Coexistence of Severe Valvular Pulmonary Stenosis and Papillary Thyroid Carcinoma: A Rare Association or Coincidence?

Papillary thyroid carcinoma (PTC) is the most common thyroid malignancy. Pulmonary valve stenosis (PVS), on the other hand, is a congenital valvular heart disease characterized by narrowing of the right ventricular outflow tract, which can lead to right-sided heart failure if severe. To date, there are no reported cases in the literature describing a co-occurrence of PTC and PVS in the same individual. Our patient was a 28-year-old male, a smoker with a five-pack-year history, who presented with a painless right neck swelling of five months duration. Physical examination revealed a firm, non-t

Automated detection of pediatric congenital heart disease from phonocardiograms using deep and handcrafted feature fusion.

Congenital heart disease (CHD) is the most common type of birth defect, impacting about 1% of live births worldwide. Echocardiography, the gold-standard diagnostic method, is costly and inaccessible in low-resource settings. Diagnosis is delayed due to limited skilled experts, whose ability to interpret pathological patterns varies significantly, causing inter- and intra-clinician variability. Therefore, we present a new method for a more accessible diagnostic modality, the digital stethoscope, to detect CHDs. Our method is based on deep feature fusion, integrating deep and handcrafted feature

Acute Rheumatic Fever in an Adult Female Patient Presenting With Chest Pain: A Case Report.

Acute rheumatic fever (ARF) is an autoimmune disease triggered by group A Streptococcus (GAS) infection, predominantly affecting children in low- and middle-income countries. Although its incidence has significantly decreased in the United States (US), ARF remains a major cause of morbidity and mortality worldwide due to complications like rheumatic heart disease (RHD). The condition arises from molecular mimicry, where the immune system targets host tissues after a GAS infection, leading to inflammation and potential damage to the heart, joints, and nervous system. Early diagnosis and treatme

Multi-Feature Decision Fusion Network for Heart Sound Abnormality Detection and Classification.

The heart sound reflects the movement status of the cardiovascular system and contains the early pathological information of cardiovascular diseases. Automatic heart sound diagnosis plays an essential role in the early detection of cardiovascular diseases. In this study, we aim to develop a novel end-to-end heart sound abnormality detection and classification method, which can be adapted to different heart sound diagnosis tasks. Specifically, we developed a Multi-feature Decision Fusion Network (MDFNet) composed of a Multi-dimensional Feature Extraction (MFE) module and a Multi-dimensional Dec

ZCHSound: Open-Source ZJU Paediatric Heart Sound Database With Congenital Heart Disease.

UNLABELLED: Congenital heart disease (CHD) is a common birth defect in children. Intelligent auscultation algorithms have been proven to reduce the subjectivity of diagnoses and alleviate the workload of doctors. However, the development of this algorithm has been limited by the lack of reliable, standardized, and publicly available pediatric heart sound databases. Therefore, the objective of this research is to develop a large-scale, high-standard, high-quality, and accurately labeled pediatric CHD heart sound database. METHOD: From 2020 to 2022, we collaborated with experienced cardiac surg

Rhinovirus-induced wheeze was associated with asthma development in predisposed children.

AIM: This study explored whether early-life factors, such as rhinovirus-induced wheeze and allergic sensitisation, were related to asthma at 11 years of age. METHODS: We focused on 107 children aged 6-48 months, who attended the paediatric emergency department at Astrid Lindgren's Children's Hospital in Stockholm, Sweden, with acute wheeze in 2008-2012. They also attended follow-up visits at 11 years of age and were compared with 46 age-matched healthy controls. Odds ratios (OR) with 95% confidence intervals (CI) were calculated with logistic regression. RESULTS: We found that 62.6% of the a

Correlations between heart sound components and hemodynamic variables.

Although the esophageal stethoscope is used for continuous auscultation during general anesthesia, few studies have investigated phonocardiographic data as a continuous hemodynamic index. In this study, we aimed to induce hemodynamic variations and clarify the relationship between the heart sounds and hemodynamic variables through an experimental animal study. Changes in the cardiac contractility and vascular resistance were induced in anesthetized pigs by administering dobutamine, esmolol, phenylephrine, and nicardipine. In addition, a decrease in cardiac output was induced by restricting the

All That Wheezes Is Not Asthma.

Wheezing is a high pitched, whistling sound generated when air flows through narrowed airways and is often equated with asthma. However, wheezing may be a presenting symptom of various other conditions including structural lesions of the airways, foreign body aspiration, pulmonary infections as well as cardiac causes. Underlying etiology of wheezing may also vary with age. Detailed history, physical examination, and laboratory investigations are often required to identify the underlying etiology of wheezing. Additional studies may sometimes be needed to accurately identify the underlying etiol

Voice reduction in cardiac auscultation sounds with reference signals measured from vocal resonators.

This study proposes the use of vocal resonators to enhance cardiac auscultation signals and evaluates their performance for voice-noise suppression. Data were collected using two electronic stethoscopes while each study subject was talking. One collected auscultation signal from the chest while the other collected voice signals from one of the three voice resonators (cheek, back of the neck, and shoulder). The spectral subtraction method was applied to the signals. Both objective and subjective metrics were used to evaluate the quality of enhanced signals and to investigate the most effective

Fully Convolutional Hybrid Fusion Network With Heterogeneous Representations for Identification of S1 and S2 From Phonocardiogram.

Heart auscultation is a simple and inexpensive first-line diagnostic test for the early screening of heart abnormalities. A phonocardiogram (PCG) is a digital recording of an analog heart sound acquired using an electronic stethoscope. A computerized algorithm for PCG analysis can aid in detecting abnormal signal patterns and support the clinical use of auscultation. It is important to detect fundamental components, such as the first and second heart sounds (S1 and S2), to accurately diagnose heart abnormalities. In this study, we developed a fully convolutional hybrid fusion network to identi

Importance of fourth heart sound and preserved left atrial function in wild-type transthyretin amyloidosis.

AIMS: A fourth heart sound (S4) was reported to be almost never present in patients with amyloid light-chain cardiomyopathy. There have been no reports on S4 in patients with wild-type transthyretin amyloid cardiomyopathy (ATTRwt-CM). This study aimed to clarify the clinical implications of S4 in patients with ATTRwt-CM. METHODS AND RESULTS: Seventy-six patients with ATTRwt-CM (mean age: 80.4 ± 5.4 years, 68 males) who had undergone phonocardiography (PCG) were retrospectively assessed. We measured S4 amplitude on digitally recorded PCG. S4 was considered to be present when its amplitude was

The heart sound classification of congenital heart disease by using median EEMD-Hurst and threshold denoising method.

Heart sound signals are vital for the machine-assisted detection of congenital heart disease. However, the performance of diagnostic results is limited by noise during heart sound acquisition. A limitation of existing noise reduction schemes is that the pathological components of the signal are weak, which have the potential to be filtered out with the noise. In this research, a novel approach for classifying heart sounds based on median ensemble empirical mode decomposition (MEEMD), Hurst analysis, improved threshold denoising, and neural networks are presented. In decomposing the heart sound

A Novel Non-invasive Predictor Measure of Left Atrial Pressure and Practical Implicates in High Frequency Ventilation: Theory.

A new prototype of ventilator mode, based on high-frequency ventilation (HFV), aims to address the interrelations among diverse respiratory variables, several intercardiovascular parameters rapports, and instantaneously the intrarespiratory and cardiac interactions. This article proposes a logarithmic network of integrated correlation ratio H/V, R correlation, that primarily relates both dependent respiratory (V) and independent hemodynamic variables (H). Given a constant steady state of the heart and vasculature, a difference between calculated ratio (R calc), and optimized ratio (R opt) coul

A Case of Pericardial Effusion in Association With Celiac Disease.

This is the case of a 22-year-old female with celiac disease-induced pericardial effusion. Celiac disease is a gastroenterological autoimmune condition that affects several organ systems. It is a disease found in both children and adults. As many systems are involved, this case presented with a unique presentation: pericardial effusion with symptoms overlapping those of cardiac chest pain such as substernal chest pain and shortness of breath. The patient had tachycardia, tachypnea, and jugular venous distention with distant S1 and S2 heart sounds. Cardiothoracic surgery was consulted and diagn

Complete atrioventricular canal in a dog-sounds like a final judgment but is it actually one? A case report.

Congenital heart disease (CHD) is an important subset of all cardiovascular diseases in dogs. Among them, there are a number of other less commonly described congenital malformations. One of these is the atrioventricular canal, which involves a defect that has several phenotypes. This report details a case of a complete atrioventricular canal (cAVC) defect. Using Rastelli's classification scheme, the diagnosis of a type-A cAVC defect was based on two-dimensional, contrast, and color Doppler echocardiography. Despite a severe defect that resulted in the atypical anatomy of the atrioventricular
