Pregnancy in Takayasu Arteritis: A Case Report of Successful Pregnancy in a Large Aortic Aneurysm.

A 29-year-old primigravida woman with a gestational age of 19 weeks visited in the cardio-obstetrics clinic for hypertension. Physical examination revealed auscultated systolic and diastolic murmurs in the second right intercostal space. Transthoracic echocardiography (TTE) showed severe left ventricle (LV) enlargement with mild systolic dysfunction. Severe aortic regurgitation (AR) and aneurysmal dilation of the proximal part of the ascending aorta were noted. Magnetic resonance imaging (MRI) without contrast showed a dilated aortic root and ascending aorta (64.4 mm at the right pulmonary art

Heart rate study using the cardiac sounds.

The purpose of this paper is to present a straightforward framework for Heart Rate (HR) estimation from a Phonocardiogram (PCG) record and study the association between murmur severity levels and HR. The system focuses primarily on data processing procedure, which is based on signal preprocessing using Maximal Overlap Discrete Wavelet Transform (MODWT) to delineate murmurs from heart sounds. We exploit the characteristics of Logistic function to derive an enhanced PCG envelope that serves as prerequisite for HR algorithm detection. In fact, the PCG envelope presents a cyclostationarity that ca

Heart Murmur Detection in Phonocardiogram Data Leveraging Data Augmentation and Artificial Intelligence.

Background/Objectives: With a 17.9 million annual mortality rate, cardiovascular disease is the leading global cause of death. As such, early detection and disease diagnosis are critical for effective treatment and symptom management. Cardiac auscultation, the process of listening to the heartbeat, often provides the first indication of underlying cardiac conditions. This practice allows for the identification of heart murmurs caused by turbulent blood flow. In this exploratory research paper, we propose an AI model to streamline this process to improve diagnostic accuracy and efficiency. Meth

MYSTETH-home-based heart monitoring.

INTRODUCTION: The MySteth is an intelligent medical tool designed for cardiac disease screening, utilizing either a stethoscope or smartphone to record heart sounds. Normal heart sounds in healthy individuals consist of "lub" and "dub" noises, while murmurs-additional sounds during heartbeats-can indicate cardiac anomalies such as valve dysfunctions and rapid blood flow, categorized as systolic or diastolic. METHOD: MySteth was developed and tested using heart sounds recorded via smartphone and digital stethoscope. For ensuring the clinical validity of the data, all heart sound samples were m

Beyond the murmur: unmasking a rare sinus venosus ASD in an adolescent with chronic respiratory infections: A case report.

RATIONALE: Overall, 5% to 10% of all atrial septal defects (ASDs) are of the sinus venosus type, a rare congenital disease. This defect must be identified and treated immediately since it might cause significant left atrial dilatation and dyspnea. PATIENT CONCERNS: A 15-year-old male presented with previously undetected sinus venosus type ASD. The patient arrived with a high-grade heart murmur and a history of recurring respiratory illnesses. DIAGNOSES: A clinical examination identified a 3/6 systolic ejection murmur in the second left intercostal gap and a fixed split second heart sound. El

Explainable attention-based deep learning for classification and interpretation of heart murmurs using phonocardiograms.

Cardiovascular diseases (CVDs) remain a leading global health challenge, necessitating diagnostic solutions that combine high accuracy with clinical interpretability and reproducibility. Traditional auscultation methods rely extensively on clinician expertise, resulting in variability and potential diagnostic delays, especially for subtle murmurs indicative of existing cardiac abnormalities. While automated methods have improved diagnostic accuracy, they frequently lack reproducibility and transparency, contributing to clinical mistrust. To address these challenges, we propose an Explainable A

Transfer learning based cardiac murmur detection in phonocardiogram signals using spectrograms.

This study uses transfer learning architectures to detect cardiac murmurs in phonocardiogram signals by denoising the signal, extracting relevant features for spectrograms generation. The Short-Time Fourier Transform, Mel-Frequency Cepstral Coefficients, and Continuous Wavelet Transform techniques were applied on Physionet's CirCor Digiscope PCG dataset. VGG16, VGG19, ResNet50, and InceptionV3 models, were trained on these spectrograms for binary classification. Fourth-order Butterworth bandpass filter, used with Savitzky-Golay filtering, gave the best results. The CWT Spectrogram and VGG19 co

Improving AI-Based Canine Heart Disease Diagnosis with Expert-Consensus Auscultation Labeling.

Noisy labels pose significant challenges for AI model training in veterinary medicine. This study examines expert assessment ambiguity in canine auscultation data, highlights the negative impact of label noise on classification performance, and introduces methods for label noise reduction. To evaluate whether label noise can be minimized by incorporating multiple expert opinions, a dataset of 140 heart sound recordings (HSR) was annotated regarding the intensity of holosystolic heart murmurs caused by Myxomatous Mitral Valve Disease (MMVD). The expert opinions facilitated the selection of 70 h

Comparative Analysis of CNN and Transformer Architectures with Heart Cycle Normalization for Automated Phonocardiogram Classification.

The automated classification of phonocardiogram (PCG) recordings represents a substantial advancement in cardiovascular diagnostics. This paper presents a systematic comparison of four distinct models for heart murmur detection: two specialized convolutional neural networks (CNNs) and two zero-shot universal audio transformers (BEATs), evaluated using fixed-length and heart cycle normalization approaches. Utilizing the PhysioNet2022 dataset, a custom heart cycle normalization method tailored to individual cardiac rhythms is introduced. The findings indicate the following AUROC values: the CNN

Deep Learning-Driven Non-Contact Sound Source Localization via Multi-Axis Analysis with Laser Doppler Vibrometry.

This study presents a non-invasive sound source localization methodology that leverages multi-axis vibration analysis combined with deep learning classification. Accurate sound source localization is critical in various fields, including clinical diagnostics, where determining the origin of acoustic signals provides valuable physiological insights. Using Laser Doppler Vibrometry (LDV), sound-induced surface vibrations are measured, and directional information is extracted from the Log Power Spectra (LPS). The proposed framework integrates theoretical modeling of multi-axis vibrations, experime

Adult Presentation of Supracardiac Total Anomalous Pulmonary Venous Connection with Direct Drainage into the Superior Vena Cava.

We report the case of a woman in her late 30 s with longstanding cyanosis diagnosed with a rare supracardiac total anomalous pulmonary venous connection (TAPVC) draining directly into the superior vena cava (SVC). Her survival into adulthood was facilitated by a large, nonrestrictive atrial septal defect, and unobstructed pulmonary venous return, allowing functional compensation despite chronic hypoxemia. Echocardiography, cardiac catheterization, and computed tomography confirmed the anatomy and surgical feasibility. This case highlights this rare anatomic variant of TAPVC with direct drainag

Frog vocal sacs-inspired soft acoustic system with continuously tunable resonance for sound emission and stethoscopic sensing.

To overcome limitations of flexible thermoacoustic devices in low-frequency emission and sensing, we present a resonance-adjustable graphene sound device (RAGSD) inspired by frog vocal sacs. Integrating laser-induced graphene (LIG) with a deformable cavity, RAGSD provides continuous tuning from 922.12 to 1762.90 hertz. A dynamic, continuously tunable electro-mechano-acoustical model explains the mechanism and predicts frequency with [Formula: see text] . In emission, a 25.34-decibel sound pressure level gain was observed at resonance, enabling controllable, frequency-selective voice amplificat

Noise reduction in abdominal acoustic recordings of maternal placental murmurs.

Fetal phonocardiography is a well-known auscultation technique for evaluation of fetal health. However, murmurs that are synchronous with the maternal heartbeat can often be heard while listening to fetal heart sounds. Maternal placental murmurs (MPM) could be used to detect maternal cardiovascular and placental abnormalities, but the recorded MPMs are often contaminated by ambient interference and noise. Objective:The aim of this study was to compare noise reduction methods to reduce noise in the recorded MPMs. Approach:1) Bandpass filtering (BPF), 2) a multichannel noise reduction (MCh) using

Transcatheter closure of anomalous systemic arterial supply to the left lung from the left subclavian artery: Diagnostic and procedural challenges.

UNLABELLED: A 26-year-old woman with chronic effort intolerance and a continuous murmur underwent evaluation for suspected congenital heart disease. While initial assessments raised suspicion for a patent ductus arteriosus, computed tomography pulmonary angiography confirmed a rare systemic arterial collateral originating from the left subclavian artery and supplying the left lung. Due to significant left heart chamber dilatation, transcatheter closure was undertaken. A 20-mm vascular plug was successfully deployed to occlude the primary collateral. However, during attempted closure of an addi

Low utility of pre-operative cardiology referrals in adolescent idiopathic scoliosis using a cutoff of Cobb > 70 degrees.

BACKGROUND: Severe adolescent idiopathic scoliosis (AIS) may be associated with cardiopulmonary issues, including previously undiagnosed congenital cardiac anomalies, valvular disease, and pulmonary hypertension. Many centers routinely refer patients with coronal Cobb angles ≥ 70° for pre-operative cardiology evaluation prior to posterior spinal fusion (PSF). The clinical utility of this practice remains uncertain. METHODS: A retrospective review was conducted of AIS patients undergoing PSF at a single pediatric orthopedic center from 2017 to 2024. All patients received pre-operative anesthes

Masquerading details behind a murmur and chest pain.

Masquerading bundle branch block (MBBB) is a rare presentation of bifascicular blocks. It is the result of a right bundle branch block associated with an advanced left anterior fascicular block due to extensive damage to the conduction system. We present the case of a 75-year-old male with late onset presentation anterior wall myocardial infarction (MI) with ongoing ischemia (which evolved into a ventricular septal defect [VSD]). Electrocardiographically, the MBBB is characterized by a prominent R in V1, left axis deviation and absence of small S waves in I and aVL. Its presence confers a poor

Beyond the usual spectrum: Atrial septal defect in a Marfan syndrome patient with severe aortic pathologies.

Marfan's syndrome is a rare connective tissue disorder which mainly affects cardiovascular, skeletal, and ocular systems. Common cardiovascular manifestation of Marfan's includes aortic root dilatation, aortic regurgitation, and mitral valve prolapse. However, presence of congenital heart defects like atrial septal defect (ASD) is very rare. Here, we report the case of a 22-year-old male with Marfan's syndrome who had an uncommon coexistence of ASD and severe aortic pathologies. The patient had clinical features like tall stature, long arm span, and musculoskeletal abnormalities such as pectus
