BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) is a resource-intensive treatment for refractory out-of-hospital cardiac arrest (OHCA), underscoring the importance of identifying patients with favorable neurological outcomes. We investigated whether waveform changes from initial shockable cardiac rhythm to alternative cardiac rhythm upon hospital arrival can predict the outcomes of patients undergoing ECPR for OHCA.
METHODS: This secondary analysis of the SAVE-J II study included patients with OHCA who received ECPR at 36 emergency departments in Japan. We identified patients

Objective: We sought to assess variations in pulse oximetry waveform amplitude (ΔP) and peak values (ΔS) separately during passive leg raising (PLR) and challenge plus maintenance crystalloid volume resuscitation over time in mechanically ventilated (MV) patients in shock. Methods: Variables were recorded and analayzed using previously described techniques. Findings were compared between the following: at baseline, during passive leg raising (PLR), with 0.9% normal saline administration (or removal), and applying tidal volume (Vt), peak, and mean airway pressure (Paw,peak and Paw,mean, respect

Background/Objectives: During cardiopulmonary resuscitation (CPR), patients must be checked for signs of return of spontaneous circulation (ROSC). Point-of-care ultrasound (POCUS) may be more reliable for detecting the ROSC. We investigated whether a POCUS pulse check algorithm could be used in compliance with the CPR guidelines. Methods: This was a prospective controlled and blinded multicentre manikin study involving staff from two tertiary clinical centres and their emergency medical services. A standard operating procedure for POCUS pulse checks during CPR was evaluated using a simulator i

BACKGROUND: Despite advances in trauma care, traumatic cardiac arrest (TCA) shows significantly poorer outcomes compared to non-traumatic cardiac arrest, with mortality rates exceeding 96%. However, no standardized protocol exists for appropriate cardiopulmonary resuscitation (CPR) duration in TCA. This study aimed to establish evidence-based CPR duration thresholds and identify factors associated with return of spontaneous circulation (ROSC) in TCA patients.
METHODS: We conducted a retrospective observational study using a single-centre trauma registry of adult patients with TCA between Janu

There is a lack of established methods to explore the etiology of sudden cardiac arrest. We herein report sudden cardiac arrest as pulseless electrical activity in a young woman triggered by coronary artery spasm. There were no health issues except for situs inversus totalis, but the patient experienced sudden cardiac arrest after suffering chest pain. ST-segment depression was observed in leads V3-V6 on electrocardiography. However, urgent angiography did not reveal any coronary stenosis. Cardiac magnetic resonance imaging (MRI) revealed myocardial edema localized to the inferior and posterio

Metformin-associated lactic acidosis (MALA) is a rare but life-threatening complication with mortality rates exceeding 10-30%. While renal replacement therapy (RRT) remains the cornerstone of treatment, mechanical circulatory support may be necessary in cases complicated by severe cardiovascular dysfunction. We report a case of severe MALA with unprecedented metabolic derangement successfully treated with veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and continuous renal replacement therapy (CRRT). A 39-year-old male with type 2 diabetes mellitus presented with altered mental sta

The Emergency Department (ED) serves as a critical entry point for patients with life-threatening conditions, yet mortality within this setting remains a significant clinical and epidemiological concern. This retrospective monocentric study investigates the primary causes of death in the ED of Emergency Municipal Hospital, Timişoara, Romania, over a five-year period (2019-2023). A total of 63 cases were analyzed, integrating clinical data at the time of presentation with post-mortem pathological findings to ensure diagnostic accuracy. Results indicate that irreversible cardiac arrest (ICA) con

Here we describe the evidence for point-of-care ultrasonography application in cardiac arrest. Most commonly, transthoracic echocardiography and transesophageal echocardiography imaging may identify reversible etiologies for arrest, guide chest compression quality, and provide prognostic information. Many arrest ultrasound protocols have been described, and emphasize qualitative assessment of cardiac and respiratory pathology while minimizing disruption to chest compressions.

BACKGROUND: Painless acute pancreatitis (PAP) is a slowly progressive disease that involves inflammation, scarring, and thickening of pancreatic cells, which can happen due to either alcohol, idiopathic, or genetic. Clinicians usually miss PAP due to lack of pain and additional symptoms of hypotension and fever can lead to an infectious work-up instead. In this case report, we discuss the importance of the rapid discovery of this condition to prevent devastating complications like diabetes, necrotizing pancreatitis, or even death.
CASE SUMMARY: A 47-years old male with past medical history of

STUDY OBJECTIVES: Ventricular fibrillation (VF) is traditionally identified on ECG but echocardiography can visualize myocardial fibrillation. The prevalence and importance of occult VF defined as a nonshockable ECG rhythm but VF by echocardiography is unknown.
METHODS: In this multicenter, prospective study, emergency department patients presenting following out-of-hospital cardiac arrest were eligible for inclusion if echocardiography and ECG were performed simultaneously. Recorded echocardiography and ECG were interpreted separately by physicians blinded to all patient and resuscitation in

Pulseless electrical activity (PEA) presents with electrical cardiac activity without a pulse and often carries a poor prognosis. Pseudo-PEA mimics true-PEA but has residual cardiac motion detectable by ultrasound. This pilot study suggests thoracic transthoracic impedance (TTI), a defibrillator-based measure, may help differentiate pseudo-PEA from true-PEA. Among 10 reviewed cases, TTI showed waveform fluctuations only in pseudo-PEA, indicating potential for improved identification and targeted resuscitation strategies.

OBJECTIVES: A Prague out-of-hospital cardiac arrest (OHCA) study has demonstrated that an invasive approach (early transport to the hospital, extracorporeal cardiopulmonary resuscitation [ECPR]) is a feasible and effective treatment strategy in refractory OHCA. This post hoc analysis of the Prague OHCA study aimed to stratify the prognosis of patients according to the detailed course of heart rhythm during prehospital and early hospital periods.
DESIGN, SETTING, AND PATIENTS: This analysis included all 256 patients (median age 58, 17% females) randomized to the Prague OHCA study. The sequence

Background and Clinical Significance: Fulminant pulmonary embolism (PE) leading to an out-of-hospital cardiac arrest (OHCA) is associated with a high mortality rate and cardiopulmonary resuscitation (CPR) frequently failing to achieve return of spontaneous circulation (ROSC). Extracorporeal CPR (eCPR) has emerged as a potential life-saving intervention. Case Presentation: A 66-year-old woman suffered an OHCA due to massive PE, presenting with pulseless electrical activity (PEA). After 90 min of pre- and in-hospital CPR without sustained ROSC, venoarterial extracorporeal membrane oxygenation (v

BACKGROUND: The decision to terminate cardiopulmonary resuscitation (CPR) remains challenging, particularly for patients with non-shockable rhythms (asystole/pulseless electrical activity). This study evaluated the role of serial point-of-care ultrasound (POCUS) in predicting the termination of resuscitation by analyzing cardiac standstill duration. METHODS: A prospective cohort study was conducted from June 2024 to March 2025 in two academic hospitals. Adult patients (n = 154) with non-traumatic cardiac arrest (out-of-hospital or in-hospital) and non-shockable rhythms were included. Serial PO

OBJECTIVES: This case series highlights the utility of Point of Care Ultrasound (POCUS) during cardiac arrest pulse checks, and how this tool can be used to identify Pulseless Rhythm with Echocardiographic Motion (PREM), also known as pseudo-pulseless electrical activity (PEA), and the potential changes in prehospital resuscitation management based on that finding.
METHODS: We've documented four cases of PREM identification in the prehospital field to draw attention to this phenomenon and ideally serve as a blueprint for other agencies to adopt PREM-specific protocols.
RESULTS: This case ser

BACKGROUND: Over 70% of patients with out-of-hospital cardiac arrest (OHCA) present with a non-shockable initial rhythm, asystole or pulseless electrical activity (PEA), which is associated with lower survival than a shockable initial rhythm. Advanced airway management and oxygen delivery are important OHCA interventions, but their impact based on different presenting rhythm groups are incompletely defined. Our objective was to determine if advanced airway strategy is associated with outcomes in shockable, PEA and asystolic OHCA.
METHODS: We performed a secondary analysis of data collected fr

We present the case of a 79-year-old male with iatrogenic adrenal insufficiency on chronic hydrocortisone who presented with acute hypoxic respiratory failure due to severe bilateral pneumonia. He underwent rapid sequence intubation with IV etomidate and rocuronium. Within hours, he developed worsening hypotension, progressing to pulseless electrical activity cardiac arrest. Return of spontaneous circulation was achieved after two rounds of CPR and epinephrine. Despite vasopressor support, hypotension persisted until a 100 mg IV bolus of hydrocortisone was administered, leading to rapid hemody

Elective intravenous infusion therapies are increasingly offered in outpatient settings, often without adequate physician oversight or protocols for safety. We describe a case of accidental iatrogenic injury following an intravenous infusion at a medical spa. An elderly woman with a solitary kidney presented to the ED with chest tightness, acute shortness of breath, and headache shortly after receiving an intravenous vitamin infusion in a liter of sterile water, followed by half a liter of normal saline. She rapidly deteriorated, developing flash pulmonary edema and pulseless electrical activi

AIM: Shockable rhythm on initial electrocardiogram is a predictor of favorable neurological outcomes of out-of-hospital cardiac arrest in patients undergoing extracorporeal cardiopulmonary resuscitation (ECPR). The present study evaluated the impact of conversion from shockable rhythm to pulseless electrical activity (PEA) before ECPR on patient outcomes.
METHODS: In this secondary analysis of the data from SAVE-J II, a retrospective multicenter registry including 36 participating institutions in Japan, patients with initial shockable rhythm were categorized into those with conversion to PEA
