Essential role of multimodality imaging in the diagnosis of a rare high-grade primary cardiac intimal sarcoma.

BACKGROUND: Primary cardiac intimal sarcoma is an exceptionally rare and aggressive malignant tumour that often mimics benign cardiac lesions, particularly atrial myxoma. Accurate diagnosis requires multimodality imaging and histopathological confirmation. CASE SUMMARY: A 58-year-old woman presented with palpitations, exertional dyspnoea, and recurrent syncope. Echocardiography and CT revealed a large, polypoid left atrial mass attached to the interatrial septum, initially suggestive of atrial myxoma. Due to haemodynamic compromise, urgent surgical resection was performed. Histopathology demo

Identifying cardiac safety signals of disproportionate reporting for CGRP antagonists: evidence from the FDA Adverse Event Reporting System.

The objective of this study was to investigate the potential association between the use of calcitonin gene-related peptide (CGRP) antagonists and the reporting of cardiac adverse events (cAEs) by analyzing data from the US Food and Drug Administration Adverse Event Reporting System (FAERS). CGRP antagonists are a novel class of effective treatments for migraine. However, given CGRP's crucial role as a potent vasodilator, concerns about the cardiac safety of its long-term blockade persist. This study aimed to assess real-world post-marketing safety signals for this drug class. FAERS data from

Massive left atrial myxoma in pregnancy: case report.

BACKGROUND: Cardiac myxomas are rarely diagnosed in pregnancy. CASE SUMMARY: A 41-year-old 18+3/40 weeks' gestation pregnant woman presented with dyspnoea and palpitations on a background of a benign pituitary microadenoma and previous tenosynovial giant cell tumour. Transthoracic echocardiogram demonstrated a 9.1 × 5.5 cm left atrial mass attached to the fossa ovalis with moderate-to-severe obstruction to flow through the mitral valve. After a multidisciplinary discussion regarding maternal and fetal risks, a decision was made to proceed with intrapartum cardiac surgery, given the evidence o

Sex-specific adverse event profiles of PDE4 inhibitors: a comparative big-data pharmacovigilance study of apremilast, crisaborole, and roflumilast in FAERS (2004-2025).

INTRODUCTION: Phosphodiesterase-4 (PDE4) inhibitors, including apremilast, crisaborole and roflumilast, are widely prescribed for chronic inflammatory diseases. However, sex-specific safety profiles of these agents in routine clinical practice remain poorly characterized. METHODS: We conducted a retrospective pharmacovigilance study using the US Food and Drug Administration Adverse Event Reporting System (FAERS) from Q1-2004 to Q1-2025. Data cleaning and deduplication were performed following the FDA Guidance for Industry: Pharmacovigilance Practices and Pharmacoepidemiologic Assessment (FDA,

Coexistence and spontaneous transition between orthodromic atrioventricular reentrant tachycardia and atrial tachycardia: The mechanism of atrial activation sequence.

UNLABELLED: The coexistence of multiple supraventricular tachycardias during electrophysiological study (EPS) is rare and can complicate diagnosis and treatment. We present a case of coexisting orthodromic atrioventricular reentrant tachycardia (AVRT) and atrial tachycardia (AT) with spontaneous transitions. A 15-year-old girl with palpitations underwent EPS revealing orthodromic AVRT via a left-sided accessory pathway (AP) and AT originating from the right atrium. The tachycardias had similar cycle lengths and exhibited spontaneous transitions. During AT, a unique cyclic atrial activation seq

Atypical Chest Pain: A Diagnostic Challenge for Takotsubo Cardiomyopathy.

Takotsubo cardiomyopathy (TTC) typically presents in post-menopausal females with chest pain, dyspnea, or palpitations and often mimics acute coronary syndrome. This case report describes a unique presentation of TTC in an elderly patient who exhibited gastrointestinal and musculoskeletal symptoms without cardiopulmonary complaints. This constellation of symptoms has not been previously described in the literature. The patient was ultimately diagnosed with TTC with evidence of ST-T wave abnormalities on ECG, elevated serum troponin levels, a reduced left ventricular ejection fraction, the abse

Borderline Arrhythmogenic Cardiomyopathy in an Athlete: Exercise Testing May Support Clinical Decision-Making.

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) bears a considerable risk of malignant arrhythmias and heart failure, but diagnosis in athletes can be challenging. CASE SUMMARY: We present a male 52-year-old long-distance runner with palpitations, a dilated right ventricle with apical akinesia, nonsustained ventricular tachycardia, and negative genetic testing. We performed combined stress echocardiography and cardiopulmonary exercise testing showing blunted exercise response and a decrease in right ventricular strain during exercise. Although the modified task force criteria were not met, a

Digital characterization and quality assessment of Yangxue Anshen tablet: A combined approach utilizing fingerprinting, multicomponent quantification, and antioxidant activity.

Yangxue Anshen Tablet (YAT), composed of seven traditional Chinese medicines (TCMs) used for the treatment of palpitations, dizziness and insomnia, has not been adequately studied concerning quality control and active ingredient determination. This research developed a multi-dimensional fingerprinting system that integrated chromatographic, spectral, and thermal analyses by combining multiple mathematical models to address this gap and establish a rapid quality assessment strategy. First, a five-wavelength fusion HPLC fingerprint, FTIR fingerprint, and DSC fingerprint were established to evalu

Syncopal Episodes in Coved ST-Segment Elevation and T-Wave Inversion in Anterior Leads.

CASE SUMMARY: We describe the case of an 88-year-old male patient referred to our cardiology outpatient clinic because of recent and recurrent syncopal episodes, often preceded by palpitations. At clinical assessment, the patient was asymptomatic, vital signs were normal, and no relevant findings at physical examination were detected. The standard 12-lead electrocardiogram showed a normal sinus rhythm with coved ST-segment elevation and T-wave inversion in anterior leads. A differential diagnosis is discussed, and the evolution of the electrocardiographic pattern throughout the course of the d

Acute Intoxication with Caffeine-Containing Tablets: A Case Report with a Fatal Outcome.

Caffeine is widely consumed and generally considered safe at customary doses. How-ever, high-dose preparations available online pose a risk of severe and potentially fatal intoxication. Although uncommon, lethal caffeine poisoning is associated with profound cardiovascular and neurological toxicity. A rare case of intentional acute caffeine intoxication with fatal outcome is presented. A 25-year-old woman ingested an estimated 60 tablets containing 200 mg of caffeine each, purchased online. She was admitted to hospital shortly after ingestion of the caffeine tablets with palpitations, agitatio

Thyroid hormone resistance beta and autoimmune thyroid disease - a family case study highlighting diagnostic and therapeutic challenges.

BACKGROUND: Resistance to thyroid hormone (RTH) is a rare endocrine disorder, most commonly caused by mutations in the thyroid hormone receptor β (TRβ) gene, resulting in the RTH beta (RTHβ) subtype. In contrast, autoimmune thyroid disease (AITD) is common. The coexistence of RTHβ with AITD has not been fully clarified. Here, we describe a family in which RTHβ coexists with AITD, highlighting the complex interplay between these disorders. CASE PRESENTATION: A 45-year-old woman, previously for years treated for hyperthyroidism, presented after a decade-long gap in follow-up, reporting palpitati

Establishing Cardiopulmonary Bypass Before General Anesthesia in a Patient Undergoing Right Atrial Myxoma Resection.

We describe a case of establishing cardiopulmonary bypass (CPB) before general anesthesia in a person who has an obstructive right atrial myxoma protruding into the right ventricle (RV). A 27-year-old female endorsed shortness of breath on exertion, dry cough, and palpitations, with a cardiac MRI exhibiting a severely enlarged right atrium (RA) with a 65 mm x 40 mm mass. The mass obstructed the tricuspid valve from closing and extended into the RV, leading to an ejection fraction (EF) of 49%. The obstruction caused concern for cardiovascular collapse due to hypotension and bradycardia that may

Incidentally Detected Cystic Pheochromocytoma on Computed Tomography in a Patient Presenting With a Urinary Tract Infection: A Report of a Rare Case.

Pheochromocytomas are rare catecholamine-secreting neuroendocrine tumors originating from the adrenal medulla, presenting with classic symptoms of headache, diaphoresis, and palpitations due to paroxysmal hypertension. Generally, pheochromocytomas are solid or sometimes solid with cystic changes. Purely cystic pheochromocytoma is very uncommon, with only a limited number of cases reported. We present the case of a 42-year-old male with cystic pheochromocytoma who presented with symptoms of urinary tract infection and, on imaging, was found to have a left adrenal purely cystic lesion. On imagin

Acute Kidney Injury and Nephrotic-Range Proteinuria as Initial Presentation of Pheochromocytoma: A Case Report.

INTRODUCTION: Pheochromocytomas are rare catecholamine-producing neuroendocrine tumors that present with the classical triad of paroxysmal hypertension, palpitations, and sweating; however, this presentation is seen in less than 1/4 of pheochromocytoma patients. Hypertension is usually the predominant manifestation. Atypical manifestations, including renal involvement, are infrequent and may delay diagnosis. CASE PRESENTATION: We report the case of a 37-year-old male with no prior comorbidities who presented with progressive abdominal pain, shortness of breath, and oliguria. Laboratory evalua

Paediatric chest pain: evaluating diagnostic testing and clinical outcomes.

BACKGROUND: Chest pain is a common reason for referral to paediatric cardiology clinics. Unlike adults, most paediatric chest pain is benign, yet uncertainty often leads to additional testing. The study aimed to evaluate the aetiology of chest pain in children referred to a tertiary paediatric cardiology clinic, determine the diagnostic yield of additional tests, and identify patients who truly benefit from further investigations using a risk-based approach. METHODS: A retrospective observational study of 368 children (5-18 years) presenting with chest pain among 1,631 outpatient visits was c

Menopausal symptom network differences between women with and without waist-to-height ratio-defined abdominal obesity.

OBJECTIVES: To compare the structure of menopausal symptom networks between women with and without abdominal obesity (AO), defined as waist-to-height ratio (WHtR). METHODS: This cross-sectional analysis included 1,150 women from visit 6 of the study of Women's Health Across the Nation (SWAN). AO was defined as a WHtR of ≥ 0.5. We adjusted for demographic and clinical differences by regressing each symptom score on covariates, and subsequently using the residualized symptom scores for network estimation. We used centrality indices and community structure analysis to characterize the networks,
