BACKGROUND: Life-threatening airway obstructions caused by tumors require immediate and effective intervention. However, the management of severe airway obstruction during therapeutic bronchoscopy is challenging, particularly when there is a potential inability to ventilate the lungs using conventional techniques, due to extensive tracheobronchial lesions or the risk of major intraoperative bleeding related to disease characteristics. For these reasons, the multidisciplinary team opted for an endoscopic debulking with the aid of a procedural veno-venous extracorporeal membrane oxygenation (VV-

Inflammatory myofibroblastic tumour (IMT) is a rare mesenchymal neoplasm of intermediate biological potential, most commonly affecting children and young adults, with the lung being one of the the most common sites. Its variable clinical, radiological and pathological features often mimic malignancy, making diagnosis challenging. We report a middle-aged man in his early 50s presenting with life-threatening haemoptysis and found to have a right main bronchus mass causing central airway obstruction. Emergency rigid bronchoscopy with cryo-debulking was performed for airway stabilisation and tissu

Laryngeal mucormycosis is an uncommon form of fungal laryngitis that typically affects immunocompromised individuals. We report a rare such case in a male in his 40s who had recently recovered from severe acute respiratory syndrome-coronavirus disease-19 (SARS CoV-2 infection-COVID 19) and presented with a 1-month history of worsening hoarseness, haemoptysis, and noisy breathing. A chewing gum-like subglottic necrotic mass was biopsied. The rarity of the diagnosis was aided by histopathological and microbial study of the mass, which confirmed mucormycosis. Management included surgical debulkin

A man in his 70s, a retired coal miner with prior asbestos exposure and no smoking history, presented with syncope and progressive exertional dyspnoea. Initial CT pulmonary angiogram (CTPA) revealed bilateral lobar pulmonary emboli, and transthoracic echocardiography (TTE) showed evidence of pulmonary hypertension. He was discharged on rivaroxaban and referred to the heart failure and respiratory clinic. At the respiratory clinic follow-up, he reported worsening breathlessness and was initiated on long-term oxygen therapy (LTOT). Outpatient investigations were arranged, including a ventilation

A woman in her early 50s presented with haemoptysis and cavitary pulmonary nodules. Initial testing showed a low-titre positive cryptococcal antigen (CrAg); however, repeat testing on two occasions was negative. Her cavitary nodules were ultimately attributed to an overlap systemic autoimmune rheumatic disease. She achieved complete clinical and radiological resolution with corticosteroid therapy. This case highlights the importance of recognising false-positive serum CrAg results.

Chronic pulmonary aspergillosis (CPA) is a slowly progressive fungal infection that commonly complicates underlying structural lung disease and is frequently misdiagnosed in tuberculosis-endemic regions. Invasive forms of CPA can closely mimic lung malignancy.A man in his late 30s with a history of repeated empirical antitubercular therapy presented with progressive dyspnoea, chronic cough with mucoid sputum, seasonal variation of symptoms and recurrent haemoptysis. Imaging revealed a large right upper lobe mass with mediastinal extension, vascular encasement and complete bronchial obstruction

Pulmonary vein stenosis (PVS) is a rare but potentially serious complication of catheter ablation for atrial fibrillation (AF), often presenting with non-specific respiratory symptoms and initially misdiagnosed. Early recognition is crucial to reduce the risks of progressive pulmonary hypertension, infarction, and long-term morbidity. We report a 65-year-old woman who presented with progressive dyspnoea on exertion, marked reduced exercise tolerance, lethargy, chest pain , and new onset haemoptysis. These symptoms began five months following an ablation procedure for symptomatic AF. Initial ev

Aspergillus fumigatus is a clinically important airborne fungal pathogen capable of causing a broad spectrum of respiratory disorders. Persistent exposure or airway colonization can provoke severe allergic diseases, particularly in patients with asthma or cystic fibrosis, whereas impaired immunity or pre-existing structural lung damage may allow the development of chronic or life-threatening invasive pulmonary aspergillosis. Accordingly, the aim of this review was to synthesize current evidence on the role of A. fumigatus in the development of allergic and non-allergic diseases, with particula

Pulmonary artery pseudoaneurysm (PAP) is a rare but potentially fatal entity caused by a variety of underlying conditions. They may be congenital in origin or occur due to infective complications or occasionally secondary to pulmonary arterial hypertension. Among infective processes, tuberculosis and bacterial infections are the most common causative agents. There are few isolated reports of angio-invasive mucormycosis causing PAP; many of them were detected postmortem. Diagnosis of specific aetiology is often overlooked due to lack of clinical suspicion, as a result of incomplete investigatio

BACKGROUND: Massive haemoptysis is a life-threatening complication of pulmonary mucormycosis, often resulting from invasive destruction of pulmonary arteries. Early predictors and preventive strategies remain poorly defined.
METHODS: We retrospectively enrolled all hospitalised patients with pulmonary mucormycosis regardless of haemoptysis between 2019 and 2024. Patients were stratified into non-massive and massive haemoptysis groups. Clinical and radiological variables were compared between groups, and univariable and multivariable logistic regression analyses were performed. Given the limit

Here we describe the novel, multi-point Comet Interceptor mission. It is dedicated to the exploration of a little-processed long-period comet, possibly entering the inner Solar System for the first time, or to encounter an interstellar object originating at another star. The objectives of the mission are to address the following questions: What are the surface composition, shape, morphology, and structure of the target object? What is the composition of the gas and dust in the coma, its connection to the nucleus, and the nature of its interaction with the solar wind? The mission was proposed t

Hydatid disease (HD) is a zoonotic parasitic infection caused by Echinococcus granulosus and remains endemic in many regions. In children, pulmonary involvement is more common than hepatic disease and is often asymptomatic or mildly symptomatic. Massive hemoptysis as an initial presentation in early childhood is extremely rare. We report a case of a previously healthy 4-year-old boy who presented with a 3-day history of productive cough and massive hemoptysis without fever or systemic symptoms. Chest radiography showed right-sided consolidation, and contrast-enhanced computed tomography reveal

BACKGROUND: Maudsley Family Therapy (MFT), and its manualised version, Family-Based Therapy (FBT), are the only well-established treatment interventions for adolescent anorexia nervosa (AN), with treatment efficacy primarily measured by improvements in eating behaviours and weight restoration. A crucial component of this therapy is an intensive home-based refeeding intervention that requires a substantial commitment from parents for up to one year. While this treatment works to restore weight in a proportion of adolescents, very little is known about its impacts on family distress, relationshi

Monkeypox is a contagious viral disease that spreads between animals and people. The UK government guidance described the first case of 'Monkey Pox' in 1958, when it was found only in monkeys used for research purposes. Fortunately, for a third world fast developing country like India, monkeypox does not spread easily in the population but spread by close physical contact between people, and there is limited information available about the impact on pregnancy. The virus can enter the body through broken skin, the respiratory tract, or mucous membranes (the moist inner lining of cavities and so

BACKGROUND: Radiation-induced cardiovascular disease may appear decades after cancer therapy and can involve multiple vascular, valvular, myocardial, and conduction structures of the heart. Management is particularly challenging when prior irradiation results in a hostile chest environment and impaired wound healing.

Atypical pulmonary carcinoids may recur many years after initial treatment, but very late central-airway recurrence is uncommon and its optimal endoscopic management is uncertain. We report a 67-year-old man with metastatic atypical pulmonary carcinoid who developed recurrent subglottic disease almost 20 years after pulmonary resection. Following mild haemoptysis and inspiratory dyspnoea, bronchoscopy showed progression of a known subglottic lesion causing approximately 80% tracheal obstruction. Laser photoresection achieved rapid debulking, haemostasis and marked airway recanalization. Residu

Rheumatic mitral stenosis remains a significant cause of cardiovascular morbidity in young adults worldwide, yet its diagnosis can be challenging when clinical presentation overlaps with concurrent infections. We present a case of a woman in her 30s who presented with dyspnoea, fever, and haemoptysis, initially attributed to community-acquired pneumonia with concurrent viral and bacterial infection. She received appropriate treatment and was discharged; however, her symptoms persisted, and she developed alveolar haemorrhage, prompting further investigations, which ultimately revealed severe mi

An immunocompetent male in his 30s with a history of bronchiectasis presented with recurrent haemoptysis (5-10 mL every 2 months) over 4 years. High-resolution CT revealed cystic bronchiectasis and ground-glass opacities. Bronchoscopy identified active bleeding from a ruptured bronchial artery, which was successfully managed with emergent embolisation. Bronchoalveolar lavage demonstrated weakly acid-fast branching filaments, later confirmed as Nocardia asteroides by matrix-assisted laser desorption-ionisation time of flight mass spectrometry. The patient responded well to trimethoprim-sulfamet

BACKGROUND: Coronary artery pseudoaneurysm (PSA) is a rare but potentially life-threatening condition, typically occurring after percutaneous coronary intervention (PCI). Giant coronary PSA in patients without prior coronary procedures is exceedingly uncommon. Mycotic involvement, particularly with Staphylococcus aureus, may accelerate pseudoaneurysm progression and significantly worsen prognosis.
CASE SUMMARY: We report the case of a 38-year-old man on long-term haemodialysis who presented with unstable angina and new electrocardiographic changes. Coronary angiography revealed severe proxima
