An efficient lung sound separation algorithm base on GIHO-VMD.

BACKGROUND AND OBJECTIVE: Lung sounds (LS) serve as a critical source of pathological information for diagnosing lung diseases. However, during clinical auscultation, LS are often contaminated by heart sounds (HS) and other noises, significantly compromising the accuracy of pathological assessment. The primary challenge lies in the spectral overlap between LS and HS, coupled with the non-stationary nature of both signals. Furthermore, abnormal LS exhibit intricate frequency components and ambiguous band boundaries, causing conventional separation methods to yield incomplete or contaminated res

Phonocardiopulmography: the future of perioperative auscultation.

Traditionally, anesthesiologists have recommended performing auscultation of breath and heart sounds with a precordial or esophageal stethoscope during anesthesia. The stethoscope is generally considered the most important single device used for monitoring. Unfortunately, many anesthesiologists neglect the perioperative use of a stethoscope, even though it provides a wealth of information about the circulatory system, heart, and lungs. Although point-of-care ultrasound appears to offer greater diagnostic value in some situations, it is premature to dismiss the stethoscope, and its diminished p

Heart Failure-Associated Tension Chylothorax: A Rare Presentation of a Common Disease.

Heart failure can present along with pleural effusion, which is usually bilateral and biochemically transudative. This pleural effusion can rarely be chylous, indicating chylothorax. Herein, we describe a case of an 86-year-old male patient with heart failure with reduced ejection fraction who presented with a two-week history of progressive dyspnea and bilateral leg swelling. Physical findings included absent breath sounds in the right mid- and lower lung zones, left basilar crackles, and 3+ pitting edema up to the knees. Chest X-ray and computed tomography of the chest revealed a large right

Assessing the Utility of Audio Foundation Models for Heart and Respiratory Sound Analysis.

Pre-trained deep learning models, known as foundation models, have become essential building blocks in machine learning domains such as natural language processing and image domains. This trend has extended to respiratory and heart sound models, which have demonstrated effectiveness as off-the-shelf feature extractors. However, their evaluation benchmarking has been limited, resulting in incompatibility with state-of-the-art (SOTA) performance, thus hindering proof of their effectiveness. This study investigates the practical effectiveness of off-the-shelf audio foundation models by comparing

Remote Photoplethysmography-Triggered Signal Averaging for Non-Contact Heart Sound Measurement.

Remote photoplethysmography- (rPPG-) triggered signal averaging, a novel approach for the non-contact heart sound measurement, was proposed. Recently, there has been a growing need for a heart sound monitoring method for the early detection of cardiovascular diseases. Therefore, we proposed a non-contact heart sound measurement approach using a signal averaging with rPPG peaks as triggers. This approach can emphasize the heart sound, the only sound synchronized in time with the heartbeat, and reduces other ambient noises. Herein, we evaluated the feasibility of our approach. As a result, it wa

Using Convolutional Neural Networks for Fetal Heart Sound Segmentation.

The development of phonocardiography processing methods is gradually becoming more important as the amount of data increases due to the ease and low cost of such measurements. To improve diagnosis and aid medical experts, heart sound-based disorder classification methods have been introduced. However, most of these methods rely on an accurately segmented phonocardiogram signal. This can be achieved in a multitude of ways, the most widely used being the hidden semi-Markov model with logistic regression. However, with the introduction of large, labeled databases, such as the PhysioNet challenge

User-Centered Redesign of Monitoring Alarms: A Pre-Post Study on Perception, Functionality, and Recognizability Following Real-Life Clinical Implementation.

Background: Auditory alarms in patient monitoring are vital for clinical safety, but their harsh acoustic properties and high frequency contribute to stress, alarm fatigue, and reduced acceptance among healthcare staff. In collaboration with Sen Sound, Philips redesigned its alarm sounds to reduce auditory harshness, particularly for low- and medium-priority alarms, while preserving the salience of high-priority alerts. This study evaluated the impact of these refined alarm sounds in a real-world clinical setting. Objective: The goal was to determine whether anesthesia professionals perceive t

Clustering lung function and symptom profiles for asthma risk stratification.

Asthma is a heterogeneous condition often studied through wheeze alone, yet the interplay between lung function and reported symptoms remains underexplored. To capture this heterogeneity, we applied Bayesian Profile Regression to data from school-age children in two prospective birth cohorts, integrating airway hyperresponsiveness, lung function, bronchodilator reversibility, allergic sensitisation, reported symptoms, and physician diagnosis. In the Manchester Allergy and Asthma Study (discovery cohort), five reproducible clusters were identified: HA-LLF (high asthma-low lung function), HA-NLF

THE EFFICACY OF SENSORY-ADAPTED DENTAL INTERVENTIONS FOR CHILDREN WITH DEVELOPMENTAL DISABILITIES AND SENSORY SENSITIVITIES.

BACKGROUND: Children diagnosed with autism spectrum disorder (ASD) are often more sensitive to environmental stimuli, particularly light, sound, and touch, which can provoke significant anxiety and behavioral resistance during dental visits, thereby contributing to poor oral health. OBJECTIVE: To assess the efficacy of sensory adapted dental environments (SADEs) and related interventions to reduce dental anxiety by improving behavioral cooperation of children with ASD based on peer-reviewed evidence. Artificial Intelligence Enabled Lung Sound Auscultation in the Early Diagnosis and Subtyping of Interstitial Lung Disease.

Background: Interstitial lung disease (ILD) involves numerous chronic pulmonary conditions that damage the lung parenchyma and alveolar interstitium. ILD has overlapping clinical and radiological features with other commonly seen cardiac and respiratory conditions. If not identified and treated in a timely manner, it may lead to irreversible fibrosis and a poor prognosis in the patient. The current diagnostic methods are either invasive or reliant on imaging or specialist interpretation, which can lead to diagnostic delay, increased radiation exposure, and healthcare costs. Lung crackles, ofte

Cardiac Adaptation in Lupus: A Case of Massive Pericardial Effusion With Preserved Hemodynamics.

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease; cardiac involvement is a recognized complication, with pericardial effusion being one of the most frequent manifestations. Here, we present a patient with massive pericardial effusion in a known SLE patient without hemodynamic instability, highlighting concepts of pericardial compliance and physiological adaptation in autoimmune disease. We present a case of a 33-year-old female with a known history of SLE who presented with progressively worsening pleuritic chest pain over three weeks. She was hemodynamically stable, with

Blocking the neonatal Fc receptor as a novel approach to prevent cardiac neonatal lupus: a proof-of-concept study.

OBJECTIVES: Novel biologic agents targeting the neonatal Fc receptor (FcRn) offer a promising strategy to prevent cardiac neonatal lupus (cardiac-NL) in pregnant patients with high-titre anti-SSA/Ro52 kD or 60 kD autoantibodies via dual effects: reducing serum immunoglobin G (IgG) levels and inhibiting placental transfer. This study was initiated to assess the feasibility of FcRn blockade as prophylactic therapy for recurrent cardiac-NL. METHODS: A 34-year-old pregnant patient with systemic lupus erythematosus and 3 prior consecutive pregnancies complicated by neonatal lupus (1 cutaneous, 1 f

Congestive Nephropathy: A Neglected Entity in Heart Failure.

Contrary to common belief, the primary cause of acute kidney injury in patients presenting to emergency departments with heart failure is not diminished renal perfusion but rather renal venous congestion and increased pressure. In patients without overt signs of heart failure during initial physical assessment, however, congestion often cannot be distinguished through physical examination alone, leading to congestive nephropathy being frequently overlooked in the differential diagnosis of worsening renal function. Physicians often avoid diuretics for fear of exacerbating renal impairment and i

Transfusion-Related Acute Lung Injury Detected by Perioperative Lung Ultrasound: Early Recognition With Point-of-Care Ultrasound (POCUS).

Transfusion-related acute lung injury (TRALI) is a rare but potentially life-threatening transfusion reaction characterized by acute hypoxemia and non-cardiogenic pulmonary edema within six hours of transfusion. We report the case of a 20-year-old female patient who developed acute hypoxemia approximately 1.5 hours after receiving one unit of packed red blood cells (PRBCs) during dilation and suction curettage under general anesthesia. Postoperatively, she experienced dyspnea, oxygen desaturation, and coarse bilateral breath sounds. Point-of-care ultrasound (POCUS) demonstrated multiple bilate

Automated system for diagnosing pulmonary fibrosis using crackle analysis in recorded lung sounds based on iterative envelope mean fractal dimension filter.

Patients with pulmonary fibrosis (PF) often experience long waits before getting a correct diagnosis, and this delay in reaching specialized care is associated with increased mortality, regardless of the severity of the disease. Early diagnosis and timely treatment of PF can potentially extend life expectancy and maintain a better quality of life. Crackles present in the recorded lung sounds may be crucial for the early diagnosis of PF. This paper describes an automated system for differentiating lung sounds related to PF from other pathological lung conditions using the average number of crac

Unveiling a Hidden Diaphragmatic Hernia Underlying a Spontaneous Hydropneumothorax: A Diagnostic Surprise.

Right-sided diaphragmatic hernias in adults are rare and often pose diagnostic challenges due to their atypical clinical and radiological presentations. The coexistence of a spontaneous hydropneumothorax can obscure the underlying pathology, initially diverting attention toward pleural management. We report a rare case of a spontaneous right-sided diaphragmatic hernia discovered incidentally beneath a spontaneous hydropneumothorax in an elderly patient presenting with chest pain, respiratory distress, and abdominal discomfort. Clinical examination revealed decreased breath sounds on the right

Prehospital needle thoracostomy and the need to implement objective criteria for intervention: A retrospective study.

BACKGROUND: Needle thoracostomy (NT) is a frontline intervention for suspected tension pneumothorax in prehospital trauma care. The necessity for intervention in patients with relative indications is unclear, and locoregional protocols guiding NT placement by prehospital personnel vary. This study aims to identify factors associated with a positive response to NT and how often objective measures are utilized to prompt intervention, which may help better define indications for the procedure. METHODS: A retrospective review of adult trauma patient who received prehospital needle decompression w

Thrombolysis in Prosthetic Valve Thrombosis: A Safe and Effective Alternative?

Prosthetic valve thrombosis (PVT) is a rare but life-threatening complication of mechanical heart valves. Surgery remains the standard of care, but thrombolysis has emerged as a valuable alternative in patients with high surgical risk or refusal. We describe a 38-year-old woman, five years after double mitral-aortic mechanical valve replacement, who had discontinued anticoagulation one month prior to admission. She presented with acute angina-like chest pain and severe dyspnea (New York Heart Association (NYHA) class IV). Clinical examination revealed diminished prosthetic clicks and pulmonary
