BACKGROUND: The benefit of expedited intra-arrest transport with ongoing resuscitation versus more extended on-scene resuscitation for refractory out of hospital cardiac arrest (OHCA) is uncertain. We aimed to determine whether expedited intra-arrest transfer to hospital in adults with refractory OHCA improves favourable neurological outcomes.
METHODS: We conducted a prospective, parallel, multi-centre, open-label randomised, superiority trial across greater Sydney, NSW, Australia. Patients aged 18-70 years with a witnessed OHCA of presumed medical cause, bystander cardiopulmonary resuscitati

BACKGROUND: Selecting appropriate patients for extracorporeal cardiopulmonary resuscitation (ECPR) in cases of out-of-hospital cardiac arrest remains challenging, particularly for those with initial nonshockable rhythms. Our aim is to develop a prediction score to identify suitable ECPR candidates in patients with initial nonshockable rhythm.
METHODS: The data were sourced from the SAVE-J II (Study of Advanced Life Support for Ventricular Fibrillation With Extracorporeal Circulation in Japan II) study, a retrospective multicenter observational study in Japan. Included were adult patients with

Photodynamic bone stabilization (PBSS) is a minimally invasive technique used in patients with metastatic bone disease who are poor traditional surgery candidates. Although the procedure has a high success rate, we present a fatal case of cardiopulmonary collapse following balloon insufflation during PBSS. The patient had advanced malignancy and preexisting cardiopulmonary conditions and ultimately experienced pulseless electrical activity and hypoxemia. We propose that the likely etiology is embolic debris from medullary reaming. This case highlights the need for perioperative risk stratifica

Plasmodium falciparum malaria remains a major cause of morbidity and mortality, particularly in endemic regions. We report the case of a 21-year-old male with recent travel to an endemic area (Guapi, Colombia), who presented with febrile symptoms, severe respiratory distress, and oxygen saturation below 75%, necessitating orotracheal intubation. During the procedure, he developed pulseless electrical activity cardiac arrest, achieving return of spontaneous circulation after advanced resuscitation. Diagnosis was confirmed by thick blood smear, demonstrating P. falciparum infection. The patient

The term cardiopulmonary resuscitation-induced consciousness (CPRIC) describes the presence of "awareness" or "consciousness" during CPR, i.e. the at least partially conscious (co)experience of one's own resuscitation measures. Depending on the circumstances surrounding the resuscitation, including the medication administered, this may or may not be accompanied by a later memory of the event (= awareness of CPR). In the literature it is unanimously criticized that this is a little-noticed phenomenon and that there are as yet no generally accepted guidelines for dealing with it with or without

BACKGROUND: Anaphylaxis caused by intravenous acetaminophen is extremely rare, but a few case reports have identified mannitol, an excipient, as the causative component. Since mannitol is widely present in medications and foods, distinguishing the true antigen is essential to prevent recurrence.
CASE PRESENTATION: A 67-year-old woman developed anaphylaxis with pulseless electrical activity during ophthalmic surgery after intravenous administration of acetaminophen (Acelio®). Allergy testing revealed positive reactions to both Acelio® and acetaminophen in skin tests and the basophil activation

OBJECTIVES: The use of extracorporeal cardiopulmonary resuscitation (ECPR) for out-of-hospital cardiac arrest (OHCA) is increasing, although including patients with non-shockable rhythms remains controversial and data addressing the influence of rhythm remains scarce. This study aimed to assess the impact of initial rhythm, its conversion, and the underlying cause of arrest on ECPR outcomes in a real-life patient cohort.
METHODS: A cohort of 281 consecutive patients undergoing ECPR for OHCA between 2016 and 2023 was retrospectively stratified based on initial and admission rhythm as well as c

Cardiac arrest is a life-threatening condition with a high mortality rate, necessitating prompt recognition and treatment of reversible causes to enhance patient survival. Point-of-care ultrasound (POCUS) has emerged as a useful tool that contributes to optimizing resuscitative efforts. This imaging modality offers real-time visualization that assists in detecting reversible causes such as cardiac tamponade, pulmonary embolism, tension pneumothorax and hypovolemia. This review aims to explore the expanding role of ultrasound in the assessment and management of cardiac arrest, emphasizing its u

BACKGROUND: Right-sided interatrial septal pouches (RSSPs) are an uncommon anatomical variant with unclear clinical significance.
CASE SUMMARY: We present a unique case of a patient who was found to have a RSSP thrombus following an initial episode of pulseless electrical activity cardiac arrest, likely due to massive bilateral pulmonary embolism.
DISCUSSION: Existing literature on atrial septal pouches predominantly focuses on left-sided presentations and their potential clinical implications. Documented cases of RSSP are limited.
TAKE-HOME MESSAGE: RSSPs are uncommon anatomical variants t

There are limited methods of differentiating between pulseless electrical activity (PEA) and pseudo-PEA in both prehospital and early hospital settings. According to the European Resuscitation Council (ERC) guidelines, point-of-care ultrasound (POCUS) is a rapid and effective method for differentiating between PEA and pseudo PEA. In addition, ERC guidelines recommend the use of POCUS in cases of non-shockable rhythms for the diagnosis of potentially reversible causes of sudden cardiac arrest (SCA). Here, we present the case of a 73-year-old patient who experienced sudden cardiac death due to a

Ultrasound is increasingly utilized during cardiac arrest in adult populations. The European Society of Pediatric and Neonatal Intensive Care and the American Academy of Pediatrics have published guidelines on the use of point-of-care ultrasound (POCUS) in neonates and children. However, there were no specific recommendations for its use during the Neonatal Resuscitation Program (NRP) algorithm. In this narrative review, we map the existing evidence to evaluate its role, potential benefits, and limitations of POCUS integration during neonatal resuscitation. The limited available studies, mostl

INTRODUCTION: Coronary artery disease is uncommon in adults under the age of 35, and studies show a lower incidence in women of this age group. Physicians should suspect myocardial infarction in all patients who present with cardiac arrest and a shockable rhythm.
CASE REPORT: We report a case of a 34-year-old female who presented after return of spontaneous circulation following both pulseless electrical activity and ventricular fibrillation. The initial emergency department 12-lead electrocardiogram (ECG) demonstrated ST-segment elevation in the anterior precordial leads. The second, more no

BACKGROUND: Survival rates after out-of-hospital cardiac arrest (OHCA) are lower at night than during the day. Cold exposure may stimulate the sympathetic nervous system and provoke and sustain ventricular tachycardia (VT)/ventricular fibrillation (VF). Diurnal temperature changes and sympathetic stimulation may explain these.
OBJECTIVE: The aim of this study was to investigate our hypothesis that the colder the ambient temperature, the worse the neurologic outcome in VT/VF OHCA patients.
METHODS: We used the All-Japan Registry (2005-2010) combined with atmospheric temperature data and enrol

BACKGROUND: Perioperative cardiac arrest after pancreaticoduodenectomy is uncommon but often catastrophic. Hypokalemia is a modifiable risk factor for malignant arrhythmia, particularly in insulin-treated diabetic patients exposed to bowel preparation and fasting. We report one of the few well-documented cases demonstrating a rapid decrease in serum potassium from a normal value to life-threatening hypokalemia within 72 h. CASE PRESENTATION: A 67-year-old woman with type 1 diabetes and hypertension underwent elective laparoscopic pancreaticoduodenectomy. The serum potassium concentration measu

INTRODUCTION: Adrenaline provides inotropic, chronotropic and vasopressor effects and is a cornerstone drug during cardiopulmonary resuscitation. This study of in-hospital cardiac arrest aimed to investigate the effect of adrenaline on the return of spontaneous circulation (ROSC) in primary pulseless electrical activity (PEA).
METHOD: In-hospital cardiac arrests episodes at St. Olav University Hospital (Norway) were prospectively registered between 2018 and 2022, among these were 73 episodes with primary PEA. Time of adrenaline administration was obtained with minutes precision by reviewing t

Out-of-hospital cardiac arrest (OHCA) is a leading cause of death worldwide, and detecting the return of spontaneous circulation (ROSC), which essentially relies on the discrimination between pulseless electrical activity (PEA) and pulse-generating rhythm (PR), is critical for patient survival. ECG-based automatic PEA/PR classifiers proposed to date, often show poor performance due to the significant similarities both rhythms present in the ECG pattern. The high temporal resolution cerebral oximetry signal obtained through near-infrared spectroscopy reflects fluctuations correlated with the QR
