Pulmonary tuberculosis (PTB) remains a major global health challenge, particularly in low- and middle-income countries where it contributes significantly to morbidity and mortality. Hemoptysis is a common complication of PTB and may result from a variety of mechanisms, including vascular involvement. Rasmussen aneurysm, a pseudoaneurysm of the pulmonary artery adjacent to a tuberculous cavity, represents a rare but potentially fatal cause of hemoptysis. We report a Malaysian patient with smear-positive PTB complicated by Rasmussen aneurysm, successfully managed with selective pulmonary artery

INTRODUCTION: Uniportal video-assisted thoracoscopic surgery (U-VATS) is emerging as a promising approach for the treatment of pulmonary aspergilloma (PA). Nevertheless, it remains technically challenging and is associated with significant morbidity and a considerable risk of conversion to thoracotomy. This study aimed to evaluate the feasibility and safety of U-VATS in patients with PA and to identify prognostic factors that influence postoperative morbidity and conversion rates.
PATIENTS AND METHODS: We retrospectively reviewed the medical records of 51 patients who underwent U-VATS resecti

COPA syndrome is a rare autosomal dominant autoimmune disorder caused by COPA gene mutations, leading to immune dysregulation and multisystem involvement. We report a 45-year-old man with recurrent respiratory failure, haemoptysis, renal dysfunction and arthritis. Symptoms began at Age 27 with diffuse alveolar damage requiring mechanical ventilation. Imaging showed progressive interstitial lung disease and biopsy revealed follicular bronchiolitis. Immunosuppression provided temporary stabilisation. In 2024, next-generation sequencing confirmed a likely pathogenic COPA variant. The patient late

Rasmussen's aneurysm is a pseudoaneurysm of a pulmonary artery branch occurring adjacent to or within a tuberculous cavity and represents a rare but potentially fatal cause of hemoptysis. Bilateral involvement is exceptionally uncommon and sparsely reported in medical literature. This case report discusses a 45-year-old female with pulmonary tuberculosis on antitubercular therapy who presented with recurrent hemoptysis and progressive breathlessness. Computed tomography pulmonary angiography demonstrated multiple bilateral cavitary lesions with associated small aneurysmal dilatations arising f

Background/Objectives: Paediatric pulmonary embolism (PE) is rare but potentially lethal, and its diagnosis is complicated by non-specific presentations and multiple predisposing risk factors. We describe a 15-year-old girl who presented with features of pulmonary hypertension and was found to have extensive pulmonary embolism with right heart strain. Methods: The diagnosis was established with echocardiography, computed tomography pulmonary angiography (CTPA), and Doppler ultrasonography. Results: Her predisposing risk factors comprised obesity (BMI 33 kg/m2), a family history of thrombosis,

Pulmonary artery pseudoaneurysm (PAP) is a rare lethal disease; central tuberculous PAP carries a mortality rate of 50%-90% and often responds poorly to conventional embolization. We report a 59-year-old tuberculous woman with recurrent haemoptysis secondary to left main PAP. Covered stent-graft exclusion was conducted after failed conservative treatment and bronchial artery embolization, with complete haemostasis and no recurrence at 1-year follow-up. Most existing literature adopts vessel-occlusive embolization for PAP, which may cause pulmonary infarction. Moreover, stent-graft implantation

Melanoma is a malignant tumour with a recognised potential for late recurrence. Although pulmonary involvement is common in advanced disease, endobronchial metastasis is rare and may present with non-specific respiratory symptoms. We report the case of an 85-year-old woman with a history of stage IIA melanoma of the right thumb treated with amputation in 2014, and no evidence of disease until loss to follow-up in 2022. Nearly ten years after initial treatment, she developed progressive cough, haemoptysis and dyspnoea. Chest CT revealed right lower-lobe atelectasis with post-obstructive changes

A woman in her 30s without prior medical history presented with 4 months of progressive back pain that evolved to paraplegia, followed by dyspnoea and haemoptysis. Imaging revealed numerous, well-circumscribed pulmonary nodules consistent with cannonball metastases, a loculated right pleural effusion, bilateral renal masses, pancreatic nodules and osteolytic L5 collapse with cord compression. The lung lesion biopsy demonstrated a poorly differentiated neoplasm, which on immunohistochemistry was most consistent with metastatic sarcomatoid carcinoma of renal origin. She required intubation, pleu

BACKGROUND: Takayasu arteritis is a rare, chronic large-vessel vasculitis that predominantly affects young women, involving the aorta and its major branches. Pseudoaneurysm formation is uncommon in this disease, particularly in the ascending aorta, with only a few such cases having been reported.
CASE SUMMARY: A 20-year-old Asian man presented with fever, cough, and haemoptysis. Contrast-enhanced computed tomography revealed a giant ascending aortic pseudoaneurysm (57 mm in diameter) compressing the pulmonary artery. Emergency total arch replacement was performed because the patient was in a

Haemoptysis can be a challenging diagnostic dilemma. The most common causes include acute respiratory infection, cancer, bronchiectasis and chronic obstructive pulmonary disease. However, in 20% of cases no clear cause can be found. We present here a rare case of a 54-year-old male who previously had a lobectomy for lung cancer and presented with chronic low-grade haemoptysis caused by endobronchial polypropylene sutures. We detail their successful removal using a novel technique with flexible endobronchial scissors and flexible fibreoptic bronchoscopy under light sedation. This case highlight

BACKGROUND: Tuberculosis is the leading cause of death in children living with HIV, and is challenging to diagnose owing to the limitations of microbiological approaches. The Pediatric Asian African Network for Tuberculosis and HIV Research (PAANTHER) treatment decision algorithm (TDA) was previously developed to improve the diagnosis of tuberculosis in children living with HIV, but has not yet been validated. We aimed to externally validate this TDA in a prospective cohort of children from four countries in Africa.
METHODS: The TB-Speed HIV study was a prospective diagnostic cohort study con

Benign esophago-pulmonary fistula (EPF) is a rare clinical condition. This report highlights the case of a 58-year-old woman with a 10-year history of recurrent episodes of cough, throat irritation, dyspepsia and recurrent haemoptysis. Upper gastrointestinal (GI) endoscopy and chest CT detected a fistulous communication between the mid-oesophagus and the right lower lobe lung parenchyma, with bronchiectatic changes in the lower lobe. This patient underwent a video-assisted thoracoscopic surgery (VATS) division of EPF with right lower lobectomy. Intraoperatively, the fistula was identified on e

Sclerosing mediastinitis (SM) is a rare condition characterised by extensive fibrous proliferation within the mediastinum. While some patients remain asymptomatic, others may present with chest pain, dyspnoea, haemoptysis, or complications such as superior vena cava syndrome or pulmonary hypertension. The aetiology of SM may be caused by infections, malignancies, autoimmune diseases, radiation therapy, or have idiopathic origins. We present a case of a 55-year-old man diagnosed with SM and granulomatosis with polyangiitis. The patient initially presented from an outside hospital with a large p

Background: Diffuse alveolar haemorrhage (DAH) is a rare but potentially life-threatening pulmonary complication of systemic autoimmune diseases. Although interstitial lung disease (ILD) is a hallmark of antisynthetase syndrome (ASyS), DAH has been only exceptionally reported in this setting. Methods: We present two patients with ASyS who developed DAH confirmed by bronchoalveolar lavage. Results: The first case involved a 52-year-old woman initially diagnosed with rheumatoid arthritis, later reclassified as rheumatoid arthritis-ASyS overlap, who developed DAH in the context of progressive ILD

The clinical data of five patients with giant pulmonary bullae (GPB) who underwent computed tomography (CT)-guided percutaneous polidocanol injection were retrospectively analysed. Pre-and post-treatment comparisons were performed with respect to the diameter of the pulmonary bullae, arterial blood gas parameters, modified Medical Research Council (mMRC) dyspnoea score, Chronic Obstructive Pulmonary Disease Assessment Test (CAT) score, six-minute walk distance (6MWD), and complications. During a 1- to 12- month follow-up, 3 cases achieved complete bulla closure, and 2 showed stable shrinkage.

We report the case of a 44-year-old woman with a typical carcinoid tumour of the left mainstem bronchus managed through a multidisciplinary and combined approach. Initial symptoms included haemoptysis and progressive wheezing. A subsequent bronchoscopy revealed a highly vascular, pedunculated mass nearly occluding the airway. En bloc resection using electrocautery and a cryoprobe enabled airway recanalization and diagnosis. Endobronchial ultrasound-guided transbronchial fine needle aspiration ruled out nodal disease. Histopathological examination confirmed a typical carcinoid tumour. Robotic-

Haemoptysis, defined as the expectoration of blood from the lower respiratory tract, can present to the emergency department (ED) with symptoms ranging from mild, self-limiting to massive haemorrhage. Severe presentations account for approximately 5% of cases, with mortality primarily because airway flooding with blood leading to hypoxia. This underscores the importance of early airway protection and structured resuscitation in emergency settings. This narrative review summarizes the current evidence to guide emergency physicians in the management of haemoptysis. Aetiologies vary geographicall

BACKGROUND: Coronary-bronchial artery fistulas are rare vascular anomalies between the coronary and bronchial circulations. They are most often detected incidentally and may be congenital or acquired, particularly in the setting of chronic pulmonary disease. Origin from the sinoatrial nodal artery is exceptional. If haemodynamically significant, these communications may lead to a coronary steal phenomenon and represent a potential cause of myocardial ischaemia. Recognition is also crucial prior to bronchial artery embolization to avoid inadvertent coronary complications. Multimodality imaging

Teratocarcinosarcoma (TCS) is a rare, highly aggressive malignant neoplasm composed of epithelial, mesenchymal and neuroectodermal elements, occurring almost exclusively in the sinonasal region. Primary involvement of the tracheobronchial tree or carina has not been previously reported. We describe a 55-year-old male chronic smoker who presented with cough, progressive dyspnea and haemoptysis. Computed tomography revealed a lobulated soft-tissue mass in the distal trachea overhanging the carina, extending into both main bronchi, causing significant airway obstruction. Bronchoscopy demonstrated
