Life-Threatening Ventricular Tachycardia and Transient Brugada-Like Electrocardiographic Abnormalities Caused by Severe Hyperkalemia Due to Urinary Retention in a 55-Year-Old Man.

BACKGROUND Hyperkalemia is a potentially fatal electrolyte disturbance that typically manifests as progressive conduction abnormalities, bradyarrhythmias, and sine-wave electrocardiographic changes. In rare cases, however, severe hyperkalemia also presents with malignant ventricular tachyarrhythmias and transient electrocardiographic abnormalities involving the right precordial leads, potentially leading to diagnostic confusion with primary arrhythmic disorders. CASE REPORT A 55-year-old man with no known cardiovascular disease presented with palpitations, dyspnea, and diaphoresis. Electrocard

VSD Device Removal Due to Intermittent Complete Heart Block.

BACKGROUND: Percutaneous closure of perimembranous ventricular septal defects (pmVSDs) has markedly increased with the development of newer, softer occlusion devices. CASE SUMMARY: We report a 12-year-old girl who underwent percutaneous pmVSD closure. One week later, she was admitted for paroxysmal complete heart block (CHB). She was started on intravenous corticosteroids. A percutaneous retrieval of the device was performed successfully. Sinus rhythm was maintained throughout the 8-month follow-up period. DISCUSSION: Paroxysmal CHB is a rare but serious complication following percutaneous p

Opioid-free versus opioid-based anesthesia for robot-assisted radical prostatectomy: a single-center randomized controlled trial.

Multimodal, opioid-sparing analgesia is standard in urological surgery, and there is additional interest in eliminating intraoperative opioids in prostate cancer surgery. Randomized evidence for opioid-free anesthesia (OFA) in robot-assisted radical prostatectomy (RARP) is lacking. In this single-center randomized controlled trial, adult men undergoing RARP were allocated 1:1 to OFA (dexmedetomidine plus ketamine, no intraoperative opioids) or a remifentanil-based technique. Both groups received identical multimodal premedication, a bilateral quadratus lumborum block, sevoflurane maintenance a

Environmentally relevant concentrations of a hexazinone-based herbicide impair zebrafish embryo-larval development by altering multiple biomarkers.

Hexazinone (HZN)-based herbicides are widely used in sugarcane crops and may pose risks to non-target organisms. This study evaluated the toxicity of environmentally relevant concentrations (0.05, 0.5, 5µgL-1) of the HZN-based herbicide Nortox® and two higher concentrations across zebrafish development, assessing lethal and non-lethal endpoints, neurotoxicity, cardiotoxicity, redox imbalance, and target gene expression. All HZN treatments increased mortality and reduced spontaneous contractions, whereas only the highest concentrations affected hatching and induced bradycardia. CAT activity inc

The effects of low-dose medetomidine and butorphanol premedication on anesthetic induction and recovery for intramuscular alfaxalone anesthesia in cats.

This study evaluated effects of premedication with low-dose medetomidine (MED), butorphanol (BTR), or their combination on intramuscular (IM) alfaxalone (ALFX) anesthesia in cats. Five healthy purpose-bred cats were used in a completely randomized crossover study with a minimum 1-week washout period. Cats received three types of premedication: MED (5 µg/kg), BTR (0.3 mg/kg) or their combination via IM injection. After 15 min, anesthesia was induced with low (1.0 mg/kg), medium (2.5 mg/kg) or high (5.0 mg/kg) doses of IM ALFX. Anesthetic depth, tracheal intubation scores, and cardiorespiratory

Ionizing radiation-induced remodelling of the heart: Cellular pathways and crosstalk relevant to stereotactic arrhythmia radioablation (STAR).

Stereotactic arrhythmia radioablation (STAR) has emerged as a promising non-invasive therapy for refractory ventricular tachycardia, yet its biological mechanisms remain poorly defined. While conventional catheter ablation achieves arrhythmia control through structural remodelling, radiotherapy was initially assumed to act similarly. However, STAR reduces ventricular arrhythmia burden within weeks, preceding the development of radiation-induced structural remodelling. This temporal mismatch indicates that early anti-arrhythmic effects are probably driven by functional adaptations. Human tissue

Catecholamine-Secreting Retroperitoneal Paraganglioma: A Case Report and Literature Review.

INTRODUCTION: Paragangliomas are rare neuroendocrine tumors arising from autonomic paraganglia. These tumors have metastatic potential, and histopathological findings alone cannot reliably predict their biological behavior. Clinical manifestations range from the classic triad of headache, diaphoresis, and palpitations to incidental findings in asymptomatic patients. CASE PRESENTATION: A 75-year-old man was incidentally found to have a 5 cm retroperitoneal mass adjacent to the right renal hilum. Biochemical testing confirmed marked catecholamine hypersecretion, and 18F-FDOPA PET/CT demonstrate

Prehospital Variables Do Not Reliably Identify Pelvic Fracture Morphologies Amenable to Pelvic Binder Application: A Retrospective Imaging Review.

BackgroundPelvic binders are used in prehospital settings for early stabilization when pelvic fractures are suspected. Although a rapid and cost-effective tool, uncertainties persist regarding their use prior to radiological confirmation of pelvic fracture.MethodsA review of adult patients with a pelvic fracture requiring trauma team activation who were admitted to a tertiary trauma center between 2020 and 2023 was conducted. The radiological studies were reviewed by four consultant orthopedic surgeons and were classified according to whether the application of a pelvic binder would be mechani

Two decades of prescription opioid-related cardiovascular safety signals: a FAERS-based pharmacovigilance study using machine learning.

INTRODUCTION: Prescription opioids are essential for pain management but have been increasingly linked to cardiovascular adverse events. Understanding these risks is critical for informed prescribing opioid drugs. METHODS: Approximately 18 million adverse event reports from 2004 Quarter 1 to 2024 Quarter 3 in the U.S. FDA Adverse Event Reporting System (FAERS) were downloaded and processed. RxNorm was used to normalize prescription opioid drug names. Empirical Bayes Geometric Mean analysis was then performed to detect signals of disproportionate reporting. Cardiac segmentation for ventricular tachycardia radioablation and radiotherapy patients with cardiac implants.

Deep-learning (DL) segmentation enables cardiac substructure (CS) delineation for advanced cardiac-sparing radiation therapy (RT). Patients presenting for RT may have implantable devices (cardioverter-defibrillators (ICDs)) and metal artifacts. Stereotactic body RT (SBRT) is used to treat ventricular tachycardia (VT), a population frequently with ICDs. We apply a DL model for RT patients with ICDs or undergoing VT-SBRT, enabling segmentation and CS-level dose analysis. CT simulation (CT-SIM) of 19 patients with ICDs and received VT-SBRT were evaluated. A validated segmentation model was retrai

Genetic landscape of paediatric channelopathies and cardiomyopathies in Hong Kong.

INTRODUCTION: Cardiac channelopathies and cardiomyopathies are important causes of sudden cardiac death. The genetic background of these conditions in Asian paediatric populations has rarely been reported. This study aimed to report the genetic underpinnings of these conditions in patients treated at a single tertiary paediatric cardiac centre in Hong Kong. METHODS: All patients diagnosed with cardiac channelopathy or cardiomyopathy who underwent genetic testing in our cardiology centre between 2007 and 2022 were included in this cohort study. Phenotype classification, genetic testing methodo

Videolaryngoscopy versus direct laryngoscopy for endotracheal intubation of obese patients: A prospective randomized trial.

BACKGROUND: Obese patients present challenging airway conditions, including reduced functional residual capacity and rapid desaturation during apnoea. Videolaryngoscopy improves glottic visualisation, but evidence comparing it with direct laryngoscopy in bariatric patients managed by experienced anaesthesiologists remains limited. We conducted a prospective randomised trial comparing these techniques. METHODS: 256 adult patients (BMI > 35 kg/m²) were randomised to intubation with a standard Macintosh laryngoscope or a hyperangulated videolaryngoscope (INSIGHT iS3-L), with a stylet routinely u

Metabolic stress exacerbates Ca2+ dysregulation and arrhythmias in catecholaminergic polymorphic ventricular tachycardia.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a genetic arrhythmic syndrome caused by mutations in the sarcoplasmic reticulum (SR) calcium (Ca2+) release channel RyR2 or its accessory proteins, including calsequestrin-2 (CASQ2). RyR2 dysfunction or leak is the hallmark feature of CPVT and directly triggers arrhythmias. Despite this electrical instability, CPVT hearts are typically anatomically normal and lack pathological remodelling. Therefore, we hypothesized that introducing additional stressors, such as metabolic stress, would worsen cardiac arrhythmias and potentially tr

First-in-human experience and one-year outcomes using a novel conformal pulsed-field ablation balloon catheter for pulmonary vein isolation in paroxysmal atrial fibrillation.

BACKGROUND: Thermal balloon catheters have improved the efficiency of pulmonary vein isolation (PVI) in atrial fibrillation (AF). Reports assessing additional safety and effectiveness of pulsed field ablation (PFA) balloon catheters for PVI are limited. This study aims to assess the safety and effectiveness of a conformal PFA balloon catheter for PVI in paroxysmal AF. METHODS: 35 patients with paroxysmal AF (PAF) were treated with a PFBalloon catheter (Biphasic, Bipolar, 750 V). The balloon was inflated with 10:1 saline/contrast, with volume adjusted for spherical wide-antral or pear-shaped o
