The link between inflammation and autonomic dysfunction in alpha-synucleinopathies - narrative review.

INTRODUCTION: Alpha-synucleinopathies are a group of disorders characterized by the accumulation of the pathological form of alpha-synuclein. This group comprises Parkinson's disease, Parkinson's disease dementia, multiple system atrophy, dementia with Lewy bodies, pure autonomic failure, and idiopathic REM sleep behavior disorder. Despite sharing a similar molecular basis, these disorders differ in their clinical presentation. Characteristic manifestations include signs of autonomic failure such as orthostatic hypotension, postural orthostatic tachycardia syndrome, constipation, gastroparesis

Multimodality Imaging-Guided Ventricular Tachycardia Ablation in a Hypoplastic Right Ventricle After Glenn Palliation.

Adults with single-ventricle physiology increasingly survive into adulthood and may develop complex ventricular arrhythmias. A 44-year-old man with severe right ventricular hypoplasia and previous bidirectional Glenn palliation presented with sustained monomorphic ventricular tachycardia. Transthoracic and intracardiac echocardiography, computed tomography, and high-density electroanatomic mapping were combined to reconstruct the right-sided anatomy and to characterize the arrhythmogenic substrate. Intracardiac echocardiographic contours were merged with the electroanatomic map to build the ge

Spiked Helmet Sign Associated With Acute Gastric Distension and Hemodynamic Instability: A Case Report.

The spiked helmet sign is an uncommon pseudo-ST-segment elevation pattern reported in critical illness and recognized by an upward baseline shift beginning before the QRS complex and a dome-and-spike contour. Its distinction from acute coronary occlusion is clinically important but can be difficult when myocardial injury, left ventricular hypertrophy, electrolyte abnormalities, or post-resuscitation changes coexist. We report a 34-year-old male with end-stage kidney disease on hemodialysis, hypertension, hypertrophic cardiomyopathy, and thrombotic microangiopathy who developed aspiration-relat

Targeting Brain-Body Processes in Postural Tachycardia Syndrome: A Pilot Trial of POts Reprocessing Therapy.

Purpose Postural tachycardia syndrome (POTS) is a chronic autonomic disorder characterized by orthostatic intolerance and frequently accompanied by a broad constellation of debilitating symptoms. Emerging evidence suggests that dysregulation within central-autonomic regulatory networks may contribute to symptom persistence. We developed POts Reprocessing Therapy (PORT), a neuroplasticity-informed behavioral intervention adapted from Pain Reprocessing Therapy. This pilot study evaluated PORT's feasibility, acceptability, and preliminary clinical outcomes. Methods We conducted a non-randomized,

Traumatic High Cervical Spine Injury Presenting as Cardiac Arrest: A Case Report.

Traumatic high cervical spinal cord injury is a potentially devastating condition that may cause profound cardiovascular and respiratory instability through disruption of descending sympathetic pathways. In severe cases, autonomic dysfunction may contribute to marked hypotension, bradycardia, and cardiac arrest. We report the case of a 47-year-old man who sustained blunt trauma of incompletely characterized mechanism in a workplace accident and was found in cardiac arrest by a prehospital emergency medical team. Advanced life support was initiated, and return of spontaneous circulation was ach

Pulsed field ablation with Renal denervation for persistent Atrial fibrIllation: Rationale and design of the PRAISE-AF randomized trial.

BACKGROUND: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia worldwide and is associated with increased risks of stroke, heart failure, hospitalization, disability, and mortality. Catheter ablation has become an established rhythm-control strategy for symptomatic AF; however, the recurrence rate after ablation remains substantial, especially in patients with persistent AF. Pulsed field ablation (PFA) is an emerging nonthermal ablation technology with myocardial tissue selectivity and a potentially improved safety profile compared with conventional thermal ablation. Right Atrium-Dominant Single-Loop Biatrial Tachycardia Mimicking Reverse Typical Atrial Flutter After Mitral Valve Repair via a Superior Transseptal Approach.

Following a superior transseptal approach, extensive postoperative septal conduction impairment facilitated a right atrium-dominant single-loop biatrial tachycardia mimicking reverse typical atrial flutter. Detailed biatrial mapping and entrainment revealed limited left atrial participation, highlighting the importance of recognizing biatrial circuits in patients with prior septal surgery.

Persistent Supraventricular Tachycardia Following Central Venous Catheter Insertion: When Withdrawal of the Mechanical Stimulus Is Not Enough.

Cardiac arrhythmias are a recognised complication of central venous catheterisation and are usually attributed to mechanical irritation of the endocardium by the guidewire or catheter tip. These arrhythmias typically resolve following withdrawal of the mechanical stimulus. We report a 42-year-old man with heart failure with reduced ejection fraction and diabetic ketoacidosis who developed an abrupt regular narrow-complex tachycardia at approximately 217 beats/min immediately following advancement of a right internal jugular central venous catheter. The arrhythmia was associated with transient

Inhibition of NOX2 Prevents Impairment of Cardiovascular Response and Inflammatory Tissue Injury Mediated by Nitrosative/ Oxidative Stress in the Lipopolysaccharide-Induced Septic Shock Model in Rats.

INTRODUCTION: It is unknown whether inhibition of nicotinamide adenine dinucleotide phosphate oxidase (NOX) 2 has protective effects against the impaired cardiovascular response and inflammatory tissue injury mediated by nitrosative/ oxidative stress in bacterial septic shock. Therefore, using the NOX2 inhibitor NOX2ds-stat, the effects of NOX2 inhibition on cardiovascular response were explored in a rat septic shock model induced by lipopolysaccharide (LPS), in relation to cardiovascular/renal inflammatory tissue injury. METHODS: Mean arterial pressure and heart rate were obtained non-invasi

Delayed Postoperative Malignant Hyperthermia After Total Intravenous Anesthesia: A Case Report.

Malignant hyperthermia (MH) is a rare pharmacogenetic disorder typically triggered by volatile anesthetics or succinylcholine, but its occurrence after total intravenous anesthesia (TIVA) remains exceptional and underreported. Propofol-based TIVA is recommended for MH-susceptible patients to avoid known triggers, yet absolute prevention cannot be guaranteed. A 24-year-old male with prior suspected MH underwent spine surgery under propofol-remifentanil TIVA after strict preventive preparation. Despite stable intraoperative parameters, he developed fever, tachycardia, rigidity, and myoglobinuria

Standardized Spark-Pattern Cut-Mode Electrosurgical Bailout Workflow for Difficult Left Bundle Branch Area Pacing Lead Advancement: Procedural and Follow-Up Outcomes From a 15-Patient Case Series.

BACKGROUND: Electrosurgical facilitation can aid difficult left bundle branch area pacing lead advancement, but standardized delivery patterns, reassessment logic, and follow-up lead performance remain insufficiently defined. METHODS: We retrospectively analyzed 15 consecutive patients undergoing standardized spark-pattern cut-mode electrosurgical bailout after conventional mechanical left bundle branch area pacing lead advancement failed. The generator was operated in unipolar cut-mode at 20 W; spark-pattern delivery denoted brief sliding contact to the exposed cathode segment followed by im

Multicenter Coronary Venous Carbon Dioxide Insufflation for Subxiphoid Epicardial Access in Ventricular Tachycardia Ablation.

Percutaneous subxiphoid epicardial access for ventricular tachycardia ablation carries a substantial risk of catastrophic complications. Intentional coronary venous carbon dioxide insufflation (Epi-CO2) creates a radiolucent pneumopericardium that provides a broad needle target, but evidence is dominated by single-center reports. We retrospectively analyzed 53 consecutive patients undergoing epicardial access for ventricular tachycardia ablation at 3 Japanese tertiary centers (Epi-CO2, n = 28; conventional access, n = 25). Major access-related complications occurred in 0 of 28 Epi-CO2 patients

Serial Artificial Intelligence ECG-Derived Atrial Fibrillation Probability and Recurrence Risk After Pulsed Field Ablation.

BACKGROUND: Pulsed field ablation is now widely used for atrial fibrillation (AF) ablation, but atrial tachyarrhythmia recurrence remains common and postablation risk tools are limited. We assessed whether a validated sinus‑rhythm artificial intelligence- enabled ECG AF probability score, and its early postablation change, predicts recurrence after pulsed field ablation. METHODS: We studied consecutive patients undergoing index pulsed field ablation across 3 Mayo Clinic sites (February 2024-March 2025) with ≥1 sinus‑rhythm ECG within 180 days preprocedure. Baseline artificial intelligence-ena

Value of Left Ventricular Low Voltage Area and Ejection Fraction in Predicting End-Stage Heart Failure Outcomes After Ventricular Tachycardia Ablation in Patients With Non-Ischemic Cardiomyopathy.

BACKGROUND: Patients with nonischemic cardiomyopathy (NICM) and ventricular tachycardia are at increased risk for adverse HF outcomes, and abnormal left ventricular (LV) substrate identified by endocardial unipolar and bipolar low-voltage areas (LVAs) may aid in risk stratification. This study assessed the association of LV unipolar and bipolar LVAs with end-stage HF outcomes in patients with NICM undergoing ventricular tachycardia ablation. METHODS: Patients with NICM and LV ejection fraction (EF) <50% undergoing ventricular tachycardia ablation from 2019 to 2023 were included. Endocardial v

Explainable deep learning-based classification of Wolff-Parkinson-White electrocardiographic signals.

INTRODUCTION: Wolff-Parkinson-White (WPW) syndrome is a cardiac electrophysiology (EP) disorder caused by the presence of an accessory pathway (AP) that bypasses the atrioventricular node, faster ventricular activation rate, and provides a substrate for atrio-ventricular reentrant tachycardia (AVRT). Accurate localization of the AP is critical for planning and guiding catheter ablation procedures. While traditional diagnostic tree (DT) methods and more recent machine learning (ML) approaches have been proposed to predict AP location from surface electrocardiogram (ECG), they are often constrai
