Postoperative Nasal Synechia as a Source of Severe Delayed Epistaxis After Closed Reduction of a Nasal Bone Fracture.

Delayed severe epistaxis after closed reduction of a nasal bone fracture is uncommon, and postoperative nasal synechia is rarely considered as a bleeding source. A 19-year-old woman developed recurrent severe epistaxis 5 weeks after closed reduction and intranasal packing. Bleeding recurred immediately after pack removal despite repeated packing. Emergency endoscopic exploration showed bilateral adhesions between the septum and inferior turbinate, with active bleeding directly visualized from right-sided synechial tissue. Suction coagulation and synechiolysis achieved hemostasis. Hemoglobin de

Tolerability and Outcome of Medical Treatment in Feline Nasal Carcinomas: A Multi-Institutional Retrospective Study.

Carcinomas are the second most common feline intranasal neoplasm. Various treatment options, including radiotherapy, surgery or a combination of both, have been described, but data on medical therapy efficacy are lacking. This retrospective study evaluated the outcome of cats with nasal carcinoma (NC) treated with antineoplastic treatment. Cats with a histopathological or cytopathological diagnosis of NC were included. Patients receiving antineoplastic treatment were compared to patients receiving palliative treatment alone. Response to therapy, median time to progression (MTTP) and median sur

Evaluating sotatercept in the treatment of pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) is a progressive disease characterized by pulmonary vascular remodeling, right ventricular overload, and premature death. Despite advances achieved through endothelin receptor antagonists, phosphodiesterase type 5 inhibitors, and prostacyclin analogs, these agents primarily act as vasodilators and do not reverse underlying vascular pathology. Sotatercept, a first-in-class activin signaling modulator, restores the balance between pro- and antiproliferative signaling within the pulmonary vasculature via the TGF-β/activin-BMPR2 pathway, offering a novel disea

Videolaryngoscope-assisted fibreoptic intubation in anticipated difficult airways of maxillofacial trauma: a prospective randomised comparison with videolaryngoscope-guided nasotracheal intubation.

BACKGROUND: Maxillofacial trauma poses airway management challenges due to edema, distorted facial anatomy and associated bleeding. Further, its fixation requires unhindered access to facial fractures, often necessitating securing the airway naso-tracheally during the intraoperative period. Traditional technique of nasotracheal intubation is associated with 17%-77% incidence of epistaxis. Fibreoptic bronchoscope helps guide nasotracheal tube under vision, decreasing incidence and severity of epistaxis. Moreover, as patients with maxillofacial trauma have difficult airways, every effort should

Navigating the efficacy, safety and outcome of intranasal platelet-rich plasma for persistent olfactory loss: a systematic review and meta-analysis.

BACKGROUND: Persistent olfactory dysfunction (OD) poses a significant clinical challenge with limited therapeutic options. Intranasal platelet-rich plasma (PRP), an autologous source of regenerative growth factors, presents a biologically plausible intervention, yet its efficacy across various OD etiologies remains synthetically underexplored. METHODS: This systematic review and meta-analysis adhered to PRISMA guidelines. A comprehensive search of PubMed/MEDLINE, Web of Science, and Cochrane Library (2010-2025) identified studies assessing intranasal PRP for persistent OD (> 6 months). Random

Anesthetic Considerations for Pulmonary Arteriovenous Malformation Embolization in a Patient With Hereditary Hemorrhagic Telangiectasia: A Case Report.

Hereditary hemorrhagic telangiectasia (HHT) is characterized by mucocutaneous telangiectases and visceral arteriovenous malformations (AVMs). Pulmonary AVMs (PAVMs) create an anatomic right-to-left shunt, increasing the risk of hypoxemia and paradoxical embolic events, and are commonly treated with transcatheter embolization. We report a 25-year-old man with suspected HHT based on Curaçao criteria, including recurrent epistaxis, prior cerebral AVM, and multiple PAVMs, presenting with exertional dyspnea and hypoxemia. He underwent embolization under general anesthesia with air management, lung

Ten-year follow-up of the first left ventricular assist device implantation program in Chile: a case series.

Heart failure (HF) affects over 64 million people worldwide, with prevalence projected to reach 3% by 2030. In advanced stages, when medical therapy fails and transplantation is limited by organ shortages, left ventricular assist devices (LVADs) represent a viable alternative. This retrospective, single-center study reports 10-year outcomes from the first Chilean LVAD program using the HeartWare (HVAD) device. Between 2013 and 2015, nine patients with advanced HF received HVAD implants. The mean age was 42.2 ± 15.8 years, with a left ventricular ejection fraction of 22% ± 4.1%. One-year surviv

Influence of hormonal status in hereditary hemorrhagic telangiectasia - analysis of an online patient questionnaire.

BACKGROUND: Hereditary Hemorrhagic Telangiectasia (HHT) is a rare vascular disorder with mucocutaneous telangiectasia. Hormones are hypothesized to play a role in reducing bleeding in these patients; however, clear evidence is currently lacking. The aim of this study was to further investigate this hypothesis by assessing the impact of altered hormonal status on HHT severity. METHODS: This cross-sectional electronic questionnaire, available in English and German, was distributed with the support of various self-help groups. It included sections on the general medical history of HHT and the inf

Subarachnoid and intraventricular dissemination of silicone oil: an under-recognised radiological diagnosis.

A woman aged in her 50s presented with sudden onset of vertigo, headaches and epistaxis, initially attributed to her background of chronic systemic hypertension. Ophthalmological history was significant for bilateral proliferative diabetic retinopathy and retinal detachment managed with vitrectomy, endolaser and intraocular silicone oil 5 years prior to the current presentation. Review of the initial non-contrast CT by our neuroradiology department identified right subretinal silicone oil, tracking along the subarachnoid space of the right optic sheath, right suprasellar cistern and non-depend
