The Real-World Safety Profile of Haloperidol: A Comprehensive Pharmacovigilance Analysis Using the Large-Scale FAERS Database.

OBJECTIVE: Haloperidol is a widely prescribed antipsychotic, but its long-term safety profile across broad populations remains insufficiently researched. Using the large-scale FDA Adverse Event Reporting System (FAERS) database, this study sought to comprehensively characterize the real-world adverse event profile of haloperidol and identify potential safety signals. METHODS: Adverse event reports from FAERS spanning Q1 2004 through Q1 2026, with haloperidol listed as the primary suspect drug, were analyzed. Disproportionality analyses employing four complementary algorithms (ROR, PRR, BCPNN,

Exogenous testosterone use in cisgender heterosexual females with postural orthostatic tachycardia syndrome: A dual case report.

Exogenous testosterone has been previously reported to have given clinical improvement in symptoms of postural orthostatic tachycardia syndrome (POTS) in reported cases of transgender patients during their transition from female to male. Herein is presented two case reports of cisgender heterosexual female patients with POTS who have used testosterone for the management of concurrent gynecologic problems, but who also found that symptoms of both POTS and joint hypermobility improved after its administration.

Heart Rate-Lowering Therapies for Tachycardia in Postural Orthostatic Tachycardia Syndrome: A Systematic Review.

BACKGROUND: Postural orthostatic tachycardia syndrome (POTS) is defined by a sustained increase in heart rate of ≥30 beats/min within 10 minutes of standing, accompanied by symptoms of orthostatic intolerance persisting for at least 3 months. Prior research has focused on determinants of tachycardia, raising the question of whether the tachycardia represents a compensatory mechanism or the primary driver of symptoms. In this review, we summarize the available literature on heart rate-lowering interventions in POTS and examine whether reducing heart rate is associated with symptom improvement.

Congenital Long QT Syndrome Type 2: Delayed Diagnosis After a Decade of Seizure-Like Episodes.

BACKGROUND: Congenital long QT syndrome (LQTS) is typically diagnosed in youth. Recognition in advanced age is rare and may be delayed when arrhythmic syncope mimics epilepsy. CASE SUMMARY: A 93-year-old man with recurrent convulsive syncope presented with marked QT prolongation, notched T waves, and pause-dependent torsades de pointes/pulseless ventricular tachycardia initiated by premature ventricular contractions. Four years earlier, torsades de pointes had been attributed to mild hypokalemia and famotidine, although QTc interval remained prolonged after correction. Genetic testing identif

Hypermobility spectrum disorder/hypermobile Ehlers-Danlos syndrome, mast cell activation syndrome and dysautonomia as risk factors for adverse events and complications.

INTRODUCTION: Hypermobility spectrum disorder/hypermobile Ehlers-Danlos syndrome (HSD/hEDS), mast cell activation syndrome (MCAS) and dysautonomia, including postural orthostatic tachycardia syndrome (POTS), are comorbid conditions encountered in clinical practice with possible overlapping immunologic, autonomic and connective tissue abnormalities. Clinicians frequently observe that affected individuals affected with these syndromes commonly experience disproportionate adverse events and complications from a variety of stressors. AREAS COVERED: We review clinical observations and emerging evi

Chronic Non-Communicable Disease Burden among Recently Arrived Refugees in Syracuse, New York.

Background: Refugees resettling in the United States often come from regions with limited healthcare infrastructure. While communicable diseases are routinely screened for, non-communicable diseases (NCDs), including hypertension and diabetes mellitus, remain underrecognized. Lifetime adverse events contributing to chronic stress further increase cardiovascular disease risk in refugees. Objective: To evaluate the chronic disease burden of blood pressure measurements in the hypertensive range and point-of-care capillary glucose values in the prediabetic and diabetic ranges among refugees arrivi

Automated Two-component Pre-CTPA ECG Risk Phenotype and 365-day Mortality among ECG-evaluated Patients with Computed Tomography-confirmed Pulmonary Embolism.

OBJECTIVE: Electrocardiographic abnormalities are common in acute pulmonary embolism, but their prognostic value in public waveform-linked cohorts remains incompletely characterized. We evaluated whether an interpretable automated two-component pre-CTPA ECG risk phenotype was associated with 365-day mortality among ECG-evaluated patients with CTPA-confirmed pulmonary embolism. METHOD: This secondary analysis used a retrospective cohort derived from MIMIC-IV-Ext-PE linked to MIMIC-IV-ECG. The source population included 1,534 unique PE-positive patients after patient-level indexing. A total of

The Vein of Marshall Promotes Re-Entry After Pulmonary Vein Reconnection.

BACKGROUND: Postablation re-entrant atrial tachycardia recurrence remains a major limitation of atrial fibrillation (AF) ablation. Pulmonary vein (PV) reconnection and epicardial connections from the left atrium (LA) to the vein of Marshall (VoM) and coronary sinus (CS) in proximity to mitral valve are important contributors. We hypothesized that VoM facilitates arrhythmia induction through interaction with the epicardial region encircled by wide antral circumferential ablation (WACA). OBJECTIVES: The objective of this study was to systematically evaluate the combined effect of PV reconnectio

Managing Multiorgan Failure and Infection Before Durable LVAD in Fulminant Myocarditis.

BACKGROUND: Fulminant myocarditis (FM) carries high rates of mortality, heart transplantation, or durable ventricular assist device (dVAD) implantation. Deciding when to transition to dVAD is challenging, particularly when complicated by multiorgan failure or infection. CASE SUMMARY: A 45-year-old man with FM was transitioned from a failing extracorporeal membrane oxygenation-Impella (combined use of venoarterial extracorporeal membrane oxygenation and Impella CP) to a surgical temporary biventricular assist device. Despite dialysis-dependent renal failure and Candida fungemia-which subsequen

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

A Case of Suspected Scombroid Poisoning in Intensive Care Staff: Is It Anaphylaxis?

Scombroid poisoning (histamine fish poisoning) is an uncommon foodborne illness that frequently mimics immunoglobulin E-mediated anaphylaxis. We report a case of a previously well 28-year-old intensive care nurse who developed abrupt facial flushing, wheeze, dizziness, and hypotension after consuming canned tuna. Initial observations demonstrated sinus tachycardia and hypotension. She was treated according to established anaphylaxis guidelines with two doses of intramuscular adrenaline, intravenous crystalloid resuscitation, and subsequent adrenaline infusion in intensive care. She recovered r

Identifying the electrically unstable NSTEMI patient: Comparative performance of four scores for short-term risk stratification of major cardiac arrhythmias.

BACKGROUND: Major cardiac arrhythmias after NSTEMI are associated with increased mortality and adverse clinical outcomes. However, evidence comparing the ability of established acute coronary syndrome risk scores to predict in-hospital arrhythmic events remains limited. METHODS: We conducted a retrospective cohort study of consecutive NSTEMI patients who underwent coronary angiography at Taipei Medical University Hospital between January 2017 and August 2022. Major cardiac arrhythmias were defined as ventricular tachycardia, ventricular fibrillation, new-onset atrial fibrillation, and high-de

High-Frequency Electrograms Abolition as a Valuable Target for Remap During Ventricular Tachycardia Ablation. The HF REMAP Study.

BACKGROUND: peak frequency (PF) analysis has recently emerged as a novel tool to assess electrogram properties through their frequency spectrum. PF is an indirect marker of local myocardial activity, and its reduction after radiofrequency ablation reflects effective tissue modification. OBJECTIVE: our study aims to evaluate PF analysis as an objective method to validate lesion efficacy during ventricular tachycardia (VT) ablation. METHODS: we retrospectively analyzed 43 (out of 247) consecutive patients who underwent VT ablation between January 2023 and December 2024, using the EnSite X mapp

Symptomatic meningioma in the third trimester of pregnancy: a case report from Kenya.

Meningiomas, the most common primary central nervous system (CNS) tumors, are typically benign and hormonally responsive. They have a higher incidence in women, particularly during the reproductive years. Pregnancy may trigger rapid tumor progression and symptom onset due to elevated hormone levels. Although rare in pregnancy, meningiomas may adversely affect maternal and fetal outcomes, increasing the risk of complications such as preterm labor and gestational hypertension. Management strategies range from conservative monitoring to surgical intervention depending on gestational age, tumor si

Successful Guidewire-Guided Ethanol Ablation for PVCs Originating From Epicardial Anterior Interventricular Groove Adjacent to LAD.

BACKGROUND: Premature ventricular complexes (PVCs) originating from epicardial anterior interventricular groove and closely adjacent to the left anterior descending artery (LAD) are difficult to eliminate and carry high risks of coronary damage or occlusion. We describe a successful guidewire-assisted ethanol ablation in this high -risk patient. CASE SUMMARY: A 30-year-old man who presented with palpitations and high-burden PVCs was admitted to our hospital. Pace mapping performed at the epicardial anterior interventricular groove yielded QRS morphology nearly identical to the native PVCs. Co

Large-scale plasma metabolic profiling identifies signatures and biomarkers associated with air pollution and arrhythmias: A prospective cohort study from UK Biobank.

BACKGROUND: Air pollution is an established modifiable risk factor for cardiac arrhythmia, yet the metabolic mechanisms underlying this association remain poorly understood. This study aimed to elucidate how air pollutants promote arrhythmias through metabolic pathway perturbations. METHODS: We conducted a prospective analysis of 449,068 UK Biobank participants without baseline arrhythmia. Exposures to PM2.5, PM10, NO2, and NOx were assessed using land use regression models. Plasma metabolomic profiling including 170 metabolites via 1H-NMR was performed at baseline. Elastic net regression, Co

Recurrent Supraventricular Tachycardia Triggered by Tea Consumption in a Postoperative Adolescent Female: A Case Report.

Supraventricular tachycardia (SVT) in adolescents may be triggered by physiological stress or caffeine. We report a 16-year-old girl who complained of palpitations, dyspnea, and regular tachycardia (170 beats/min) after tea intake. ECG showed SVT and intravenous adenosine restored sinus rhythm. Recognizing dietary stimulants as potential precipitants of SVT is important.
