Severe Bicytopenia and Neutropenic Fever in a Patient With COVID-19 Pneumonia: A Diagnostic Challenge.

Coronavirus disease 2019 (COVID-19) is a multisystemic viral infection that primarily affects the respiratory system but is widely recognized for its diverse extrapulmonary manifestations. Hematologic abnormalities, especially those associated with inflammation and disrupted hemostasis, are prevalent and have been extensively reported. However, few cases of profound neutropenia and severe thrombocytopenia have been documented in the literature, making their occurrence extremely rare. Herein, we present a rare case of a 60-year-old woman who presented with a two-week history of painful oral ulc

Unveiling the invisible: the role of multimodality imaging in diagnosing primary cardiac lymphoma.

Primary Cardiac Lymphoma (PCL) is a rare malignancy that frequently masquerades as other cardiac conditions or may be missed due to nonspecific clinical presentations. This diagnostic challenge necessitates a comprehensive imaging approach to ensure accurate differentiation and management. A 54-year-old female presented with palpitations, chest discomfort, slight troponin elevation and lateral ST-segment depressions mimicking acute coronary syndrome. Initial transthoracic echocardiography (TTE) revealed a peri-atrial mass. Cardiac Computed Tomography (CCT) ruled out coronary artery disease. In

Peritoneal implantation of pheochromocytoma 8 years after adrenalectomy: A rare cause of delayed recurrence.

Peritoneal implantation of pheochromocytoma following adrenalectomy is an exceptionally rare mechanism of recurrence, typically related to intraoperative tumor spillage or capsular rupture. It may present after a prolonged latency and mimic metastatic disease. We report the case of a 43-year-old man who presented with occasional palpitations, headache, and hypertension 8 years after right adrenalectomy for pheochromocytoma. Imaging revealed multiple peritoneal soft-tissue lesions. I-123 MIBG scintigraphy demonstrated avid uptake in these lesions, confirming peritoneal implantation of pheochrom

Allergy to Semen: A Rare Cause of Anaphylaxis.

A previously healthy lady in her mid-30s presented to the medical emergency with recurrent episodes of flushing, rash, wheezing, chest tightness, palpitations, dyspnoea, and loss of consciousness. After five months of intensive medical investigations, she was referred to the gynaecological team, and a detailed clinical history made it clear that these episodes always occurred after unprotected penetrative sexual intercourse. The patient was thoroughly investigated with cardiological evaluation and imaging. Eventually, a semen allergy test was performed by injecting 1 cc of semen subcutaneously

Identifying Symptom Clusters Predictive of Acute Myocardial Infarction for Aboriginal and Torres Strait Islander and Non-Indigenous Australians.

OBJECTIVES: This study examines the symptoms experienced by patients investigated for Acute Coronary Syndrome (ACS). It aims to determine whether symptom clusters exist, whether these predict acute myocardial infarction (AMI), and whether symptom patterns differ between Aboriginal and Torres Strait Islander people and non-Indigenous Australians. METHODS: This analysis of the LEGEND trial included 2185 patients (22.7% of whom identified as Aboriginal and Torres Strait Islander). Data on presenting symptoms and outcomes were collected from patient medical records. Regression analyses examined w

QT monitoring in chemotherapy.

This review provides a comprehensive overview of cardiac electrophysiology principles as they apply to oncology care, with a focus on QT interval assessment and monitoring. Accurate measurement of the QT interval and appropriate heart rate correction-particularly using the tangent method and Fridericia formula-are essential for evaluating arrhythmic risk in cancer patients, who are especially vulnerable due to the cardiotoxic potential of many chemotherapeutic agents. We highlight key risk factors for QT prolongation, including electrolyte imbalances, underlying cardiac conditions, and concurr

Unique Shield: An Obstructive Huge Tricuspid Pouch Associated With Ventricular Septal Defect Developing Into Adulthood.

BACKGROUND: Tricuspid valve (TV) pouch is a rare entity associated with ventricular septal defect (VSD). It is even rarer to cause right ventricular (RV) inflow obstruction. CASE SUMMARY: A 43-year woman presented with exertional palpitations for 2 years, diagnosed with a large TV pouch causing significant RV inflow obstruction without pulmonary arterial hypertension (PAH). Cardiac magnetic resonance revealed a large perimembranous VSD with bidirectional flow into the TV pouch, which had no effective communication with the right ventricle, preventing a significant left-to-right shunt and PAH.

Diet and environment: unexplored influences on sleep quality in postural orthostatic tachycardia syndrome (POTS).

Postural orthostatic tachycardia syndrome (POTS) is a chronic autonomic disorder characterized by an excessive increase in heart rate upon standing, accompanied by symptoms such as dizziness, fatigue, palpitations, cognitive impairment, and sleep disturbances. Sleep dysfunction is increasingly recognized as a significant contributor to symptom burden and reduced quality of life among individuals with POTS. Emerging evidence suggests that patients with POTS may experience heightened sympathetic activity during sleep, leading to disrupted autonomic regulation and fragmented sleep patterns. While

Patient-Reported Outcome Measures Following Pulsed-Field Ablation Versus Thermal Ablation of Atrial Fibrillation: The PROMs-PFA Study.

BACKGROUND: Pulsed-field ablation (PFA) has transformed atrial fibrillation (AF) ablation, but how the early patient experience compares with thermal techniques remains unclear. This study aimed to assess patient-reported outcome measures (PROMs) in the first 3 months after PFA or thermal AF ablation, using a novel smartphone-based application. METHODS: Patients undergoing first-time pulmonary vein isolation prospectively completed the Cardiac Ablation Recovery Evaluation (CARE) symptom questionnaire at five timepoints: baseline, day 1, day 7, day 30, and day 90. Symptoms were rated on a 7-po

A 78-Year-Old Woman With Dyspnea on Exertion, Hypotension, and Telangiectasias.

A 78-year-old woman with gastroesophageal reflux disease was admitted with 2 months of progressive dyspnea on exertion and leg swelling. At baseline, she could walk 10 blocks; at presentation, she managed less than 6 steps. For a month, she had been sleeping upright in a chair. She experienced no fever, cough, palpitations, or chest pain. With prompting, she shared a several-year history of discomfort and skin changes in her fingers; she attributed these symptoms to chemical exposures as a housecleaner.

Transfusion in Hospice-Palliative Care.

Transfusion in hospice and palliative care remains a complex issue as it is viewed as either a futile life-prolonging treatment or a legitimate intervention for symptom relief. These differing views result in inconsistent clinical practices. Anemia is common in terminally ill patients and is associated with fatigue, dyspnea, dizziness, palpitations, cognitive decline, and an impaired quality of life. Although red blood cell transfusion may provide short-term symptomatic improvement in selected patients, its benefits are often transient; repeated transfusions may be necessary, and its effect on

Case Report: Hyperthyroidism in a patient with spotty skin pigmentation and atrial myxoma (Carney complex): coincidence, association or cause?

BACKGROUND: Carney complex (CNC) is a rare autosomal-dominant multiple neoplasia syndrome characterised by spotty skin pigmentation, cardiac myxomas and endocrine overactivity. Although thyroid follicular adenomas are common in CNC, the presence of CNC in combination with Graves' disease-related hyperthyroidism is rare, and the pathogenic association between the two conditions remains poorly understood. CASE PRESENTATION: An 18-year-old Chinese woman presented with palpitations and worsening fatigue over 2 months. She had a 2-year history of Graves' disease (treated with iodine-131 1 year pri

When Palpitations Unmask Crista Terminalis Hypertrophy: A Case Report and Review of Current Literature.

Background and Clinical Significance: The crista terminalis (CT) is a physiological fibromuscular ridge in the right atrium. While benign, rare cases of CT hypertrophy present a diagnostic challenge, as it can mimic a pathological right atrial mass on cardiac imaging. The CT also presents arrhythmogenic potential and is known to be associated with right atrial tachyarrhythmias. Case Presentation: We present the case of a 58-year-old female that presented with rapid, irregular palpitations, accompanied by hypertension. Holter electrocardiography (ECG) confirmed self-limiting episodes of atrial

A Rare Cardiac Triple Siege in Youth: Right Atrial Lipoma, Coronary Stenosis, and Myocardial Bridge.

Right atrial lipoma combined with severe coronary stenosis and myocardial bridge is an extremely rare triad in young adults. We report a 34-year-old male presenting with acute palpitations and chest tightness. Multimodal imaging diagnosed a right atrial lipoma, severe left anterior descending artery stenosis, and myocardial bridge. One-stage surgical resection and coronary bypass were successfully performed, with histopathology confirming benign lipoma. At 1-year follow-up, there was no tumor recurrence and cardiac function was normal. This case demonstrates the value of multimodal imaging and

Utilization of cardiopulmonary bypass for resection of a massive left retrotracheal thyroid goiter: a case report and review of literature at a tertiary care hospital in Saudi Arabia.

Substernal goiter consists of thyroid tissue extending into the thoracic cavity and is more common in females. Total thyroidectomy is standard, though some cases require advanced approaches. Cardiopulmonary bypass (CPB) may be considered in selected cases for hemodynamic stability. We aimed to describe a rare case of retrotracheal thyroid goiter managed using CPB and review reported surgical approaches. A 39-year-old male presented with hoarseness and palpitations due to a left-sided retrotracheal thyroid goiter. Initial cervical access was unsuccessful, requiring median sternotomy with CPB fo

Frequency of kratom-related poison center calls before and after kratom legalization in Thailand: current trends.

INTRODUCTION: Kratom (Mitragyna speciosa) has traditionally been used in Thailand; however, it was illegal. Following decades of prohibition, kratom was removed from Thailand's list of narcotics on 24 August 2021, effectively legalizing its use. This study aims to describe the frequency, clinical characteristics, and trends of kratom-related poison center calls before and after legalization. METHODS: We conducted a retrospective review of data from the Ramathibodi Poison Center covering three 6-month periods: 24th of August, 2020 to 23rd of February, 2021; 24th of February, 2021 to the 23rd o

Severe acute myocarditis induced by 5-fluorouracil with successful rechallenge after multidisciplinary cardio-oncology evaluation: a case report.

BACKGROUND: Fluoropyrimidines are among the most common chemotherapeutic agents associated with cardiotoxicity, typically presenting as angina due to coronary vasospasm. Severe manifestations such as myocarditis and cardiogenic shock are rare, and rechallenge after cardiotoxicity is generally discouraged. We report a case of acute 5-fluorouracil-induced myocarditis with successful rechallenge after multidisciplinary evaluation. CASE SUMMARY: A 30-year-old man with unresectable rectal adenocarcinoma developed acute cardiotoxicity during the first FOLFOX-6 cycle, presenting with abdominal pain

Intramural left ventricular posterior superior process premature ventricular complex ablation with intracardiac echocardiography.

Intramural premature ventricular complexes (PVCs) are notoriously challenging during catheter ablation due to the depth of the arrhythmogenic substrate. We present a case utilizing intracardiac echocardiography (ICE) integrated with 3D electroanatomical mapping to ablate an intramural PVC in a patient with methamphetamine-associated cardiomyopathy. A 22-year-old female with a history of stimulant use disorder presented with debilitating palpitations and a 46% PVC burden. PVCs originating from the posterior superior process of the left ventricle (PSP-LV) were successfully targeted under ICE gui
