Asymptomatic central venous occlusion secondary to central venous catheter-use complicating pacemaker implantation: a leadless solution.

Abnormal central venous anatomy can obstruct cardiac-device implantation. We report a 54-year-old patient found to be in complete heart-block after identification of profound bradycardia, requiring cardiac-pacing. Echocardiography revealed systolic dysfunction, prompting a plan for cardiac resynchronization therapy. Conventional lead placement was impossible due to failed guide-wire advancement bilaterally. Peri-procedural venography demonstrated attenuation of both brachiocephalic veins and contrast-enhanced computed tomography confirmed complete central venous occlusion (CVO). Further histor

Severe, Non-apneic Respiratory Dysfunction and Hypoxia following Generalized Convulsive Seizures.

OBJECTIVE: Sudden unexpected death in epilepsy (SUDEP) is a devastating consequence of some generalized convulsive seizures (GCS). Recent work has focused on seizure related apnea as a biomarker of SUDEP risk, frequently without characterizing the adequacy of non-apneic ventilation or identifying other dysfunctional breathing patterns. We hypothesized that GCS frequently induce immediate, severe, non-apneic respiratory dysfunction that can induce critical hypoxia and bradycardia and sought to characterize breathing patterns after GCS. METHODS: Adult patients admitted to an epilepsy monitoring

Ventricular Asystole During Le Fort I Orthognathic Surgery: A Case Consistent with Trigeminocardiac Reflex and a Mini Review.

INTRODUCTION: The trigeminocardiac reflex (TCR) is a brainstem reflex in which trigeminal stimulation precipitates abrupt vagally mediated cardiovascular changes, ranging from bradycardia to asystole. While classically described during down-fracture or pterygomaxillary disjunction in Le Fort I osteotomy, rhinocardiac events from lateral nasal wall manipulation are less emphasized in orthognathic surgery. CASE PRESENTATION: A 32-year-old man undergoing Le Fort I osteotomy developed ventricular asystole during lateral nasal osteotomy. The maneuver was stopped immediately; chest compressions and

Does Regional Anesthesia Improve Recovery After vNOTES Hysterectomy? A Comparative Observational Study.

Background and Objectives: Vaginal natural orifice transluminal endoscopic surgery, vNOTES, has become an increasingly preferred minimally invasive option for benign hysterectomy. General anesthesia is still the routine choice, yet regional methods such as combined spinal epidural anesthesia may support a smoother postoperative course. Although the use of vNOTES is expanding, comparative information on anesthetic approaches remains limited, and its unique physiologic setting requires dedicated evaluation. To compare combined spinal epidural anesthesia with general anesthesia for benign vNOTES

The safety of high dose continuous propofol infusion in the pediatric intensive care unit at an academic medical center.

INTRODUCTION: Adequate sedation is crucial in the care of critically ill children. Propofol, known for its rapid onset, reliable patient response, and short duration of action, offers an attractive pharmacologic profile. However, concerns for propofol-related infusion syndrome (PRIS) have limited its use in pediatric critical care. This study examined the relationship between propofol administration and PRIS characteristics in a single medical-surgical pediatric intensive care unit (PICU). We hypothesized that continuous propofol infusion, even when judiciously exceeding current recommended do

Minimal oil exposure causes sublethal damage in early larval stages of a key prey species, lesser sandeel (Ammodytes marinus).

The lesser sandeel (Ammodytes marinus) is a vital forage species in the North Sea, supporting numerous seabird, fish, and marine mammal populations. Its life cycle is characterized by benthic dwelling during juvenile and adult stages and a pelagic larval stage, which makes it especially sensitive to environmental disturbances. This study investigated the sublethal effects of crude oil exposure on early-stage lesser sandeel larvae (2 days post hatch), with a focus on developmental morphology, cardiac function, pigmentation, gene expression, and lipid content. Larvae were exposed to environmenta

Tether Entrapment and Fracture Preventing Release and Redocking of a Helix-Fixation Atrial Leadless Pacemaker: Successful Resolution With a Protective Sleeve-Assisted Bailout Maneuver.

BACKGROUND: Helix-fixation dual-chamber leadless pacemakers reliably provide bradycardia pacing, yet mechanical complexity of the delivery system can rarely lead to procedural failure. CASE PRESENTATION: During atrial leadless pacemaker implantation, resistance felt when transitioning to tether mode suggested tether entrapment within a damaged torque shaft, resulting in mechanical distortion or fracture and preventing both device release and redocking. A controlled bailout maneuver-advancing the protective sleeve near the helix and rotating the entire catheter system counterclockwise-enabled

Carfilzomib-induced sinus bradycardia: A case report and literature review.

Carfilzomib is a second-generation irreversible proteasome inhibitors commonly used in the treatment of relapsed or refractory multiple myeloma. However, cardiac toxicity (heart failure, hypertension, coronary heart disease, etc.) has become the main factor leading to its reduction or withdrawal. We report a case of a 55-year-old female patient with multiple myeloma who developed sinus bradycardia following the administration of carfilzomib, along with a review of the relevant literature. The correlation between carfilzomib and cardiotoxicity was assessed by reviewing the medical history, appl

Mechanistic Insights into the Cardiotoxic Effects of Dextromethorphan and Its Metabolite Dextrorphan in Zebrafish Using Network Toxicology and Molecular Dynamics.

The high incidence of respiratory diseases has driven a substantial increase in the use of antitussive dextromethorphan (DXM), leading to the frequent detection of its primary human metabolite, dextrorphan (DXO), in aquatic environments. However, existing research has paid limited attention to the toxic effects of DXM and its metabolite, DXO, on fish. In this study, zebrafish embryos were exposed to DXM and DXO at concentrations ranging from 100 to 10,000 ng/L for 72 hours post-fertilization (hpf) to evaluate their impacts on cardiac function and elucidate underlying molecular mechanisms using

Aligning cardiac monitoring with American Heart Association Guidelines: Impact on utilization, hemodynamic monitoring, and outcomes.

BACKGROUND: Overuse of continuous cardiac monitoring can lead to poor patient experience, increased costs, and decreased efficiency. Because significant variation in continuous cardiac monitoring ordering exists, implementation strategies that promote care in alignment with practice standards and an examination of use cases that fall outside of standards are needed. The purpose of this study, therefore, was to evaluate if implementation of American Heart Association (AHA) practice standards on continuous cardiac monitoring could reduce utilization without jeopardizing patient safety. METHODS:

Oxytocin and Vasopressin Cross Talk Within the Brain Increases Blood Pressure.

BACKGROUND: The paraventricular nucleus of the hypothalamus (PVN) orchestrates neuroendocrine and autonomic output to maintain systolic blood pressure (SBP). Emerging evidence suggests that the PVN utilizes paracrine signals to modulate neighboring neurons. Here, we test the hypothesis that OXT (oxytocin)-synthesizing neurons of the PVN (PVNOXT) release paracrine signals that regulate SBP via modulation of AVP (arginine vasopressin)-synthesizing neurons of the PVN. METHODS: To test the hypothesis, experiments were conducted ex vivo and in vivo in mice with the expression of ChR2 (channelrhodo

Bradycardia in the setting of postpartum preeclampsia and influenza A: A case report.

Postpartum bradycardia is a rare clinical finding, with limited guidance regarding its evaluation and management. Prior literature suggests that postpartum bradycardia is most commonly associated with preeclampsia, underlying cardiac disease, medication effects, or neuraxial anesthesia. In patients with preeclampsia, management may be particularly challenging, as magnesium sulfate-used for seizure prophylaxis-is theorized to exacerbate bradycardia. This report concerns the case of a 33-year-old woman (G3P3003) with an otherwise uncomplicated pregnancy and cesarean delivery who was diagnosed wi

Inflammatory and chemotactic signals of the brainstem solitary tract mediate the morphine exacerbation of impaired reflex chronotropism in septic rats.

INTRODUCTION: The interplay between opioid analgesics and sepsis in intensive care units (ICUs) is multifaceted, often amplifying immune dysregulation and adversely affecting cardiovascular outcomes. AIM: We investigated whether morphine, the prototypical opioid, influences sepsis-induced impairment of arterial baroreceptor function and the accompanying inflammation. METHODS: Rats were implanted with indwelling catheters in femoral vessels and intracisternal (i.c.) space, and sepsis was induced using the cecal ligation and puncture (CLP) technique. The baroreceptor-mediated control of chrono

Safety profile of ticagrelor: an analysis based on literature and database review.

OBJECTIVE: Ticagrelor, a reversible P2Y12 receptor antagonist, is widely used in acute coronary syndromes (ACS) for its superior efficacy over clopidogrel. However, emerging evidence highlights unique adverse effects, including dyspnea, bradyarrhythmias, QT prolongation, and ventricular pauses. Studies on the real-world safety of ticagrelor are limited. This study aimed to explore the adverse drug reactions (ADRs) linked to ticagrelor and assess their clinical features and management strategies. METHODS: We retrieved data on ADRs related to the P2Y12 receptor antagonist inhibitors ticagrelor f

Early heart-rate trajectory phenotypes predict short-term mortality in critically ill patients: a dynamic time-warping cluster analysis.

Heart rate (HR) reflects illness severity in critically ill patients, but the prognostic significance of early HR changes is unclear. We aimed to identify HR trajectory phenotypes during the first 24 h of ICU admission and assess their association with short-term mortality. Adults admitted to the intensive care unit (ICU) between January 2018 and December 2024 with ≥ 12 h of HR data were eligible; patients with early discharge/death, mechanical support, or sustained bradycardia were excluded. Hourly HR means from the first 24 h were imputed and clustered via Dynamic Time Warping-based TimeSeri

The Effect of Some Complementary Medicine Active Components on Rat Embryonic Heart Rate In Vitro.

INTRODUCTION: Most complementary and alternative medicines (CAMs) lack clinical safety regarding use in pregnancy, yet they are freely available and do not require a prescription. While all new conventional medicines are assessed for safety during pregnancy, CAMs are exempt. In vitro testing potentially provides a rapid screening method to support risk assessment. Whole rat embryo culture can be used to identify the impact of chemicals on the embryonic heart rate. There is clear evidence that slowing of the embryonic heart rate (bradycardia) can cause spontaneous abortions and fetal malformati
