Failure of Implantable Cardioverter-defibrillator Pacing Capture in BRASH Syndrome with Severe Hyperkalemia.

This case report describes a 54-year-old woman with end-stage renal disease and heart failure who presented with BRASH syndrome: a synergistic cycle of bradycardia, renal failure, atrioventricular nodal blockade, shock, and hyperkalemia. The patient, who was adherent to metoprolol and amiodarone, presented with a profound ventricular escape rhythm of 16 bpm and hemodynamic instability after missing 4 days of hemodialysis. An interesting complication was the failure of her implantable cardioverter-defibrillator (ICD) to achieve pacing capture, a phenomenon likely driven by a potassium level of

Rapid Progression of Cardiac Allograft Vasculopathy With Sudden Cardiac Death Within the First Year of Heart Transplantation: A Case Report.

Cardiac allograft vasculopathy (CAV) is a major cause of mortality and morbidity within the first 3 years of heart transplantation (HT). CAV results in progressive coronary artery stenosis and may lead to arrhythmias and sudden cardiac death. In some cases, sudden cardiac death represents the initial presentation of CAV, as cardiac denervation blunts anginal symptoms that otherwise could have preceded sudden cardiac death (SCD). Here, we describe the case of a 56-year-old man with non-ischemic cardiomyopathy. He underwent left ventricular assist device implantation followed by orthotopic HT. F

Anti-N-Methyl-D-Aspartate Receptor Encephalitis With Autonomic Instability and Focal Seizures Without Detectable Ovarian Teratoma.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a severe but potentially reversible form of autoimmune encephalitis, and it typically affects young women. It can be challenging to differentiate from other conditions that cause symptoms such as seizures and psychiatric and autonomic manifestations. We present the case of a previously healthy young woman without any significant past medical history who developed acute-onset, progressive neuropsychiatric disturbance, focal impaired awareness seizures, cognitive impairment, and intermittent autonomic disturbance manifesting as sinu

Primary left ventricular intimal sarcoma: a case report of multimodality imaging and molecular confirmation.

Primary cardiac intimal sarcoma is a rare malignant mesenchymal neoplasm whose intracavitary appearance may mimic other cardiac masses. We report a woman in her early 60s with recurrent palpitations and occasional exertional dyspnea. Transthoracic echocardiography (TTE) demonstrated a giant, highly mobile, irregular multilobulated mass occupying most of the left ventricular cavity, with slit-like interlobular clefts and a probable broad-based attachment near the apical interventricular septum. Low-velocity color Doppler demonstrated residual flow around and between lobules, while continuous-wa

Intracerebral Hemorrhage as the Initial Manifestation of Brain Metastasis from Left Atrial Intimal Sarcoma in a Young Adult: A Case Report.

Intratumoral hemorrhage caused by metastatic brain tumors originating from intimal sarcoma is extremely rare. We report a diagnostically challenging case of cerebral metastasis from a left atrial intimal sarcoma that initially presented as subcortical hemorrhage in a young adult. A 24-year-old woman presented with right homonymous hemianopia and was diagnosed with a subcortical hemorrhage in the left parietal lobe. No obvious source of bleeding was identified, and the patient was managed conservatively and discharged. One month later, fever, dyspnea, and palpitations developed in the patient.

Antispasmodic and analgesic strategies for renal colic in pregnancy: a comprehensive analysis.

BACKGROUND: Analgesia remains the paramount priority in the clinical management of gestational renal colic, yet established protocols are frequently constrained by contraindications. While antispasmodic intervention emerges as a viable alternative, the definitive pharmacological choice remains elusive. Here, we evaluated the effectiveness and safety of commonly used antispasmodic and analgesic strategies for pregnancy-related renal colic using network and pairwise meta-analyses. METHOD: A literature review was undertaken utilizing three English-language databases and four Chinese-language dat

Increasing paced heart rate reduces cerebral pulsatility.

Cerebral pulsatility is a key index of cerebral haemodynamics, a central determinant of intravascular shear stress within intracranial vessels, and is associated with cerebral small vessel disease. It increases with a spontaneously falling heart rate, but the effect of altering heart rates on cerebral haemodynamics is unknown. We therefore tested whether increasing paced heart rate would reduce middle cerebral artery Gosling's pulsatility index (PI)on transcranial ultrasound, during emergency or elective implantation of a pacemaker for bradycardia. In 20 participants (14 males, mean age 78), p

Tachycardia Initiation by Dual-Mode PVC on Ambulatory ECG: Pattern Discovery and Invasive Electrophysiological Characteristics.

BACKGROUND: Mechanisms of premature ventricular complex (PVC)-induced long-RP tachycardia warrant investigation. CASE SUMMARY: A 57-year-old man with palpitations underwent 24-hour ambulatory electrocardiogram (ECG). The recording showed 4,150 monomorphic PVCs and 800 episodes of long-RP tachycardia, all triggered by the same PVC morphology. Two initiation patterns emerged. One involved direct retrograde conduction, the other showed retrograde conduction only after PR prolongation. In both patterns, induction occurred across a broad range of PVC coupling intervals. Invasive electrophysiology

Cellular consequences of non-ablative radiotherapy, a novel approach to ventricular tachycardias.

BACKGROUND AND AIMS: Radiotherapy (RT) with a single application of ionizing radiation (STAR) has been suggested as a non-invasive alternative to radiofrequency in ablating ventricular tachycardia (VT). Emerging data reveal that STAR may suppress VT without substrate destruction, by enhancing impulse conduction instead, through increased expression of NaV1.5 channels and connexin 43. The study aims to investigate electrophysiology and intracellular Ca2+ dynamics in cardiomyocytes (CMs) from mice subjected to in-vivo RT. The emerging data led us to evaluate biochemical changes potentially linki

Clinical Application and Safety of Intrathecal Adjuvants in Single-Shot Thoracic Segmental Spinal Anesthesia: A Narrative Review.

Thoracic segmental spinal anesthesia (TSSA) is an emerging regional anesthesia (RA) technique. TSSA with local anesthetic (LA) alone is associated with a relatively short duration of surgical anesthesia and postoperative analgesia, which may limit its clinical utility. Intrathecal adjuvants have been widely used in lumbar spinal anesthesia (LSA) to prolong block duration and improve block quality. However, their role in TSSA remains poorly defined. This review summarizes commonly used intrathecal adjuvants during TSSA, their doses, adverse effects, and safety considerations. A comprehensive li

Prehospital extracorporeal cardiopulmonary resuscitation for refractory out-of-hospital cardiac arrest: a protocol for the Rapid prehospital ACcess to ECPR in Prague (RACE Prague) single-arm pilot feasibility study.

AIM: The benefit of extracorporeal cardiopulmonary resuscitation (ECPR) in refractory out-of-hospital cardiac arrest (OHCA) is strongly time dependent, yet in established in-hospital ECPR systems, including Prague, the median collapse-to-flow time has remained around 60 min or more, dominated by intra-arrest transport. Evidence for prehospital ECPR remains limited, particularly its comparison with in-hospital ECPR. RACE Prague evaluates whether dispatching a mobile cannulation team to the scene shortens this interval within the same ECPR system. METHODS: Single-centre, prospective, interventi

Palpitations in children and adolescents in the emergency department: a state-of-the-art narrative review of arrhythmia risk, diagnostic strategy, and disposition.

BACKGROUND: Palpitations are not an uncommon complaint in the paediatric emergency department, causing significant anxiety for families concerned about serious heart disease. However, most cases are benign, often stemming from normal physiological responses like fever or anxiety. Primary cardiac arrhythmias are rare in this clinical setting. PROBLEM: The main challenge is identifying the few children with dangerous arrhythmias among the many benign cases. Fear of missing a life-threatening condition often leads to extensive, costly, and stressful investigations, even in low-risk patients, wit

Umbilical Vein Elastic Fiber Deficiency as an Emerging Risk Factor for Catastrophic Umbilical Cord Hemorrhage: A Case Report.

BACKGROUND: Umbilical cord hemorrhage (UCH) is a rare and fatal event involving the rupture of the umbilical vein. UCH is associated with mechanical reasons, such as cord entanglement, but new evidence suggests congenital structural weakness such as a lack of elastic fibers in the vein wall (umbilical vein dysplasia) as a predisposing factor, though it is not well understood clinically. This case report describes a term breech neonate who died from umbilical cord hemorrhage; autopsy revealed a marked deficiency of elastic fibers in the umbilical vein wall, suggesting a potential association wi

Clinical and Genetic Characteristics of Hereditary Arrhythmias and Cardiomyopathies in the Middle East.

Inherited arrhythmias and cardiomyopathies are leading causes of sudden cardiac death in young individuals. Although global registries and genetic databases have advanced diagnosis and prevention globally, data from the Middle East (ME) remain limited and fragmented. This review synthesizes clinical and genetic evidence on long QT syndrome, catecholaminergic polymorphic ventricular tachycardia, Brugada syndrome, arrhythmogenic right ventricular cardiomyopathy, hypertrophic cardiomyopathy, and dilated cardiomyopathy across the ME. A defining feature of ME cohorts is the high prevalence of consa

Comparison of Sugammadex and Neostigmine for Reversal of Vecuronium-Induced Neuromuscular Blockade in Pediatric Patients: A Prospective Randomized Study.

Background and objective Neuromuscular blocking agents (NMBAs) used during general anesthesia require effective reversal to prevent residual paralysis. Neostigmine, the conventional reversal agent, is associated with slower recovery and muscarinic side effects. Sugammadex selectively encapsulates amino-steroidal NMBAs, providing rapid and predictable reversal. This study compared the efficacy, safety, and recovery profile of sugammadex and neostigmine for reversal of vecuronium-induced neuromuscular blockade in children. Methods In this prospective, randomized, double-blind trial, 100 children
