We report a case of bronchial Dieulafoy presenting with haemoptysis, initially resembling an endobronchial tumour on bronchoscopy. This case highlights the critical role of EBUS in the assessment of suspicious endobronchial lesions and in preventing potentially life-threatening complications from inadvertent biopsy.

Pulmonary vein (PV) stenosis is a rare but serious complication after transcatheter ablation for atrial fibrillation, potentially leading to massive haemoptysis. We present a case of severe PV stenosis resulting in haemoptysis. A 57-year-old man presented with haemoptysis 18 months after catheter ablation. His left PV was almost completely occluded and required the left pneumonectomy. Pathological examination revealed irreversible fibrosis of the PV intima, which led to pulmonary congestion. Surgical intervention resolved the haemoptysis. Anatomical pulmonary resection is required for massive

INTRODUCTION/OBJECTIVES: Metastatic tracheal melanoma is rare, with fewer than 20 reported cases. This case describes a 62-year-old female with a history of cutaneous melanoma excised 10 years prior, initially misdiagnosed with severe COPD. We highlight the diagnostic challenges when rare metastases mimic common conditions.
DESCRIPTION: Diagnosed with COPD based on dyspnoea and spirometry, the patient later developed worsening symptoms, including haemoptysis, requiring hospitalisation. A chest radiograph was unremarkable, but CT pulmonary angiogram revealed a 1.6 × 1.3 cm tracheal mass. Bronc

Proximal Interruption of the Pulmonary Artery (PIPA) is a rare congenital condition with an incidence of 1 in 200,000-300,000 individuals. We report the case of a 67-year-old woman with PIPA who presented with massive haemoptysis. Imaging revealed a small calibre right main pulmonary artery, absence of upper/middle lobe pulmonary arteries, and tortuous right systemic collateral arteries. A multidisciplinary meeting favoured bronchial artery embolisation over right pneumonectomy, due to the bleeding risk associated with extensive transpleural systemic collateral arteries. The patient underwent

Henoch-Schoenlein purpura (HSP) in children is the most prevalent form of vasculitis. While the lungs are recognised as potential target organs in several vasculitides during childhood, pulmonary involvement in HSP is exceptionally rare. A 5-year-old girl presented with haemoptysis during week 3 of HSP vasculitis. Radiological findings from a pulmonary computed tomography scan confirmed alveolar haemorrhage. The patient was administered pulse corticosteroid therapy at a dose of 30 mg/kg/day for 3 days, followed by a regimen of 2 mg/kg/day and azathioprine. During follow-up, the corticosteroid

Pulmonary artery pseudoaneurysms (PAAs) are exceedingly uncommon. It is a life-threatening diagnosis, and often associated with poor long-term prognosis. We report a case of PAA in a 74-year-old man presenting with severe respiratory failure and haemoptysis with a history of advanced lung cancer. Though the intent was to manage the PAA with coil embolisation, this was unsuccessful and the patient passed away 72 h after hospital admission. There is limited evidence for treatment of PAA, and once a patient is symptomatic, has a high mortality.

OBJECTIVE: Massive haemoptysis due to central pulmonary artery (CPA) rupture in patients with advanced lung cancer is a fatal complication with limited early diagnostic tools. This study aimed to identify risk factors associated with pulmonary artery rupture and to propose a grading model to facilitate early diagnosis and timely endovascular intervention.
MATERIALS AND METHODS: This retrospective study included patients with advanced lung cancer who experienced in-hospital sudden haemoptysis-related death and received endovascular treatment for CPA abnormalities. Propensity score matching (PS

Pulmonary hamartomas are the most common benign tumours of the lung and are often detected incidentally. Endobronchial hamartomas, though rare, can cause significant symptoms such as dyspnoea, haemoptysis, and recurrent respiratory infections due to bronchial obstruction. This report describes the successful bronchoscopic management of symptomatic endobronchial hamartomas in two young Indian males aged 29 and 34 years. Both cases were diagnosed based on radiological and histopathological findings. The first patient underwent eight sessions of electrocautery fulguration using a flexible broncho

Pulmonary aspergilloma can cause life-threatening haemoptysis. Surgical resection and/or bronchial artery embolization (BAE) are established treatment modalities, but both can be risky in frail patients with comorbidities. Spontaneous regression of aspergilloma with antifungal agents alone is rare. We report an elderly gentleman with a background history of treated pulmonary tuberculosis, who presented with haemoptysis due to a large left upper lobe aspergilloma. He declined surgery and BAE due to perceived risks. Oral voriconazole and later isavuconazole were prescribed, which led to clinical

Massive haemoptysis is a life-threatening condition requiring immediate and systematic management. Initial steps should focus on airway stabilisation, haemodynamic support and rapid identification of the bleeding source. Due to the complexity in diagnosis and management, along with a high associated mortality rate, massive haemoptysis offers an excellent opportunity for simulation training. Herein we present a simulated case of massive haemoptysis in a hospitalised patient. The scenario is crafted to help learners develop proficiency in the rapid recognition and multidisciplinary management of

BACKGROUND: The COVID-19 pandemic may have created an 'immunity debt', altering the epidemiology and aetiology of paediatric community-acquired pneumonia (CAP). However, multi-centre data on these changes and contemporary risk factors for severe CAP remain limited. We therefore aimed to analyse clinical characteristics and pathogen composition of CAP in children before, during, and after the COVID-19 pandemic, and to identify independent risk factors associated with severe CAP.
METHODS: We collected clinical data from paediatric inpatients diagnosed with CAP at 13 hospitals in 13 provinces of

BACKGROUND: Major aorto-pulmonary collateral arteries are congenital remnants of the segmental arteries that supply the lungs. They are commonly associated with congenital heart disease, particularly in cases where the main pulmonary artery branches are not well developed, these segmental collaterals do not regress and continue to supply oxygenated blood to the pulmonary vasculature. Over time, these collaterals can become problematic, particularly if they enlarge, leading to clinically significant symptoms such as haemoptysis, segmental pulmonary hypertension, and left heart volume overload.

Haemoptysis is an alarming symptom of a wide spectrum of underlying diseases, ranging from indolent chronic conditions to life-threatening states. Among the strategies to manage pulmonary bleeding is bronchial artery embolisation (BAE), an interventional radiology procedure. The objective of this scoping review was to map the current evidence on embolic agents used in BAE for haemoptysis management, with a focus on their clinical applications, and decision-making factors. Studies published between 2019 and 2024 were included if they specified the embolic material used and reported outcomes of

BACKGROUND: This case report highlights pulmonary artery rupture as a rare but severe complication during transcatheter valve-in-ring implantation (TVIRI). The case offers practical preventive strategies to enhance procedural safety in high-risk patients undergoing TVIRI.
CASE SUMMARY: A 54-year-old female patient presented with severe tricuspid regurgitation due to annuloplasty ring failure. The TVIRI was complicated by pulmonary artery rupture, resulting in massive haemoptysis and haemodynamic collapse. The bleeding was successfully controlled by prompt management with balloon tamponade and

Chronic thromboembolic pulmonary hypertension (CTEPH) is a rare but serious consequence of acute pulmonary embolism occurring in about 0.4%-6.2% of cases. Pulmonary infarction is an infrequent complication due to the lung's dual blood supply. Cavitary pulmonary infarction without infection is an exceedingly rare manifestation in CTEPH. We are reporting a 44-year-old woman who presented with lower limb swelling, cough, haemoptysis, and dyspnoea. Evaluation confirmed CTEPH with an aseptic cavitary infarct in the context of a heterozygous Factor V Leiden (1691G > A) mutation. She responded well t

We present a patient in his 40s with a history of melanoma excised 14 years prior, who developed persistent cough, haemoptysis, and chest pain. Imaging revealed a right hilar mass extending into the pulmonary artery and left atrium. PET-CT confirmed high 18F-fluorodeoxyglucose (FDG) uptake consistent with active tumour, while brain MRI showed metastatic lesions and cardiac MRI revealed intracardiac extension. An initial CT-guided lung biopsy was non-diagnostic. Due to diagnostic uncertainty, full anticoagulation (given the difficulty in distinguishing tumour from thromboembolism), and high pro
