A comparison of standard cardiopulmonary resuscitation and active compression-decompression resuscitation for out-of-hospital cardiac arrest. French Active Compression-Decompression Cardiopulmonary Resuscitation Study Group.

BACKGROUND: We previously observed that short-term survival after out-of-hospital cardiac arrest was greater with active compression-decompression cardiopulmonary resuscitation (CPR) than with standard CPR. In the current study, we assessed the effects of the active compression-decompression method on one-year survival. METHODS: Patients who had cardiac arrest in the Paris metropolitan area or in Thionville, France, more than 80 percent of whom had asystole, were assigned to receive either standard CPR (377 patients) or active compression-decompression CPR (373 patients) according to whether

Evaluation of biphasic transthoracic defibrillation in an animal model of prolonged ventricular fibrillation.

OBJECTIVES: To determine whether a biphasic defibrillation waveform (BDW) would produce a superior rate of converting prolonged ventricular fibrillation (VF) into a perfusing rhythm and delay the occurrence of asystole and/or pulseless electrical activity (PEA) during the resuscitation attempt, when compared with a monophasic defibrillation waveform (MDW). METHODS: The authors performed a prospective, randomized, blinded experiment using an established swine model of prolonged VF. Thirty-four mixed-breed domestic swine (mean mass 22.9 kg) were sedated (ketamine/xylazine), anesthetized (isoflu

Out-of-hospital cardiopulmonary resuscitation: a population-based Norwegian study of incidence and survival.

The Trondheim region's (315 km2, population 154,000) emergency medical service (EMS) provides advanced cardiac life support (ACLS) with combined paramedic and physician response. This EMS system is commonly employed in Norway, yet no population based study of outcome in cardiac arrest has been published to date. This retrospective study reports incidence and outcome from every attempted out-of-hospital cardiopulmonary resuscitation (CPR) during 1990 through 1994 according to the Utstein template. Information on the patient's pre-morbid conditions and final outcome was obtained from hospital re

Cardiovascular collapse associated with extreme iatrogenic PEEPi in patients with obstructive airways disease.

Chronic obstructive pulmonary disease (COPD) is commonly associated with positive alveolar pressure at end-expiration (intrinsic PEEP or PEEPi) caused by a prolonged expiratory time constant. Positive pressure ventilation (PPV) with large tidal volumes and high ventilatory frequencies may cause pulmonary hyperinflation, with increases in intrathoracic pressure and cardiopulmonary effects. We report two cases, one of fatal pulseless electrical activity, the other of life-threatening hypotension, both during vigorous manual PPV, in patients with severe COPD. This phenomenon has been well-recogni

Bolus thrombolytic infusions during CPR for patients with refractory arrest rhythms: outcome of a case series.

Thrombolytic therapy has been accepted in the treatment of acute myocardial infarction. Given historical recommendations that thrombolytic therapy is contraindicated in patients receiving CPR, its potential clinical benefit for facilitating conversion of rhythm in patients in refractory cardiac arrest has not been investigated. We present three case reports in which patients with confirmed acute myocardial infarction had a witnessed cardiac arrest in the ED. Standard Advanced Cardiac Life Support measures failed in all three cases. A bolus infusion of tissue plasminogen activator was administe

Accurate recognition and effective treatment of ventricular fibrillation by automated external defibrillators in adolescents.

OBJECTIVES: To evaluate the accuracy and efficacy of automated external defibrillators (AEDs) in patients <16 years old. BACKGROUND: AEDs are standard therapy in out-of-hospital resuscitation of adults and have led to higher success rates. Their use in children and adolescents has never been evaluated, despite recommendations from the American Heart Association that they be used in children >8 years of age. METHODS: This was a retrospective cohort study of children <16 years old who underwent out-of-hospital cardiac resuscitation and on whom an AED was used during the resuscitation. The sett

Effects of adding links to "the chain of survival" for prehospital cardiac arrest: a contrast in outcomes in 1975 and 1995 at a single institution.

STUDY OBJECTIVE: The concept of a "chain of survival" to improve outcome from prehospital cardiac arrest has been defined and promulgated over the last two decades. The purpose of this study was to compare outcomes of prehospital cardiac arrest in 1975 and 1995 at a single institution. METHODS: This longitudinal, before-after study compares published data collected at our municipal, tertiary care in 1974-1975 with data collected prospectively in 1995. The 1975 study group served as control subjects (n = 120). We enrolled an equal number of consecutive patients who met inclusion criteria in th

Epidemiology and survival rate of out-of-hospital cardiac arrest in north-east Italy: The F.A.C.S. study. Friuli Venezia Giulia Cardiac Arrest Cooperative Study.

The results of the first epidemiological, prospective, multicentric study on cardiac arrest in a geographical Italian region are reported. On 708 consecutive cardiac arrests, 438 underwent cardiopulmonary resuscitation (CPR). Of these, 344 were identified of cardiac aetiology. The underlying initial rhythm was: 166 asystole (48.3%), 104 ventricular fibrillation (30.2%), 74 pulseless electrical activity (21.5%). The best outcome occurred in patients whose cardiac arrest was witnessed by the EMS (49% return of spontaneous circulation (ROSC), 21% hospital discharge). When cardiac arrest was witne

In-hospital first-responder automated external defibrillation: what critical care practitioners need to know.

Despite the development and widespread implementation of Basic Life Support and Advanced Cardiac Life Support, the percentage of patients who survive in-hospital cardiac arrest has remained stable at approximately 15%. Although survival rates may approach 90% in coronary care units, survival rates plummet outside of these units. The lower survival rates for cardiac arrest that occur outside of the coronary care unit may relate to the time elapsed between the onset of ventricular fibrillation and first defibrillation. The advent of automated external defibrillators has made it possible to decre

Vasopressin combined with epinephrine decreases cerebral perfusion compared with vasopressin alone during cardiopulmonary resuscitation in pigs.

BACKGROUND AND PURPOSE: It is unknown whether a combination of vasopressin and epinephrine may be superior to vasopressin alone by targeting both nonadrenergic and adrenergic receptors. METHODS: After 15 minutes of cardiac arrest (13 minutes of ventricular fibrillation and 2 minutes of pulseless electrical activity) and 3 minutes of chest compressions, 16 animals were randomly treated with either 0.8 U/kg vasopressin (n = 8) or 0.8 U/kg vasopressin combined with 200 microg/kg epinephrine (n = 8). RESULTS: Comparison of vasopressin with vasopressin and epinephrine at 90 seconds and 5 minutes

[Extrahospital cardiac arrest: the experience of Mestre Emergency Service].

BACKGROUND: Out-of-hospital cardiac arrest (AC) is one of the leading causes of death in industrialized countries. AC-related mortality can be reduced by rapid intervention. We report the experience of the emergency medical service (EMS) of Mestre on the management of out-of-hospital AC. METHODS: We analyzed 80 cases of out-of-hospital AC observed consecutively by the EMS of Mestre from February 1996 to September 1997: 72 cases (90.0%) involved cardiac etiology and 8 (10.0%) non-cardiac etiology. The 72 cases involving cardiac etiology were divided in three groups: group A) 12 unwitnessed ACs

Prehospital pulseless, unconscious penetrating trauma victims: field assessments associated with survival.

BACKGROUND: This study was designed to determine whether out-of-hospital clinical signs could be associated with functional survival for pulseless, unconscious victims of penetrating trauma. METHODS: A retrospective review of medical data and outcome for pulseless, unconscious penetrating urban trauma victims during 1993-1994. For comparison with the penetrating study group, data for blunt pulseless, unconscious trauma victims for the same period are reported. Logistic regression, odds ratios, positive predictive values, sensitivity, and specificity were used to determine the possible associa

Outcome from out-of-hospital cardiac arrest caused by nonventricular arrhythmias: contribution of successful resuscitation to overall survivorship supports the current practice of initiating out-of-hospital ACLS.

STUDY OBJECTIVE: Studies indicate that ventricular tachycardia (VT) and ventricular fibrillation (VF) are no longer the most common rhythms initially documented in out-of-hospital sudden cardiac death. Although the outcome from asystole and rhythms designated as pulseless electrical activity (PEA) is reported as poor (approximately 1% survival), resuscitative efforts for these patients are still encouraged. The purpose of this study was to determine the potential contribution that this patient group makes to overall survivorship. METHODS: During this 2-year prospective study, all patients in

A comparison of repeated high doses and repeated standard doses of epinephrine for cardiac arrest outside the hospital. European Epinephrine Study Group.

BACKGROUND: Clinical trials have not shown a benefit of high doses of epinephrine in the management of cardiac arrest. We conducted a prospective, multicenter, randomized study comparing repeated high doses of epinephrine with repeated standard doses in cases of out-of-hospital cardiac arrest. METHODS: Adult patients who had cardiac arrest outside the hospital were enrolled if the cardiac rhythm continued to be ventricular fibrillation despite the administration of external electrical shocks, or if they had asystole or pulseless electrical activity at the time epinephrine was administered. [Interposed abdominal compression in mechanical cardiopulmonary resuscitation. Description of a clinical case].

Among the different techniques proposed to integrate the standard cardiopulmonary resuscitation (sCPR) protocol, mechanical CPR (mCPR) and interposed abdominal compression (IAC) were found to be particularly effective for the simplicity of the procedure and the significant results obtained. A case of a 54-year old male with cardiogenic shock following viral infection, in which prolonged mechanical cardiopulmonary resuscitation with interposed abdominal compression was performed, is presented. Five hours after admission in the ICU, the patient's condition worsened with subsequent cardiac arrest
