Patterns of Respiratory Symptoms and Asthma Diagnosis in School-Age Children: Three Birth Cohorts.

BACKGROUND: Many studies used information on wheeze presence/absence to determine asthma-related phenotypes. We investigated whether clinically intuitive asthma subtypes can be identified by applying data-driven semi-supervised techniques to information on frequency and triggers of different respiratory symptoms. METHODS: Partitioning Around Medoids clustering was applied to data on multiple symptoms and their triggers in school-age children from three birth cohorts: MAAS (n = 947, age 8 years), SEATON (n = 763, age 10) and ASHFORD (n = 584, age 8). 'Guided' clustering, incorporating asthma d

Navigating heart murmurs in children.

BACKGROUND: Murmurs are a common finding in the paediatric population, with 99% being non-pathological. Cardiovascular examination is recommended to aid the detection of serious structural cardiac defects as a routine part of neonatal and child health checks. Incidentally noted murmurs are also a common finding, with a significant proportion of children developing an innocent murmur at some stage of childhood. It is therefore important for primary care physicians to be able to appropriately identify concerning features of both screening detected and incidentally noted murmurs in children. OBJ

Emulating the EPIC trial using VetCompass primary-care data: causal effects of pimobendan in UK dogs with grade IV/VI heart murmurs.

Target trial emulation applies design principles from randomised controlled trials (RCTs) to the analysis of observational data, potentially replicating RCT results in real-world settings. The EPIC trial reported that pimobendan delays the onset of congestive heart failure (CHF) and extends survival in dogs with preclinical degenerative mitral valve disease (DMVD). The current study aimed to explore the extent to which target trial emulation approximates the EPIC trial results in a primary-care setting. Grade IV/VI murmur diagnosis was defined as the treatment intervention stage. There were 92

Impact of Visual and Auditory Relaxation Techniques on Autonomic Responses During Dental Procedures: A Randomized Controlled Trial.

INTRODUCTION: Relaxation methods for managing anxiety can help overcome and prevent dental treatment phobia. We investigated the effects of relaxation techniques targeting visual and auditory domains, which remain understudied, on dental treatment-related stress. METHODS: Thirty adult volunteers with dental anxiety were enrolled in a randomized, open-label, four-sequence, four-period crossover study evaluating the effects of the following interventions: wearing a hot eye mask with a heat- and steam-generating (HSG) sheet, wearing an inactivated HSG sheet, listening to nature sounds, and weari

The first report of quadricuspid aortic valve in cattle: clinical, anatomical and histological findings in three Holstein cattle.

Quadricuspid aortic valve (QAV) is a rare congenital heart condition in which the aortic valve is comprised of four, rather than three, sinuses and cusps. The clinical significance of QAV is unclear; it is diagnosed most frequently in humans and has not been reported in cattle. QAV was identified in three female Holstein cattle at necropsy for unrelated clinical conditions. Although heart murmurs were identified on clinical examination, no animal had heart failure. In all three cases the non-coronary cusp of the aortic valve was comprised of two separate cusps, resulting in a quadricuspid valv

Double-Chambered Right Ventricle and Intraventricular Thrombosis Mimicking Double-Chambered Right Ventricle in Cynomolgus Monkeys (Macaca fascicularis).

Reports of congenital heart disease in nonhuman primates are rare, and double-chambered right ventricle (DCRV), which is a rare congenital heart disease, in which an abnormal muscle bundle divides the right ventricle into 2 chambers (a proximal high-pressure chamber and a distal low-pressure chamber), has not been previously reported. We diagnosed DCRV antemortem in 2 monkeys bred at The Tsukuba Primate Research Center that presented with cardiac murmurs. Subsequent diagnostic evaluation confirmed DCRV in one case, with the other diagnosed with midventricular obstruction having a pathophysiolo

Method for Predict Stenosis of Arteriovenous Fistula Patients Based on Machine Learning.

OBJECTIVES: Arteriovenous fistula (AVF) is the most ideal vascular access for hemodialysis. People with AVF have a longer vascular access survival rate and a lower complication rate. Thrombosis and stenosis are the most common complications of AVF. The annual thrombosis event rate is 10%-50%. Appropriate identification of AVF stenosis and management could reduce the risk of thrombosis and access loss. Guidelines recommended physical examinations as the first line of AVF stenosis monitoring. However, even for health professionals, the diagnosis rate by hearing the bruit varied. The sound waves

Missing what's right under your nose: Failed appetitive and aversive audio-olfactory conditioning in humans.

The comparison of physiological mechanisms underlying appetitive and aversive conditioning is often challenging due to the involvement of stimuli from different modalities with potentially disparate effective mechanisms (e.g., pain stimuli versus monetary rewards). The olfactory system offers a unique opportunity to examine both types of conditioning in humans, as isointense odors can serve as comparably pleasant and unpleasant stimuli. To study physiological and behavioral responses during appetitive and aversive learning, we employed odors as unconditioned stimuli (US) in a within-subjects d

Assessment of remote digital auscultation for identifying significant aortic stenosis: A pilot study.

BACKGROUND: Aortic stenosis (AS) is the most common valvular disease in the Western world. Technological advances, such as the TytoCare™ device, have introduced remote telemedicine-based cardiac auscultation. Its utility in diagnosing and grading AS remains unexplored. This study aims to evaluate the reliability of the TytoCare™ for assessing AS. METHODS: Sixty patients were classified by standard echocardiogram into three groups: significant AS (at least moderate AS; n=20), non-significant AS (aortic sclerosis or mild AS; n=20) and controls (n=20). Heart sounds were self-recorded using the T

A dual branch feature extraction network for heart sound signal analysis.

The analysis of heart sound signals is critical in the early diagnosis of cardiovascular disease. However, the complexity and diversity of these signals pose significant challenges for accurate recognition. In this paper, we propose a novel heart sound dual-branch feature extraction network (HSDFE-Net) to address these challenges. The model first extracts both conventional audio features and bi-spectrum features from preprocessed heart sound signals, enabling comprehensive characterization of their linear spectral and nonlinear properties. Unlike conventional single-branch networks, HSDFE-Net

An Uncommon Cause of Respiratory Distress: Ruptured Tension Gastrothorax.

A gastrothorax occurs when the stomach becomes displaced into the thoracic cavity. Rarely, this gastrothorax can rupture, which can lead to respiratory distress, hemodynamic compromise, and cardiac arrest. Here, we discuss the case of a 69-year-old male who presented to the emergency department (ED) for evaluation of generalized weakness and shortness of breath without associated upper respiratory symptoms. On physical exam, the patient was hemodynamically unstable with absent lung sounds on the left. A portable chest X-ray (CXR) was obtained, which initially revealed concern for tension hydro

A Novel Method for ECG-Free Heart Sound Segmentation in Patients with Severe Aortic Valve Disease.

Severe aortic valve diseases (AVD) cause changes in heart sounds, making phonocardiogram (PCG) analyses challenging. This study presents a novel method for segmenting heart sounds without relying on an electrocardiogram (ECG), specifically targeting patients with severe AVD. Our algorithm enhances traditional Hidden Semi-Markov Models by incorporating signal envelope calculations and statistical tests to improve the detection of the first and second heart sounds (S1 and S2). We evaluated the method on the PhysioNet/CinC 2016 Challenge dataset and a newly acquired AVD-specific dataset. The meth

Haemorrhagic Pericardial Effusion Following Intrapleural Fibrinolysis Therapy With Alteplase/Dornase Alfa for Complicated Parapneumonic Effusion: A Case Report.

Standard treatment for complicated parapneumonic effusion is antibiotic therapy and drainage of the infected pleural fluid. Complicated effusions refractory to standard treatment require further interventions such as intrapleural fibrinolysis. This is effective for patients with loculated effusions as local fibrinolytics administered via a chest drain can break down the septations. Common complications include intrapleural bleeding, hypersensitivity, pain and discomfort with fibrinolytics, but no studies, to our knowledge, have reported pericardial haemorrhage. In this case report, we describe

Pulse deficits resulting in apparent bradycardia in atrial bigeminy.

Supraventricular premature beats are premature activations of the atria outside of the sinus node that is often benign. They typically present with irregular pulses and dynamic changes to pre-existing murmurs between sinus beats and ectopic beats, but rarely with bradycardia. We describe an elderly male with atrial bigeminy who presented for evacuation of a soft tissue collection in a regional hospital. On examination, he appeared bradycardic due to a significant pulse deficit, despite remaining haemodynamically stable and asymptomatic. This led to two emergency response teams being called and
