Acceptability and feasibility of biomarkers of airway eosinophilic inflammation for the management of preschool wheeze: a qualitative study.

OBJECTIVE: This study aimed to examine whether biomarker tests-finger-prick, skin-prick and offline fractional exhaled nitric oxide (FeNO)-are acceptable and feasible as a guide to treatment decisions, uniquely combining the perspectives of parents of preschool children with wheeze and healthcare professionals (HCPs) working in National Health Service (NHS) primary care. DESIGN: Qualitative interview study. Criterion sampling was used to recruit 16 parents from 16 families, and convenience sampling to recruit 16 HCPs (doctors and nurses) from 14 primary care NHS practices. Qualitative data we

Parental knowledge, awareness, and intended healthcare-seeking responses toward congenital heart disease in the West Bank, Palestine: a cross-sectional study.

BACKGROUND: Congenital heart disease (CHD) is a major contributor to infant morbidity and mortality worldwide. Although prenatal screening, neonatal assessment, pulse oximetry screening, and routine pediatric evaluation contribute to CHD detection, some cases may remain unrecognized until after discharge or later in infancy. Parental recognition of warning signs may support timely healthcare-seeking, but it should be considered one component of a broader diagnostic and referral pathway. This study assessed parental knowledge, awareness, and intended healthcare-seeking responses toward CHD amon

Prior-aligned frequency-domain explanations for heart sound classification: a scale-consistent attribution approach.

OBJECTIVE: Deep learning models for phonocardiogram (PCG) classification often achieve high accuracy but lack transparency in relation to clinically meaningful features. Existing explainable artificial intelligence (XAI) methods rarely ensure alignment between model attributions and cardiac physiology. METHODS: We propose Scale-Consistent Attribution (SCA), a training-time regularization method that incorporates domain knowledge into model learning. SCA aligns spectral attention with a soft clinical prior, distinguishing low-frequency heart sounds from high-frequency murmurs. The method is ev

A Dual Classifier-Regressor Architecture for Heart Sound Onset/Offset Detection.

OBJECTIVE: Identifying the first and second heart sounds (S1 and S2) from phonocardiogram (PCG) signals is an essential step in automating the diagnosis of cardiac conditions such as irregular heartbeat, valve misfunctions, and heart failure. Recent research inspired by image segmentation has shown promise in utilising deep neural networks for point-wise PCG segmentation with the support of synchronised electrocardiogram (ECG). This paper shifts the focus from point-wise segmentation to identifying the onset/offset of S1 and S2 heart sounds in the PCG signal. METHODS: We incorporate the ECG s

Non-contact Heart Sound Measurement by Defocused Speckle Imaging.

Phonocardiogram (PCG) is a graphical representation of heart sounds and murmurs, playing a vital role in the early screening and prevention of cardiovascular diseases (CVDs). In this study, we propose a first camera-stethoscope that is capable of recording PCG contactlessly, enabled by the principle of defocused speckle imaging (DSI). By illuminating the chest surface adjacent to the heart with a laser beam and employing a defocused camera to capture the reflected laser speckle interference, the subtle cardiac vibrations involved in heart sound generation are magnified and measured. By adjusti

A method for heart sound classification using sample augmentation and INDANet.

The number of deaths due to cardiovascular diseases has been steadily increasing, with the majority occurring in underdeveloped regions. Doctors in less developed regions often have limited clinical experience to diagnose these diseases by listening heart sounds, whereas artificial intelligence can serve as a valuable tool to assist them in conducting auxiliary diagnoses. However, small samples and strong noise reduce the diagnosis accuracy. Therefore, in this paper, a novel method is put forward for heart sound classification, which integrates sample augmentation and injected noise dual atten

Patterns of morbidity, multimorbidity, and mortality in aging cats: findings from seven years of the Cat Prospective Ageing and Welfare Study.

INTRODUCTION: Aging is associated with an increased risk of morbidity, multimorbidity and mortality in cats. However, longitudinal cohort studies are somewhat limited in this species. Information about the prevalence and incidence of age-related disease, combinations of morbidities, and factors associated with mortality could provide insights into the impact of aging on the welfare of domestic cats. METHODS: The prevalence and incidence of common age-related diseases, patterns of multimorbidity and causes and risk factors for mortality were investigated in a prospective longitudinal cohort st

Attention and physiological responses to task-irrelevant sounds in misophonia.

INTRODUCTION: Misophonia is a disorder in which certain sounds (known as triggers) cause a negative and often overwhelming emotional response. Common triggers include human-produced and repetative sounds such as chewing and tapping. The extent to which this response is modulated by task focus - such as whether a sound is attended or not - is largely unknown. Similarly, the extent to which these responses are specific to triggers or found, to some degree, for a wider range of stimuli is unclear. METHODS: In this study, two groups (misophonics, controls) engage in a visuo-motor task during the

Distinguishing Between Innocent and Pathologic Pediatric Heart Murmurs: A Case Report.

Pediatric heart murmurs are common findings during physical examination, but only a small number of cases require intervention. This case report highlights a routine physical examination for a 10-year-old male patient that revealed a grade III/VI harsh systolic murmur heard in all areas of auscultation and a fixed split S2. A subsequent transthoracic echocardiogram showed an atrial septal defect that required catheter-based closure. This case proved to be one of the few cases in which urgent treatment of the heart murmur is required, and it helps highlight the features of pathologic murmurs th

Exploratory clinical trial on the acoustic variability index for predicting fluid responsiveness during hepatectomy.

The acoustic variability index (AVI), derived from continuous phonocardiographic analysis of heart sounds via an esophageal stethoscope, is a novel minimally invasive method for intraoperative hemodynamic monitoring. We evaluated the ability of AVI to predict fluid responsiveness in patients undergoing hepatectomy and compared its performance with that of the conventional indices. Forty adult patients who underwent hepatectomy were enrolled in this prospective, single-center observational study. After major surgical resection and stabilization, a 500 mL crystalloid fluid challenge was administ

Northwest Axis on Electrocardiogram: Decoding an Unusual Cardiac Axis in a Young Patient.

BACKGROUND: We describe a patient with a "northwest" cardiac axis and electrocardiographic features of right ventricular (RV) pressure overload in the setting of Fontan circulation. TAKE-HOME MESSAGES: In atrioventricular canal defects with Fontan circulation, a northwest cardiac axis on electrocardiography reflects altered RV activation and remodeling, often persisting from childhood into adulthood and secondary to RV outflow tract obstruction. CASE SUMMARY: A 28-year-old woman presented for routine medical follow-up. Medical history was notable for Fontan procedure, with the last operation

Pupil-based arousal self-regulation: impact on physiological and affective responses to emotional stimuli.

Pupil-based biofeedback has been shown to enable healthy participants to volitionally control locus coeruleus-mediated arousal. The locus coeruleus is considered a critical player in the central stress circuitry and dysfunctions in the system, causing dysregulated arousal levels, have been tightly linked to neuropsychiatric disorders such as anxiety and stress-related disorders and stress-induced cardiovascular vulnerability. Yet, it remains unclear whether physiological and self-rated affective responses to emotional stimuli are influenced by volitional control of arousal levels. In this stud

Nurse-led heart murmur clinic at Sheffield Children's Hospital: a review.

Anurse-led heart murmur clinic was introduced at Sheffield Children's Hospital in 2023 to reduce waiting times, support the clinical nurse specialist (CNS) role development, and improve cost-efficiency. By triaging appropriate referrals to a CNS-led pathway with pre-arranged echocardiography and telephone follow-up, the clinic reduced wait times from 16 to 5-6 weeks and saved over £3,000 in its first year. The model demonstrates that nurse-led services can safely and effectively manage selected paediatric referrals, improve access to care, and deliver measurable service efficiencies.

Exploring the acceptability and feasibility of implementing VR relaxation in naturalistic settings.

Stress increased markedly in recent years, highlighting the need for effective and accessible relaxation strategies. Virtual Reality (VR) has emerged as a promising intervention. While most prior VR studies were conducted under controlled conditions, the present study explored the acceptability and feasibility of VR as a relaxation tool in naturalistic settings. Additionally, it explored the effects on the perceived level of relaxation and mood, and whether heart rate (HR) data obtained from consumer-grade wearables correlated with changes in psychological state. The study involved 160 partici

Classic Murmur, Unexpected Diagnosis: A Case Report and Pathophysiological Review.

A systolic murmur best heard at the aortic area with radiation to the neck is considered a hallmark of aortic stenosis. We report a patient with this auscultatory pattern in whom investigation revealed an unexpected diagnosis. Accompanying symptoms reinforced the initial suspicion, adding another layer of complexity to the diagnostic process. This case highlights the limitations of relying solely on symptom profile and murmur topography, and underscores the importance of comprehensive physical examination and diagnostic imaging in evaluating valvular heart disease.

Neural and cardiorespiratory responses in vocalization and slow breathing: Contrasting brain and autonomic responses.

Singing and vocalization practices are reportedly associated with increased relaxation, better emotional regulation, and enhanced social closeness. Here, we investigate the neural underpinnings of such effects and ask whether they are related to processes linked to the vocalization aspect or are a result of modified breathing patterns. Toning is a form of vocalization involving the expression of vocal sounds for the duration of the exhalation, often resulting in a slower breathing frequency compared to rest. It is increasingly used in therapeutic environments, but its neural underpinnings are
