Sensorimotor Synchronization in Healthy Aging and Neurocognitive Disorders.

Sensorimotor synchronization (SMS), the coordination of physical actions in time with a rhythmic sequence, is a skill that is necessary not only for keeping the beat when making music, but in a wide variety of interpersonal contexts. Being able to attend to temporal regularities in the environment is a prerequisite for event prediction, which lies at the heart of many cognitive and social operations. It is therefore of value to assess and potentially stimulate SMS abilities, particularly in aging and neurocognitive disorders (NCDs), to understand intra-individual communication in the later sta

Breaking Bad News in the Emergency Department.

AUDIENCE: The primary audience for this simulation is emergency medicine (EM) residents, but this curriculum could also be used for EM-bound medical students. INTRODUCTION: Breaking bad news is a difficult but necessary skill for EM physicians. Bad news can range from informing family that a patient is in the emergency department (ED), to shared decision making regarding a life-threatening situation, to family notification of patient death.1 Although there are many structured approaches to death notification and breaking bad news, such as GRIEV_ING2 and SPIKES,3 EM physicians often lack confi

A Comparison of Heart Pulsations Provided by Forcecardiography and Double Integration of Seismocardiogram.

Seismocardiography (SCG) is largely regarded as the state-of-the-art technique for continuous, long-term monitoring of cardiac mechanical activity in wearable applications. SCG signals are acquired via small, lightweight accelerometers fixed on the chest. They provide timings of important cardiac events, such as heart valves openings and closures, thus allowing the estimation of cardiac time intervals of clinical relevance. Forcecardiography (FCG) is a novel technique that records the cardiac-induced vibrations of the chest wall by means of specific force sensors, which proved capable of monit

Artificial Intelligence-Based Stethoscope for the Diagnosis of Aortic Stenosis.

BACKGROUND: The diagnostic accuracy of the stethoscope is limited and highly dependent on clinical expertise. Our purpose was to develop an electronic stethoscope, based on artificial intelligence (AI) and infrasound, for the diagnosis of aortic stenosis (AS). METHODS: We used an electronic stethoscope (VoqX; Sanolla, Nesher, Israel) with subsonic capabilities and acoustic range of 3-2000 Hz. The study had 2 stages. In the first stage, using the VoqX, we recorded heart sounds from 100 patients referred for echocardiography (derivation group), 50 with moderate or severe AS and 50 without valvu

Unsupervised detection and classification of heartbeats using the dissimilarity matrix in PCG signals.

BACKGROUND AND OBJECTIVE: Auscultation is the first technique applied to the early diagnose of any cardiovascular disease (CVD) in rural areas and poor-resources countries because of its low cost and non-invasiveness. However, it highly depends on the physician's expertise to recognize specific heart sounds heard through the stethoscope. The analysis of phonocardiogram (PCG) signals attempts to segment each cardiac cycle into the four cardiac states (S1, systole, S2 and diastole) in order to develop automatic systems applied to an efficient and reliable detection and classification of heartbea

Classification of Children's Heart Sounds With Noise Reduction Based on Variational Modal Decomposition.

PURPOSE: Children's heart sounds were denoised to improve the performance of the intelligent diagnosis. METHODS: A combined noise reduction method based on variational modal decomposition (VMD) and wavelet soft threshold algorithm (WST) was proposed, and used to denoise 103 phonocardiogram samples. Features were extracted after denoising and employed for an intelligent diagnosis model to verify the effect of the denoising method. RESULTS: The noise in children's phonocardiograms, especially crying noise, was suppressed. The signal-to-noise ratio obtained by the method for normal heart sounds

Successful adrenaline treatment of perioperative severe bronchospasm combined with a silent lung: two case reports.

BACKGROUND: Silent lung is a rare and potentially fatal disease. It is a critical sign of strong bronchospasm or extensive mucus plug blockage, which can result in the obvious weakening of breathing sounds or even disappearance of breathing sounds. Silent lung has an acute onset and rapid progress, which seriously threatens the life of patients. It needs early diagnosis, timely and effective treatment to reverse the persistent severe bronchospasm of patients. If not handled in time, silent lung can cause rapid onset of severe hypoxemia, hypoxic brain injury, and even cardiac arrest. Few studie

The Effect of Low Frequency Sound on Heart Rate Variability and Subjective Perception: A Randomized Crossover Study.

BACKGROUND: Vibroacoustic therapy (VAT) uses low-frequency sound, often combined with listening to music, for therapeutic purposes. However, the impact of low-frequency vibration (LFV) on physiological functions and subjective perception is relatively unknown. METHODS: We conducted a randomized cross-over study with the aim of comparing the effect of constant LFV of 40 Hz, its amplitude modulation, and the placebo condition on heart rate variability (HRV), stress perception (measured by visual analogue scales for stress) and mood (measured by UWIST Mood Adjective Check List). RESULTS: Resear

Towards Multimodal Equipment to Help in the Diagnosis of COVID-19 Using Machine Learning Algorithms.

COVID-19 occurs due to infection through respiratory droplets containing the SARS-CoV-2 virus, which are released when someone sneezes, coughs, or talks. The gold-standard exam to detect the virus is Real-Time Polymerase Chain Reaction (RT-PCR); however, this is an expensive test and may require up to 3 days after infection for a reliable result, and if there is high demand, the labs could be overwhelmed, which can cause significant delays in providing results. Biomedical data (oxygen saturation level-SpO2, body temperature, heart rate, and cough) are acquired from individuals and are used to

Clinical and Viral Factors Associated With Disease Severity and Subsequent Wheezing in Infants With Respiratory Syncytial Virus Infection.

UNLABELLED: Respiratory syncytial virus (RSV) causes substantial morbidity and mortality in infants and young children worldwide. Here we evaluated host demographic and viral factors associated with RSV disease severity in 325 RSV-infected infants under 1 year of age from 3 European countries during 2017-2020. Younger infants had a higher clinical severity (ReSViNET) score and were more likely to require hospitalization, intensive care, respiratory support, and/or mechanical ventilation than older infants (<3 months vs 3 to <6 months and 3 to <6 months vs ≥6 months). Older age (≥6 months vs <3

Asthma and asthma-related comorbidity: effects on nocturnal oxygen saturation.

STUDY OBJECTIVES: Nocturnal symptoms are very common in asthma, which is associated with worse sleep quality. The nocturnal oxygen saturation may be decreased in asthma; however, whether this association is dependent on nocturnal asthma symptoms, lung function, coexisting obstructive sleep apnea (OSA), or other asthma-related comorbidities is unknown. The objective of this study was to examine the effects of asthma, OSA, lung function, airway symptoms, and asthma-related comorbidities on the nocturnal oxygen saturation in a cross-sectional community-based population study. METHODS: In total,

On the analysis of data augmentation methods for spectral imaged based heart sound classification using convolutional neural networks.

BACKGROUND: The application of machine learning to cardiac auscultation has the potential to improve the accuracy and efficiency of both routine and point-of-care screenings. The use of convolutional neural networks (CNN) on heart sound spectrograms in particular has defined state-of-the-art performance. However, the relative paucity of patient data remains a significant barrier to creating models that can adapt to a wide range of potential variability. To that end, we examined a CNN model's performance on automated heart sound classification, before and after various forms of data augmentatio

Real-world evaluation of the Stemoscope electronic tele-auscultation system.

BACKGROUND: With the spread of COVID-19, telemedicine has played an important role, but tele-auscultation is still unavailable in most countries. This study introduces and tests a tele-auscultation system (Stemoscope) and compares the concordance of the Stemoscope with the traditional stethoscope in the evaluation of heart murmurs. METHODS: A total of 57 patients with murmurs were recruited, and echocardiographs were performed. Three cardiologists were asked to correctly categorize heart sounds (both systolic murmur and diastolic murmur) as normal vs. abnormal with both the Stemoscope and a t

Heart Sound Classification based on Residual Shrinkage Networks.

Heart sound classification is one of the non-invasive methods for early detection of the cardiovascular diseases (CVDs), the leading cause for deaths. In recent years, Computer Audition (CA) technology has become increasingly sophisticated, auxiliary diagnosis technology of heart disease based on CA has become a popular research area. This paper proposes a deep Convolutional Neural Network (CNN) model for heart sound classification. To improve the classification accuracy of heart sound, we design a classification algorithm combining classical Residual Network (ResNet) and Long Short-Term Memor

A Federated Learning Paradigm for Heart Sound Classification.

Cardiovascular diseases (CVDs) have been ranked as the leading cause for deaths. The early diagnosis of CVDs is a crucial task in the medical practice. A plethora of efforts were given to the automated auscultation of heart sound, which leverages the power of computer audition to develop a cheap, non-invasive method that can be used at any time and anywhere for measuring the status of the heart. Nevertheless, previous works ignore an important factor, namely, the privacy of the user data. On the one hand, learnt models are always hungry for bigger data. On the other hand, it can be difficult t

Respiratory-Induced Amplitude Modulation of Forcecardiography Signals.

Forcecardiography (FCG) is a novel technique that records the weak forces induced on the chest wall by cardio-respiratory activity, by using specific force sensors. FCG sensors feature a wide frequency band, which allows us to capture respiration, heart wall motion, heart valves opening and closing (similar to the Seismocardiogram, SCG) and heart sounds, all simultaneously from a single contact point on the chest. As a result, the raw FCG sensors signals exhibit a large component related to the respiratory activity, referred to as a Forcerespirogram (FRG), with a much smaller, superimposed com

Tracheal Sound Analysis.

Tracheal sound sensors provide multiple respiratory signals that are valuable for studying upper airway characteristics. This chapter reviews the original work and ongoing research on tracheal sound analysis in relation to upper airway obstruction during sleep. Past and current research suggest that being associated with other sleep study recording sensors and advanced signal processing techniques, tracheal sound analysis can extensively contribute to the diagnosis and assessment of sleep-disordered breathing.
