Effects of different administration routes of dexmedetomidine on sleep quality in patients undergoing total knee arthroplasty.

Postoperative insomnia affects up to 42% of patients undergoing total knee arthroplasty. Dexmedetomidine has been widely used in perioperative anesthesia and previous reports suggested that dexmedetomidine may be associated with the improvement of sleep quality after surgery. We hypothesized that intranasal administration of dexmedetomidine could improve sleep quality compared to postoperative intravenous infusion. A total of 72 participants were allocated into three groups: an intranasal dexmedetomidine group (intervention group 1, 1 µg/kg), an intranasal normal saline group (control group),

Efficacy and safety of dexmedetomidine versus midazolam for sedation in mechanically ventilated pediatric patients: a randomized clinical trial.

AIMS: The benefits of Dexmedetomidne in sedation of mechanically ventilated pediatric patients are still challenging. In this study, we compared the efficacy and safety of Dexmedetomidine versus Midazolam for achieving deep sedation in pediatric patients receiving mechanical ventilation. METHODS: A randomized clinical trial was conducted across three affiliated hospitals during February 2023 until February 2024. Eligible mechanically ventilated children aged 1 month to 18 years were randomized 1:1 to receive either Dexmedetomidine (0.25-1 mcg/kg/h) or midazolam (1-4 mcg/kg/min) for sedation,

Dexmedetomidine for Adult Patients Undergoing Electroconvulsive Therapy: A Systematic Review and Meta-Analysis.

INTRODUCTION: Electroconvulsive therapy (ECT) triggers a pronounced sympathetic surge that may increase cardiovascular risk, especially in susceptible patients. Dexmedetomidine, an α2-adrenergic agonist with sympatholytic properties, has been proposed to blunt this response, but its overall clinical impact and optimal dosing remain unclear. METHODS: We conducted a systematic review and meta-analysis of double-blind randomized controlled trials evaluating dexmedetomidine administered before anesthesia induction in adult ECT. Primary outcomes were heart rate (HR) and mean arterial pressure (MAP

Retrieval of a dislodged helix-fixation leadless pacemaker using the double-snare technique: A case report.

UNLABELLED: Leadless pacemakers (LPs) reduce the risk of lead-related complications, but early dislodgement remains a rare yet serious adverse event. We report the case of an 88-year-old woman with bradycardia-tachycardia syndrome who underwent implantation of an AVEIR™ VR LP (Abbott, Chicago, IL, USA). The procedure was uneventful, with stable electrical parameters and confirmed fixation. However, pacing failure was detected the following day, and imaging revealed that the device had dislodged into the right pulmonary artery. Given the position and orientation of the dislodged device, the ded

Sinus bradycardia as an atypical presentation of severe hypercalcemia: a case report and a literature review.

INTRODUCTION AND IMPORTANCE: Severe hypercalcemia typically produces QT interval shortening or ventricular arrhythmias, whereas bradyarrhythmias are exceptionally rare. The coexistence of hypercalcemia with acute kidney injury (AKI) - a condition that usually causes hypocalcemia - further underscores diagnostic complexity. CASE PRESENTATION: An 85-year-old man presented with AKI, confusion, and symptomatic sinus bradycardia (40 bpm) without atrioventricular block or QT prolongation. Laboratory studies showed markedly elevated corrected calcium, suppressed parathyroid hormone, and increased pa

Bradycardia, Hyperkalemia, Renal Dysfunction, and Hypoglycemia in Guideline-Directed Medical Therapy for Heart Failure: When to Tolerate and When to Worry.

A paradox persists in contemporary heart failure (HF) care, whereby the therapies most clearly proven to save the most lives are also those most frequently interrupted, often for reasons that are more physiological than pathological. Indeed, during HF medical therapy bradycardia, modest increases in creatinine or potassium levels, mild reductions in blood pressure, and concern regarding hypoglycemia are frequently perceived as dangerous adverse effects of drugs therapy, leading to premature dose reductions or discontinuation. However, when interpreted within their pharmacological and physiolog

Exploring Mechanical Synchrony and Myocardial Work Across Different Pacing Locations Within the Left Bundle Branch.

BACKGROUND: Left bundle branch pacing (LBBP) enables physiological activation by capturing the left conduction system at various sites. However, whether mechanical synchrony and efficiency vary depending on the pacing location remains unclear. OBJECTIVE: The study aim was to evaluate whether left ventricular (LV) mechanical performance varies among different LBBP sites and whether paced mechanical synchrony replicates intrinsic LV activation across locations. METHODS: This prospective cohort study included 114 patients with preserved LV ejection fraction who received LBBP for bradycardia ind

Efficacy and safety of fospropofol disodium versus propofol for general anesthesia induction: a systematic review and meta-analysis.

BACKGROUND: Propofol is a widely used agent for general anesthesia induction, with well-established anesthetic efficacy. However, its use is frequently associated with adverse reactions such as injection pain and circulatory depression. Fospropofol disodium, a water-soluble prodrug of propofol, has the potential to alleviate these issues. This study aimed to systematically review and meta-analyze the differences in efficacy and safety between fospropofol disodium and propofol for general anesthesia induction. METHODS: A comprehensive search was conducted in PubMed, Web of Science, Embase, Coc

Impact of outpatient preanesthetic consultation on perioperative outcomes in 700 urological patients in a tertiary hospital: a retrospective observational study.

BACKGROUND: Outpatient preanesthetic consultation improves patient assessment and anesthetic planning, enhancing safety and reducing complications. Conducted in advance, it allows systematic, individualized planning. Uncontrolled hypertension, common in 60% of people over 60, is a major cause of surgery cancellation and increases cardiovascular risk. This study evaluates its impact on preventing perioperative complications and optimizing clinical and surgical outcomes in urological patients. METHODS: This retrospective observational study analyzed 700 patients (≥ 18 years) who attended outpat

Ultrasound-guided measurement of cross-sectional area of dural sac to titrate the dosage of local anaesthetic in geriatric orthopaedic surgery under spinal anaesthesia: A prospective randomised study.

BACKGROUND AND AIMS: Dural sac cross-sectional area (DSCSA)-based intrathecal dosing aims to optimise block spread and improve haemodynamic stability during spinal anaesthesia (SA). Ultrasound-guided DSCSA measurement helps in local anaesthetic (LA) dose reduction compared with conventional fixed-dose practice. The primary objective was to determine LA dosage proportional to DSCSA reduction, while the secondary objectives included comparison of block characteristics and intra-operative haemodynamic parameters [heart rate and mean arterial pressure (HR and MAP)] between the two approaches. A Lifeline Through the Umbilicus: Emergency Pacing in Neonatal Complete Heart Block.

Complete congenital atrioventricular heart block (CHB) is a rare but serious, life-threatening condition in neonates, most often associated with maternal autoimmune connective tissue disease. Definitive management typically requires epicardial pacing; however, in cases of life-threatening bradycardia and hemodynamic compromise, temporary transvenous pacing may be lifesaving. Securing vascular access in premature or low-birth-weight infants often makes vascular access technically challenging, and conventional routes may not always be feasible. In such situations, the umbilical vein offers a rap

Efficacy and Safety of Dexmedetomidine-Esketamine Versus Dexmedetomidine Alone as Premedication for Pediatric Anesthesia Induction: A Systematic Review, Meta-Analysis, and Trial Sequential Analysis.

BACKGROUND: Emergence delirium (ED) is a common complication in pediatric anesthesia. Although intranasal dexmedetomidine (DEX) is widely used, its application is constrained by a slow onset, residual risk of ED in some patients, and risks such as bradycardia and hypotension. Esketamine (ESK), an NMDA receptor antagonist, may provide a faster onset and reduce these side effects. OBJECTIVE: This study compared the efficacy and safety of intranasal DEX-ESK combination versus DEX alone as premedication for anesthesia induction in pediatric patients undergoing surgery. METHODS: Electronic databa
