The trigger-timing framework for long COVID: using symptom triggers and temporal patterns to guide mechanistic stratification and treatment planning.

Long COVID is characterized by substantial clinical heterogeneity, with similar symptoms often arising from different underlying physiological mechanisms. Fatigue, cognitive dysfunction, palpitations, dyspnea, gastrointestinal symptoms, sleep disruption, and exertional intolerance may each reflect autonomic dysfunction, mitochondrial or bioenergetic impairment, endothelial or microvascular dysfunction, mast cell or histamine-mediated signaling, gut dysbiosis, neuroendocrine dysregulation, or interactions among these domains. Symptom-based classification alone may therefore be insufficient for

Malignancy-Associated Microangiopathic Hemolytic Anemia Mimicking Thrombotic Thrombocytopenic Purpura in a Patient With Metastatic Breast Cancer: A Case Report.

Microangiopathic hemolytic anemia (MAHA) is a form of Coombs-negative hemolysis characterized by mechanical fragmentation of erythrocytes, producing schistocytes on the peripheral blood smear. When accompanied by thrombocytopenia and neurological features, the presentation closely resembles thrombotic thrombocytopenic purpura (TTP), a hematological emergency requiring urgent plasma exchange. However, advanced malignancy can produce an almost identical thrombotic microangiopathy (TMA) that responds poorly to plasma exchange, and for which the definitive treatment is control of the underlying ca

Genes Do Not Read DSM: Tuberous Sclerosis Complex May Dominate with Multi-Neuropsychiatric Symptoms.

Tuberous sclerosis complex (TSC) is a multisystem genetic disorder frequently accompanied by epilepsy and TSC-associated neuropsychiatric disorders. Psychiatric-predominant seizure presentations may, however, delay recognition. This clinical challenge describes a 26-year-old woman with recurrent panic-like episodes characterized by dyspnea, choking sensations, palpitations, and derealization, followed by depressive symptoms. The episodes persisted despite treatment with selective serotonin reuptake inhibitors and benzodiazepines. Further examination revealed hypomelanotic macules, prompting ne

Acute oral Chagas disease following armadillo blood ingestion: a case report.

A 70-year-old man from rural Colombia presented with new-onset atrial fibrillation, dyspnea, and palpitations. A peripheral blood smear revealed a Trypanosoma cruzi trypomastigote, which was confirmed by molecular testing. He reported ingesting armadillo blood 3 weeks earlier, identifying oral transmission as the likely route. Treatment with nifurtimox, oral metoprolol, and enoxaparin resulted in conversion to sinus rhythm and symptomatic improvement, although a parasitological cure could not be confirmed. Because the patient had several competing risk factors for atrial fibrillation-advanced

Safety assessment of ambrisentan based on the FDA Adverse Event Reporting System.

Ambrisentan is an endothelin receptor antagonist broadly used for the treatment of pulmonary arterial hypertension. However, multiple adverse events (AEs) were reported with its use, and we intended to evaluate potential ambrisentan-associated AE signals through the US Food and Drug Administration Adverse Event Reporting System. Reports from March 2019 to December 2022 were selected for disproportionality analysis. Four signal detection methods were applied, namely the reporting odds ratio, the proportional reporting ratio, the information component, and the empirical Bayes geometric mean, to

Disproportionality analysis of cardiovascular adverse events reported to the FDA adverse event monitoring system with monoclonal antibodies targeting type 2 inflammation.

BACKGROUND: Type 2 inflammation leads to changes in the airways pathophysiology and several monoclonal antibodies targeting type 2 inflammation have been developed for allergic and eosinophilic diseases. However, long-term safety data for these medications are limited. Among the medications, only omalizumab has a safety warning for potential serious cardiovascular and cerebrovascular adverse events (AEs). OBJECTIVE: The aim was to analyse the disproportionality of cardiovascular AEs reported to the FDA Adverse Event Monitoring System (AEMS) database with these medications. METHODS: AEs relat

Swiss obesity clinical practice guidance.

Obesity is a chronic, relapsing, and multifactorial disease that poses a significant public health challenge in Switzerland, where approximately 43% of adults are overweight or obese. This new clinical practice guidance establishes a structured, multidisciplinary framework for healthcare professionals, emphasising that assessment, treatment and care should focus on improving overall health metrics, resolving comorbidities and achieving functional gains, rather than solely on numerical weight loss. Crucially, the guidance mandates a non-stigmatising, empathetic approach to combat weight bias, r

A Rare Presentation of Left Ventricular False Tendon Across the Mitral Valve Connecting the Left Atrium and Ventricle: A Case Report.

INTRODUCTION: Cardiac False tendons are anatomical variants of fibromuscular structures that generally should not exist in the heart. The left ventricular tendon, which crosses the left ventricular cavity, is typically seen in most cases of false cardiac tendons; however, our case differs from other cardiac false tendons. This case involves a fibrous tendon that spans the mitral valve, connecting the left atrium and ventricle. This unusual presentation has only been documented once. Based on our knowledge and research, this is the first documented occurrence in the United Arab Emirates and the

Ivabradine as a Treatment for Postural Orthostatic Tachycardia Syndrome: A Systematic Review.

Ivabradine is one of several off-label treatments for patients with postural orthostatic tachycardia syndrome (POTS). We conducted a systematic review of the literature to identify studies that evaluate ivabradine treatment in patients with POTS. The results were narratively synthesized given methodological heterogeneity among the identified studies. There were 11 studies included in the analysis with 305 participants between 2008 and 2020. The only randomized trial of 22 patients showed that ivabradine improved heart rate compared with placebo, and there were symptomatic benefits in terms of

The cardiac conduction system: development, function and therapeutic targets.

The cardiac conduction system (CCS) has a vital role in initiating and coordinating nearly 3 billion heartbeats throughout a person's lifetime. The CCS comprises two primary tissue types: the impulse-generating, slow-conducting nodes and the fast-conducting components of the ventricular conduction system. Dysfunction in this system can give rise to a spectrum of clinical symptoms, including palpitations, syncope, heart failure and even sudden cardiac death. Owing to the limited therapeutic options other than electronic pacemakers, substantial research efforts have been aimed at uncovering the

Management of Ablation-Refractory Atrial Fibrillation During Pregnancy.

Development of new atrial fibrillation (AF) is uncommon in pregnancy with an incidence of 0.03%. The occurrence of drug-refractory AF during pregnancy in individuals with structurally normal hearts is even rarer, and there are no reports of ablation-refractory AF during pregnancy in the literature. Early recognition and treatment of AF are crucial, as it causes hemodynamic changes that can adversely impact both the mother and fetus, potentially resulting in poor outcomes. We report a case of a 35-year-old woman at 19 weeks of gestation who presented with recurrent paroxysmal AF. Two years earl

Accessory pathway ablation: When the going gets tough, the tough go epicardial.

A 38-year-old lady presented with recurrent episodes of palpitations since the last four years. The electrocardiogram (ECG) during tachycardia indicated pre-excited atrial fibrillation, while the baseline ECG revealed preexcitation with QS waves in the inferior leads. Initial attempts at endocardial ablation and ablation via the coronary sinus approach did not yield successful results. Subsequently, we successfully ablated the inferoparaseptal (posteroseptal) pathway by utilizing a percutaneous epicardial approach. This method should be considered in cases where other techniques fail.
