Primary Pulmonary Endobronchial Mucoepidermoid Carcinoma Treated With Sleeve Resection in a 23-Year-Old Woman: Case Report.

Pulmonary Mucoepidermoid Carcinoma (PMEC) is a rare, slow-growing, low-grade primary intrathoracic cancer of the tracheobronchial tree that manifests as an exophytic endobronchial tumour with luminal obstruction. Radical surgery is the cornerstone of treatment for low-grade PMEC. We report a case of a 23-year-old woman who presented with intermittent haemoptysis. On imaging, an endoluminal mass was seen in the bronchus intermedius, also visualised on bronchoscopy, although biopsies were inconclusive. Final histology after surgical resection revealed a low-grade PMEC with tumour-free margins. T

Pancreatic cancer unveiled by multisystemic manifestations.

Pancreatic cancer is characterised by its aggressive progression, late-stage diagnosis and poor prognosis. This case report illustrates the intricate interplay between pancreatic adenocarcinoma, disseminated intravascular coagulation with diffuse alveolar haemorrhage and bone metastasis in an elderly woman. Presenting with asthenia, significant hip pain and haemoptysis, her work-up revealed multiple supradiaphragmatic adenopathies and infradiaphragmatic adenopathies, pancreatic parenchymal heterogeneity and lytic bone lesions. Despite extensive diagnostic efforts, a diagnosis of adenocarcinoma

Severe Haemoptysis due to Pulmonary Artery Agenesis, About Two Rare but Threatening Case Reports.

Unilateral pulmonary artery agenesis (UPAA) is a rare condition that may be asymptomatic or present with recurrent infections and haemoptysis. A 17-year-old patient with severe haemoptysis who was diagnosed with right pulmonary artery agenesis, associated with hypertrophy of the homolateral bronchial arteries and numerous systemic collaterals. Despite three embolisation procedures, a high-risk of recurrence required pneumonectomy. A 45-year-old patient with tetralogy of Fallot and a known left pulmonary artery agenesis presented with 300 mL haemoptysis. The attempt of embolisation and closure

Recurrent Endobronchial Coil Migration Associated With Needle Biopsy Following Pulmonary Artery Pseudoaneurysm Embolization.

Endobronchial coil migration is a rare complication of pulmonary artery embolization, occurring when coils erode through a vessel into an airway. We describe a patient who underwent embolization of pulmonary artery pseudoaneurysms associated with a lung mass. The unique aspect of this case is the development of recurrent endobronchial coil migration following a CT-guided core needle biopsy and a subsequent endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) of the mass. In both circumstances, the portion of the coil that migrated into the large airways was cut with end

Delayed Pulmonary Artery Stump Thrombosis Presenting With Haemoptysis: A Rare Presentation of a Rare Phenomenon.

Pulmonary artery stump thrombosis (PAST) is a complication following lobectomy and pneumonectomy where a thrombus is formed at the site of the pulmonary stump. It remains a rare condition, but the clinical significance is debatable. Delayed presentation of PAST is very rare. Most cases are incidental and are treated as benign. However, here we report a very rare case of PAST presenting as haemoptysis and associated with pulmonary hypertension.

Primary pulmonary synovial sarcoma in the bronchial cavity: A case report.

Synovial sarcoma, a mesenchymal spindle cell malignancy commonly seen in young and middle-aged adults, typically occurs in the soft tissues around the major joints of the extremities, with some reports of occurrences outside the joints. However, primary pulmonary cases are rare. This study reports a 52-year-old female patient who was admitted due to coughing and recurrent haemoptysis and was ultimately diagnosed with primary pulmonary synovial sarcoma (PPSS) through surgery and pathology after a series of examinations, including computed tomography scans and bronchoscopy. The tumour was locate

A Case of Bronchial Artery Aneurysm Incidentally Discovered as a Pulsatile Oesophageal Submucosal Mass.

Bronchial artery aneurysm (BAA) is often detected through haemoptysis or chest pain, and its rupture can be life-threatening. However, due to the rarity of BAAs, the optimal management of unruptured cases remains uncertain. Here, we report a case of an asymptomatic BAA that was incidentally discovered and successfully treated with bronchial artery embolization (BAE). The BAA was identified during an upper gastrointestinal endoscopy performed as a follow-up after Helicobacter pylori eradication, where a pulsatile oesophageal submucosal mass was observed. The diagnosis of BAA was subsequently co

Pulmonary Sarcomatoid Carcinoma Initially Misdiagnosed as Aspergillosis: A Diagnostic Challenge.

Pulmonary sarcomatoid carcinoma (PSC) is a rare and aggressive malignancy that often presents atypically, leading to incomplete diagnosis. We report a 55-year-old male with chronic obstructive pulmonary disease who presented with productive cough and haemoptysis. Initial bronchoscopy revealed fungal hyphae consistent with Aspergillus infection. The misleading clinical presentation led to an incomplete diagnosis. Subsequent lung biopsy and immunohistochemistry confirmed PSC harbouring an EGFR L858R mutation. Despite the presence of a targetable mutation, the patient showed a poor response to EG

Pericardial Agenesis: The Significance of Multimodality Imaging in Diagnosis.

Pericardial agenesis is a rare pericardial defect that can be complete or partial. Often an incidental finding, most patients are asymptomatic at time of detection but some can present with symptoms of atypical chest pain, dyspnoea, palpitations or dizziness. There can be a delay in diagnosis as imaging is essential to confirm the diagnosis although there may be subtle signs on radiographic imaging and electrocardiogram. Both cardiac magnetic resonance imaging and computed tomography can be used to diagnose the condition and distinguish between complete and partial pericardial agenesis, althou

Sweet's syndrome as a presenting symptom of granulomatosis with polyangiitis.

A patient presented with fatigue, weight loss, haemoptysis, bullous rash and spiculated pulmonary nodules. Skin biopsy was consistent with Sweet's syndrome (SS). Pertinent labs included a positive c- anti-neutrophil cytoplasmic antibody (ANCA) and positive proteinase 3 (PR3) antibody. Additional work-up included a lung biopsy which revealed multinucleated giant cells and repeat skin biopsy with small and medium vasculitis, leading to a diagnosis of granulomatosis with polyangiitis (GPA). SS is a neutrophilic dermatosis that often presents with malignancy, drugs or infection. SS has rarely been

Use of nebulised tranexamic acid in adult patients with cystic fibrosis: A retrospective audit.

BACKGROUND: Haemoptysis is common in adults with cystic fibrosis (CF). Tranexamic acid (TA), an antifibrinolytic agent, blocks the binding of plasminogen and plasmin to fibrin to inhibit clot breakdown. Despite its theoretical benefits, there is limited data on the use of inhaled TA for management of haemoptysis in CF. METHODS: A 3-year retrospective audit of inhaled TA in CF patients with haemoptysis was conducted. Baseline demographics, hospitalisation status, haemoptysis volume, adverse events, adjuvant therapies and time to resolution of haemoptysis were extracted. We report on the safety

Idiopathic Pulmonary Haemorrhage, Is It Really Idiopathic?

Idiopathic pulmonary haemorrhage (IPH) is a rare cause of haemoptysis that most commonly presents in children. By definition, this condition is idiopathic in nature, however, exclusion of autoimmune capillaritis creates a diagnostic challenge. If autoimmunity is present, treatment is with immunosuppression. We present a 4-year-old girl with the classic clinical triad of IPH with haemoptysis, bilateral pulmonary infiltrate on chest radiography and iron deficiency anaemia. Her clinical course was complicated by frequent relapses in symptoms and critical pulmonary haemorrhage despite standard tre

Long term outcomes following embolisation of bronchial and non-bronchial systemic arteries for the management of haemoptysis - a 20-year experience.

BACKGROUND: Bronchial artery embolisation (BAE) is considered the most effective non-surgical technique for management of moderate-massive haemoptysis. Associated risks include neurological compromise such as stroke and spinal cord ischaemia. We aim to evaluate post-procedural outcomes and complication rates. MATERIALS AND METHODS: A single-centre retrospective observational study was conducted for BAE cases performed between January 2002-June 2022 in a London teaching hospital. Data was collected from electronic medical records and Picture Archiving Communications System (PACS). Primary outc

Fire: argon plasma coagulation, diathermy and laser.

Interventional pulmonology offers a range of minimally invasive techniques to manage airway pathologies, including central airway obstruction. This review examines three commonly used hot ablation therapies: argon plasma coagulation (APC), diathermy and laser. These modalities play a crucial role in treating intrinsic central airway obstruction caused by both benign and malignant conditions, as well as managing haemoptysis by achieving effective haemostasis. APC is a non-contact technique that delivers ionised argon gas to coagulate tissue, making it useful for superficial lesions and bleeding
