Perioperative Hemodynamic Events following Intrathecal Fentanyl versus Buprenorphine as Adjuvants to Hyperbaric Bupivacaine in Infraumbilical Surgery: A Prospective Observational Study.

BACKGROUND: Hemodynamic instability is a recognized consequence of spinal anesthesia, but comparative evidence on intrathecal fentanyl and buprenorphine under routine clinical conditions remains limited. OBJECTIVE: To compare the incidence of hypotension and bradycardia and to explore the timing of these events with intrathecal fentanyl 25 μg versus buprenorphine 60 μg as adjuvants to hyperbaric bupivacaine in elective infraumbilical surgery. METHODS: This prospective observational study included 192 American Society of Anesthesiologists Physical Status I-II adults (96 per group). The attend

PFDoA induces developmental and immune toxicity in zebrafish through ROS-mediated inflammation-apoptosis signaling.

Perfluorododecanoic acid (PFDoA) is a persistent and bioaccumulative long-chain perfluoroalkyl acid, yet the mechanisms underlying its immunotoxicity remain poorly understood. This study investigated PFDoA-induced immunotoxicity in zebrafish embryos, demonstrating that 72 h exposure caused dose-dependent developmental toxicity (reduced survival, bradycardia, and shortened body length), markedly decreased neutrophil and macrophage populations, and induced thymic atrophy, indicating impairment of both innate and adaptive immunity. Transcriptomic analysis revealed enrichment of PPAR signaling, ap

Thiopentone in Combination with Xylazine: An Experimental Investigation for Induction of Surgical Anesthesia and Safe Recovery in Experimental Rats.

INTRODUCTION: Achieving reliable anesthesia induction and rapid postoperative recovery are crucial aspects of animal research, particularly in small experimental animals. Thiopentone is still a widely used anesthetic drug, but its safety is a primary concern in delivering a safe dose to experimental animals. However, the use of thiopentone in combination with xylazine for surgical procedures in small experimental animals remains underexplored. Therefore, this study assesses the efficacy and safety of combining thiopentone and xylazine for inducing anesthesia and facilitating recovery in Spragu

E-cigarette exposure during pregnancy impairs uterine artery blood flow and feto-placental function.

The popularity of electronic cigarettes (e-cigs) has risen in recent years, largely due to perceptions of reduced harm compared with traditional tobacco cigarettes. E-cig use has spread into vulnerable populations such as adolescents and individuals of reproductive age, yet their safety during pregnancy remains poorly understood. E-cigs differ from conventional cigarettes in composition, and their aerosolized components may uniquely affect the uterine environment and placental function. Proper placental development, maternal vascular remodeling, and tone are essential for healthy pregnancy out

Pharmacotherapy Management of Beta-Blocker Toxicity in the Emergency Department.

Beta-blocker (BB) overdose presentations to the emergency department represent a significant contributor to cardiovascular toxic exposures and associated morbidity and mortality. Overdose is characterized primarily by bradycardia, hypotension, atrioventricular blockade, and cardiogenic shock, alongside potential central nervous system depression. The severity of clinical manifestations is influenced by agent-specific pharmacologic properties, formulation (e.g., extended-release), and dose. Specific therapies discussed include atropine, glucagon, hyperinsulinemic euglycemia therapy, intravenous

Why Temporary Pacing in Acute Inferior Myocardial Infarction or No-Reflow Becomes a Trigger-Not a Remedy-For Ventricular Fibrillation: Undersensing, "R-On-T", Catheter Trauma and the Ischaemic Substrate.

Temporary pacing during acute inferior MI or no-reflow can trigger ventricular fibrillation rather than prevent it. Four mechanisms are highlighted: (1) acute ischemia lowers VF threshold and creates repolarization heterogeneity; (2) fragmented electrograms cause undersensing and asynchronous spikes; (3) bradycardia-related long RR cycles position spikes on the T-wave ("R-on-T"); and (4) catheter micro-displacement induces mechanical extrasystoles. We propose a bedside decision framework-three questions before pacing-and a prevention bundle focused on urgent ischemia reversal, continuous elect

Selective Estrogen Receptor Degraders Induce Bradycardia by Modulating Nuclear Estrogen Signaling.

Selective estrogen receptor degraders (SERDs) are emerging therapies for estrogen receptor-positive (ER+) breast cancer. However, certain oral SERDs, including giredestrant and camizestrant, have been associated with bradycardia in clinical trials. To elucidate the underlying molecular mechanism, we used chemical biology and genetic approaches in a zebrafish model of SERD-induced bradycardia. Giredestrant and camizestrant induced significant bradycardia in wild-type zebrafish embryos, whereas fulvestrant and amcenestrant (SERDs that do not induce bradycardia in patients) did not alter heart ra

Pacemaker and ICD Programming Adherence With Evidence-Based Recommendations.

INTRODUCTION: Despite data-driven 2015 expert consensus statement (consensus) guidelines on recommended PPM and ICD parameters, patients often fall short of optimal device programing. The purpose of this retrospective, observational, single-center study is to review device implant registry data both before and after the consensus update to determine whether there is an improvement in adherence to consensus-recommended device programming over time. METHODS: McKesson software was used to identify all patients pre- (1/1/2012-12/31/2013) and post- (1/1/2020-12/31/2022) consensus who underwent a s

Prevalence of clinically significant bradyarrhythmias in patients implanted with loop recorders for cryptogenic stroke: A manifestation of atrial cardiomyopathy?

BACKGROUND: Cryptogenic stroke (CS) is often associated with atrial fibrillation (AF), but may also reflect a broader atrial pathology such as atrial cardiomyopathy. Although implantable loop recorders (ILRs) are used primarily to detect AF in these patients, the burden of bradyarrhythmias is not well established. OBJECTIVE: This study aimed to evaluate the prevalence and clinical significance of bradyarrhythmias in patients implanted with ILRs after CS or transient ischemic attack (TIA) compared with a matched control population with ILRs for other indications. METHODS: We retrospectively a

Antiarrhythmic Potential of the β-Adrenoceptor Blocker Metoprolol against Dofetilide-Induced Torsades de Pointes in Anesthetized Bradycardic Rabbits.

We have established a proarrhythmic rabbit model of acute atrioventricular block (AVB) with severe bradycardia to detect delayed rectifier K+ channel current (IKr) blocker-induced torsade de pointes (TdP). To better understand the role of β-adrenoceptors in this model, we investigated the effect of the β-adrenoceptor blocker metoprolol against an IKr blocker dofetilide-induced TdP. AVB was induced by catheter ablation under isoflurane anesthesia, and the ventricles were electrically paced at a constant rate of 60 beats/min throughout the experiments. Monophasic action potentials (MAPs) were re

The effect of opioid-free anesthesia on postoperative pain in orthopedic surgery: A systematic review and meta-analysis of randomized controlled trial.

BACKGROUND: Opioid-free anaesthesia (OFA) is increasingly recognized as a perioperative approach that may offer effective pain control with reduced risk of adverse effects. Considering that orthopaedic surgeries often lead to at least moderate postoperative pain, we assessed the effectiveness and safety of OFA in orthopaedic procedures. METHODS: We searched PubMed, ScienceDirect, ProQuest, and EuropePMC for relevant randomised controlled trials (RCT) and supplemented the data with citation tracking. Data were analysed using RevMan version 5.4. The risk of bias and the certainty of evidence we
