Tongsai Granules Ameliorate Airway Mucus Hypersecretion in AECOPD by Remodeling LIPA-Driven Ceramide Metabolism.

ETHNOPHARMACOLOGICAL RELEVANCE: Tongsai Granules (TSG) is a traditional Chinese medicine formula traditionally used for the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with phlegm-heat obstructing lung syndrome, a condition characterized by cough, excessive sputum production, and dyspnea; however, its underlying therapeutic mechanisms remain unclear. AIM OF THE STUDY: To investigate the mechanisms by which TSG alleviates airway mucus hypersecretion in AECOPD through regulation of the LIPA/ceramide axis. MATERIALS AND METHODS: A murine AECOPD model and ai

Staged multi-target therapy for anti-MDA5-positive dermatomyositis-associated rapidly progressive interstitial lung disease: a case report.

Anti-melanoma differentiation-associated gene 5 antibody-positive dermatomyositis-associated rapidly progressive interstitial lung disease is associated with acute respiratory deterioration and high mortality. A man in his 40s presented with fever, dry cough, progressive dyspnea, mechanic's hand-like lesions and severe hypoxemic respiratory failure. Chest high-resolution computed tomography demonstrated rapidly progressive bilateral ground-glass and reticular opacities. Myositis-specific antibody testing showed strong anti-MDA5 positivity, with concomitant anti-EJ, anti-TIF1γ and anti-Ro52 pos

Indications, procedural standards, and training requirements for flexible nasolaryngoscopy in pulmonology - a Delphi study protocol.

Flexible nasolaryngoscopy (FNL) is a well-established, minimally invasive procedure for assessing the nasal passages, nasopharynx, and larynx. Its role in pulmonology is expanding, particularly for evaluating vocal cord dysfunction/inducible laryngeal obstruction, chronic cough, and other upper airway conditions relevant to the "united airways" model of care. However, no consensus-based guidance currently exists on the indications, procedural standards, or training requirements for FNL performed by pulmonologists. This protocol describes a modified two-round Delphi study that will survey an in

[Specific respiratory tract infections in acute cough-when is targeted diagnosis and treatment required?].

In most cases, acute cough is caused by a self-limiting respiratory infection. However, symptoms, pre-existing conditions, or environmental factors may indicate a cause requiring treatment; red flags point to dangerous courses, the NEWS2 score can assess the risk of septic deterioration, and the CRB-65 score supports decision-making regarding hospital admission in pneumonia. Pneumonia cannot be reliably diagnosed clinically but is unlikely in the absence of clinical indicators. Laboratory tests can support the decision for antibiotic treatment, while the diagnosis is confirmed by chest X‑ray.

Beyond Pneumonia: A Case of Steroid-Responsive Pulmonary Eosinophilia With Atypical p-ANCA Positivity.

Persistent pulmonary opacities despite antimicrobial therapy should prompt consideration of diagnoses beyond infection. We present a 76-year-old man with heart failure with moderately reduced ejection fraction, coronary artery disease, type 2 diabetes mellitus, and eczema treated with dupilumab who presented with progressive dyspnea, dry cough, weakness, hypoxemia, and a recent episode of hemoptysis. He had recently been hospitalized for presumed pneumonia and underwent bronchoscopy at an outside hospital before being discharged on amoxicillin, without significant improvement. Chest computed t

Self-reported prevalence of tuberculosis and selected associated vulnerability factors among rural populations in six districts of India: A community-based cross-sectional study.

BACKGROUND: Tuberculosis is a major public health challenge in India, contributing to one-fourth of global burden of TB disease. Comorbidities in uninfected individuals make them vulnerable to TB disease. This study estimates the self-reported prevalence of tuberculosis and selected vulnerability factors (HIV, current smoking, silicosis, occupational silica-dust exposure, diabetes mellitus and persistent cough >2 weeks) among the rural demography of six districts in India, with the aim of informing targeted active case finding under the National Tuberculosis Elimination Programme (NTEP). An Atypical Presentation of Richter's Transformation in Chronic Lymphocytic Leukemia: Stroke-Like Onset With Multiorgan Involvement Including the Heart and Seminal Vesicles.

Chronic lymphocytic leukemia (CLL) is usually an indolent lymphoproliferative disorder. However, a small proportion of patients may develop Richter's transformation (RT), in which CLL transforms into an aggressive lymphoma. This transformation is often associated with rapid clinical progression and extranodal disease. Here, we describe a 72-year-old man diagnosed with CLL during evaluation for persistent cough and leukocytosis. Peripheral blood smear demonstrated smudge cells, and the diagnosis was confirmed by peripheral blood flow cytometry. As the patient was asymptomatic, he was managed wi

Endocardial thickening in dilated cardiomyopathy associated with FHL2 variant: a case report.

BACKGROUND: FHL2 is critical for cardiac development, and its variants are associated with early-onset severe dilated cardiomyopathy (DCM), though such reports remain rare. CASE PRESENTATION: The patient was a 30-day-old male infant admitted with a 3-day history of cough, a 2-day history of poor feeding, and lethargy lasting half a day. Echocardiography revealed left ventricular enlargement, with a minimum left ventricular ejection fraction (LVEF) of only 13%, and endocardial thickening. Genetic testing identified a heterozygous missense variant c.391C>T (p.Arg131Cys) in FHL2. This variant wa

Tropheryma whipplei as the Dominant Organism in Bronchoalveolar-Lavage Metagenomic Next-Generation Sequencing: A Single-Centre Case Series of 28 Patients.

Background/Objectives: Metagenomic next-generation sequencing (mNGS) of bronchoalveolar lavage fluid (BALF) increasingly reports Tropheryma whipplei (T. whipplei) in patients without classical Whipple disease, but the significance of these detections is often uncertain. We aimed to quantify relative sequencing abundance, characterise the accompanying clinical, radiological, and routine laboratory phenotype, and develop a structured framework for interpretation. Methods: We retrospectively reviewed 944 consecutive BALF-mNGS tests performed at Tianjin Chest Hospital between October 2022 and Marc

Massive Esophageal Dilatation Following Laparoscopic Adjustable Gastric Banding Presenting as Acute Respiratory Distress After Swallowing Effervescent Granules: A Case Report.

Esophageal dilatation is a recognized late complication of laparoscopic adjustable gastric banding (LAGB), resulting from chronic outflow obstruction and esophageal dysmotility. Although most cases are clinically mild, severe dilatation can produce mass effect on adjacent mediastinal structures and rarely cause acute cardiopulmonary compromise. A 63-year-old female with a history of obesity, hypertension, glaucoma, and LAGB placement more than 15 years earlier presented with acute respiratory distress during an upper gastrointestinal contrast study. Shortly after swallowing effervescent granul

Nocardiosis: death from the ground.

Nocardiosis is a rare but potentially life-threatening infection caused by aerobic, filamentous, gram-positive bacteria of the genus Nocardia found in soil and water. Although traditionally found in immunocompromised patients, up to one-third of cases occur in immunocompetent individuals, contributing to frequent diagnostic delays. Historically fatal before the antibiotic era, outcomes have substantially improved with modern antimicrobial therapy and diagnostic advances. However, nocardiosis remains underrecognized due to its nonspecific presentation and ability to mimic more common diseases.

Large Carinal Chondrosarcoma: Resection of the Main Carina, Distal Trachea, and Orifices of the Mainstem Bronchi and Construction of a New Carina.

BACKGROUND Chondrosarcoma of the trachea is an extremely rare malignancy, and tumors located at the main carina are even rarer. Carinal resection and subsequent reconstruction of a new carina constitute a demanding surgical procedure that requires extensive expertise and close collaboration between the anesthetic and surgical teams. In this case report, we aim to highlight the rarity of chondrosarcoma of the main carina and emphasize the critical role of surgical intervention despite the complexity of the procedure. Furthermore, we aim to share our clinical experience and contribute an additio

Effectiveness of Diphtheria and Tetanus Toxoids and Acellular Pertussis Vaccine in Children Aged 6 Months-6 Years During a Statewide Pertussis Outbreak - Alaska, May-November 2024.

Widespread use of pertussis vaccines in children has resulted in a >90% reduction in the number of annual pertussis cases in the United States. After a period of low U.S. pertussis incidence during and shortly after the COVID-19 pandemic, incidence increased in 2024. Data on the real-world effectiveness of diphtheria and tetanus toxoids and acellular pertussis vaccine (DTaP) against pertussis during this disease resurgence are limited. In response to documentation of a record high pertussis incidence in Alaska during May-November 2024, DTaP vaccine effectiveness (VE) against pertussis in Alask

[Frecuency of Bordetella pertussis infection in adults: a multicenter study in Argentina].

INTRODUCTION: The epidemiology of Bordetella pertussis (Bp) has changed following universal pediatric vaccination, with an increase in cases among adolescents and adults, who may have a higher risk of complications and behave as a reservoir for infection in infants. Reported prevalence in adults ranges from 2% to 32% worldwide. The aim of this study was to assess the frequency of Bp infection in adults with cough in Argentina. MATERIALS AND METHODS: This was a prospective, multicenter, observational cross-sectional study conducted between August 2023 and August 2024, including individuals age

RSV Molecular Epidemiology Pre- and Post-COVID-19 Pandemic Periods in Seattle, Washington, USA.

BACKGROUND: Globally, RSV declined to historically low levels following stay-at-home orders in 2020 and re-emerged as restrictions lifted, with large inter-seasonal outbreaks and other shifts in epidemiology. The effect of the pandemic restrictions on genomic diversity has not been well studied. METHODS: Individuals with respiratory symptoms were enrolled into community-based surveillance studies and provided nasal swabs over three respiratory virus seasons from 2019 to 2022 in the Seattle metropolitan area in Washington State, USA. Samples were tested for RSV by RT-qPCR, with whole genome se

[Systemic sclerosis with anti-human neutrophil cytoplasm antibody associated vasculitis: A case report].

Systemic sclerosis (SSc) is an autoimmune disease involving microvessels and a wide range of connective tissues. Anti-human neutrophil cytoplasm antibody-associated vasculitis (AAV) is a primary, systemic necrotizing small vasculitis. The coexistence of AAV and SSc is rare. We report a case of SSc combined with AAV complicated with alveolar hemorrhage, gastrointestinal hemorrhage, and cerebral hemorrhage. The patient was a 56-year-old woman with a 20-year history of Raynaud phenomenon, skin sclerosis on the fingertip and face. She developed dry cough for 1 year and shortness of breath after ac

Influenza: Prevention, Diagnosis, and Treatment.

Influenza is typically a self-limiting viral infection that causes symptoms such as fever, chills, cough, rhinitis, headache, sweats, myalgias, and malaise. However, serious illness and complications (eg, pneumonia, death) can occur, particularly in individuals with risk factors. Annual influenza vaccination is recommended for people 6 months and older, including pregnant individuals, who do not have a contraindication to the vaccine. Rapid diagnosis of influenza using point-of-care testing in select patients increases antiviral prescribing and decreases antibiotic prescribing in those with a

Dissecting Adult Peripheral Cellular Immunity to Bordetella pertussis: Innate-Associated IFN-γ Responses, Transient T-Cell Recall, and HLA-A*02-Associated CD8+ Reactivity.

Bordetella pertussis elicits both innate and adaptive cellular responses, yet their relative contributions to peripheral interferon-γ (IFN-γ) measurements and the temporal dynamics of circulating T-cell reactivity in adults remain incompletely defined. B. pertussis-reactive cellular immunity was investigated in 42 adult donors, including 26 healthy donors and 16 donors with a history of pertussis, using standard and modified IFN-γ enzyme-linked immunospot (ELISpot) assays with a chemically inactivated whole-cell B. pertussis preparation, bacterial innate-biased stimuli, and a peptide-based T-c
