1. Blood pressure was measured both directly and indirectly in seven hypertensive patients before and after a single oral dose of 100 mg metoprolol, at rest and during sustained handgrip. 2. Sixteen patients with uncomplicated hypertension were studied in a double-blind within-patient trial comparing slow release oxprenolol (SRO) with conventional oxprenolol (CO). The anti-hypertensive effect of SRO once daily was greater than that of CO once daily, although this did not reach statistical significance. CO once daily was less effective than CObd and this difference was statistically significant for lying diastolic blood pressure. After exercise testing 26 hours post-dose, blood pressure and pulse rate were marginally lower on SRO than on CO, but this difference was not significant.

A 35-year-old man was treated with maximum medical therapy for open-angle glaucoma. A diurnal curve revealed that the pressure was not controlled as might have been thought by a single reading. Reference is also made to the value of Goldmann visual field testing, since in this patient visual field defects were discovered with normal, healthy-appearing optic discs. A careful regimen including precise visual field testing and the plotting of the diurnal curve is most important in evaluating ocular hypertension and glaucoma.

The effect of propranolol on diurnal variability of BP and plasma noradrenaline levels was assessed in 15 patients with uncomplicated essential hypertension. While total variability and average noradrenaline level remained unaltered, the pressor range (difference between basal and maximum BP readings) was reduced in most patients. An inverse relationship was observed between changes in pressor range and noradrenaline.

As part of an epidemiological study of hypertension, an analysis was made of the general practitioner records of all attenders at a screening survey. A blood pressure recording, made before screening, was found in 37-9 per cent of cases. I performed trabeculectomies to reduce the introacular pressure to 20 mm Hg or lower, in 56 eyes with medically uncontrollable primary open-angle glaucoma or secondary angle-closure glaucoma. Detection of high blood pressure can be a major public health service contribution by the dental profession. The pioneer program at Fairleigh Dickinson University, in conjunction with the Bergen County Health Department and the Bergen County Dental Society, is encouraging dentists to participate in the National High Blood Pressure Program. Participants are urged to measure the blood pressures of all their adolescent and adult patients and to refer those with sustained elevated readings to their physicians, preferably by the arrangements of appointments.
