Effects of endothelium-derived catecholamines on heart rate, blood pressure and gill ventilation in the rainbow trout.

In most fish and tetrapods, regulation of cardiovascular system is traditionally attributed to the actions of the sympathetic and parasympathetic nervous systems on the heart and vasculature. Beyond the classic neurotransmitters (adrenaline, noradrenaline, and dopamine), the vascular endothelium can synthesize newly discovered catecholamines that modulate vascular tone and cardiac function independently of neuronal release, but their cardiorespiratory effects in fish remain unknown. Therefore, we characterized the cardiorespiratory responses of conscious rainbow trout (Oncorhynchus mykiss; n =

A Longitudinal Study of the Effects of Ketogenic Diet on Seizures, Cardiorespiration, Sleep Architecture and Mortality in the Kv1.1 Knockout Mouse Model of Sudden Unexpected Death in Epilepsy (SUDEP).

Background: Sudden unexpected death in epilepsy (SUDEP) causes significant mortality, affecting approximately 1 in 1000 people with epilepsy. Clinical and preclinical studies have identified severe seizures, bradycardia, apnea, severe postictal hypoxia, and sleep deficiency that emerge prior to SUDEP and thus may represent temporal biomarkers. The metabolic ketogenic diet (KD) therapy increases longevity in preclinical SUDEP models. Here, the hypothesis that KD therapy would determine whether the emergent sleep deficiency, bradycardia, apnea and/or hypoxemia persist as temporal biomarkers in p

A patient with a history of syncope experiencing cardiac arrest after brachial plexus block.

A 65-year-old male with a history of hypertension and diabetes, experienced syncope and cardiac arrest during shoulder arthroscopy after receiving a brachial plexus block. The patient, who was sedated with dexmedetomidine, developed bradycardia after being positioned in the beach chair position, which lead to cardiac arrest. Prompt cardiopulmonary resuscitation restored the patient's heart rate and consciousness. This report discusses the possible causes, including the vasovagal reflex, dexmedetomidine effects, and Bezold- Jarisch reflex, and proposes preventive measures. Recognising high-risk

Electrocardiographic Profile of Adult Patients with Coronavirus Disease (COVID-19) who were Given Remdesivir and Admitted in the University of the Philippines-Philippine General Hospital (UP-PGH).

BACKGROUND AND OBJECTIVE: Severe Acute Respiratory Syndrome - Coronavirus-2 (SARS-CoV-2) was initially known to affect the respiratory system and has been reported to also involve the cardiovascular system leading to myocardial damage. Remdesivir is one of the approved treatments for COVID-19, wherein viral replication is inhibited by terminating the RNA transcription prematurely. According to studies, the primary electrocardiographic effect of remdesivir in COVID-19 patients are sinus bradycardia and QT prolongation. The use of electrocardiogram (ECG) is an essential diagnostic tool in assess

Effect of deep analgosedation vs. intubated general anesthesia on perioperative sedation-related adverse events in older adults undergoing ERCP: protocol for a randomized controlled trial.

BACKGROUND: Deep analgosedation (DAS) and general anesthesia (GA) are currently the predominant anesthetic approaches for endoscopic retrograde cholangiopancreatography (ERCP). In contrast to DAS, GA typically requires endotracheal intubation for airway management. To determine the optimal analgosedation strategy, this study is designed to compare the effect of nasopharyngeal airway-assisted DAS (NPA-assisted DAS) vs. endotracheal intubation GA on perioperative sedation-related adverse events (SRAEs) in elderly ERCP patients. METHODS: Patients scheduled for ERCP will be randomly assigned in a

Comparative Study of Fixed Dose versus Height-adjusted Dose of Heavy Bupivacaine in Spinal Anesthesia for Elective Cesarean Section.

BACKGROUND: Maternal hypotension is the most common and significant complication of spinal anesthesia for cesarean delivery, increasing maternal nausea, vomiting, and risk of fetal acidosis. The traditional fixed intrathecal dose of 0.5% hyperbaric bupivacaine does not account for patient height, an important determinant of block spread and sympathetic blockade. MATERIALS AND METHODS: In this single-center, prospective, double-blind, randomized controlled trial, 60 American Society of Anesthesiologists I-II parturients undergoing elective lower-segment cesarean section received either a fixed

Case Report: Recurrent sinus arrest induced by repeated vomiting.

Neuromyelitis optica spectrum disorder (NMOSD) can affect autonomic centers in the dorsal medulla, but presentation with recurrent sinus arrest is extremely rare. We report a 71-year-old woman with 10 days of persistent hiccups and vomiting who developed repeated syncopal episodes. ECG monitoring revealed vomiting-induced bradycardia progressing to sinus arrest, and Holter monitoring showed nocturnal sinus pauses up to 40.6 s. Routine cardiac and neurological evaluations were unremarkable, but serum anti-aquaporin-4 antibodies were markedly elevated, and brain MRI demonstrated a dorsal medulla

Efficacy and safety of different vasopressor infusions on feto-maternal outcomes in normotensive patients undergoing caesarean delivery: a systematic review and network meta-analysis of randomised controlled trials.

INTRODUCTION: Phenylephrine infusion is widely endorsed by guidelines as the preferred prophylactic drug for spinal hypotension in patients undergoing caesarean delivery; however, clinical practice continues to show marked variability in the selection of vasopressor drugs. To address this, we aimed to synthesise current evidence from randomised controlled trials comparing vasopressor infusions for various feto-maternal outcomes in normotensive adult patients undergoing caesarean delivery. METHODS: Randomised controlled trials evaluating maternal and fetal outcomes associated with prophylactic

Amitraz poisoning mimicking organophosphate toxicity in a young woman: a clinically diagnosed case report from Western India.

BACKGROUND: Amitraz is a formamidine insecticide and acaricide widely used in veterinary and agricultural settings. Human poisoning with amitraz is uncommon but potentially serious, primarily due to its alpha-2 adrenergic agonist activity, monoamine oxidase inhibition, and prostaglandin synthesis inhibition. It is frequently misdiagnosed as organophosphate poisoning, leading to inappropriate management. CASE PRESENTATION: We present the case of a 26-year-old female from Himmatnagar, Gujarat, India, who ingested an amitraz formulation (12.5% w/v in xylene) with suicidal intent following a famil

Phenotyping the Structure and Function of the Heart of Elite Sailors: Implications for Pre-Participation Cardiac Screening.

Background: Structural and functional adaptation of the heart to chronic exercise is dependent on multiple factors, including the volume and type of training, and has direct implications for pre-participation cardiac screening (PPCS). Sailing is a unique multi-training modality sport with limited prior description of cardiac adaptation to training. The aims of this study are (1) to describe electrocardiogram (ECG) changes in sailors, informing PPCS guidelines; (2) to assess structural and functional cardiac changes in sailors; and (3) to examine sex- or discipline-specific cardiac adaptations

Differential responses of J waves to heart rate changes in idiopathic ventricular fibrillation and the general population in Japan.

Previous case studies have reported bradycardia-dependent augmentation of J waves in patients with idiopathic ventricular fibrillation (VF). However, the response patterns of J waves to changing heart rates have rarely been evaluated in the general population. In this review, the response patterns of J waves to varying RR intervals were analyzed in the general population during premature beats or pacing and compared with the patterns observed in idiopathic VFs. Furthermore, the mechanism and clinical importance of the J waves are discussed. J waves associated with idiopathic VFs showed bradyca

Low-dose dexmedetomidine improves postoperative sleep and pain in gynecological surgery: a randomized trial.

BACKGROUND: Postoperative sleep disturbances often lead to a vicious cycle with pain, severely hindering the recovery of patients. Women, due to fluctuations in sex hormones and their unique pain modulation mechanisms, are particularly vulnerable to both postoperative sleep disorders and pain. Dexmedetomidine (DEX) has shown potential in promoting sleep and providing analgesia. Therefore, exploring its application in optimizing postoperative pain management for gynecological patients is of great significance in enhancing recovery outcomes. OBJECTIVE: This study aimed to assess the impact of a
