All-fiber wireless auscultation garment for cardiopulmonary diagnostics.

Daily apparel could enable continuous, noninvasive monitoring of cardiopulmonary health for the early detection and treatment of cardiopulmonary diseases, the leading causes of death and disability worldwide. Here we develop an all-fiber wireless auscultation system seamlessly integrated into garments. An ~150-μm-diameter fiber contact microphone is sewn, knitted or woven into clothing to capture deep-body acoustic signals while overcoming acoustic impedance mismatch and electromagnetic noise. An ~500-μm knitted fiber computer enables real-time signal processing and Bluetooth transmission to a

Advancing Point-of-Care Still's Murmur Identification: Evaluating the Efficacy of ConvNets and Transformers Using the StethAid Multicenter Heart Sound Database.

BACKGROUND: Primary care providers (PCPs) are not successful in accurately identifying Still's murmur with no available clinical tools to aid in the process. Existing deep learning (DL) methods primaryly focused on adult pathological murmurs or murmur detection, lacking dedicated approaches for Still's murmur. Furthermore, the absence of a specialized database hampers the development and validation of AI models for pediatric populations. OBJECTIVE: To collect a pediatric dataset and evaluate DL methods in identifying Still's murmur in pediatric patients from phonocardiograms (PCGs) acquired u

DPPAT: Dual-Level Periodic Pattern-Aware Transformer for Heart Sound Murmur Identification.

Developing heart sound classification algorithms for murmur identification is critical for early screening of heart diseases. However, identifying murmurs in long-duration heart sound signals can be challenging due to their weak features and interference from noise. Considering the periodic patterns of heart sounds and murmurs, periodic priors can be introduced to enhance murmur identification, an approach that remains underutilized in current methods. In this study, we propose a novel Dual-level Periodic Pattern-Aware Transformer (DPPAT) to implicitly leverage the periodic priors of heart sou

The utility of continuous high-enhanced-contrast electrocardiography-gated cardiac computed tomography method evaluating accurate minimal ductal diameter and ductus arteriosus morphology in a dog with patent ductus arteriosus.

A three-year-old, 4.1-kg, female Shih tzu was referred for investigation of exercise intolerance and heart murmurs. At the first visit, the dog was diagnosed with patent ductus arteriosus and a suspected persistent left cranial vena cava using echocardiography. Electrocardiography (ECG)-gated non-helical, stationary 320-slice multidetector cardiac computed tomography (CCT) was performed to classify the morphology and to measure the minimal ductal diameter. However, the ductal ostium at the pulmonary artery was not clearly visualized. To overcome this problem, we developed the continuous high-e

Deep learning-driven automated detection of canine cardiac murmurs via digital wireless stethoscope auscultation.

A heart murmur is a key indicator of cardiovascular disease, making auscultation essential. Nonetheless, its accuracy is influenced by the clinician’s experience and subjective interpretation. This study developed a deep learning–based algorithm (CNN6) for automated detection of heart murmurs using phonocardiogram (PCG) data acquired via a digital wireless stethoscope. A total of 2,269 recordings (over 20 h) from 406 dogs were used for model development and validation, and 297 recordings from 60 dogs were reserved for independent testing. The model achieved 89.9% sensitivity, 92.7% specificity

Heyde syndrome secondary to hypertrophic obstructive cardiomyopathy: a case report.

BACKGROUND: Heyde syndrome is an uncommon clinical condition characterized by the triad of aortic stenosis, gastrointestinal angiodysplasia, and acquired von Willebrand syndrome. Its occurrence in the context of hypertrophic obstructive cardiomyopathy is rare. Common in individuals over 65, diagnosis is challenging due to the prevalence of these conditions. Hypertrophic obstructive cardiomyopathy increases shear stress, leading to von Willebrand Factor degradation and a higher risk of gastrointestinal bleeding. CASE PRESENTATION: A 69-year-old male presented to the outpatient clinic with mele

Subaortic Stenosis in Four Free-Ranging Eurasian Lynx (Lynx lynx) from the Alpine and Jura Populations in Switzerland.

As part of wildlife conservation efforts, the reintroduced Eurasian lynx (Lynx lynx) populations in Switzerland are the subject of ongoing health surveillance. Cardiac pathologies, such as heart murmurs and histopathologic changes in the cardiac tissue, including intramural cardiac vessels, are frequently observed. Here we describe four cases of subaortic stenosis (SAS), a congenital heart disease rarely reported in felids, in free-ranging adult male lynx from Switzerland. The affected lynx presented macroscopically with grades 1 to 3 of SAS. Histopathology confirmed characteristic subvalvular

Advancing cardiovascular screening: deep learning-based heart-sound classification using SMOTE and temporal modeling.

Early and reliable detection of cardiac murmurs from phonocardiogram (PCG) recordings is essential for improving cardiovascular screening and supporting diagnosis in primary care. However, automated murmur classification remains challenging due to signal variability, class imbalance, and temporal dependence within heart-sound sequences. This study presents a leakage-safe heart-sound classification framework that combines peak-based segmentation, Mel-Frequency Cepstral Coefficient (MFCC) feature extraction, Synthetic Minority Over-sampling Technique (SMOTE)–based class balancing, and Recurrent

Complete atrioventricular septal defect in Thoroughbred foals: Echocardiographic and postmortem findings.

Complete atrioventricular septal defect (AVSD) is rarely reported in horses. We retrospectively reviewed three Thoroughbred foals (2017-2023) with necropsy-confirmed complete AVSD and summarized their clinical presentation, transthoracic echocardiographic findings, and postmortem anatomy. All foals showed early signs of congenital cardiac disease including weakness, failure to thrive and cardiac murmurs. Echocardiography demonstrated a common atrioventricular junction with a single atrioventricular valve and combined atrial and ventricular septal defects; all were classified as Rastelli type C

Extraction of Acoustic Features via Empirical Wavelet Transform to Determine Stenosis Degree of the Left Anterior Descending Artery Based on the Diastolic Heart Sounds of 75 Participants.

OBJECTIVES: This study aimed to develop a method for extracting acoustic features to assess left anterior descending artery (LAD) stenosis severity. METHODS: Heart sound data were collected from 75 participants (10 diastoles per participant) using a high-signal-to-noise ratio micro-electro-mechanical systems stethoscope. The diastolic signals were preprocessed, and empirical wavelet transform was applied to decompose their power spectra into three modes (0-150, 150-500, and > 500 Hz). The spectral energies (e(1), e(2), e(3)) of these modes were analyzed, and support vector machine (SVM) and e

Parental knowledge, awareness, and intended healthcare-seeking responses toward congenital heart disease in the West Bank, Palestine: a cross-sectional study.

BACKGROUND: Congenital heart disease (CHD) is a major contributor to infant morbidity and mortality worldwide. Although prenatal screening, neonatal assessment, pulse oximetry screening, and routine pediatric evaluation contribute to CHD detection, some cases may remain unrecognized until after discharge or later in infancy. Parental recognition of warning signs may support timely healthcare-seeking, but it should be considered one component of a broader diagnostic and referral pathway. This study assessed parental knowledge, awareness, and intended healthcare-seeking responses toward CHD amon

Prior-aligned frequency-domain explanations for heart sound classification: a scale-consistent attribution approach.

OBJECTIVE: Deep learning models for phonocardiogram (PCG) classification often achieve high accuracy but lack transparency in relation to clinically meaningful features. Existing explainable artificial intelligence (XAI) methods rarely ensure alignment between model attributions and cardiac physiology. METHODS: We propose Scale-Consistent Attribution (SCA), a training-time regularization method that incorporates domain knowledge into model learning. SCA aligns spectral attention with a soft clinical prior, distinguishing low-frequency heart sounds from high-frequency murmurs. The method is ev

Non-contact Heart Sound Measurement by Defocused Speckle Imaging.

Phonocardiogram (PCG) is a graphical representation of heart sounds and murmurs, playing a vital role in the early screening and prevention of cardiovascular diseases (CVDs). In this study, we propose a first camera-stethoscope that is capable of recording PCG contactlessly, enabled by the principle of defocused speckle imaging (DSI). By illuminating the chest surface adjacent to the heart with a laser beam and employing a defocused camera to capture the reflected laser speckle interference, the subtle cardiac vibrations involved in heart sound generation are magnified and measured. Patterns of morbidity, multimorbidity, and mortality in aging cats: findings from seven years of the Cat Prospective Ageing and Welfare Study.

INTRODUCTION: Aging is associated with an increased risk of morbidity, multimorbidity and mortality in cats. However, longitudinal cohort studies are somewhat limited in this species. Information about the prevalence and incidence of age-related disease, combinations of morbidities, and factors associated with mortality could provide insights into the impact of aging on the welfare of domestic cats. METHODS: The prevalence and incidence of common age-related diseases, patterns of multimorbidity and causes and risk factors for mortality were investigated in a prospective longitudinal cohort st
