Pilsicainide Intoxication Presenting With Alternating Accelerated Idioventricular and Junctional Escape Rhythms During Acute Kidney Injury: A Case Report.

Pilsicainide is a class Ic sodium channel blocker that is eliminated predominantly by the kidneys and may accumulate when renal function deteriorates. An 85-year-old man receiving pilsicainide 150 mg/day developed acute kidney injury (AKI) during hospitalization for hemorrhagic colitis. He subsequently developed impaired consciousness, dyspnea, pulmonary congestion, and alternating episodes of junctional escape rhythm at 30-37 beats/min and a regular, wide QRS rhythm at 79 beats/min. The QRS duration was approximately 150 ms, and the serum pilsicainide concentration was 3.19 µg/mL. The wide QR

Excessive supraventricular ectopic activity and new-onset ischemic stroke: a retrospective cohort study with prospective outcome ascertainment.

BACKGROUND: Excessive supraventricular ectopic activity (ESVEA), a marker of both paroxysmal atrial fibrillation (pAF) and underlying atrial cardiomyopathy, may serve as an earlier electrographic predictor of cryptogenic ischemic stroke than pAF itself. This study aimed to investigate the association between ESVEA and the risks of new-onset ischemic stroke and all-cause mortality in an Asian population, and to explore a potential data-driven threshold that requires external validation. METHODS: In this retrospective cohort study with prospective outcome ascertainment, data were collected from

Intraoperative Dobutamine Plus Norepinephrine to Prevent Postoperative Acute Kidney Injury in Septic Shock: A Randomized Clinical Trial.

IMPORTANCE: International guidelines recommend dobutamine for management of septic shock and cardiac dysfunction with persistent hypoperfusion in adults based on very low-certainty evidence. Randomized trials are lacking on the effect of intraoperative dobutamine combined with norepinephrine vs norepinephrine on postoperative outcomes in patients with septic shock undergoing surgical source control. OBJECTIVE: To determine whether intraoperative dobutamine combined with norepinephrine reduces acute kidney injury (AKI) within 7 postoperative days in patients with septic shock undergoing surgic

Paradoxical bradycardia from occult venous hemorrhage mimicking vasovagal syncope - A case report.

BACKGROUND: Hemorrhagic shock typically presents with tachycardia; however, profound hypovolemia can paradoxically induce bradycardia via the Bezold-Jarisch reflex. This can lead to misdiagnosis of vasovagal syncope, causing dangerous treatment delays. CASE: A 50-year-old woman developed profound bradycardia and hypotension after vomiting in the post-anesthesia care unit after undergoing hysterectomy. Despite initial management of vasovagal syncope, her condition remained refractory to vasopressors. A critically low hemoglobin level of 5.5 g/dL strongly suggested massive occult hemorrhage. Sh

Cholinergic urticaria following active forced-air warming in the post-anesthesia care unit: a case report and literature review.

INTRODUCTION: Cholinergic urticaria (CU), a subtype of chronic inducible urticaria, is triggered by elevations in core body temperature. Postoperative shivering and active warming measures may increase core body temperature in the post-anesthesia care unit (PACU), yet CU in this setting remains underrecognized and has rarely been reported. CASE PRESENTATION: We report a case of an 18-year-old male who developed acute CU in the PACU following cochlear implantation under general anesthesia. After extubation, the patient exhibited postoperative shivering and was treated with a forced-air warming

Conduction Gap of Left Atrial Anterior Wall Linear Ablation Undetected by Left Atrial Appendage Pacing but Revealed by Left Lateral Ridge Pacing.

Confirmation of counterclockwise conduction block across the left atrial anterior wall linear lesion is usually performed during left atrial appendage pacing. However, even when endocardial conduction block across the left atrial anterior wall linear lesion is suggested during left atrial appendage pacing, an epicardial connection between the left lateral ridge and the left atrial anterior wall can be a cause of residual conduction gap. In such a situation, assessment during pacing with the left lateral ridge capture should be considered.

LAA occluder device impeding pulsed field ablation of an anterior mitral line: a case report about the role of a dual energy source.

BACKGROUND: Pulsed field ablation (PFA) and percutaneous closure of the left atrial appendage are increasingly used in patients with atrial fibrillation. An interaction between this form of energy and metallic structures in the heart is an emerging problem. We first report a left atrial appendage closure (LAAC)-limiting PFA using a novel dual energy ablation catheter, solved by switching to the classic radiofrequency ablation. CASE SUMMARY: An 85-year-old woman with heart failure with preserved ejection fraction and a history of persistent atrial fibrillation (status post pulmonary vein isola

Impact of 18FDG-Avidity and Immunosuppression on Idiopathic and Genetic Cardiomyopathies.

BACKGROUND: Myocardial 18fluorodeoxyglucose (18FDG)-avidity is frequently seen in patients with genetic cardiomyopathy, as well as a growing "idiopathic 18FDG-avidity" group of genotype-negative patients who do not clearly have cardiac sarcoidosis. The prognostic implications of 18FDG-avidity in patients with and without genetic cardiomyopathy, and the effects of immunosuppression in the latter, remain uncertain. METHODS: This multicenter, retrospective study included all patients who were referred for both 18FDG-PET and cardiomyopathy genetic testing. Patients with acute myocarditis, biopsy-

Refractory Ventricular Tachycardia-Induced Severe Cardiomyopathy in a Child: Successful Radiofrequency Ablation With ECMO Support.

BACKGROUND: Ventricular tachycardia (VT)-induced cardiac dysfunction is a recognized but uncommon cause of cardiomyopathy in children. CASE SUMMARY: A 7-year-old girl presented with severely depressed left ventricular function and incessant VT that was refractory to multiple antiarrhythmics and attempted cardioversions. Because of hemodynamic instability, the patient required extracorporeal membrane oxygenation (ECMO) support and ultimately radiofrequency ablation of a focal VT that was localized to the right midventricular septum near the moderator band. The patient had a subsequent return o

Identifying the High-Risk Arrhythmic Mitral Valve Prolapse Phenotype by Imaging.

Within arrhythmogenic mitral valve prolapse (AMVP), a distinct subset of mitral valve prolapse (MVP), there is a small group that carries a higher risk of malignant ventricular arrhythmias and sudden cardiac death called malignant MVP. The malignant MVP phenotype is over-represented in young women with unexplained sudden cardiac death on autopsy. AMVP is defined by myxomatous, often bileaflet MVP with mitral annular disjunction, abnormal valvular-myocardial mechanics, complex ventricular ectopy, and nonsustained ventricular tachycardia. Excessive mechanical force from systolic annular expansio

From call to home: The role of dispatcher-assisted CPR in patient survival.

BACKGROUND: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) is a key strategy to reduce no-flow time in out-of-hospital cardiac arrest (OHCA), yet its impact on long-term survival remains various across emergency medical systems. PURPOSE: We aimed to evaluate the association between DA-CPR and resuscitation outcomes in a nationwide cohort of OHCA patients presenting with an initial shockable rhythm. METHODS: We conducted a retrospective observational study including all adult OHCA cases with ventricular fibrillation or pulseless ventricular tachycardia (VF/VT) in which resuscitati

Going around phase defects: Addressing a fundamental flaw of phase singularity detection.

Phase mapping is a widespread method for identifying rotational electrical activity sustaining cardiac arrhythmias. However, conventional implementations assume that the cardiac phase map is continuous, leading to ill-defined phase indices in regions affected by functional conduction block, fibrosis, or anatomical boundaries. These regions of discontinuous or undefined phase, termed phase defects, lead to both false positive and false negative detections of rotational drivers. This work introduces an improved phase mapping implementation termed extended phase mapping that explicitly detects an

Electrophysiologic Phenotype Outperforms Left Ventricular Ejection Fraction for Mortality Prediction in Immune Checkpoint Inhibitor Myocarditis.

BACKGROUND: Immune checkpoint inhibitor (ICI)-associated myocarditis carries mortality rates approaching 50%. Risk stratification relies heavily on left ventricular ejection fraction (LVEF), yet many fatal cases occur despite preserved systolic function. OBJECTIVES: The study aims to compare the prognostic value of electrophysiologic involvement vs LVEF in ICI-associated myocarditis. METHODS: We systematically searched PubMed for ICI-associated myocarditis cases with extractable patient-level data. Electrophysiologic phenotype (EP) was defined by clinically significant conduction disease or

Elevated lithium levels in drinking water are associated with higher prevalence of Wolff-Parkinson-White diagnoses.

BACKGROUND: Supraventricular tachycardia (SVT) and Wolff-Parkinson-White (WPW) are potentially highly symptomatic and, rarely, lethal arrhythmias associated with atrioventricular accessory pathways. While the etiology of accessory pathways remains largely unknown, they are frequently observed with Ebstein's Anomaly, a congenital heart disease associated with maternal medicinal lithium use. Therefore, we evaluated whether in utero exposure to lithium from environmental sources is associated with the prevalence of accessory pathways. METHODS: We conducted a county-level ecological comparison of
