ESR Essentials: imaging in nasal obstruction and epistaxis-practice recommendations by the European Society of Head and Neck Radiology.

Nasal obstruction and epistaxis are common otorhinolaryngologic complaints with multiple etiologies ranging from benign anatomical variations and mucosal inflammation to severe neoplastic and vascular disorders. Clinical evaluation and nasal endoscopy are first-line diagnostic tools, with imaging reserved for selected indications. Imaging differentiates uncomplicated from complicated presentations, guiding management and surgical planning. Computed tomography (CT) is the primary modality for assessing sinonasal anatomical variants, bony pathology, and inflammatory or neoplastic disease. Magnet

Trends in Laboratory Evaluation and Risk Factors for Transfusion Therapy in Pediatric Epistaxis.

OBJECTIVES: This study examines practices in laboratory evaluation and transfusion therapy for pediatric epistaxis in the emergency department (ED). METHODS: We conducted a single-center retrospective study of children (<21 years) evaluated in the ED for epistaxis from 2013 to 2023. Extracted data included demographics, medical history, laboratory evaluation, and treatments. Laboratory testing included complete blood count (CBC) and coagulation tests. We performed descriptive analyses to describe factors associated with laboratory testing and transfusion therapy. RESULTS: Of 944 children (me

Impact of Floseal and NasoPore in Managing Epistaxis in the Emergency Department: A Quality Improvement Analysis.

BACKGROUND: Epistaxis is a common ear, nose, throat (ENT) presentation in the emergency department (ED), with the majority of the population having experienced an episode during their lifetime and required hospitalization. Like many district general hospitals (DGHs) in the country, St Peter's Hospital does not offer adult inpatient or out-of-hours emergency cover for ENT, necessitating patients who require intranasal packing with a Rapid Rhino® (Smith & Nephew, Hull, UK) to be transferred to another hospital for assessment and admission, with most transfers being via ambulance. Apart from bein

Tranexamic Acid in Pediatric and Emergency Medicine: Evidence Review and Practical Applications.

Tranexamic acid (TXA) is an antifibrinolytic medication that helps stabilize clots and reduce bleeding. Although FDA approval is limited, it has become widely used off-label in pediatric and emergency medicine. Evidence shows that TXA decreases blood loss, reduces transfusion requirements, and may improve survival in trauma. It has also been beneficial in pulmonary hemorrhage, post-tonsillectomy bleeding, epistaxis, abnormal uterine bleeding, and secondary postpartum hemorrhage. TXA is generally safe and well tolerated; common side effects include headache and abdominal discomfort, while serio

Characterizing Self-Reported Daily Diary Epistaxis Measures in Hereditary Hemorrhagic Telangiectasia.

OBJECTIVES: To characterize a proposed clinical trial outcome measure of weekly cumulative bleeding duration from daily nosebleed diaries, termed PRO-CB (patient-reported outcome of cumulative bleeding). We also assessed cumulative weekly gushing bleeding duration (PRO-CGB) and evaluated the accuracy of patient recall on Epistaxis Severity Score (ESS) items 1-3 compared with daily diary data. METHODS: In this prospective observational study at the Toronto HHT Centre (St. Michael's Hospital, Unity Health Toronto), 23 patients from two clinical trials were observed during three-month run-in per

An Interesting Case of Hereditary Hemorrhagic Telangiectasia in a Patient with Recurrent Epistaxis.

Hereditary hemorrhagic telangiectasia is a disorder of blood vessels characterized by angiogenesis and chronic dilatation. Hereby presenting a case of an elderly female presenting with recurrent epistaxis with spontaneous resolution, multiple episodes of per rectal bleed since the past 10 years, leading to chronic anaemia and required blood transfusions. Further evaluation revealed the presence of multiple nasal, gastric, and colonic telangiectasias without active bleeding and complex pulmonary arteriovenous malformations. The patient was transfused with five pints of packed cell volume, caute

Navigating Risk in Atrial Fibrillation and Hereditary Hemorrhagic Telangiectasia: Antithrombotic Versus Antifibrinolytic Therapy.

Hereditary hemorrhagic telangiectasia (HHT) is characterized by mucocutaneous telangiectasias and arteriovenous malformations, often complicated by chronic epistaxis. Patients with HHT and comorbid atrial fibrillation (AF) present a clinical challenge in the concurrent management of bleeding and clotting risks. We retrospectively reviewed HHT patients with AF to evaluate the tolerability of anticoagulant (AC), antiplatelet (AP), and TXA therapies. Of 33 patients, AC was attempted in 13 cases, with 62% requiring premature discontinuation due to bleeding. AP therapy was initiated in 9 patients,

BMPR2 Splice-Site Variant in a Patient With Pulmonary Arteriovenous Malformation and Delayed-Onset Pulmonary Arterial Hypertension: A Case Report and Mechanistic Phenocopy Hypothesis.

Pulmonary arteriovenous malformations (PAVMs) are rare vascular anomalies most commonly seen in hereditary hemorrhagic telangiectasia (HHT), a condition associated with mutations in ENG, ACVRL1, SMAD4, or GDF2. In contrast, BMPR2 variants are well-established in heritable pulmonary arterial hypertension (PAH), but their relationship to PAVMs remains poorly understood. We report the case of a 41-year-old woman with an incidentally discovered PAVM, initially treated with embolization and subsequent surgical resection. She remained asymptomatic for several years until progressive exertional dyspn

Preventing Hypoxia in Pediatric MRI Sedation: A Randomized Controlled Trial of Prophylactic Nasopharyngeal Airway Placement.

BACKGROUND: Deep procedural sedation for magnetic resonance imaging (MRI) is associated with airway collapse, causing hypoxia. The nasopharyngeal airway (NPA) is a simple rescue device, but there is limited literature on its prophylactic use to prevent hypoxia. This study evaluates the incidence of hypoxic episodes with and without prophylactic NPA placement in children undergoing MRI under sedation. METHODS: Two hundred and sixty-two American Society of Anesthesiologists (ASA) I and II children aged 1-10 years scheduled for MRI under sedation were randomized into the NPA and control groups.

A Novel Homozygous ITGA2B Variant Associated With Recurrent Epistaxis in a Four-Year-Old Girl: A Case Report.

Glanzmann thrombasthenia (GT) is a rare, autosomal recessive platelet aggregation disorder caused by mutations in the ITGA2B and ITGB3 genes. These mutations result in quantitative or qualitative deficiencies in the alpha IIb beta 3 integrin complex, impairing platelet aggregation and leading to recurrent mucocutaneous bleeding. The key findings include absent or severely reduced platelet aggregation with adenosine diphosphate (ADP), epinephrine, and collagen, while aggregation with ristocetin remains normal. While hundreds of pathogenic variants have been identified, the genetic landscape of

Endovascular Embolization of Sinonasal Tumors: A Report of Two Cases and a Technical Review.

Hypervascular sinonasal tumors frequently cause recurrent epistaxis and pose a risk of significant intraoperative bleeding during resection. Preoperative embolization can improve operative visualization and safety; however, optimal technical strategies remain incompletely defined, particularly regarding embolic particle size and adjunctive flow-control techniques. We describe a structured preoperative embolization approach for two patients with hypervascular sinonasal tumors. In both cases, superselective microcatheterization of sphenopalatine artery feeders was performed after confirming the

Clinical spectrum of hereditary hemorrhagic telangiectasia: data from the Comprehensive HHT Outcomes Registry of the US (CHORUS).

Hereditary hemorrhagic telangiectasia (HHT), an autosomal dominant vasculopathy affecting 1 in 5000 individuals, is the second most common inherited bleeding disorder worldwide. Despite this prevalence, comprehensive data on disease manifestations and complications remain limited. To address this gap, the US Congress allocated funding leading to the Comprehensive HHT Outcomes Registry of the United States (CHORUS), a prospective, 15-center longitudinal registry enrolling unselected patients with confirmed HHT. In this initial report, we describe findings from the first 600 participants, with a

Bernard Soulier Syndrome Misdiagnosed and Treated as Immune Thrombocytopenia Purpura: A Case Report.

Bernard Soulier syndrome (BSS) is a rare autosomal recessive disorder that presents with giant platelets, prolonged bleeding time, and mucocutaneous bleeding. As it is a rare disorder and shares overlapping clinical features with other bleeding disorders, it is often misdiagnosed as immune thrombocytopenia purpura (ITP), which can result in inappropriate and aggressive management. This report explores the case of a young female patient who presented with recurrent epistaxis and ecchymotic episodes since childhood. She was misdiagnosed and treated for ITP, and due to refractory symptoms, she un

Isolated Hepatic Hemangiomatosis Leading to Fatal Consumptive Coagulopathy in an Elderly Patient: A Case Report.

BACKGROUND The most common vascular lesions of the liver are hepatic hemangiomas (HH), which are generally asymptomatic and do not require medical intervention. However, when the lesions are large or multiple, they can be a significant clinical problem. This report describes a unique case of an elderly patient with isolated hepatic hemangiomatosis associated with the onset of consumptive coagulopathy and hemolytic anemia, ultimately resulting in liver failure and death. CASE REPORT A 74-year-old man presented with jaundice, progressive weight loss, fatigue, lower-limb edema, and recurrent epis
