DTF-STCANet: A Dual Time-Frequency Swin Transformer and ConvNeXt Attention Network for Heart Sound Classification.

Background/Objectives: Cardiovascular diseases are the leading cause of death worldwide. Therefore, early diagnosis and treatment of these diseases are of critical importance. Stethoscopes are the easiest and fastest medical devices for the initial diagnosis of cardiovascular diseases. However, interpreting heart sounds requires considerable expertise. The use of artificial intelligence in healthcare for decision support has increased and become popular recently. Methods: The popular 2016 PhysioNet/CinC Challenge dataset, consisting of phonocardiogram (PCG) signals, was used to implement the p

An empirical comparison of deep CNN architectures for heart sound classification from phonocardiogram signals.

Heart disease continues to be the world's number one killer, making low-cost, early screening a priority. A phonocardiogram (PCG), a simple recording of sounds made by the heart, provides valuable clues to conditions such as murmurs and valve defects and can be recorded cheaply using a digital stethoscope. With less accessibility to specialists, the automatic analysis of heart sounds is preferred for screening and tele-health environments. We compare five deep convolutional neural networks in this work, namely LeNet5, AlexNet, VGG16, ResNet-50 and a hybrid LSTM-CNN, on the task of separating n

Congenital Pulmonary Airway Malformation of the Left Upper Lobe Associated With Dextrocardia in a Neonate: A Case Report.

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung anomaly that may present in the neonatal period with respiratory distress, and its association with dextrocardia is exceptional, potentially leading to diagnostic challenges. We report the case of a 26-day-old male neonate, born at term, admitted for cyanosis and respiratory distress. Clinical examination revealed tachypnea (68 breaths per minute), oxygen desaturation (SpO₂ 65% in room air), and signs of respiratory distress, with cardiac auscultation suggesting right-sided heart sounds. Imaging studies demonstrated a

Upper airway microbiome interacts with GSDMB and ORMDL3 asthma risk SNPs to influence early-life wheeze risk.

BACKGROUND: Single-nucleotide polymorphisms (SNPs) in the chromosome 17q12-q21 region and, independently, early-life nasal microbiota dominated by Moraxella, Streptococcus, or Haemophilus (MSH) increase risk of chronic wheeze and asthma development. OBJECTIVE: We sought to determine whether 17q12-q21 risk SNPs and nasal microbiota interact to modulate childhood wheeze risk. METHODS: Nasal wash samples from 12-month-old infants in 2 birth cohorts, COAST (Childhood Origins of Asthma; n = 180) and URECA (Urban Environment and Childhood Asthma; n = 139), underwent 16S ribosomal RNA variable regi

AMI-SSS01 portable phonocardiographic examination with AI-assisted assessment for detecting heart failure exacerbations in home-based medical care in Japanese primary care clinics: a study protocol for a randomised controlled feasibility trial.

INTRODUCTION: The global prevalence of heart failure continues to increase, particularly in ageing populations. Many older patients receiving home-based medical care have limited access to standard diagnostic tools, such as chest radiography and echocardiography, which can delay the detection of disease progression. AMI-SSS01, an artificial intelligence (AI)-based portable phonocardiography device, enables rapid and reproducible analysis of heart sounds, potentially allowing earlier identification of heart failure exacerbations in home-based medical settings. This trial aims to evaluate the fe

Psychophysiological effects of low-frequency sound mediated by room size, room shape, and stimulus duration.

Music and sounds elicit a wide range of emotions and activate numerous psychological and physiological effects associated with the functioning of the autonomic nervous system (ANS) and involved in the maintenance of homeostasis. As such, sound interventions can play an important role in supporting human wellbeing and improve outcomes in medical patients. Psychophysiological responses are dependent on the stimulus type, frequency, duration, and spatial conditions of the acoustic environment. To arrive at a more articulate understanding of these dependencies, the effects of singing bowl sounds a

Physiological Responses to Auditory, Visual, and Auditory Imagery Triggers in Misophonia.

Misophonia is characterized by decreased tolerance to specific trigger sounds, yet the underlying physiological mechanisms remain poorly understood. Misophonic responses are also often reported to be elicited not only by the auditory triggers themselves, but also by visual cues associated with those sounds. However, direct empirical evidence for visual cues associated with auditory triggers remains limited. Moreover, it is not known if misophonic responses can be elicited in the absence of any sensory input, such as during auditory mental imagery. The present study systematically examined phys

Congenital Peritoneopericardial Diaphragmatic Hernia in a Dog: Ventral Advancement Diaphragmatic Technique for Repair.

Peritoneopericardial diaphragmatic hernia (PPDH) is a rare congenital anomaly that requires surgical treatment. Its clinical presentation varies and can affect one or more abdominal organs. This case report refers to a recently described surgical technique called ventral advancement of the diaphragm (VADT). Previously, a complete diagnostic approach was made in a 4-month-old, 6 kg (13.22 lbs.), uncastrated, mixed breed dog, with a history of cough from the time of adoption, with no known record of trauma. Physical examination revealed tachycardia, tachypnea, body condition score of 5/9, positi

Iatrogenic ventricular septal defect following papillary muscle premature ventricular contraction ablation: a case report.

BACKGROUND: Catheter ablation of monomorphic premature ventricular contractions (PVCs) in symptomatic or haemodynamically compromised patients has a Class I recommendation in cardiomyopathy and a Class IIa recommendation in patients comprising non-outflow tract PVC morphology and a physiologic anatomy according to current ESC guidelines. It is therefore widely performed and generally considered safe. Rare but clinically significant complications can occur and may present considerable diagnostic and therapeutic challenges. CASE SUMMARY: A 77-year-old woman underwent radiofrequency (RF) cathete

Adult Diagnosis of Solitary Kidney and Renal Dysplasia in a Male Born Prematurely as a Twin: A Case Report.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) are a common cause of chronic renal disease and usually manifest either antenatally or in the early years of life. Solitary Kidney with Renal Dysplasia presents early in life due to regular antenatal sonographic scans. The rarity of diagnosis being deferred until adulthood has important implications when there is prematurity and other associated congenital anomalies, such as Congenital Heart Disease. CASE PRESENTATION: We present a 23-year-old male experiencing sequential generalized edematous changes, decreased urine ou

Emergency Management of Blunt Cardiac Tamponade: A Report of Two Cases.

Blunt cardiac tamponade is an uncommon but life-threatening consequence of thoracic trauma. Early recognition and prompt surgical management are essential to reduce mortality. In Case 1, a 35-year-old male presented nine hours after a fall from 10 ft with hypotension, bradycardia, and muffled heart sounds. Extended focused assessment with sonography in trauma (EFAST) revealed pericardial effusion suggestive of tamponade. The patient was transferred to the operating room and underwent a clamshell thoracotomy with pericardial decompression, evacuating 100 mL of hematoma. He stabilized and was su

Multilevel Physiological Responses to the Hypersonic Effect: Concurrent Electroencephalographic, Heart Rate Variability, and Subjective Evaluation in Healthy Adults in a Participant-Blind Crossover Study.

Background and objective Sounds carrying inaudible high-frequency components (HFCs) above 20 kHz can alter brain activity, autonomic tone, and subjective state, a cluster of responses termed the hypersonic effect. However, only single physiological domains have been examined till now; therefore, whether central, autonomic, and subjective indices move together under one stimulus in the same participants remains unresolved. This study measured electroencephalographic (EEG), heart rate variability (HRV), and subjective responses simultaneously to characterize the layered response to HFC-containin

Acute Myocardial Infarction Secondary to Triple Vessel Coronary Artery Disease in a 31-year-old Female with Systemic Lupus Erythematosus: Case Report and Review of Literature.

Cardiovascular (CV) disease is the leading cause of mortality in systemic lupus erythematosus (SLE). The risk of myocardial infarction (MI) in SLE is twice the incidence and ten years earlier in onset than in the general population. We present the first known case in the Philippines of acute MI from triple vessel coronary artery disease (CAD) in a young female patient with SLE. This aims to increase recognition and improve preventive strategies for this rare lupus complication. A 31-year-old female with SLE for thirteen years, antiphopspholipid syndrome (APS) and controlled hypertension (HTN)

Language-supervised electrocardiogram interpretation with vision transformer.

PURPOSE: Most artificial intelligence models for electrocardiogram (ECG) interpretation rely on task-specific supervised learning, requiring dedicated labeled datasets for each diagnostic task and limiting scalability across diverse cardiac conditions. This study investigated whether vision-language learning from paired ECG recordings and summary reports could enable transferable ECG representations without task-specific supervised training. METHODS: We developed ECG-VLM, a contrastive vision-language model consisting of a Transformer-based image encoder and a text encoder, trained on 800,034

Determinants of systolic murmurs without significant valvular heart disease in an unselected Norwegian adult population: the Tromsø Study.

BACKGROUND: Systolic murmurs are common in adults without significant valvular heart disease (VHD) and are more prevalent in women than in men, but the determinants of murmur audibility remain unclear. We aimed to identify the physiological determinants of innocent murmurs and examine the basis for sex differences in murmur prevalence. METHODS: In this cross-sectional population-based study, we analysed data from 1680 participants in Tromsø7 without significant VHD who underwent echocardiography and heart sound recording. We used multivariable logistic regression with complete-case data from

A Tripartite Framework of Conflict-Induced Stress, Acoustic Hypervigilance, and Ramadan Observance as Convergent Determinants of Circadian Decoherence in Athletes.

Athletes living and training in active conflict zones face a combination of distinct biological pressures that disrupt the body's internal 24-hour clock and stress-hormone systems. The constant psychological threat of conflict keeps the body's stress-hormone axis chronically active, raising cortisol levels, heightening alertness, and making it harder to fall and stay asleep. A key problem in the existing research is that nighttime sleep disruption in conflict zones has often been incorrectly described as ordinary screen-related sleep loss. In reality, government-issued missile warning sirens a
