormal valvular-myocardial mechanics, complex ventricular ectopy, and nonsustained ventricular tachycardia. Excessive mechanical force from systolic annular expansio

From call to home: The role of dispatcher-assisted CPR in patient survival.

BACKGROUND: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) is a key strategy to reduce no-flow time in out-of-hospital cardiac arrest (OHCA), yet its impact on long-term survival remains various across emergency medical systems. PURPOSE: We aimed to evaluate the association between DA-CPR and resuscitation outcomes in a nationwide cohort of OHCA patients presenting with an initial shockable rhythm. METHODS: We conducted a retrospective observational study including all adult OHCA cases with ventricular fibrillation or pulseless ventricular tachycardia (VF/VT) in which resuscitati
