Multiscale analysis of heart sound signals in the wavelet domain for heart murmur detection.

A heart murmur is an atypical sound produced by blood flow through the heart. It can indicate a serious heart condition, so detecting heart murmurs is critical for identifying and managing cardiovascular diseases. However, current methods for identifying murmurous heart sounds do not fully utilize the valuable insights that can be gained by exploring different properties of heart sound signals. To address this issue, this study proposes a new discriminatory set of multiscale features based on the scaling and complexity properties of heart sounds, as characterized in the wavelet domain. Scaling

A New Heartbeat-Respiration Coupling Metric for Relaxation/Stress Monitoring.

OBJECTIVE: Identification and continuous monitoring of relaxation/stress level through easily measurable physiological signals has become an urgent need. Relaxation/stress is mainly mediated by autonomic nervous system (ANS) which can hardly be noninvasively-measured, but can be reflected in heartbeat and respiration because the human body is a strongly coupling system. However, existing metrics based on heart rate and respiration present low accuracy for relaxation assessment. Our main objective is to propose a reliable metric for relaxation/stress monitoring. METHODS: By improving the quant

Adult Patient With Systolic Murmur: An Unexpected Diagnosis of Congenital Heart Disease.

We present a case of a 50-year-old male patient with shortness of breath and progressive edema. Echocardiography demonstrated dilation of the four cavities, left ventricular dysfunction, and a patent arterial duct (PDA) with systolic predominant flow due to pulmonary hypertension. Cardiac magnetic resonance imaging (MRI) confirmed the PDA. This case is of particular interest since the typical continuous heart murmur found in PDA is lost due to equalization of the diastolic pressure between the pulmonary and aortic circulation.

Perimembranous Ventricular Septal Defect Concurrent with an Aorto-Right Ventricular Fistula Caused by a Ruptured Sinus of Valsalva in Three Dogs.

An aorto-right ventricular fistula, a rare condition in humans, is characterized by communication between the ascending aorta and the right ventricle through a defect in the aortic wall. This report describes three cases of dogs with continuous murmurs: a 6-month-old Coton de Tulear, a 5-year-old Maltese, and a 6-month-old Jindo. Notably, all of the dogs presented with no severe clinical signs. Echocardiography revealed a turbulent jet through restrictive perimembranous ventricular septal defects (VSD) during systole and aorto-right ventricular fistulas secondary to ruptured sinuses of Valsalv

Point-of-Care Ultrasound in Early Identification of Tamponade: A Case Series.

Cardiac tamponade is a life-threatening condition resulting from fluid accumulation within the pericardial sac, leading to impaired cardiac filling and output. Signs described in the literature, such as Beck's triad (hypotension, jugular venous distension (JVD), muffled heart sounds) or pulsus paradoxus can be absent especially in the early stages, particularly in non-trauma patients, making early diagnosis challenging. In this case series, we present three patients who developed tamponade with subtle symptoms, including shortness of breath and tachycardia, but without hypotension or other hal

Systolic Ejection Murmur in a Case of Patent Ductus Arteriosus and Pulmonary Hypertension With Liver Cirrhosis.

Patent ductus arteriosus (PDA) is a congenital anomaly characterized by a persistent connection between the descending aorta and the pulmonary artery. While patients with PDA typically present with a continuous murmur, atypical cases, such as those with systolic murmurs or no murmurs (although the exact mechanism remains unclear), pose diagnostic challenges. We report a case of a 38-year-old woman with alcoholic liver cirrhosis, referred for liver transplantation, in whom a systolic ejection murmur prompted further evaluation. Diagnostic imaging and right heart catheterization confirmed PDA wi

Responding to clinical alarms in unfolding simulated clinical scenarios: auditory icons perform better than tonal alarms.

BACKGROUND: The international medical device safety standard IEC 60601-1-8 now recommends use of auditory icon alarms. Auditory icon alarms are alarms that act as metaphors for the problems that they signal. These are compared with traditional tonal alarms. METHODS: Two sets of three auditory alarms were compared, one consisting of auditory icons and one consisting of variants of older tonal alarms. Volunteer participants were required to monitor three clinical scenarios each lasting 4 min with different problems (cardiac, oxygenation, and technical) occurring during each of these scenarios t

AI-Enhanced Detection of Heart Murmurs: Advancing Non-Invasive Cardiovascular Diagnostics.

Cardiovascular diseases (CVDs) are the leading cause of death worldwide, claiming over 17 million lives annually. Early detection of conditions like heart murmurs, often indicative of heart valve abnormalities, is critical for improving patient outcomes. Traditional diagnostic methods, including physical auscultation and advanced imaging techniques, are constrained by their reliance on specialized clinical expertise, inherent procedural invasiveness, substantial financial costs, and limited accessibility, particularly in resource-limited healthcare environments. This study presents a novel con

Approaches to reduce the risk of severe asthma in children with preschool wheeze.

INTRODUCTION: Asthma is a common, serious condition. We can treat the symptoms of mild-moderate disease, but severe asthma is life-threatening despite treatment. We cannot cure asthma and have no specific preventive strategies. AREAS COVERED: We performed a PubMed search using the terms 'Severe asthma' and 'Prevention' and 'Preschool wheeze' limited to children, humans and English language over the previous five years. We searched the bibliographies of relevant references and also our personal archives. We cover transgenerational, antenatal and early life factors which increase the risk of pr

Surgical correction of ventricular septal defect in a cat.

A two-year-old intact male British shorthair cat, weighing 4.6 kg, was referred for surgical correction of a ventricular septal defect (VSD). The cat was treated with pimobendan, amlodipine, furosemide, and clopidogrel for tachypnea, and no other clinical signs of cardiac disease were observed. Physical examination revealed heart murmurs. Radiographic and echocardiographic evaluations indicated generalized cardiomegaly and left atrial enlargement. Two-dimensional echocardiography revealed a large left-to-right shunt through a 5.8-mm perimembranous VSD with a septal aneurysm. The pulmonary-to-s

Triple cardiovascular disease detection with an artificial intelligence-enabled stethoscope (TRICORDER): design and rationale for a decentralised, real-world cluster-randomised controlled trial and implementation study.

INTRODUCTION: Early detection of cardiovascular disease in primary care is a public health priority, for which the clinical and cost-effectiveness of an artificial intelligence-enabled stethoscope that detects left ventricular systolic dysfunction, atrial fibrillation and cardiac murmurs is unproven but potentially transformative. METHODS AND ANALYSIS: TRICORDER is a pragmatic, two-arm, multi-centre (decentralised), cluster-randomised controlled trial and implementation study. Up to 200 primary care practices in urban North West London and rural North Wales, UK, will be randomised to usual ca

Comparative Analysis of Coronary Artery Anomalies Originating From the Pulmonary Artery: Clinical Presentation, Imaging Findings, and Surgical Outcomes in Diverse Age Groups.

BACKGROUND & AIM: Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), a rare congenital heart defect, shows progression closely tied to patient age and the development of coronary artery collateral circulation. Understanding of this condition is largely derived from case reports and remains limited. This study aims to thoroughly investigate the differential manifestations of ALCAPA across various age groups, with a central emphasis on the evaluation of the coronary collateral circulation system, thereby contributing to a more nuanced approach in the clinical manage

Genetic detection and discrimination of closely related oyster pathogens for biosecurity and shellfish stock management.

Disease outbreaks among shellfish cause significant declines in both wild fisheries and aquaculture production globally. In Aotearoa New Zealand, a long history of flat oyster infections by the haplosporidian parasite Bonamia exitiosa has influenced fishery dynamics in the Foveaux Strait. B. ostreae, which causes high host mortality, was detected in both wild and farmed oysters in the Marlborough Sounds in 2015 and at Stewart Island in 2017, prompting the development of rapid diagnostic tests with high sensitivity and specificity that can be used to simultaneously monitor both Bonamia species.

Dynamic brain-heart-gut coupling during sleep: a continuous physiological signal analysis.

BACKGROUND: Investigating brain-heart-gut coupling during sleep is crucial for understanding the coordinated regulatory mechanisms of multiple systems during sleep. Non-invasive continuous physiological signal acquisition techniques have been widely applied in brain-heart dynamic assessment. However, current research on gut function primarily focuses on gut microbiota, with a lack of systematic investigation into the macroscopic dynamic changes of gut function. This study, therefore, based on multiple non-invasive physiological signals, aims to explore the dynamic changes and underlying mechan

Temporal convolutional neural network-based feature extraction and asynchronous channel information fusion method for heart abnormality detection in phonocardiograms.

BACKGROUND AND OBJECTIVE: Auscultation-based cardiac abnormality detection is valuable screening approach in pediatric populations, particularly in resource-limited settings. However, its clinical utility is often limited by phonocardiogram (PCG) signal variability and a difficulty in distinguishing between pathological and innocent murmurs. METHODS: We proposed a framework that leverages temporal convolutional network (TCN)-based feature extraction and information fusion to integrate asynchronously acquired PCG recordings at the patient level. A probabilistic representation of the pathologic
