Comparison of Tocolytic Agents for Successful External Cephalic Version: A Bayesian Network Meta-Analysis of Sixteen Randomized Controlled Trials.

BACKGROUND: Cesarean section is frequently performed for breech presentation; however, external cephalic version (ECV) is recommended as an alternative strategy to increase the likelihood of vaginal birth. Tocolytic agents are commonly administrated to improve ECV success, yet the comparative effectiveness of different regimens remains inadequately characterized. AIMS: To systematically evaluate and compare the efficacy and safety of various tocolytic agents in facilitating successful ECV through a Bayesian network meta-analysis. STUDY DESIGN: Bayesian network meta-analysis. METHODS: Bayesi

Management of Ablation-Refractory Atrial Fibrillation During Pregnancy.

Development of new atrial fibrillation (AF) is uncommon in pregnancy with an incidence of 0.03%. The occurrence of drug-refractory AF during pregnancy in individuals with structurally normal hearts is even rarer, and there are no reports of ablation-refractory AF during pregnancy in the literature. Early recognition and treatment of AF are crucial, as it causes hemodynamic changes that can adversely impact both the mother and fetus, potentially resulting in poor outcomes. We report a case of a 35-year-old woman at 19 weeks of gestation who presented with recurrent paroxysmal AF. Two years earl

Accessory pathway ablation: When the going gets tough, the tough go epicardial.

A 38-year-old lady presented with recurrent episodes of palpitations since the last four years. The electrocardiogram (ECG) during tachycardia indicated pre-excited atrial fibrillation, while the baseline ECG revealed preexcitation with QS waves in the inferior leads. Initial attempts at endocardial ablation and ablation via the coronary sinus approach did not yield successful results. Subsequently, we successfully ablated the inferoparaseptal (posteroseptal) pathway by utilizing a percutaneous epicardial approach. This method should be considered in cases where other techniques fail.

Left bundle branch pacing combined with atrioventricular node ablation in atrial fibrillation with severe aortic stenosis: A case report.

RATIONALE: In patients with atrial fibrillation (AF) complicated by severe aortic stenosis (AS), atrioventricular node ablation (AVNA) combined with permanent pacemaker implantation is an effective treatment strategy, especially when conventional rhythm control methods fail. PATIENT CONCERNS: An 88-year-old female with a 40-year history of paroxysmal palpitations and chest tightness, with significant worsening of symptoms over the past 20 days. Despite undergoing AF ablation 5 years ago, her symptoms and arrhythmia burden persisted. DIAGNOSIS: Long-standing (24 years) AF, severe AS, and a hi

Management of anomalous aortic origin of coronary arteries in children: A single-center experience.

BACKGROUND: Anomalous aortic origin of coronary arteries (AAOCA) is the second leading cause of sudden cardiac death in young athletes and poses significant diagnostic and management challenges due to its variable anatomy and unpredictable clinical course. METHODS: We retrospectively analyzed 17 pediatric patients (10 male, 7 female; median age 12 years) diagnosed with AAOCA between 2016 and 2025. All patients underwent echocardiography and electrocardiogram-gated computed tomography angiography. Risk stratification considered interarterial course, intramural segment, and ostial morphology.

Aortic Valve Myxoma in a Resource-Limited Setting.

BACKGROUND: Cardiac myxoma is one of the most common benign cardiac tumors, typically arising in the left atrium. Valvular myxomas are exceedingly rare, with aortic valve involvement being among the rarest. Aortic valvular myxomas often present with embolic events or left ventricular outflow tract (LVOT) obstruction. CASE SUMMARY: We present a case of a 32-year-old man with a 3-month history of recurrent bilateral calf pain, associated with recurrent syncope, palpitations, and fatigue. Echocardiography revealed a large subaortic valve mass obstructing the LVOT. The patient underwent successfu

Apparent Sinus Pause and Premature Atrial Complexes Attributable to an Accessory Pathway.

BACKGROUND: A concealed accessory pathway is typically not characterized on a sinus rhythm electrocardiogram (ECG). However, in exceptionally rare cases, it is misdiagnosed as sinus pause because retrograde conduction via an accessory pathway generates a P' wave that is concealed within the T wave and not conducted to the ventricles; such cases are rarely reported. CASE SUMMARY: A 54-year-old man presented to a local hospital with recurrent chest tightness and palpitations in 2023. Holter ECG results at another hospital suggested sinus pause with junctional escape beats, and pacemaker implant

Smartphone Application-Derived Clusters of Persistent Symptoms in Patients After Atrial Fibrillation Ablation: Data From the ISOLATION Study.

BACKGROUND: Atrial fibrillation (AF) is characterized by a heterogeneous presentation of symptoms. AF ablation reduces symptom burden. However, persistent symptoms following AF ablation are common independently of AF recurrence. OBJECTIVES: This study sought to perform a cluster analysis to identify clinically relevant AF subphenotypes based on persistent symptoms following AF ablation and evaluate their associations with clinical characteristics and AF recurrence. METHODS: Patients were instructed to perform smartphone app-based simultaneous symptom and photoplethysmography heart rhythm mon

Life-Threatening Diagnosis or Completely Benign? A School Nurse's Guide to Assessing and Caring for Students With Cardiac Complaints.

This article, developed in collaboration with pediatric cardiologists and school nurses, reviews common student complaints that may have a cardiac etiology including palpitations, chest pain, and syncope. We discuss the initial steps that a school nurse should perform for each of these complaints, including relevant history taking and physical exam. We identify red flags which should raise suspicion for a cardiac etiology and outline the initial steps a school nurse should take in managing these students. We discuss which cases require assessment with a bedside automatic external defibrillator

Inferior vena cava anomalous drainage into the left atrium with severe pulmonary arterial hypertension: a case report and review of management strategies.

Anomalous drainage of the inferior vena cava (IVC) into the left atrium is a rare congenital anomaly seldom associated with pulmonary arterial hypertension (PAH). This condition may correlate with atrial septal defect (ASD), pulmonary arteriovenous fistula, and chronic hypoxemia. We present a case of a 34-year-old female admitted with recurrent chest tightness, dyspnea, and palpitations. Comprehensive evaluations, including transthoracic echocardiography, computed tomography angiography, and right heart catheterization, confirmed the diagnosis of IVC anomalous drainage into the left atrium com

Clinical features and inflammatory signatures of patients with persistent gastrointestinal long COVID two years after severe SARS-CoV-2 infection.

Persistent gastrointestinal (GI) symptoms are increasingly recognized as part of long COVID, yet their underlying mechanisms remain poorly defined. We conducted an exploratory case-series study of 80 adults hospitalized with severe COVID-19 in March-May 2020 in Manaus, Brazil. Two years post-infection, participants underwent structured clinical interviews and longitudinal cytokine analysis (IL-1β, IL-6, IL-8, IL-10, IL-12, and TNF-α). Overall, 30 participants reported ongoing GI symptoms (GI group) predominantly gastroesophageal reflux (63%), abdominal pain (43%), and diarrhea (37%). Compared

Atrial fibrillation among master athletes: A complex relationship.

BACKGROUND: Atrial fibrillation (AF) is the most common sustained arrhythmia with a notable impact on morbidity and mortality. While moderate physical activity is generally protective against cardiovascular disease, the relationship between endurance sports and AF, especially in master athletes (aged ≥45), remains unclear. Some evidence suggests a U-shaped association between exercise intensity and AF risk. OBJECTIVE: To investigate the association between endurance sport participation and AF occurrence among master athletes, and to identify potential risk factors or predisposing conditions.
