Prevalence of masked hypertension in patients with diabetes: a study in Mozambique.

BACKGROUND: Patients with type 2 diabetes mellitus (T2DM) are at an increased risk of masked hypertension (normal office blood pressure with increased out-of-office readings) and its cardiovascular complications. However, data from Africa remain scarce. OBJECTIVES: To describe the occurrence of masked hypertension and its associated factors among Black patients with T2DM. METHODS: Patients with T2DM diagnosed for at least 1 year, with office SBP less than 140 mmHg and DBP less than 90 mmHg and not under antihypertensive treatment were consecutively selected from primary healthcare and outpat

Terminal Digit Preference and Threshold Avoidance in Digital Blood Pressure Measurements During Pregnancy: Secondary Analysis of Data From the CLIP and PRECISE Cohorts.

BACKGROUND: Screening for, detecting, and managing pregnancy hypertension is a core function of antenatal care. To reduce both training requirements and the risks of measurement error in blood pressure (BP) values, automated and semiautomated BP devices have been validated in pregnant women with normal BP and pregnant women with hypertension and introduced for serial antenatal measurement of BP. OBJECTIVES: The study aimed to (1) determine whether or not repeated BP measurements reduced the presence of terminal digit preference and (2) discern whether or not there was evidence of threshold av

The Effect of Veteran Race and Socioeconomic Status on Enrollment in Remote Patient Monitoring for Hypertension: Retrospective Observational Cross-Sectional Study.

BACKGROUND: Black veterans and veterans from lower socioeconomic backgrounds are more likely to have uncontrolled hypertension. One potential explanatory factor is reduced access to specific treatments that result in improved chronic disease management. In the Veterans Health Administration (VHA), veterans with hypertension may enroll in a remote patient monitoring (RPM) program, which consists of patient education, daily home blood pressure (BP) monitoring, health coaching, and case management. Barriers for socioeconomically disadvantaged patients may exist for similar programs in other healt

Agreement between various non-invasive blood pressure measurement sites in the obese population using the VitalStream system as control.

BACKGROUND: The incidence of obesity and hypertension, with all their associated complications, is on the rise globally. Non-invasive blood pressure (NIBP) is often difficult to measure in obese individuals owing to the increased circumference and conical shape of the upper arm. The forearm and ankle may serve as convenient alternatives to measure NIBP. The aim of this study was to identify the most accurate site for NIBP monitoring in obese patients. OBJECTIVES: To statistically determine an agreement between different NIBP sites (upper arm, forearm and ankle) and our control blood pressure

Reading Between the Guidelines: Perioperative Implications of the 2025 American Heart Association/American College of Cardiology Hypertension Update.

The 2025 American Heart Association/American College of Cardiology Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults introduces important updates to the classification, risk assessment, and treatment of hypertension. Notable changes include adoption of the Predicting Risk of Cardiovascular Disease EVENTs risk calculator, revised criteria for initiating pharmacotherapy, and a shift toward lower treated blood pressure targets. Yet these updates have limited direct application to the perioperative setting. Recommendations for surgical patients re

Discrepancies in office blood pressure measurement protocols across international guidelines: "Houston, we've got a problem.".

Office blood pressure (OBP) remains a cornerstone of hypertension management. However, contemporary hypertension guidelines recommend different OBP measurement protocols, which vary in procedural details and analytical approaches, and the clinical implications of these differences remain unclear. We compared eight guideline-recommended OBP protocols (Japanese, Korean, ISH, NICE, ESC/Brazilian, ESH, Chinese, and ACC/AHA) in 826 treated hypertensive outpatients (age = 60.2 ± 14.3 years; 40.4% men) from two Brazilian cohorts, using aneroid ( n  = 394) and oscillometric devices ( n  = 432). Neck circumference in screening for elevated blood pressure.

INTRODUCTION: Elevated blood pressure (EBP) in childhood is associated with excess adiposity. The aim was to compare anthropometric and body composition indicators to identify EBP (≥90th percentile) in schoolchildren. MATERIALS AND METHODS: Cross-sectional study in 497 rural Andalusian schoolchildren (6-15 years). Weight, height, circumferences (waist and neck) and skinfolds (bicipital, tricipital, subscapular, and suprailiac) were recorded to calculate body mass index, waist-to-height ratio, the sum of 4 skinfolds and body fat percentage. Blood pressure was measured by oscillometry (3 readin

Improving case-finding of long-term health problems in disadvantaged communities: A Policy-focused Evidence Brief.

CURRENT CHALLENGES: Case finding, which actively identifies undiagnosed conditions, is key to tackling the growing chronic disease burden, especially in disadvantaged groups. Diseases like type 2 diabetes, cardiovascular disease, and chronic obstructive pulmonary disease already cause millions of preventable deaths, with numbers rising due to aging populations, urbanization, and widening inequalities. The poorest communities bear the heaviest load, with higher disease rates and lower diagnosis. Traditional healthcare models, which wait for patients to seek care, leave millions undiagnosed and

Asymmetric interarm blood pressure differences on sequential measurements.

Guidelines recommend initial blood pressure assessment on both arms, with the higher-reading arm used for future measurements. As simultaneous measurement devices are often unavailable, we evaluated if sequential measurements are appropriate for assessing interarm differences. Blood pressure was measured in Shanghai using an automated system. Consecutive readings were taken with 1-min intervals, beginning with the right followed by the left arm, with a third reading if the difference between the first two exceeded 5 mmHg. In 7838 participants, right arm readings were consistently higher, with

Diagnostic performance of simplified ambulatory nocturnal blood pressure monitoring for nocturnal hypertension and target organ damage assessment.

OBJECTIVE: Nocturnal blood pressure (BP) is a more sensitive predictor of cardiovascular risk. However, frequent nighttime measurements may disrupt sleep and reduce patient compliance. Therefore, this study aimed to investigate whether simplified nocturnal or awake BP readings could effectively diagnose nocturnal hypertension (NHT) and assess associated target organ damage (TOD). METHODS: Two thousand four hundred seventy-five hypertensive patients with complete 24-h ambulatory BP monitoring (ABPM) data between January 2021 and December 2023 were recruited in this retrospective study. Six SBP

Effect of Health Literacy on use of a Virtual Coach to Promote Home Blood Pressure Monitoring.

Health literacy and hypertension control are more limited in rural U.S. residents relative to metropolitan counterparts. Contemporary hypertension guidelines advocate for health education and home blood pressure monitoring (HBPM) to advance hypertension management. We conducted a preliminary assessment of the effect of health literacy on utilization of a virtual coaching agent, a computerized animation to converse with patient users, to promote HBPM. We recruited individuals from primary care practices in rural Pennsylvania; measured health literacy with the Brief Health Literacy Screen (BRIEF

Mid-aortic syndrome in two Saudi children with refractory hypertension: a case report.

BACKGROUND: Middle aortic syndrome (MAS) is a rare clinical condition in children that is associated with a significant risk of morbidity and mortality. Delayed diagnosis and inadequate treatment can lead to severe complications, resulting in life-threatening consequences. CASE PRESENTATION: In this paper, we present two cases involving two 10-year-old Saudi girls diagnosed with MAS who presented with systemic hypertension. Echocardiography demonstrated a patent aortic arch; however, it showed abnormal diastolic flow in the descending aorta, which facilitated the diagnosis of MAS. CONCLUSION

Central Systolic Blood Pressure and Left Ventricular Mass: Findings From the International 24-Hour Aortic Blood Pressure Consortium.

BACKGROUND: Central systolic blood pressure (cSBP) reflects the pressure experienced by vital organs and may be more closely related to hypertensive cardiac damage than brachial SBP. Twenty-four-hour ambulatory blood pressure monitoring provides additional insight into cardiovascular load beyond office readings. This study investigated the association of 24-hour cSBP, measured noninvasively with a validated device, with left ventricular mass index (LVMi) and LV hypertrophy. METHODS: We analyzed data from 21 centers worldwide (n=2367; 45.5% women; mean age 49.5 years) participating in the Inte

Optimizing Home Blood Pressure Monitoring Protocols in Older Adults: The Atherosclerosis Risk in Communities (ARIC) Study.

BACKGROUND: Current clinical guidelines recommend home blood pressure monitoring (HBPM) to confirm hypertension diagnosis, guide therapy, and support long-term control. However, the optimal number of HBPM measurements needed for valid assessment is not established. METHODS: At ARIC Visit 10 (2023), participants completed an eight-day HBPM protocol with morning and evening measurements and three readings per session. We evaluated the validity between the eight-day mean (reference) and streamlined protocols (fewer days, less than three readings per session, or only morning or evening measuremen

How useful is a casual BP measurement?

Blood pressure (BP) varies continuously dependent on numerous personal and external factors as well as on the way it is measured and the methodology used. A casual BP reading may give some clues about a person's cardiovascular health, but risk can only be determined with repeated careful measurements. If high (≥140/90), systematic measurement following approved guidelines using a validated monitor is necessary, especially if the person is symptomatic and/or BP ≥180/120. A low casual BP (<90/60) should be similarly rechecked, especially if related symptoms are present. Despite the availability
