Assessing consciousness in acute coma using name-evoked responses.

Detecting consciousness in clinically unresponsive patients remains a significant challenge. Existing studies demonstrate that electroencephalography (EEG) can detect brain responses in behaviorally unresponsive patients, indicating potential for consciousness detection. However, most of this evidence is based on chronic patients, and there is a lack of studies focusing on acute coma cases. This study aims to detect signs of residual consciousness in patients with acute coma by using bedside EEG and electromyography (EMG) during an auditory oddball paradigm. We recruited patients with acute br

Deep Learning in Heart Sound Analysis: From Techniques to Clinical Applications.

Importance: Heart sound auscultation is a routinely used physical examination in clinical practice to identify potential cardiac abnormalities. However, accurate interpretation of heart sounds requires specialized training and experience, which limits its generalizability. Deep learning, a subset of machine learning, involves training artificial neural networks to learn from large datasets and perform complex tasks with intricate patterns. Over the past decade, deep learning has been successfully applied to heart sound analysis, achieving remarkable results and accumulating substantial heart s

Diaphragmatic Dysfunction Following Endoscopic Coronary Artery Bypass Grafting in a Patient With Concomitant Sleep Disordered Breathing: A Case Report.

Unilateral diaphragmatic paralysis (UDP) is a frequent complication following cardiac surgery, usually affecting the left hemidiaphragm. Here, we present a case of a right-sided UDP following endoscopic coronary artery bypass grafting (CABG), which is far more uncommon. A 60-year-old male patient presented at our outpatient clinic with exertional dyspnoea and orthopnoea. Breathing sounds were diminished upon auscultation of the right lung base. He recently underwent a CABG through video-assisted thoracoscopic surgery (VATS). We documented a new right-sided UDP as well as severe obstructive sle

[Heart sound classification algorithm based on bispectral feature extraction and convolutional neural networks].

Cardiovascular disease (CVD) is one of the leading causes of death worldwide. Heart sound classification plays a key role in the early detection of CVD. The difference between normal and abnormal heart sounds is not obvious. In this paper, in order to improve the accuracy of the heart sound classification model, we propose a heart sound feature extraction method based on bispectral analysis and combine it with convolutional neural network (CNN) to classify heart sounds. The model can effectively suppress Gaussian noise by using bispectral analysis and can effectively extract the features of he

A Case Report of Methicillin-Resistant Staphylococcus aureus-Related Pericardial Empyema: A Deadly Cardiac Risk.

Pericardial empyema, a rare but often fatal condition, is frequently diagnosed postmortem. Staphylococcus aureus is the most common causative organism. This case involves a 73-year-old male with diabetes and hypothyroidism who presented with chills, fever, and a persistent skin infection. Cultures revealed Methicillin-resistant Staphylococcus aureus (MRSA), and he was treated with intravenous Vancomycin. On the third day, the patient experienced sudden chest pain, and examination revealed muffled heart sounds, jugular venous distension, sinus tachycardia, and diffuse ST elevation. Transthoraci

Impact of a music intervention on heart rate variability in very preterm infants.

AIM: Infants born very preterm spend their early postnatal life in a neonatal intensive care unit, where irregular and unpredictable sounds replace the structured and familiar intrauterine auditory environment. Music interventions may contribute to alleviate these deleterious effects by reducing stress and providing a form of environmental enrichment. MATERIAL AND METHODS: This was an ancillary study as part of a blinded randomised controlled clinical trial entitled the effect of music on preterm infant's brain development. It measured the impact of music listening on the autonomic nervous sy

A review on deep learning methods for heart sound signal analysis.

INTRODUCTION: Application of Deep Learning (DL) methods is being increasingly appreciated by researchers from the biomedical engineering domain in which heart sound analysis is an important topic of study. Diversity in methodology, results, and complexity causes uncertainties in obtaining a realistic picture of the methodological performance from the reported methods. METHODS: This survey paper provides the results of a broad retrospective study on the recent advances in heart sound analysis using DL methods. Representation of the results is performed according to both methodological and appl

Association of urinary eosinophilic protein X at age 3 years and subsequent persistence of wheezing and asthma diagnosis in adolescence.

BACKGROUND: Wheezing is common in early life, but most children stop wheezing by school age. However, the prediction of course of wheezing through childhood is difficult. OBJECTIVE: To investigate whether urinary EPX (a marker of eosinophil activation) in children at age 3 years may be useful for the prediction of wheeze persistence and future asthma diagnosis. METHODS: U-EPX was measured at age 3 years (radioimmunoassay) in 906 participants in the population-based birth cohort. Children attended follow-ups to age 16 years. We investigate the discriminative ability of u-EPX and other factors

Pontine Osmotic Demyelination Syndrome in a Pregnant Woman With Sepsis and Wernicke's Encephalopathy.

A pregnant woman was brought to the emergency department looking starved and neglected. She was diagnosed with sepsis and started on intravenous antibiotics. She was also disoriented and hypernatremic. When the fetal heart sounds were found to be absent, the patient was diagnosed with septic miscarriage, which was managed by misoprostol, and after the expulsion of the products of conception, she was taken for evacuation of the uterus under anesthesia. Her brain CT was normal; however, due to her disorientation and altered mental status, a brain MRI was done, which revealed findings suggestive

Enhanced heart sound classification using Mel frequency cepstral coefficients and comparative analysis of single vs. ensemble classifier strategies.

This paper seeks to enhance the performance of Mel Frequency Cepstral Coefficients (MFCCs) for detecting abnormal heart sounds. Heart sounds are first pre-processed to remove noise and then segmented into S1, systole, S2, and diastole intervals, with thirteen MFCCs estimated from each segment, yielding 52 MFCCs per beat. Finally, MFCCs are used for heart sound classification. For that purpose, a single classifier and an innovative ensemble classifier strategy are presented and compared. In the single classifier strategy, the MFCCs from nine consecutive beats are averaged to classify heart soun

Exploring Pre-trained General-purpose Audio Representations for Heart Murmur Detection.

To reduce the need for skilled clinicians in heart sound interpretation, recent studies on automating cardiac auscultation have explored deep learning approaches. However, despite the demands for large data for deep learning, the size of the heart sound datasets is limited, and no pre-trained model is available. On the contrary, many pre-trained models for general audio tasks are available as general-purpose audio representations. This study explores the potential of general-purpose audio representations pre-trained on large-scale datasets for transfer learning in heart murmur detection. Exper

Dextrocardia with complete atrioventricular block-the right clinical approach to pace a heart on the right hemithorax: a case report.

BACKGROUND: A heart positioned on the right side of the thorax can be more a complex situation than it seems, also the potentially associated congenital cardiopathies are variable. In this regard, patients with dextrocardia presenting with complete atrioventricular block require a thorough anatomical investigation to map the veno-arterial system and potential cardiovascular defects before proceeding with a permanent pacing procedure. CASE PRESENTATION: A 73-year-old Caucasian woman with no significant medical history presented with syncope. Physical examination showed regular heart sounds wit
