Enhanced heart sound anomaly detection via WCOS: a semi-supervised framework integrating wavelet, autoencoder and SVM.

Anomaly detection is a typical binary classification problem under the condition of unbalanced samples, which has been widely used in various fields of data mining. For example, it can help detect heart murmurs when the heart is structurally abnormal, to tell if a newborn has congenital heart disease. Due to the low time and high efficiency, most work focuses on the semi- supervised anomaly detection method. However, the anomaly detection effect of this method is not high because of massive data with uneven samples and different noise. To improve the accuracy of anomaly detection under unbalan

AI-facilitated home monitoring for cystic fibrosis exacerbations across pediatric and adult populations.

BACKGROUND: AI-aided home stethoscopes offer the opportunity of continuous remote monitoring of cystic fibrosis (CF) patients, reducing the need for clinic visits. AIM: This study aimed to analyze the possibility of detecting CF pulmonary exacerbations (PEx) at home using an AI-aided stethoscope (AIS). MATERIALS AND METHODS: In a six-month study, 129 CF patients (85 children, 44 adults) used AIS for at least weekly self-examinations, recording various parameters: wheezes, rhonchi, crackles intensity, respiratory and heart rate, and inspiration-to-expiration ratio. Health state surveys were a

Update in paediatric asthma.

PURPOSE OF REVIEW: The field of paediatric asthma is rapidly moving, with the advent of new biologicals for severe asthma and increased understanding of preschool wheeze amongst other developments and insights. RECENT FINDINGS: There is increasing evidence of efficacy in children for biologics directed against Type 2 inflammation (especially mepolizumab and dupilumab) as well encouraging evidence that Tezepelumab may be effective against Type 2 low phenotypes. The importance of airway remodelling and infection in the pathophysiology of preschool wheeze is increasingly appreciated. The treatme

Point-of-Care Blood Eosinophils to Predict Preschool Wheeze Attacks.

BACKGROUND: Post hoc analysis of clinical trials shows blood eosinophil counts (BEC) predict future preschool wheeze attacks; however, prospective usefulness in a clinical setting is unreported. We assessed the feasibility of point-of-care (POC) eosinophil measurements in preschool wheezers and related BEC to symptoms, lung function, and utility in predicting attacks. METHODS: Children aged 1-5 years with recurrent wheeze underwent finger-prick sampling during the outpatient clinic for POC eosinophils, forced oscillation technique (FOT) and/or spirometry, and symptom score (TRACK questionnair

The Impact of Disgusting Sounds on Pupil Diameter of Misophonic and Non-Misophonic Listeners.

Everyday sounds can elicit a range of emotional and physiological responses. For individuals with misophonia, some sounds can produce strong feelings of disgust, annoyance, and anger, often accompanied by increased perspiration and heart rate. Presently, methods of diagnosing misophonia rely on clinical interviews and self-assessment scales. Coupling these subjective measures with an objective, physiological method like pupillometry may lead to improved understanding of misophonia and inform efficacy of treatments. Previous studies show that both unpleasant and pleasant sounds increase pupil d

A New Heartbeat-Respiration Coupling Metric for Relaxation/Stress Monitoring.

OBJECTIVE: Identification and continuous monitoring of relaxation/stress level through easily measurable physiological signals has become an urgent need. Relaxation/stress is mainly mediated by autonomic nervous system (ANS) which can hardly be noninvasively-measured, but can be reflected in heartbeat and respiration because the human body is a strongly coupling system. However, existing metrics based on heart rate and respiration present low accuracy for relaxation assessment. Our main objective is to propose a reliable metric for relaxation/stress monitoring. METHODS: By improving the quant

Point-of-Care Ultrasound in Early Identification of Tamponade: A Case Series.

Cardiac tamponade is a life-threatening condition resulting from fluid accumulation within the pericardial sac, leading to impaired cardiac filling and output. Signs described in the literature, such as Beck's triad (hypotension, jugular venous distension (JVD), muffled heart sounds) or pulsus paradoxus can be absent especially in the early stages, particularly in non-trauma patients, making early diagnosis challenging. In this case series, we present three patients who developed tamponade with subtle symptoms, including shortness of breath and tachycardia, but without hypotension or other hal

Responding to clinical alarms in unfolding simulated clinical scenarios: auditory icons perform better than tonal alarms.

BACKGROUND: The international medical device safety standard IEC 60601-1-8 now recommends use of auditory icon alarms. Auditory icon alarms are alarms that act as metaphors for the problems that they signal. These are compared with traditional tonal alarms. METHODS: Two sets of three auditory alarms were compared, one consisting of auditory icons and one consisting of variants of older tonal alarms. Volunteer participants were required to monitor three clinical scenarios each lasting 4 min with different problems (cardiac, oxygenation, and technical) occurring during each of these scenarios t

Approaches to reduce the risk of severe asthma in children with preschool wheeze.

INTRODUCTION: Asthma is a common, serious condition. We can treat the symptoms of mild-moderate disease, but severe asthma is life-threatening despite treatment. We cannot cure asthma and have no specific preventive strategies. AREAS COVERED: We performed a PubMed search using the terms 'Severe asthma' and 'Prevention' and 'Preschool wheeze' limited to children, humans and English language over the previous five years. We searched the bibliographies of relevant references and also our personal archives. We cover transgenerational, antenatal and early life factors which increase the risk of pr

Aortic Valve Endocarditis in an Intravenous Drug User With Psychiatric History: A Diagnostic Challenge.

Infective endocarditis (IE), a life-threatening cardiac infection, can present atypically, complicating diagnosis. We present a case of a 45-year-old male patient with schizophrenia and intravenous drug use (IVDU) who presented with suicidal ideation. Though afebrile, he exhibited tachycardia, bilateral pedal edema, bronchial breath sounds, and a diastolic murmur. Echocardiography identified an extensive aortic valve vegetation and a reduced ejection fraction. Despite negative resected valve cultures and Gram staining, emergency aortic valve replacement and left atrial appendage clipping were

Genetic detection and discrimination of closely related oyster pathogens for biosecurity and shellfish stock management.

Disease outbreaks among shellfish cause significant declines in both wild fisheries and aquaculture production globally. In Aotearoa New Zealand, a long history of flat oyster infections by the haplosporidian parasite Bonamia exitiosa has influenced fishery dynamics in the Foveaux Strait. B. ostreae, which causes high host mortality, was detected in both wild and farmed oysters in the Marlborough Sounds in 2015 and at Stewart Island in 2017, prompting the development of rapid diagnostic tests with high sensitivity and specificity that can be used to simultaneously monitor both Bonamia species.

Dynamic brain-heart-gut coupling during sleep: a continuous physiological signal analysis.

BACKGROUND: Investigating brain-heart-gut coupling during sleep is crucial for understanding the coordinated regulatory mechanisms of multiple systems during sleep. Non-invasive continuous physiological signal acquisition techniques have been widely applied in brain-heart dynamic assessment. However, current research on gut function primarily focuses on gut microbiota, with a lack of systematic investigation into the macroscopic dynamic changes of gut function. This study, therefore, based on multiple non-invasive physiological signals, aims to explore the dynamic changes and underlying mechan

Patterns of Respiratory Symptoms and Asthma Diagnosis in School-Age Children: Three Birth Cohorts.

BACKGROUND: Many studies used information on wheeze presence/absence to determine asthma-related phenotypes. We investigated whether clinically intuitive asthma subtypes can be identified by applying data-driven semi-supervised techniques to information on frequency and triggers of different respiratory symptoms. METHODS: Partitioning Around Medoids clustering was applied to data on multiple symptoms and their triggers in school-age children from three birth cohorts: MAAS (n = 947, age 8 years), SEATON (n = 763, age 10) and ASHFORD (n = 584, age 8). 'Guided' clustering, incorporating asthma d

Method for Predict Stenosis of Arteriovenous Fistula Patients Based on Machine Learning.

OBJECTIVES: Arteriovenous fistula (AVF) is the most ideal vascular access for hemodialysis. People with AVF have a longer vascular access survival rate and a lower complication rate. Thrombosis and stenosis are the most common complications of AVF. The annual thrombosis event rate is 10%-50%. Appropriate identification of AVF stenosis and management could reduce the risk of thrombosis and access loss. Guidelines recommended physical examinations as the first line of AVF stenosis monitoring. However, even for health professionals, the diagnosis rate by hearing the bruit varied. The sound waves
