Effect of Indian Music as an Auditory Stimulus on Physiological Measures of Stress, Anxiety, Cardiovascular and Autonomic Responses in Humans-A Randomized Controlled Trial.

Among the different anthropogenic stimuli humans are exposed to, the psychological and cardiovascular effects of auditory stimuli are less understood. This study aims to explore the possible range of change after a single session of auditory stimulation with three different ‘Modes’ of musical stimuli (MS) on anxiety, biomarkers of stress, and cardiovascular parameters among healthy young individuals. In this randomized control trial, 140 healthy young adults, aged 18−30 years, were randomly assigned to three MS groups (Mode/Raga Miyan ki Todi, Malkauns, and Puriya) and one control group (natur

Early childhood wheezing phenotypes and determinants in a South African birth cohort: longitudinal analysis of the Drakenstein Child Health Study.

BACKGROUND: Developmental trajectories of childhood wheezing in low-income and middle-income countries (LMICs) have not been well described. We aimed to derive longitudinal wheeze phenotypes from birth to 5 years in a South African birth cohort and compare those with phenotypes derived from a UK cohort. METHODS: We used data from the Drakenstein Child Health Study (DCHS), a longitudinal birth cohort study in a peri-urban area outside Cape Town, South Africa. Pregnant women (aged ≥18 years) were enrolled during their second trimester at two public health clinics. We followed up children from b

Optimized Acoustic Phantom Design for Characterizing Body Sound Sensors.

Many commercial and prototype devices are available for capturing body sounds that provide important information on the health of the lungs and heart; however, a standardized method to characterize and compare these devices is not agreed upon. Acoustic phantoms are commonly used because they generate repeatable sounds that couple to devices using a material layer that mimics the characteristics of skin. While multiple acoustic phantoms have been presented in literature, it is unclear how design elements, such as the driver type and coupling layer, impact the acoustical characteristics of the p

Changes in cardiac acoustic biomarkers before and after cardiac events in a patient with right-sided heart failure due to cor pulmonale.

UNLABELLED: Chronic heart failure (HF) has various phenotypes. It is accompanied by repeated hospitalizations over a long period. Therefore, accumulating long-term observational data of patients with various backgrounds is important to establish a prediction technology for the exacerbation of HF. In a patient with chronic right-sided HF caused by cor pulmonale, heart sounds and electrocardiograms were recorded at home or our hospital twice a week for 7 months including the stable (31 days), pre-exacerbation (2 weeks just before the onset of exacerbation), and hospitalization periods and quanti

Automated identification of innocent Still's murmur using a convolutional neural network.

BACKGROUND: Still's murmur is the most prevalent innocent heart murmur of childhood. Auscultation is the primary clinical tool to identify this murmur as innocent. Whereas pediatric cardiologists routinely perform this task, primary care providers are less successful in distinguishing Still's murmur from the murmurs of true heart disease. This results in a large number of children with a Still's murmur being referred to pediatric cardiologists. OBJECTIVES: To develop a computer algorithm that can aid primary care providers to identify the innocent Still's murmur at the point of care, to subst

Case Report: Coexistence of generalized arterial calcification of infancy (GACI) and maternal infections with cytomegalovirus and Toxoplasma gondii-unexpected fatal complication in a newborn.

Generalized arterial calcification of infancy (GACI) is an extremely rare autosomal recessive condition characterized by the storage of calcium at the level of internal elastic membrane of arteries. The main consequences are intimal fibrous thickening and arterial occlusion. We present the case of a preterm male infant, born from an improperly dispensed pregnancy. At birth, the newborn presented generalized edema and hypotonia, and abolished heart sounds, without response to stimulation. Despite the mechanical ventilation, the infant died 2 h after birth. The death was clinically presumed to b

Primary salivary gland-type polymorphous adenocarcinoma in the lung: A case report and literature review.

RATIONALE: Polymorphous low-grade adenocarcinoma is a low-risk infiltrative malignant tumor of the salivary glands. However, some of these tumors are more malignant than the low-grade tumors and therefore, according to the most recent recommendation of the World Health Organization, they are renamed as polymorphous adenocarcinomas (PACs). Primary polymorphous low-grade adenocarcinomas/PACs of the lungs are rare. Herein, we report a case of primary PAC of the lung with bronchial cartilage and perineural invasion, and lymph node metastasis. PATIENT CONCERNS: A 58-year-old man had developed feve

Temporal-Framing Adaptive Network for Heart Sound Segmentation Without Prior Knowledge of State Duration.

OBJECTIVE: This paper presents a novel heart sound segmentation algorithm based on Temporal-Framing Adaptive Network (TFAN), including state transition loss and dynamic inference. METHODS: In contrast to previous state-of-the-art approaches, TFAN does not require any prior knowledge of the state duration of heart sounds and is therefore likely to generalize to non sinus rhythm. TFAN was trained on 50 recordings randomly chosen from Training set A of the 2016 PhysioNet/Computer in Cardiology Challenge and tested on the other 12 independent databases (2,099 recordings and 52,180 beats). And fur

The assessment of blood pressure in pregnant women: pitfalls and novel approaches.

Accurate assessment of blood pressure is fundamental to the provision of safe obstetrical care. It is simple, cost effective, and life-saving. Treatments for preeclampsia, including antihypertensive drugs, magnesium sulfate, and delivery, are available in many settings. However, the instigation of appropriate treatment relies on prompt and accurate recognition of hypertension. There are a number of different techniques for blood pressure assessment, including the auscultatory method, automated oscillometric devices, home blood pressure monitoring, ambulatory monitoring, and invasive monitoring

Timing the opening and closure of the aortic valve using a phonocardiogram envelope: a performance test for systolic time intervals measurement.

OBJECTIVE: This work explored the reliability of using points on the heart sounds envelope as indicators of the opening and closure of the aortic valve (AVO, AVC) to measure the pre-ejection period (PEP) and the left ventricular ejection time (LVET). APPROACH: 36 phonocardiograms (PCGs) from healthy subjects and cardiovascular disease subjects were denoised using single-channel independent component analysis (SCICA) and, from the Hilbert envelopes, the positions of the S1 and S2 peaks were detected (pS1, pS2). Complementarily, the positions of the local maxima of S1 and S2 (mS1, mS2) and the

Heart sound recognition technology based on convolutional neural network.

The mortality rate of heart disease continues to rise each year: developing mechanisms to reduce mortality from heart disease is a top concern in today's society. Heart sound auscultation is a crucial skill used to detect and diagnose heart disease. In this study, we propose a heart sound signal classification algorithm based on a convolutional neural network. The algorithm is based on heart sound data collected in the clinic and from medical books. The heart sound signals were first preprocessed into a grayscale image of 5 seconds. The training samples were then used to train and optimize the

Automated, multiparametric monitoring of respiratory biomarkers and vital signs in clinical and home settings for COVID-19 patients.

Capabilities in continuous monitoring of key physiological parameters of disease have never been more important than in the context of the global COVID-19 pandemic. Soft, skin-mounted electronics that incorporate high-bandwidth, miniaturized motion sensors enable digital, wireless measurements of mechanoacoustic (MA) signatures of both core vital signs (heart rate, respiratory rate, and temperature) and underexplored biomarkers (coughing count) with high fidelity and immunity to ambient noises. This paper summarizes an effort that integrates such MA sensors with a cloud data infrastructure and

Meditation Monologue can Reduce Clinical Injection-Related Anxiety: Randomized Controlled Trial.

BACKGROUND: Strategies to reduce anxiety prior to injection procedures are not well understood. The purpose is to determine the effect of a meditation monologue intervention delivered via phone/mobile application on pre-injection anxiety levels among patients undergoing a clinical injection. The following hypothesis was tested: patients who listened to a meditation monologue via phone/mobile application prior to clinical injection would experience less anxiety compared to those who did not. METHODS: A prospective, randomized controlled trial was performed at an orthopedics and sports medicine

The Use of Physostigmine in the Diagnosis and Treatment of Anticholinergic Toxicity After Olanzapine Overdose: Literature Review and Case Report.

BACKGROUND: Second-generation antipsychotic agents are commonly used by clinicians for the treatment of various psychiatric and medical conditions. Despite their presumed safety, an overdose with olanzapine may lead to the development of anticholinergic toxicity. The anticholinergic toxidrome is characterized by both central and peripheral physical findings. Central anticholinergic syndrome, a term used to describe the symptoms that arise from reduced cholinergic activity in the central nervous system, is characterized primarily by signs and symptoms consistent with hyperactive delirium. Signs

Evaluation of a digitally enhanced cardiac auscultation learning method: a controlled study.

BACKGROUND: Cardiac auscultation remains an efficient and accessible diagnostic tool, especially in resource-limited countries where modern diagnostic devices like cardiac ultrasound are expensive and difficult to access. However, cardiac auscultation skills of medical students and physicians are declining, mainly because of an ineffective teaching method for this technique. The objective of this study is to evaluate the effect of a digitally enhanced cardiac auscultation learning method on participants' theoretical knowledge and auscultation skills. METHODS: This will be a controlled study w

Sound Effects on Physiological State and Behavior of Drivers in a Highway Tunnel.

Driving behavior in a highway tunnel could be affected by external environmental factors like light, traffic flow, and acoustic environments, significantly when these factors suddenly change at the moment before and after entering a tunnel. It will cause tremendous physiological pressure on drivers because of the reduction of information and the narrow environment. The risks in driving behavior will increase, making drivers more vulnerable than driving on the regular highways. This research focuses on the usually neglected acoustic environment and its effect on drivers' physiological state and

Sounds of Silence in Times of COVID-19: Distress and Loss of Cardiac Coherence in People With Misophonia Caused by Real, Imagined or Evoked Triggering Sounds.

The extreme, unprecedented situations in the current COVID-19 pandemic are risk factors for psychosocial stress for the entire population. However, strict confinement had a particular impact on people suffering from misophonia and their families. Misophonia is a condition in which hearing certain sounds triggers intense anger, disgust and even severe autonomic nervous system responses. This prospective cohort study examined the impact of strict confinement (Spain, March 14-June 21, 2020) on a sample of 24 people (16 women and eight men) who had been diagnosed with moderate to extreme misophoni
