Anterior Spinal Artery Aneurysms Associated with Coarctation of the Aorta: A Focused Narrative Review with an Illustrative Case.

Anterior spinal artery (ASA) aneurysms are rare causes of subarachnoid hemorrhage (SAH), and coarctation of the aorta (CoA) may recruit the spinal arterial circulation into a high-flow collateral network. We report the case of a 58-year-old woman who was resuscitated after pulseless electrical activity arrest. Noncontrast CT demonstrated SAH concentrated in the cervical spinal canal with limited intracranial extension. Cervically extended CT angiography identified a C5-6 ventral spinal aneurysm, severe thoracic CoA with extensive collateralization, and diffuse left subclavian artery hypoplasia

Prehospital predictors of clinical outcomes in out-of-hospital cardiac arrest with pulseless electrical activity: a JCS-ReSS report.

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) with pulseless electrical activity (PEA) has a poor prognosis and is aetiologically heterogeneous. We aimed to identify prehospital predictors of a favourable outcome, overall and by presumed aetiology. METHODS: This nationwide cohort study analysed 442,630 adults with OHCA and an initial rhythm of PEA from the All-Japan Utstein Registry between 2005 and 2024. The primary outcome was a favourable neurological outcome at 1 month. Two models were fitted, at the scene and at hospital arrival, each also within each aetiology. Epinephrine and advan

Prehospital extracorporeal cardiopulmonary resuscitation for refractory out-of-hospital cardiac arrest: a protocol for the Rapid prehospital ACcess to ECPR in Prague (RACE Prague) single-arm pilot feasibility study.

AIM: The benefit of extracorporeal cardiopulmonary resuscitation (ECPR) in refractory out-of-hospital cardiac arrest (OHCA) is strongly time dependent, yet in established in-hospital ECPR systems, including Prague, the median collapse-to-flow time has remained around 60 min or more, dominated by intra-arrest transport. Evidence for prehospital ECPR remains limited, particularly its comparison with in-hospital ECPR. RACE Prague evaluates whether dispatching a mobile cannulation team to the scene shortens this interval within the same ECPR system. METHODS: Single-centre, prospective, interventi

Catastrophic Bone Cement Implantation Syndrome Identified Real Time with Transesophageal Echocardiography.

Characterized by unexpected hypoxia, hypotension, or hemodynamic collapse, bone cement implantation syndrome (BCIS) is a rare complication of orthopedic surgeries. During the process of utilizing cement to aid in prosthetic implantation, increased intramedullary pressure can lead to embolization of mixed contents (cement, fat, air,) into the heart. By mechanical obstruction and inflammatory response, BCIS can lead to catastrophic cardiovascular collapse. Herein, we present an 81-year-old man with grade three BCIS with pulseless electrical activity during a left hip arthroplasty with the aid of

Traumatic High Cervical Spine Injury Presenting as Cardiac Arrest: A Case Report.

Traumatic high cervical spinal cord injury is a potentially devastating condition that may cause profound cardiovascular and respiratory instability through disruption of descending sympathetic pathways. In severe cases, autonomic dysfunction may contribute to marked hypotension, bradycardia, and cardiac arrest. We report the case of a 47-year-old man who sustained blunt trauma of incompletely characterized mechanism in a workplace accident and was found in cardiac arrest by a prehospital emergency medical team. Advanced life support was initiated, and return of spontaneous circulation was ach

Spectral Characteristics and Parameters of Pulse Oximetry Waveforms for Assessing Resuscitation Status.

BACKGROUND: Real-time assessment of physiological resuscitation status is critical for guiding personalised cardiopulmonary resuscitation (CPR) and avoiding unnecessary harm from prolonged chest compressions. Non-invasive, objectively trackable tools for this purpose remain limited. METHODS: This multicentre retrospective study (2013-2018) included 451 adult patients with cardiac arrest (497 CPR episodes) from 14 Chinese tertiary hospitals, comprising 158 Out-of-Hospital Cardiac Arrest (OHCA) episodes with prehospital SpO2/ECG monitoring continued upon ED arrival and full pulse oximetry pleth

Impact of Cardiac Rhythm Dynamics during Cardiopulmonary Resuscitation on Clinical Outcomes in Patients Treated with Extracorporeal Cardiopulmonary Resuscitation (ECPR): A Systematic Review and Meta-analysis.

BACKGROUND: Although the initial presenting cardiac rhythm is an established prognostic factor for extracorporeal cardiopulmonary resuscitation, heart rhythm dynamics during conventional cardiopulmonary resuscitation remain poorly studied. We evaluated how the evolution of rhythm affects neurological outcomes and survival among recipients of extracorporeal cardiopulmonary resuscitation. METHODS: We systematically searched PubMed and Embase for studies evaluating rhythm dynamics, focusing on Rhythm Improvement (from non-shockable to shockable) or Rhythm Decay (from shockable to non-shockable).

Post In-Hospital Cardiac Arrest Simulation to Improve Medical Decision Making and Communication in Internal Medicine Residents.

INTRODUCTION: Cardiac arrest is a common cause of morbidity and mortality in hospitalized patients, and is a critical event that hospital-based physicians must be prepared to manage. While cardiac arrest care follows established algorithms, management of patients after return of spontaneous circulation (ROSC) is more dynamic and nuanced. Resident physicians therefore need structured training to provide high-quality post-ROSC care. METHODS: We developed a 90-minute simulation session on postarrest care for internal medicine interns. The session included 2 simulation cases: one involving a pati

Pericardial tamponade during surgical ventriculoperitoneal shunt placement : a case report and review of the literature.

BACKGROUND: Cardiac tamponade is a catastrophic hemodynamic emergency. While ventriculoperitoneal (VP) shunt placement is a common neurosurgical procedure, its typical complications include obstruction and infection. No prior cases of intraoperative cardiac tamponade during VP shunt placement have been reported. CASE PRESENTATION: We present a case of life-threatening cardiac tamponade that occurred during VP shunt insertion in a patient with severe cerebral edema. The patient developed abrupt hemodynamic collapse, progressing to pulseless electrical activity requiring emergency cardiopulmonar

Respiratory and cardiac toxicity of xylazine and fentanyl overdose in rats.

INTRODUCTION: Xylazine, a veterinary sedative, is an adulterant increasingly found in illicit fentanyl supplies. Yet, the dose-dependent effects of xylazine on fentanyl overdose remain poorly characterized. We sought to determine how xylazine modifies fentanyl-induced mortality, respiratory depression, and cardiovascular dysfunction in rats. METHODS: Adult male and female Sprague-Dawley rats received non-lethal xylazine alone or combined with fentanyl (150 μg/kg) intravenously. For xylazine, three doses (0.1 mg/kg, 1 mg/kg, or 10 mg/kg) were chosen that produced a dose-dependent transition fr

Cardiac Arrest From Sulfur Hexafluoride Contrast Anaphylaxis After Prior Perflutren Hypersensitivity.

BACKGROUND: Ultrasound-enhancing agents (UEAs) are generally considered safe, with severe reactions exceptionally rare. Cross-reactivity between agents remains poorly understood. CASE SUMMARY: A 62-year old man with prior hypersensitivity to perflutren lipid microspheres contrast agent developed anaphylaxis with pulseless electrical activity arrest immediately after administration of sulfur hexafluoride lipid microspheres contrast agent. After resuscitation, return of spontaneous circulation occurred within 2 minutes. An elevated tryptase level confirmed mast cell activation. DISCUSSION: Cur

Pushing the limits of death-Rapid ultraoxygenated recovery without preimplant heart reanimation in circulatory death with prolonged "no-touch" period.

Cardiac allografts from donation after circulatory death (DCD) donors have increased the donor pool in adult heart transplantation. Historically, techniques to reanimate DCD hearts before implantation have been used to ensure graft viability and function. Our group has recently described a novel method for the rapid ultraoxygenated recovery of DCD hearts for transplantation without the need for preimplant reanimation. Concerns remain regarding its worldwide application in prolonged "no-touch" scenarios, such as the 20-minute asystole observation period in Italy. Here, we highlight the use of o

Prolonged pulseless electrical activity warm ischemia predicts mortality and graft dysfunction in donation after circulatory death heart transplant.

OBJECTIVE: Prolonged asystolic warm ischemic time during donation after circulatory death (DCD) heart transplantation worsens outcomes. Despite recommendations from the American Society of Transplant Surgeons, declaration of death upon pulseless electrical activity (PEA) instead of asystole remains inconsistent. This study evaluated the association between prolonged PEA warm ischemic time (PWIT) and outcomes in adult recipients of cardiac allografts recovered using thoracoabdominal normothermic regional perfusion (TA-NRP). METHODS: Adult heart transplants from DCD allografts recovered with TA

Survival following high-voltage electrical injury with out-of-hospital cardiac arrest: rapid ROSC and full recovery in a resource-limited setting: a case report.

BACKGROUND: High-voltage electrical injuries can cause sudden cardiac arrest, most commonly due to ventricular fibrillation, and are associated with high mortality and poor neurological outcomes. Survival with complete neurological recovery following out-of-hospital cardiac arrest remains uncommon, particularly in low-income and resource-limited settings. CASE PRESENTATION: We report the case of a 22-year-old previously healthy male who sustained a high-voltage electrical injury at a construction site and collapsed immediately. On arrival at the emergency department, he was unresponsive, pulse

Takotsubo cardiomyopathy with cardiogenic shock misdiagnosed as old MI, successfully treated with TAVR: a case report.

BACKGROUND: Differentiating Takotsubo cardiomyopathy (TCM) from myocardial infarction (MI) remains challenging, especially in elderly patients with structural heart disease. When complicated by severe aortic stenosis (AS), cardiogenic shock can rapidly develop, posing both diagnostic and therapeutic dilemmas. CASE SUMMARY: A 91-year-old woman presented with progressive dyspnoea and was admitted for acute decompensated heart failure. Transthoracic echocardiography revealed severe AS and a reduced left ventricular ejection fraction (LVEF) of 30% with apical wall thinning, suggestive of an old M

Dynamic ST-Segment Elevation in Massive Pulmonary Embolism Captured by Holter Electrocardiography: A Case Report.

ST-segment elevation on a post-cardiac arrest electrocardiogram (ECG) is traditionally viewed as a hallmark of acute coronary occlusion. However, this finding can also arise from non-coronary etiologies like massive pulmonary embolism (PE), creating a high-stakes diagnostic dilemma. An 83-year-old woman hospitalized for trauma developed cardiac arrest with pulseless electrical activity (PEA). Immediate post-resuscitation ECG showed marked ST-segment elevation in leads V1-V4, II, III, and aVF, suggesting extensive myocardial infarction. A point-of-care echocardiogram showed severe right ventric

COVID-19-Associated Acquired Hemophilia A With an Exceptionally High Inhibitor Titer: A Case of Remission and Overwhelming Sepsis.

Acquired hemophilia A (AHA) is a rare autoimmune bleeding disorder caused by autoantibodies against factor VIII. COVID-19 infection has emerged as a potential trigger, though reported cases remain limited, with variable inhibitor titers. We report a 70-year-old African American man who developed AHA following documented COVID-19 infection in October 2023, presenting with an exceptionally high factor VIII inhibitor titer of 561 Bethesda Units (BU) that peaked at 677 BU during initial treatment. His course was complicated by migratory spontaneous hematomas and a massive chest wall hematoma leadi

Deep learning-based ROSC prediction and ECG phenotyping in out-of-hospital cardiac arrest.

INTRODUCTION: Electrocardiogram (ECG) signals during cardiac arrest contain detailed information on cardiac rhythm characteristics and have been associated with resuscitation outcomes. We aimed to develop a convolutional neural network (CNN) model to predict return of spontaneous circulation (ROSC) and identify novel ECG phenotypes in patients with out-of-hospital cardiac arrest (OHCA). METHODS: This retrospective cohort study used Korean OHCA Registry and ECG data from Seoul emergency medical services between July 2021 and December 2023. Adult patients with nontraumatic OHCA who had prehospi
