Fulminant Pulmonary Hypertension Due to Pulmonary Lymphangitic Carcinomatosis and Tumor Embolism From Occult Pulmonary Adenocarcinoma.

Pulmonary lymphangitic carcinomatosis (PLC) and pulmonary tumor embolism (PTE) are rare manifestations of metastatic malignancy that may mimic more common causes of dyspnea and pulmonary hypertension, often leading to delayed diagnosis. Both conditions are associated with poor prognosis and may present with rapidly progressive right ventricular failure. A 45-year-old female lifelong non-smoker with no significant past medical history presented with a two-week history of cough, pleuritic chest pain, and worsening fatigue. CT pulmonary angiography was negative for central macrovascular pulmonary

VenoArterial Extracorporeal Membrane Oxygenation in Cardiac Arrest Suspected due to Massive Pulmonary Embolism: A Case Report.

INTRODUCTION: High-risk pulmonary embolism (PE) is an uncommon but potentially reversible cause of out-of-hospital cardiac arrest, frequently presenting as pulseless electrical activity (PEA). Early identification and multidisciplinary intervention are critical, yet confirmatory imaging is often delayed. Current guidelines recommend consideration of venoarterial extracorporeal membrane oxygenation (VA-ECMO) support in specialized centers when high-risk PE is suspected and the patient presents with cardiogenic shock or cardiac arrest. CASE REPORT: We describe a 50-year-old woman with a remote

Pulseless electrical activity versus asystole as initial pulseless rhythm in children: challenging convention regarding non-shockable rhythms.

BACKGROUND: Pulseless electrical activity (PEA) and asystole are the predominant initial rhythms in pediatric in-hospital cardiac arrest (IHCA) and are often grouped as "non-shockable." Whether they represent distinct entities in children remains unclear. METHODS: We analyzed pediatric (<18 years) IHCA cases from the American Heart Association Get With The Guidelines-Resuscitation (GWTG®-R) registry (2000-2023). Patients with an initial rhythm of PEA or asystole were included. Outcomes were return of spontaneous circulation (ROSC), restoration of circulation (ROC, defined as ROSC or ROC with

Impact of nurse empowerment for early adrenaline administration for in-hospital cardiac arrest resuscitation: A pre-post intervention study.

BACKGROUND: Timely administration of adrenaline during in-hospital cardiac arrest (IHCA) with non-shockable rhythms is recommended, yet delays are common when drug administration depends on physician authorisation. AIM: To evaluate whether empowering nurses to administer the first dose of adrenaline could reduce delays and improve outcomes. METHODS: In this study of prospectively collected registry data, the primary outcome of time to first dose of adrenaline in IHCA events with non-shockable rhythms (asystole/pulseless electrical activity) was compared between the pre-intervention phase (Se

The Arrest Is a Trajectory, Not a Snapshot: Rethinking Point-of-Care Ultrasound During Cardiac Arrest.

Point-of-care ultrasound (POCUS) is now routine during resuscitation, and clinicians may interpret a single intra-arrest finding, such as cardiac standstill, pseudo-pulseless electrical activity (PEA), or organized motion, as a fixed description of the patient's physiological state. This perspective argues that this assumption deserves scrutiny. The physiological state during cardiac arrest may evolve over time; cardiac mechanical activity may follow a trajectory that unfolds over minutes or a longer resuscitation. Published intra-arrest POCUS protocols generally define when and how imaging sh

Pediatric Out-of-Hospital Cardiac Arrest in a Physician-Staffed EMS System: A 13-Year Retrospective Descriptive Study from Southern Italy.

Background: Pediatric out-of-hospital cardiac arrest (OHCA) is rare and associated with poor outcomes. Evidence from physician-staffed EMS systems remains limited. This study aimed to describe the incidence, presenting rhythms, EMS response intervals, and outcomes of pediatric OHCA, and to describe incidence, presenting rhythms, EMS response intervals, and prehospital outcomes in a local physician-staffed EMS system. Methods: We conducted a retrospective study of all pediatric (0-17 years) OHCA cases managed by the ASL Lecce physician-staffed EMS (southern Italy) between 2013 and 2025. Data we

Unintentional fatal overdose of veterinary levamisole in a human subject.

Cases of levamisole overdose without coingestants in human subjects are rare. Existing knowledge of human toxicity depends in part on records of adverse events of therapeutic use such as nausea, vomiting, headache, and rarer central nervous system effects. Much of our knowledge also comes from levamisole's role as an adulterant in cocaine, where it is known to have immune modulatory properties and is associated with vasculitis. The few cases of levamisole-only toxicity are either non-fatal or lack clinical details. We present the first case of a fatal, oral ingestion of levamisole in a patient

Prehospital factors associated with return of spontaneous circulation after nonshockable out-of-hospital cardiac arrest: a retrospective analysis of the Polk County Fire Rescue CARES registry.

BACKGROUND: Non-shockable out-of-hospital cardiac arrest (OHCA), including pulseless electrical activity (PEA) and asystole, is associated with poor outcomes. Identifying prehospital factors associated with return of spontaneous circulation (ROSC) may inform emergency medical services (EMS) quality-improvement priorities. METHODS: We performed a retrospective analysis of a Polk County Fire Rescue Cardiac Arrest Registry to Enhance Survival (CARES) export. Patients whose first monitored rhythm was PEA or asystole were included. Outcomes were any documented ROSC and sustained ROSC through the e

Pseudo-Pulseless Electrical Activity With Anaphylaxis in a Pediatric Surgical Patient: A Case Report.

Pseudo-pulseless electrical activity (p-PEA) is a clinical condition where cardiac output drops severely enough to produce a non-palpable pulse in the setting of organized electrical activity of the heart, but imaging or monitors display evidence of cardiac motion. It may be seen in shock states such as anaphylactic shock. We describe a case where a previously healthy teenage patient presenting for operative removal of orthopedic hardware developed p-PEA after anaphylaxis to antibiotics. Despite loss of pulses and severe mottling of the skin, the patient's pulse oximetry (plethysmography) and

A Prospective Randomized Controlled Simulation Study to Test a Proof-of-Concept Real-Time Periprocedural Decision Support System to Aid in Timely Diagnosis and Intervention on Patient Deterioration.

OBJECTIVES: The recent pilot simulation study by Greenberg and colleagues introduced a proof-of-concept audio-visual support system for anesthesiology professional management of perioperative patient deterioration. The results demonstrated that real-time decision support for providers may enhance diagnostic accuracy/efficiency. In collaboration with Greenberg and colleagues, the authors from Massachusetts General Brigham (MGB) similarly developed a realistic operating room simulation environment at Massachusetts General Hospital (MGH) Learning Center. Here, the ability of anesthesiology traine

Resuscitation following emergency room clamshell thoracotomy and cardiac laceration repair.

The resuscitative thoracotomy (RT) is a time-sensitive procedure performed on trauma patients with impending fatality, particularly those with penetrating thoracic injuries. We present a case of a male in his twenties with pulseless electrical activity following a gunshot wound (GSW) to the right chest. This patient experienced rapid haemodynamic deterioration in concurrence with signs of cardiac tamponade, leading to the performance of a clamshell thoracotomy, followed by a pericardiotomy, and subsequent myocardial repair. Direct haemorrhage control, cardiac massage and rapid transfusion achi

Sudden Cardiac Arrest in Proximal Femur Fracture: The Role of Admission Blood Parameters.

Proximal femur fractures in the elderly are associated with high mortality rates due to reduced physiological reserve and a high risk of thromboembolic events, where admission laboratory parameters can serve as early indicators of clinical vulnerability. This case highlights a catastrophic clinical deterioration in a 92-year-old man who was admitted following a fall resulting in a displaced right intertrochanteric fracture. On admission, the patient was hemodynamically stable, but laboratory findings revealed an elevated neutrophil-to-lymphocyte ratio (NLR) of 10.1 and significant hyponatremia

Incidence and risk factors for cardiac arrest following upper cervical spine fractures in the elderly after minor trauma.

INTRODUCTION: Cardiac etiologies are one of the main causes for out of hospital cardiopulmonary arrest (OHCA). Other etiologies might be easily overlooked, leading to delayed causal treatment. One such representative is neurogenic shock following upper cervical spine fractures after minor impact trauma in the elderly with little data on prevalence and risk factors, a drawback that we address in the present study. MATERIAL AND METHODS: Between 01/2013 and 12/2023, all patients ≥ 65 years with a fracture of the first and/or second vertebra after minor impact trauma were retrospectively reviewed

The impact of emergency intubation on surgical outcomes in patients with acute aortic dissection AADA (DeBakey I). Is a prompt treatment justified?

INTRODUCTION: An acute aortic dissection Stanford type A (AADA) is a life-threatening disease and one of the urgent emergencies in cardiovascular surgery. Furthermore, mortality increases when further risk factors like malperfusion, cardiac tamponade or preoperative cardiopulmonary resuscitation (CPR) occur. This study retrospectively evaluates the impact of emergency intubation, performed prior to hospital admission, on early and long-term surgical outcomes in patients with acute aortic dissection Stanford type A (AADA, DeBakey Type I). METHODS: Between January 2000 and January 2018, 430 pat

Severe Lactic Acidosis in Decompensated Cirrhosis Despite Nondiagnostic Imaging.

Lactic acidosis is a strong predictor of mortality in cirrhosis, reflecting both impaired hepatic clearance and systemic tissue hypoxia. We describe a 38-year-old man with decompensated alcohol-associated cirrhosis who developed severe lactic acidosis despite stable hemodynamics and initially nondiagnostic vascular imaging. Repeat computed tomography demonstrated rectal wall thickening with mesenteric fat stranding, concerning for ischemic injury in the setting of a low-flow state. The patient subsequently experienced pulseless electrical activity arrest and died. This case highlights the prog

Exploration of Arrhythmia Burden in Cardiac Amyloidosis Using Implantable Loop Recorders: The EXCALIBUR Study.

BACKGROUND: Cardiac amyloidosis (CA) is a progressive infiltrative cardiomyopathy associated with conduction disease and arrhythmias, although their true burden and relationship with disease phenotype remain incompletely defined. OBJECTIVES: The purpose of this study was to prospectively characterize arrhythmic burden using implantable loop recorders and explore associations with amyloid subtype and disease characteristics. METHODS: In this prospective single-center observational study, 110 treatment-naïve patients with a new diagnosis of transthyretin amyloid cardiomyopathy (ATTR-CM) or lig

Rapid Interval Development of a Left Hepatic Artery Pseudoaneurysm During Acute Interstitial Edematous Pancreatitis.

Visceral artery pseudoaneurysms are uncommon but potentially life-threatening vascular complications of acute pancreatitis. They are most frequently reported in association with necrotizing or chronic pancreatitis and typically develop weeks after disease onset. Hepatic artery pseudoaneurysms are particularly rare. We report a 70-year-old man with presumed idiopathic acute interstitial edematous pancreatitis whose serial contrast-enhanced computed tomography (CT) examinations documented interval development of a left hepatic artery pseudoaneurysm within four days of presentation despite the ab

Pneumoperitoneum Following Cardiopulmonary Resuscitation in a Dog.

A 14-year-old intact female mongrel dog presented for periodontal disease underwent anesthesia for dental procedures. During surgery, the dog developed cardiopulmonary arrest (CPA) characterized by a sudden drop in end-tidal carbon dioxide and pulseless electrical activity. Cardiopulmonary resuscitation was initiated and return of spontaneous circulation was achieved after 20 min. Post-resuscitation imaging revealed moderate pneumoperitoneum, hepatic portal venous gas, and gas within the caudal vena cava without evidence of gastrointestinal rupture or peritonitis. Takotsubo syndrome triggered by perioperative and post-resuscitation stress: Case report.

Takotsubo syndrome or "broken heart syndrome," is an uncommon but increasingly recognized condition that can mimic acute coronary syndrome and is characterized by transient regional left ventricular dysfunction, often with apical ballooning. Perioperative physiological stress, catecholamine surges, and vasopressor exposure are recognized triggers. This case describes suspected perioperative takotsubo syndrome in a high-risk surgical patient and emphasizes the need for vigilance and early recognition. A 66-year-old woman with hypertension, type B aortic dissection, prior stroke, carotid disease
