Pilot study of intravenous magnesium sulfate in refractory cardiac arrest: safety data and recommendations for future studies.

UNLABELLED: Recent case reports have evidenced a temporal association between administration of i.v. magnesium sulfate (M) and resuscitation from prolonged cardiac arrest refractory to standard (S) ACLS attempts. However, speculation has arisen that M as a vasodilator, may decrease aortic diastolic and coronary perfusion pressure (CPP), aortic systolic and cerebral perfusion pressures (CePP), which may decrease resuscitation rates and neurologic recovery, as compared to standard ACLS alone (SA). OBJECTIVE: To resolve positive beginning evidence vs. negative theory, we conducted a pilot study

Advanced Cardiac Life Support.

In brief Changes in the advanced car-brief diac life support guidelines in-elude a new classification system for interventions, training in the use of automated defibrillators, and required early defibrillation for ventricular fibrillation. The algorithms for treating ventricular fibrillation and pulseless ventricular tachycardia, pulseless electrical activity, and asystole are reviewed. Cardiac arrest caused by special circumstances-hypothermia, near drowning, trauma, and electrical injuries-presents additional treatment considerations.

Standard and higher doses of atropine in a canine model of pulseless electrical activity.

OBJECTIVE: To determine whether standard or increased doses of atropine improve the return of spontaneous circulation (ROSC) rate in a canine model of pulseless electrical activity (PEA). METHODS: A prospective, controlled, blinded laboratory investigation was performed using an asphyxial canine cardiac arrest model. After the production of asphyxial PEA, 75 dogs remained in untreated PEA for 10 minutes and then were randomized to receive placebo (group 1) or one of four doses of atropine (group 2, 0.04 mg/kg; group 3, 0.1 mg/kg; group 4, 0.2 mg/kg; group 5, 0.4 mg/kg). The hemodynamic and arterial blood gas response to asphyxiation: a canine model of pulseless electrical activity.

OBJECTIVE: Asphyxiation is a time-honored animal model for producing pulseless electrical activity cardiac arrest. To date, there has not been a detailed description of the hemodynamic and arterial blood gas response to asphyxiation in a large number of animals. Our objective was to describe a single laboratory's experience with a standardized canine model of asphyxial pulseless electrical activity arrest. METHOD: Design--Data from 4 separate research protocols using a standardized asphyxial model were retrospectively reviewed. Setting--Resuscitation research laboratory. Participants--169 mix

Efficacy of audio-prompted rate guidance in improving resuscitator performance of cardiopulmonary resuscitation on children.

OBJECTIVE: To evaluate the effect of audio-prompted rate guidance during chest compressions on the performance of cardiopulmonary resuscitation (CPR) on children. METHODS: This 24-month prospective study occurred in the pediatric intensive care units of a university hospital and a children's hospital. Intubated children with nontraumatic cardiac arrest were eligible. After placement of an infrared capnometer between the endotracheal tube and resuscitation bag, an audiotape instructed the resuscitator to perform chest compressions at 100 per minute or 140 per minute for one minute, followed by

Adrenaline-responsive electromechanical dissociation.

Electromechanical dissociation (EMD) occurs when there is no detectable blood pressure in the presence of organised electrocardiographic activity. This condition excludes pulseless ventricular tachycardia, and traditionally was thought to result from a failure of electrical depolarisation in the myocardium to elicit mechanical contraction. It is now understood from echocardiographic evidence that 'pseudo-EMD' is more common-where electrical activity is associated with myocardial contractions too weak to sustain a detectable blood pressure [1]. Three cases of EMD in association with subarachnoi

Need for sedation in a patient undergoing active compression--decompression cardiopulmonary resuscitation.

The authors report the case of a 57-year-old man with a history of ischemic heart disease who presented to the emergency department with an acute myocardial infarction and hypotension. Despite aggressive pharmacotherapy, the patient's heart rate decreased, and he developed pulseless electrical activity within 15 minutes of his arrival. Cardiopulmonary resuscitation (CPR) was begun with an active compression-decompression (ACD) device, and the patient became agitated, making purposeful movements. When ACD-CPR was discontinued for a rhythm check, the patient had no pulse and became motionless. A

Comparison of adrenergic agonists for the treatment of ventricular fibrillation and pulseless electrical activity.

The primary role of epinephrine for the treatment of ventricular fibrillation (VF) and pulseless electrical activity (PEA) is to increase blood flow to the myocardium and central nervous system and ultimately improve survival. However, despite the administration of epinephrine, survival following VF or PEA is low. In an attempt to improve outcome from VF and PEA, alternative adrenergic agonists (methoxamine, phenylephrine, norepinephrine) which have different pharmacological properties than epinephrine have been evaluated. In order to determine the role of alternative adrenergic agonists for t

Sympathetic modulation of rabbit aortic baroreceptors in vitro.

Previous studies indicate that sympathetic efferent nerves to the carotid sinus can modulate carotid sinus baroreceptor (BR) activity. There are, however, no studies that demonstrate this in the aortic arch. Since application of exogenous norepinephrine to the aortic arch can alter BR activity, we examined whether electrically stimulating the efferent innervation would likewise be effective. We used an in vitro preparation from anesthetized rabbits. This consisted of the aortic arch and sections of the aortic afferent and sympathetic efferent nerves. The aorta was perfused at a constant pulsel

High-dose corticosteroids in the treatment of pulseless idioventricular rhythm.

Pulseless idioventricular rhythm (PIVR) is a slow electrical rhythm of ventricular origin without mechanical activity, and it is associated with a mortality rate that approaches 100%. In 1976 high-dose corticosteroids were advocated as beneficial in the treatment of PIVR. A critical review of the early literature suggests that the favorable outcome may have been attributable to variables other than treatment with high-dose steroids. Recent prospective clinical trials have not confirmed the salutory effect of steroids in this dysrhythmia.

Developments in transcutaneous and transthoracic pacing during bradyasystolic arrest.

Transthoracic cardiac pacing historically has been relegated to the role of the technique of last resort in treating cardiac arrest. Recent studies have shown that this technique has a high rate of successful electrical capture, but often without mechanical activity. Survival rates have been shown to be dismal when the technique is used late in cardiac arrest. Results of several recent studies of patients paced by the transcutaneous technique have suggested that electrical capture can often be rapidly obtained in asystolic or pulseless bradycardic patients. Even though electrical capture can o

Cerebral and hemodynamic variables during cough-induced CPR in dogs.

To assess the potential of self administered, cough-induced cardiopulmonary resuscitation (CICPR) to sustain cerebral function during sudden onset ventricular fibrillation (VF), the authors studied four groups of six dogs each. Cough was simulated by: spontaneous gasping (group 1); cough elicited by bilateral electrical stimulation of the vagi (group 2); gasping with artificial glottic closure (group 3); and as a control, apnea under paralysis (group 4). Sudden onset of VF during apnea (group 4) resulted in cessation of arterial blood flow in 11 +/- 3 sec (mean +/- SD) and an isoelectric EEG i
