Biatrial cardioneuroablation guided by robotic magnetic navigation and artificial intelligence-based mapping in vagal bradyarrhythmias: A controlled observational study.

PURPOSE: Reflex bradyarrhythmias and syncope related to excessive vagal tone may be refractory to conservative therapy and significantly impair quality of life. Cardioneuroablation (CNA) has emerged as a device-sparing alternative, but real-world outcome data remain limited. METHODS: We retrospectively evaluated 12 consecutive patients (aged 23-55 years) with drug-refractory, vagally mediated bradyarrhythmias confirmed by tilt-table testing. All underwent biatrial CNA guided by ultra-high-density electroanatomical mapping with automated fragmented-electrogram detection and robotic magnetic na

Biological age acceleration and bradyarrhythmia: evidence from clinical and epigenetic perspectives.

BACKGROUND: To date, effective preventive and therapeutic strategies for early-stage bradyarrhythmia remain limited. METHODS: We first calculated two well-established biological aging (BA) measures (Klemera-Doubal Method [KDM] and Phenotypic Age [PhenoAge]), with BA acceleration defined as the residual from regressing each BA measure on chronological age. Cox proportional hazards models were used to evaluate the effects of BA acceleration on bradyarrhythmia risk. We then conducted epigenetic Mendelian randomization (MR) and colocalization analyses to prioritize aging-related DNA methylation C

Oculocardiac Reflex during Electrode Placement in Trigeminal Schwannoma Surgery via Cavernous Sinus Approach.

The oculocardiac reflex (OCR) is well known in ophthalmic surgery, but its occurrence during neurosurgical procedures is rare and often unexpected. Mechanical stimulation of trigeminal pathways at the skull base can cause significant hemodynamic changes; however reflex activation during preparatory steps rather than tumor manipulation is rarely described. We report an intraoperative event in a 22-year-old male undergoing resection of a cystic trigeminal schwannoma via a subtemporal transcavernous approach under general anesthesia, during which periorbital needle electrodes were placed for cran

Percussion Pacing as a Rescue for Peri-Stimulus Asystole During Electroconvulsive Therapy.

Electroconvulsive therapy (ECT) can provoke a brief parasympathetic surge, commonly causing transient sinus bradycardia or a short sinus pause. Most events last only seconds and resolve without intervention. Rarely, however, bradyarrhythmia or asystole may persist long enough to threaten cerebral perfusion, requiring anesthesia teams to respond immediately. We report the case of a male patient with treatment-resistant major depressive disorder who underwent bilateral ECT under methohexital and succinylcholine. Baseline sinus bradycardia was present at 47 beats per minute. Immediately after the

VSD Device Removal Due to Intermittent Complete Heart Block.

BACKGROUND: Percutaneous closure of perimembranous ventricular septal defects (pmVSDs) has markedly increased with the development of newer, softer occlusion devices. CASE SUMMARY: We report a 12-year-old girl who underwent percutaneous pmVSD closure. One week later, she was admitted for paroxysmal complete heart block (CHB). She was started on intravenous corticosteroids. A percutaneous retrieval of the device was performed successfully. Sinus rhythm was maintained throughout the 8-month follow-up period. DISCUSSION: Paroxysmal CHB is a rare but serious complication following percutaneous p

Opioid-free versus opioid-based anesthesia for robot-assisted radical prostatectomy: a single-center randomized controlled trial.

Multimodal, opioid-sparing analgesia is standard in urological surgery, and there is additional interest in eliminating intraoperative opioids in prostate cancer surgery. Randomized evidence for opioid-free anesthesia (OFA) in robot-assisted radical prostatectomy (RARP) is lacking. In this single-center randomized controlled trial, adult men undergoing RARP were allocated 1:1 to OFA (dexmedetomidine plus ketamine, no intraoperative opioids) or a remifentanil-based technique. Both groups received identical multimodal premedication, a bilateral quadratus lumborum block, sevoflurane maintenance a

Environmentally relevant concentrations of a hexazinone-based herbicide impair zebrafish embryo-larval development by altering multiple biomarkers.

Hexazinone (HZN)-based herbicides are widely used in sugarcane crops and may pose risks to non-target organisms. This study evaluated the toxicity of environmentally relevant concentrations (0.05, 0.5, 5µgL-1) of the HZN-based herbicide Nortox® and two higher concentrations across zebrafish development, assessing lethal and non-lethal endpoints, neurotoxicity, cardiotoxicity, redox imbalance, and target gene expression. All HZN treatments increased mortality and reduced spontaneous contractions, whereas only the highest concentrations affected hatching and induced bradycardia. CAT activity inc

The effects of low-dose medetomidine and butorphanol premedication on anesthetic induction and recovery for intramuscular alfaxalone anesthesia in cats.

This study evaluated effects of premedication with low-dose medetomidine (MED), butorphanol (BTR), or their combination on intramuscular (IM) alfaxalone (ALFX) anesthesia in cats. Five healthy purpose-bred cats were used in a completely randomized crossover study with a minimum 1-week washout period. Cats received three types of premedication: MED (5 µg/kg), BTR (0.3 mg/kg) or their combination via IM injection. After 15 min, anesthesia was induced with low (1.0 mg/kg), medium (2.5 mg/kg) or high (5.0 mg/kg) doses of IM ALFX. Anesthetic depth, tracheal intubation scores, and cardiorespiratory

Videolaryngoscopy versus direct laryngoscopy for endotracheal intubation of obese patients: A prospective randomized trial.

BACKGROUND: Obese patients present challenging airway conditions, including reduced functional residual capacity and rapid desaturation during apnoea. Videolaryngoscopy improves glottic visualisation, but evidence comparing it with direct laryngoscopy in bariatric patients managed by experienced anaesthesiologists remains limited. We conducted a prospective randomised trial comparing these techniques. METHODS: 256 adult patients (BMI > 35 kg/m²) were randomised to intubation with a standard Macintosh laryngoscope or a hyperangulated videolaryngoscope (INSIGHT iS3-L), with a stylet routinely u

Anaphylaxis Following Intravenous Methylprednisolone Sodium Succinate: A Case Report.

BACKGROUND: Immediate hypersensitivity to systemic corticosteroids is rare but potentially life-threatening, and it may be overlooked when the cardiovascular presentation is atypical. Identifying the responsible determinant, whether the corticosteroid molecule, a solubilizing ester, or a formulation excipient, is often difficult. CASE PRESENTATION: A 45-year-old woman with rheumatoid arthritis developed a generalized pruritic, erythematous, raised eruption, dyspnea, hypoxemia, bradycardia, and reduced consciousness within 5 minutes of starting intravenous methylprednisolone sodium succinate 2

Remimazolam ED95 for Preoperative Sedation in Children with Obstructive Sleep Apnea, with Obesity or a Reference-Range BMI: A Biased-Coin Dose-Finding Study.

BACKGROUND: Obesity may alter total-body-weight dosing of remimazolam, whereas obstructive sleep apnea narrows the respiratory safety margin. We estimated the ED95 for protocol-defined preoperative sedation in children with obesity or a reference-range body mass index (BMI). METHODS: In this prospective, single-center sequential dose-finding study, 80 children aged 3-17 years with a clinical OSA diagnosis undergoing adenoidectomy and/or tonsillectomy entered study-defined obesity (BMI-for-age >95th percentile) or reference-BMI (25th-84th percentile) groups (40 each). Holt-Oram syndrome presenting with failure to thrive due to a hemodynamically significant patent ductus arteriosus: the importance of early cardiac evaluation in infants with radial ray abnormalities.

BACKGROUND: Holt-Oram syndrome (HOS) is a rare autosomal dominant disorder caused by pathogenic variants in the TBX5 gene and characterized by radial ray malformations associated with congenital heart defects and/or cardiac conduction abnormalities. Although atrial and ventricular septal defects are the most common cardiac manifestations, other congenital cardiac lesions may occur. Early recognition of cardiac involvement is essential to prevent complications and improve outcomes. CASE PRESENTATION: We report the case of an 8-month-old female infant presenting with failure to thrive, feeding

Traumatic High Cervical Spine Injury Presenting as Cardiac Arrest: A Case Report.

Traumatic high cervical spinal cord injury is a potentially devastating condition that may cause profound cardiovascular and respiratory instability through disruption of descending sympathetic pathways. In severe cases, autonomic dysfunction may contribute to marked hypotension, bradycardia, and cardiac arrest. We report the case of a 47-year-old man who sustained blunt trauma of incompletely characterized mechanism in a workplace accident and was found in cardiac arrest by a prehospital emergency medical team. Advanced life support was initiated, and return of spontaneous circulation was ach

A scoping review of adverse events associated with concomitant beta-adrenoreceptor blockers and selective serotonin reuptake inhibitors.

Beta-adrenoreceptor blockers (beta-blockers) and selective serotonin reuptake inhibitors (SSRIs) are commonly used medications and often used together, such as for the treatment of anxiety. However, there is no recent review of any potential adverse events related to concomitant use of medicines from each group. This scoping review therefore aimed to provide this narrative synthesis. Key databases were searched for articles published in English from 1 January 1990 to 24 November 2025 reporting any adverse event due to the concomitant use of beta-blockers and SSRIs in people aged 18 or over. Opioid use and naloxone-identified suspected opioid-related adverse events in a tertiary pediatric hospital: a retrospective study.

OBJECTIVE: Opioids are essential for analgesia and sedation in pediatric inpatient care, particularly in perioperative and intensive care settings. However, their use requires careful monitoring because children, especially infants and critically ill patients, may be vulnerable to respiratory, cardiovascular, and neurological adverse effects. METHODS: This retrospective observational study evaluated opioid administration records and naloxone-identified suspected opioid-related adverse-event episodes at a tertiary pediatric hospital in Brazil from January 2019 to September 2024. Opioid adminis

Research progress on the use of beta-blockers in the progression prevention, and treatment of retinopathy of prematurity.

Retinopathy of prematurity (ROP) is a proliferative retinal vascular disorder unique to preterm infants with extremely low birth weight, ranking as the top cause of preventable childhood blindness. Retinal hypoxia, excessive secretion of vascular endothelial growth factor (VEGF) and dysregulation of insulin-like growth factor-1 (IGF-1) constitute the core pathogenic cascades of ROP. As competitive antagonists of β-adrenergic receptors (β-AR), beta-blockers suppress the HIF-1α/VEGF and PI3K/Akt/VEGF signaling axes to inhibit pathological retinal neovascularization, while exerting auxiliary anti
