Open-Irrigated Versus Non-Irrigated Catheters in SVT Ablation: Substrate-Specific Outcomes in AVRT and AVNRT.

BACKGROUND: Open-irrigated catheters (OIC) provide improved cooling, deeper lesion formation, and enhanced safety during radiofrequency catheter ablation (RFCA). However, comparative evidence regarding substrate-specific outcomes with OIC versus non-irrigated catheters (NIC) for supraventricular tachycardia (SVT) remains limited. This study compared acute ablation success (AAS) and recurrence after first-time RFCA using OIC versus NIC, with particular emphasis on differential outcomes in atrioventricular re-entrant tachycardia (AVRT) and atrioventricular re-entrant tachycardia (AVNRT). METHOD

Duration of surfactant administration and its impact on stabilisation of vital parameters in very preterm neonates <28 weeks of gestational age (SurfStab): 1 min versus 5 min-study protocol for a prospective randomised-controlled phase IV trial.

BACKGROUND: Respiratory distress syndrome remains one of the leading causes of morbidity and mortality in extremely preterm neonates. Surfactant replacement therapy substantially improves survival. Current European Consensus Guidelines recommend the less invasive surfactant administration (LISA) technique. However, no evidence exists on whether the duration of administration affects physiological stability. Rapid surfactant administration may provoke transient hypoxia or bradycardia due to airway obstruction, whereas slower delivery could potentially reduce such fluctuations. METHODS/DESIGN:

Mitochondrial DNA depletion syndrome 12B: role of photon-counting computed tomography in detection of cardiac involvement-a case report.

BACKGROUND: Mitochondrial DNA depletion syndrome 12B (MDS12B) is an ultra-rare autosomal recessive disorder caused by SLC25A4 mutations, which encode the mitochondrial adenine nucleotide translocator. Hypertrophic cardiomyopathy (HCM) is the predominant manifestation, yet detailed cardiac tissue characterization remains limited. CASE SUMMARY: A 28-year-old male presented with a 17-year history of exertional palpitations and progressive fatigue. Echocardiography revealed severe concentric left ventricular hypertrophy with moderately reduced ejection fraction (42%). Photon-counting computed tom

Surgery to treat autonomic dysreflexia due to cubital tunnel syndrome in cervical spinal cord injury: a case report.

INTRODUCTION: Autonomic dysreflexia (AD) is a potentially fatal syndrome in people with spinal cord injury (SCI) marked by episodic hypertension with flushing, diaphoresis, and bradycardia. Common etiologies include bladder distension and bowel impaction. Compressive neuropathies lack attention as a potential precipitant of AD. This report describes an individual with cervical SCI and with recurrent AD treated with ulnar nerve decompression surgery. CASE PRESENTATION: A 74-year-old woman with a remote history of C6 SCI (American Spinal Injury Association Impairment Scale Grade B) presented to

Pulsed field ablation for persistent atrial fibrillation with atrioventricular reentrant tachycardia under tubeless general anesthesia: a case report.

BACKGROUND: Radiofrequency ablation (RFA) combined with left atrial appendage closure (LAAC) has long been used in the treatment of atrial fibrillation (AF). Pulsed field ablation (PFA) is now increasingly adopted due to its great precision and improved safety profile. This report presents a rare case of PFA performed for persistent AF and atrioventricular reentrant tachycardia (AVRT), along with LAAC, under non-intubated general anesthesia. CASE DESCRIPTION: A 36-year-old male patient presented with a 10-year history of recurrent chest tightness and palpitations, which had worsened during th

Opioid-free blended analgesia in patients undergoing thoracic surgery: Preliminary results.

OBJECTIVE: Thoracic surgery is recognized as one of the most painful surgical procedures. Opioids represent the most effective analgesic drug for pain management after these procedures. However, opioid use is related to side effects such as nausea, constipation, drowsiness, hypotension, bradycardia. Because of this, recently, studies have focused on blended analgesia, which combines general and loco-regional anesthesia (LRA).This study aims to demonstrate the effectiveness of opioid-free blended anesthesia in pain management in patients undergoing open or endoscopic thoracic surgery. DESIGN:

Urogenital abscess with a complex etiology: a case report.

BACKGROUND: Scrotal abscesses requiring hospitalization are uncommon, with progression to sepsis being even rarer. A feared complication of a scrotal abscess is Fournier gangrene, an uncommon type of necrotizing fasciitis that starts in the scrotum or perineum. Fournier gangrene is associated with a mortality rate of about 40% and accounts for less than 0.02% of all hospital admissions. The patient in this report was hospitalized for a complex scrotal abscess and, during repeat incision and drainage (I&D), was found to have Fournier gangrene. CASE DESCRIPTION: The patient was a 49-year-old ma

Perineural versus intravenous dexmedetomidine as an adjuvant for ilioinguinal/iliohypogastric nerve block in open inguinal hernia repair: a randomised controlled trial.

PURPOSE: Dexmedetomidine is widely added to peripheral nerve blocks, but whether the perineural or intravenous route provides better analgesia-and at what haemodynamic cost-is unclear in inguinal hernia repair. We compared the two routes as adjuncts to an ilioinguinal/iliohypogastric (II/IH) nerve block. METHODS: This single-centre, triple-blind randomised controlled trial enrolled 90 adults (ASA I-III) undergoing elective open inguinal hernia repair. All received an ultrasound-guided II/IH block (0.5% bupivacaine with epinephrine, 20 mL) and standardised tumescent local anaesthesia, and were

Acute Lymphocytic Myocarditis Complicated By Bidirectional Ventricular Tachycardia Treated With Early Steroid Use.

Acute lymphocytic myocarditis can precipitate malignant ventricular arrhythmias, including the rare presentation of bidirectional ventricular tachycardia (BDVT), a rhythm associated with significant hemodynamic instability. Although BDVT is classically linked to catecholaminergic polymorphic ventricular tachycardia and digitalis toxicity, it is increasingly recognized in myocarditis, likely reflecting diffuse inflammatory involvement of the myocardium and Purkinje system. Emerging evidence suggests that early corticosteroid therapy may rapidly reduce arrhythmia burden and improve ventricular f

Thyroid storm revealing suspected evolving autoimmune polyglandular syndrome type 2 with steroid-limited assessment of autoimmune adrenal insufficiency: a case report.

BACKGROUND: Autoimmune polyglandular syndrome type 2 (APS-2) is a rare autoimmune disorder characterized by the coexistence of autoimmune adrenal insufficiency with autoimmune thyroid disease and/or type 1 diabetes mellitus. Its heterogeneous and progressive presentation frequently delays diagnosis, particularly during acute endocrine emergencies where one dominant manifestation may obscure concurrent glandular dysfunction. Recognition of evolving autoimmune disease before overt adrenal insufficiency develops remains particularly challenging, especially when emergency corticosteroid therapy is

Pilsicainide Intoxication Presenting With Alternating Accelerated Idioventricular and Junctional Escape Rhythms During Acute Kidney Injury: A Case Report.

Pilsicainide is a class Ic sodium channel blocker that is eliminated predominantly by the kidneys and may accumulate when renal function deteriorates. An 85-year-old man receiving pilsicainide 150 mg/day developed acute kidney injury (AKI) during hospitalization for hemorrhagic colitis. He subsequently developed impaired consciousness, dyspnea, pulmonary congestion, and alternating episodes of junctional escape rhythm at 30-37 beats/min and a regular, wide QRS rhythm at 79 beats/min. The QRS duration was approximately 150 ms, and the serum pilsicainide concentration was 3.19 µg/mL. The wide QR

Excessive supraventricular ectopic activity and new-onset ischemic stroke: a retrospective cohort study with prospective outcome ascertainment.

BACKGROUND: Excessive supraventricular ectopic activity (ESVEA), a marker of both paroxysmal atrial fibrillation (pAF) and underlying atrial cardiomyopathy, may serve as an earlier electrographic predictor of cryptogenic ischemic stroke than pAF itself. This study aimed to investigate the association between ESVEA and the risks of new-onset ischemic stroke and all-cause mortality in an Asian population, and to explore a potential data-driven threshold that requires external validation. METHODS: In this retrospective cohort study with prospective outcome ascertainment, data were collected from

Intraoperative Dobutamine Plus Norepinephrine to Prevent Postoperative Acute Kidney Injury in Septic Shock: A Randomized Clinical Trial.

IMPORTANCE: International guidelines recommend dobutamine for management of septic shock and cardiac dysfunction with persistent hypoperfusion in adults based on very low-certainty evidence. Randomized trials are lacking on the effect of intraoperative dobutamine combined with norepinephrine vs norepinephrine on postoperative outcomes in patients with septic shock undergoing surgical source control. OBJECTIVE: To determine whether intraoperative dobutamine combined with norepinephrine reduces acute kidney injury (AKI) within 7 postoperative days in patients with septic shock undergoing surgic

Paradoxical bradycardia from occult venous hemorrhage mimicking vasovagal syncope - A case report.

BACKGROUND: Hemorrhagic shock typically presents with tachycardia; however, profound hypovolemia can paradoxically induce bradycardia via the Bezold-Jarisch reflex. This can lead to misdiagnosis of vasovagal syncope, causing dangerous treatment delays. CASE: A 50-year-old woman developed profound bradycardia and hypotension after vomiting in the post-anesthesia care unit after undergoing hysterectomy. Despite initial management of vasovagal syncope, her condition remained refractory to vasopressors. A critically low hemoglobin level of 5.5 g/dL strongly suggested massive occult hemorrhage. Sh

Cytochrome P450 2D6 Metabolizer Phenotype Is Associated With Metoprolol Pharmacodynamics But Not With Metoprolol Discontinuation: A Retrospective Cohort Study in the All of Us Research Program.

BACKGROUND: The 2024 Clinical Pharmacogenetics Implementation Consortium (CPIC) guideline assigns a high level of evidence to the association between cytochrome P450 2D6 (CYP2D6) metabolizer status and both metoprolol exposure and heart-rate response and recommends genotype-informed dosing for poor metabolizers. Whether this well-characterized pharmacodynamic effect translates into a patient-level tolerability consequence, differential discontinuation has not been established at population scale in a diverse cohort. The aims of this study were to estimate the association between CYP2D6 metabol

Cholinergic urticaria following active forced-air warming in the post-anesthesia care unit: a case report and literature review.

INTRODUCTION: Cholinergic urticaria (CU), a subtype of chronic inducible urticaria, is triggered by elevations in core body temperature. Postoperative shivering and active warming measures may increase core body temperature in the post-anesthesia care unit (PACU), yet CU in this setting remains underrecognized and has rarely been reported. CASE PRESENTATION: We report a case of an 18-year-old male who developed acute CU in the PACU following cochlear implantation under general anesthesia. After extubation, the patient exhibited postoperative shivering and was treated with a forced-air warming

Biatrial cardioneuroablation guided by robotic magnetic navigation and artificial intelligence-based mapping in vagal bradyarrhythmias: A controlled observational study.

PURPOSE: Reflex bradyarrhythmias and syncope related to excessive vagal tone may be refractory to conservative therapy and significantly impair quality of life. Cardioneuroablation (CNA) has emerged as a device-sparing alternative, but real-world outcome data remain limited. METHODS: We retrospectively evaluated 12 consecutive patients (aged 23-55 years) with drug-refractory, vagally mediated bradyarrhythmias confirmed by tilt-table testing. All underwent biatrial CNA guided by ultra-high-density electroanatomical mapping with automated fragmented-electrogram detection and robotic magnetic na
