[Epistaxis].

Epistaxis, also known as a nosebleed, is common and usually stops spontaneously. However, when a patient visits the general practitioner, various self-care advice can be provided and several treatment options are available. Underlying causes of nosebleeds should be considered. Referral to an otolaryngologist is necessary if nosebleeds are persistent or recurrent. This article discusses the incidence, etiology, treatment, and prevention of nosebleeds.

Building CauterClay, a low-cost, high-fidelity training model for nasal cautery: a technical report.

OBJECTIVES: This study aimed to design and evaluate a low-cost, high-fidelity nasal cautery model (CauterClay) for increasing user confidence in carrying out nasal cautery. METHODS: We created CauterClay with an air-dry clay scaffold, simulated nasal mucosal lining and bleeding point. We carried out single-centre assessment with a variety of nasal cautery-naive and experienced doctors. Doctors were asked to complete pre- and post-induction surveys regarding confidence, prior experience and model fidelity. RESULTS: CauterClay use was associated with a statistically significant increase in use

Safety Profile of Intranasal Corticosteroids in Allergic Rhinitis: A Comprehensive Review.

Intranasal corticosteroids (INCS) remain the cornerstone of pharmacologic treatment for allergic rhinitis (AR) because of their well-established anti-inflammatory efficacy and generally favorable benefit-risk profile. Nevertheless, concerns regarding local and systemic corticosteroid-related adverse events (AEs) continue to influence patient adherence, prescribing practices, and long-term treatment acceptance. In routine clinical practice, safety perception and corticosteroid-related concerns frequently influence adherence and formulation selection to a greater extent than differences in clini

Association Between Systolic Blood Pressure at Emergency Department Arrival and Time to Bleeding Cessation in Adult Non-Traumatic Epistaxis: A Retrospective Time-to-Event Analysis.

Objectives: Epistaxis is a common emergency department (ED) presentation, but the prognostic significance of triage blood pressure (BP) remains unclear. We evaluated whether elevated systolic BP (SBP) at ED arrival was associated with delayed bleeding cessation in adults with non-traumatic epistaxis. Methods: We conducted a retrospective cohort study of adults (≥18 years) presenting with non-traumatic epistaxis to a tertiary ED in Northern Thailand between 2015 and 2025. Patients with documented triage SBP were included (n = 482). The exposure was SBP at ED arrival, categorized as ≥140 versus

Geroderma Osteodysplasticum Due to a Recurrent Golgi Ras-Associated Binding (GORAB) GTPase-Binding Protein Variant: A Report of Two Cases From Morocco.

Geroderma osteodysplasticum (GO) is an ultra-rare inherited connective tissue disorder. We report a retrospective case series of two Moroccan patients with geroderma osteodysplasticum carrying the same recurrent Golgi Ras-associated binding (RAB) GTPase-binding protein (GORAB) variant. Patient 1 was an 11-year-old boy who presented in January 2026, and Patient 2 was a five-year-old girl who presented in September 2024; both were born to consanguineous parents. They harbored the identical homozygous pathogenic truncating GORAB variant (NM_152281.3:c.79C>T; p.Arg27*), confirmed by targeted Sange

Comparing the incidence of epistaxis during nasotracheal intubation with Ring-Adair-Elwyn tubes using virtual versus flexible nasal endoscopy in patients undergoing elective oral and maxillofacial surgeries - A randomized clinical trial.

BACKGROUND AND AIMS: Nasotracheal intubation is commonly used in maxillofacial surgeries but often causes complications, with epistaxis being the most frequent. Selecting the more patent nostril and appropriate tube size can help reduce bleeding. Flexible nasal endoscopy (FNE) is traditionally used for this assessment but is invasive. Virtual nasal endoscopy (VNE), a non-invasive imaging technique, may offer an alternative. This randomized controlled study aimed to compare the incidence of epistaxis during nasotracheal intubation when guided by VNE vs FNE. MATERIAL AND METHODS: Sixty-four pat

Clinical approach to pediatric epistaxis.

Epistaxis is a common concern in the pediatric population. Although the underlying cause is often benign, epistaxis may also be a symptom of more concerning pathology. Despite the frequency with which epistaxis arises in these patients, a standardized approach to assessment and management has not been established, leading to potential gaps in care. This article outlines a systematic approach to evaluating and managing pediatric epistaxis with the aim of improving prompt recognition and ensuring timely intervention. To inform this approach, the article provides an overview of the broad differen

Attitudes Regarding De-Implementation of Bubble Humidification for Low-Flow Oxygen Delivery in a Children's Hospital.

BACKGROUND: Bubble humidification is often utilized with low-flow nasal cannulas despite a lack of evidence to support its use. The practice was discontinued in our children's hospital based on recommendations from the American Association for Respiratory Care clinical practice guideline for management of pediatric patients with oxygen in the acute care setting and other available literature. The aim of this study was to evaluate staff feedback post de-implementation of the practice. METHODS: Following staff and stakeholder communication, bubble humidification for low-flow oxygen therapy was

Effectiveness of a near-peer teaching model in an otolaryngology bootcamp for medical students: a pilot study.

BACKGROUND: Studies show that near-peer, simulation-based training can help develop technical skills. We sought to examine the effectiveness of near-peer instruction in otolaryngology emergencies by assessing student knowledge and procedural confidence before and after a 2-hour workshop. METHODS: We designed a pilot study for an otolaryngology workshop led by a second-year resident and 2 third-year medical students. The workshop consisted of lectures for anatomy and procedural overview, followed by hands-on simulation stations. Participants completed pre- and post-workshop quizzes and surveys

As-needed vs regular intranasal corticosteroid therapy in pediatric perennial allergic rhinitis: A randomized double-dummy trial.

BACKGROUND: Intranasal corticosteroids (INCS) are the most effective monotherapy for allergic rhinitis (AR), but daily adherence remains challenging. Evidence comparing regular vs as-needed INCS use in pediatric perennial AR (PAR) is limited. OBJECTIVE: To evaluate the efficacy, safety, and anti-inflammatory effects of regular vs as-needed INCS in pediatric PAR. METHODS: In this 8-week, randomized, double-blind, double-dummy trial, 70 children aged 6 to 18 years with PAR were assigned (1:1) to regular or as-needed intranasal fluticasone furoate. Key outcomes included total, total nasal, and

Case Report: acute and long-term effects of a massive beta-mercaptoethanol inhalation.

INTRODUCTION: β-mercaptoethanol is widely used in biochemistry and industrial applications. Animal toxicity has been demonstrated but very limited data exists regarding its human effects. CASE DESCRIPTION: A 40-year-old male patient was exposed to β-mercaptoethanol vapours after an accidental occupational spillage. He developed dizziness, blurred vision, a headache and epistaxis within minutes alongside dyspnoea and cough in the following hours. Lung function tests displayed bronchial hyperreactivity and increased resistances, consistent with a reactive airways dysfunction syndrome. Neuropsyc

Doxycycline for epistaxis in hereditary hemorrhagic telangiectasia: genotype-stratified outcomes.

Patients with hereditary hemorrhagic telangiectasia (HHT) experience recurrent epistaxis. Doxycycline has been proposed as a possible treatment, although its efficacy remains controversial. Whether the most common genotypes (activin receptor-like kinase 1, endoglin, and SMAD4) contribute to a differential response has not been investigated. In this study, we evaluate the effectiveness of doxycycline among the different HHT genotypes. A retrospective cohort study was conducted at the University of Florida's Hereditary Hemorrhagic Telangiectasia Center. Forty-one adult patients (aged ≥18 years)

Bilateral Post-Traumatic Carotid-Cavernous Fistula: A Case Report.

Background and Clinical Significance: Carotid-cavernous fistulas (CCFs) are pathological communications between the carotid artery and the cavernous sinus, most commonly traumatic when direct and high-flow (Barrow type A). Bilateral traumatic CCFs are rare, occurring in approximately 1-2% of cases. Case Presentation: We report a 45-year-old male with polytrauma after a road traffic accident who presented with right-sided chemosis, pulsatile exophthalmos, and ocular bruit. Digital subtraction angiography revealed bilateral direct CCFs. The right fistula was treated with transarterial coil embol
